Fraser syndrome is a rare autosomal recessive disorder characterized by multiple congenital malformations, including cryptophthalmos, syndactyly, and renal agenesis, which can lead to severe complications beginning at the embryonic stage. Mutations in genes encoding extracellular matrix proteins such as FRAS1, FREM1, FREM2, and the associated trafficking protein GRIP1, are implicated in Fraser syndrome. These proteins are critical for maintaining epithelial integrity during embryogenesis, with deficiencies leading to tissue detachment and blistering phenotypes in mouse models. The FREM2 protei

INTRODUCTION: Pfeiffer syndrome is characterized by craniosynostosis, midfacial hypoplasia, and severe ocular proptosis, with complex surgical management and a high risk of relapse. Despite recent advances in surgical techniques and multidisciplinary care, evidence on the craniofacial morphology and growth patterns remains limited. This study aims to provide a comprehensive analysis of cranial base and midfacial morphology and growth pattern in patients with Pfeiffer syndrome.
MATERIAL AND METHODS: Twelve CT scans from patients with Pfeiffer syndrome and ten age-matched controls were retrospe

Syndactyly is a common congenital malformation of the hand, characterized by fusion of adjacent digits. Early surgical correction is recommended to prevent functional limitations and esthetic concerns. We report the case of a three-year-old girl with congenital simple complete cutaneous syndactyly between the third and fourth fingers of the left hand. The fusion was limited to soft tissues, with fully formed and separate fingernails. Surgical separation was performed using interdigital dorsal and palmar Z-plasty incisions, followed by full-thickness skin grafting harvested from the groin donor

Congenital hallux varus (CHV) is an uncommon deformity of the forefoot, defined by the medial deviation of the great toe at the metatarsophalangeal joint, often accompanied by a wide gap between the first and second toes. This condition is associated with footwear difficulties, pain impacting quality of life, and aesthetic concerns. CHV may occur in isolation or alongside other congenital foot abnormalities, such as polysyndactyly or a longitudinal epiphyseal bracket (LEB). We present the case of a 19-month-old boy with bilateral CHV featuring a shortened first metatarsal and significant funct

BACKGROUND: This study aimed to investigate the clinical effects of bone fusion distal to the phalangeal epiphyseal plate in treating asymmetrical type III thumb polydactyly, focusing on growth and scar healing.
METHODS: In patients with asymmetrical type III thumb syndactyly, the proximal phalanx of the accessory finger, distal epiphyseal plate, and some soft tissue were excised to create a pedicled tissue flap. This flap, containing part of the nail and bone distal to the epiphyseal plate, was subsequently attached to the distal segment of the main finger. The therapeutic effect was evaluat

We report the extremely rare case of a digital and plantar retractile keloid in a 32-year-old patient with a history of spontaneous keloids. Since childhood, she has developed hypertrophic scars following minor trauma, extending beyond the original wound margins. She currently presents a left IV-V syndactyly and a complete flexion contracture of the fifth finger (Tubiana stage IV), as well as a retraction of the right hallux, resulting in functional impairment. The goal of surgical treatment is to restore extension. Retractile keloids occurring in the context of spontaneous keloids are unusual

Keloid formation following elective syndactyly surgery is an uncommon but difficult complication to manage. This report presents a case in which oral methotrexate was successfully used as an adjunctive postoperative therapy to prevent keloid recurrence in a paediatric patient. A 12-month-old Somali girl developed extensive, treatment-resistant keloids after syndactyly release of the second web space on her right hand. Conventional treatments with silicone gel and intralesional triamcinolone proved ineffective. Surgical excision of the keloids and reconstruction with full-thickness skin grafts

The purpose of this investigation was to investigate short finger and cleft type symbrachydactyly to identify the defining features of these two subtypes. This retrospective study extracted data from the Congenital Upper Limb Differences (CoULD) Registry. Five paediatric hand surgeons reviewed photographs and radiographic imaging to evaluate the characteristics of each subtype and the reliability of diagnosis and evaluated the recorded diagnosis in the registry. Final analysis included 50 short finger and 54 cleft type symbrachydactyly. Key characteristic patterns were identified for each type

