Syndactyly is one of the most common congenital malformations of the hand/foot, characterized by varying degrees of soft tissue and/or skeletal fusion between adjacent fingers or toes. It may also occur alongside polydactyly, camptodactyly, brachydactyly, or congenital joint fusion. The heterogeneity in its clinical phenotypes and incomplete penetrance makes it challenging to establish clear diagnostic and treatment protocols/procedures. Conventionally, the management of syndactyly has mainly relied on clinical experience, with limited reference to cutting-edge genetic research and systematic

BACKGROUND: Ectrodactyly, also called split hand/foot malformation, is a rare birth defect where some middle fingers or toes are missing or not formed properly, making the hand or foot look split. This condition can happen on its own or as part of more complicated syndromes such as ectrodactyly-ectodermal dysplasia-cleft syndrome. Surgery for young patients is usually tailored to their specific needs on the basis of how much the condition affects their ability to function and their appearance.
CASE PRESENTATION: In this manuscript, we reported a case of a Punjabi 6-year-old female presenting

Amniotic Band Sequence is a rare congenital disorder that can result in a vast array of limb deformities, including pathognomonic fenestrated syndactyly. While current surgical approaches prioritize the early release of constriction bands to address ischemia and lymphoedema, syndactyly release is often delayed until the child is 3-6 months old. This letter advocates for earlier (less than 3 months) fenestrated syndactyly release when feasible to enhance joint mobility, encourage finger growth, and support overall hand development in affected newborns. We present the minimally invasive procedur

Surgical correction of polysyndactyly of the toes requires not only functional separation of the digits but also creation of an aesthetically acceptable interdigital web, as postoperative appearance strongly affects psychosocial outcomes. Although traditional techniques, including Z-plasty and dorsal rectangular flaps as well as web-preserving flaps with or without skin grafting, have been described, complications such as web creep, wound dehiscence, and valgus deformity of the residual toes remain problematic. Here, we report a case of polysyndactyly involving the fourth, fifth, and sixth toe

Cell identity plays a pivotal role in embryo development, guiding the process of cellular differentiation essential for tissue and organ formation. Post-translational modification by the ubiquitin-related SUMO protein acts as a chromatin barrier to cell fate conversions. While SUMOylation deficiency is incompatible with mammalian embryonic development, haploinsufficiency for the SUMOylation machinery's E1 enzyme, UBA2, leads to various phenotypic traits in humans, including craniofacial malformations and aplasia cutis congenita. To investigate SUMO's role in organogenesis, SUMOylation was tran

We present a rare case of a pediatric patient diagnosed with Gaucher disease, a lysosomal storage disorder, co-occurring with hallux duplication and syndactyly, congenital foot malformations. The patient exhibited extreme and uncommon manifestations of Gaucher disease, including massive splenomegaly and severe osteoporosis localized to the affected extremities, which complicated the treatment of the foot deformities. This case is notable due to the unique interaction between two genetically distinct conditions. We explore the clinical implications, treatment challenges, and potential genetic f

BACKGROUND: Bardet-Biedl Syndrome (BBS) is a rare obesogenic disorder affecting multiple organs. The diagnosis of BBS is usually difficult and delayed due to this syndrome's wide variety of clinical features. This study aims to assess the rate of congenital melanocytic nevi (CMN) in the BBS population in an effort to bring light to an easily assessable and early manifestation of BBS to aid in earlier diagnosis.
METHODS: We utilized a survey distributed to patients with BBS registered within the Clinical Registry Investigating Bardet-Biedl Syndrome database. Analysis was performed to identify

INTRODUCTION: Nasal deviations have been observed in patients following midface surgery. Therefore, the purpose of this study was to evaluate the development of clinically visible nasal deviations following midface surgery and to assess the natural course of the nose over time.
METHODS: This retrospective study included all Apert and Crouzon patients who underwent midface surgery (Le Fort III (LF3), monobloc (MB), or facial bipartition (FB)). Clinical nasal deviation was assessed on preoperative, short-term postoperative (≤ 1 year), and long-term postoperative (> 1 year) 2D facial photographs

Children with syndromic craniosynostoses face high risk of obstructive sleep apnoea (OSA), but the underlying mechanisms remain unclear. This study investigated how Crouzon and Apert syndromes affect airway morphology and contribute to OSA, controlling for age and posture. We also examined the covariation between the upper airways and the craniofacial region (skeleton and soft tissues). We analysed 37 patients with syndromic craniosynostoses, and 53 control children aged from 0 to 49 months old. A set of 52 landmarks was placed on the 3D models of the upper airways and collected, when availabl

BACKGROUND: Syndactyly of the toes is a common congenital malformation, yet there are few reports of long-term outcomes after toe syndactyly repair and its effect on well-being. In this study, we surveyed patients and parents of patients who underwent toe syndactyly repair to investigate the functional and psychological outcomes of their surgery.
METHODS: We reviewed the medical records of all of the patients who underwent toe syndactyly repair using a plantar flap technique between January 1, 2010, and January 1, 2021. Parents or patients completed a survey assessing their opinion regarding

The separation of individual digits is dependent on establishment of digit-interdigit periodicity, remodeling of the interdigital mesenchyme, and invagination of interdigital epithelial tongues. In Protein O-glucosyltransferase 2 and 3 double knockout (Poglut2/3 DKO) mice, digits 2 and 3 are fused, suggesting a defect in one or more processes. POGLUT2/3 add O-linked glucose to epidermal growth factor-like (EGF) repeats. Syndactyly is also observed when genes encoding the POGLUT2/3 substrates fibrillin 2 (FBN2) or both Nidogen 1 and 2 (NID1/2) are knocked out, suggesting that O-glucosylation is

Genetic mutations are closely linked to various renal diseases, revealing important molecular mechanisms that contribute to kidney dysfunction. Here, we reported a 35-year-old Chinese female diagnosed of glomerulotubular nephropathy with multiple extra-renal manifestations including ptosis, corneal dystrophy, macular degeneration, right foot syndactyly. Whole-exome sequencing identified a homozygous frameshift variant in FAT1 (NM_005245: c.7444_7445delGT, p.Val2482AsnfsTer16). Further analysis revealed that this mutation caused translation repression of FAT1. RNA sequencing showed dysregulatio

Epidermolysis Bullosa (EB) is a rare disease characterized by the formation of blisters and vesicles on the skin and mucous membranes. There are 4 types: simple, junctional, dystrophic, and Kindler syndrome. They can have serious complications such as difficult airway, syndactyly, wound superinfection, or squamous cell carcinoma. We present the case of a 6-year-old male patient with severe generalized recessive dystrophic EB, surgically intervened for syndactyly in the left upper limb and cures in both lower limbs. These patients have a high risk of suffering adverse events related to the surg

PURPOSE: Epidermolysis bullosa is a rare inherited skin disorder that leads to extreme sensitivity and blistering from even slight injuries. Frequent blistering on the hands can result in deformities, and in severe cases, the hands may become enclosed in epithelial tissue, a condition referred to as "mitten deformity." In this study, we aimed to develop a modified surgical technique for the treatment of hand deformities and pseudosyndactyly in patients with epidermolysis bullosa without the aid of any skin grafts or allografts.
METHODS: We performed a retrospective review of 14 hands affected

Apert syndrome is a rare congenital defect characterised by bicoronal synostosis, midface hypoplasia and syndactyly. Fronto-orbital advancement (FOA) is a technique for surgical correction of craniosynostosis in Apert syndrome, typically done on infants under a year old. Aside from the concern for a difficult airway and other associated systemic anomalies, thorough and meticulous anaesthetic care must be employed in the management of physiologic changes during prolonged, complex surgeries such as FOA. This case report describes the anaesthetic management of a female infant with Apert syndrome