Syndactyly-polydactyly-ear lobe (SPEL) syndrome is a rare, genetic, congenital limb malformation syndrome that can be characterized by 6 different human phenotypes. We report a rare case of SPEL syndrome in a young woman with right earlobe malformations and asymptomatic preaxial polydactyly with partial duplication of the right hallucal distal phalanx without a supernumerary toe. Unique aspects of our patient's clinical presentation include lack of a supernumerary digit with a bifurcated distal phalanx, associated earlobe malformations, adult age, and no reported familial history of SPEL syndr

BACKGROUND: Polydactyly is a prevalent limb deformity with an autosomal dominant inheritance pattern, manifesting in both syndromic and nonsyndromic forms. It exhibits significant etiological and clinical diversity. This study aims to identify the pathogenic cause in two patients with sub-PHS (sub-Pallister-Hall Syndrome) and PAP (postaxial polydactyly), respectively, from two Chinese pedigrees.
METHODS: Exome sequencing was performed on patients to screen for potential pathogenic variants. Subsequently, these variants were validated by Sanger sequencing. The c.3342dupC and c.4431dupT mutant

Background: Syndactyly, the congenital fusion of digits, compromises hand function and esthetics. Although surgical separation is the standard treatment, the optimal timing of the intervention remains controversial. Methods: We prospectively analyzed 20 pediatric patients (86 operated fingers) undergoing syndactyly repair, comparing early (≤24 months) versus delayed (>24 months) surgery. Outcome measures included range of motion (ROM) at the metacarpophalangeal (MP), proximal interphalangeal (PIP), and distal interphalangeal (DIP) joints; complications (synostosis, nail deformities, finger len

Recurrent somatic mosaic pathogenic variants of RHOA have been observed in a newly identified neuroectodermal syndrome, Ectodermal Dysplasia with Facial Dysmorphism and Acral, Ocular, and Brain Anomalies, Somatic Mosaic [EDFAOB]. All 12 previously reported patients had somatic mosaicism for RHOA variants. Conversely, no patients with non-mosaic germline variants of RHOA have been reported. The absence of non-mosaic patients has been explained by the presumed lethal effect of all RHOA variants in non-mosaic status. Here we report an 11-month-old female with EDFAOB-like features but without Blas

INTRODUCTION: Symbrachydactyly is a unilateral hand abnormality characterized by incomplete finger formation and rudimentary nubbins containing nail plate, bone, and cartilage. Surgical intervention typically targets web space contractures, digit instability, and pincer dysfunction. This study examines the relationship between the severity of symbrachydactyly and the choice of operative or nonoperative approaches.
METHODS: A retrospective comparative analysis was conducted on patients diagnosed with symbrachydactyly at a pediatric referral center from 2005 to 2024. Collected data included dem

Syndactyly release is a surgical procedure aimed at restoring hand and finger function while maintaining the natural appearance of the interdigital web space (commissure) as much as possible. The timing of surgery depends on the complexity of the fusion, the patient's age, and their overall medical condition. The traditional technique is based on a zigzag incision and the use of skin grafts combined with local flaps. However, it is associated with a relatively high incidence of complications, such as web creep, hypertrophic scars, flap necrosis, and contractures. This article provides a genera

BACKGROUND: The BHLHA9 gene, a member of the basic helix-loop-helix (bHLH) family of transcription factors, plays a critical role in limb development. Mutations in BHLHA9 have been associated with various limb malformations, including syndactyly and split-hand/foot malformation. This study aimed to identify and characterize novel BHLHA9 variants in a fetus with complex limb and renal abnormalities, providing further insights into the genetic basis of developmental disorders.
METHODS: We performed Exome sequencing (ES) on a fetus with severe limb malformations and renal anomalies, along with t

CASE: An 11-month-old male child presented with severe congenital thumb hypoplasia, syndactyly of the thumb and index finger, and radial longitudinal deficiency. A novel three-stage surgical approach was performed to achieve functionality while respecting familial preference for thumb preservation using sequential syndactyly release, structural reconstruction using bone graft, and muscle and tendon transfers.
CONCLUSION: The reconstructed thumb demonstrated early wound healing, stable alignment, and maintained a 45-degree palmar abduction even up to 5 years postoperatively. Despite minor fine
