POGLUT2 and POGLUT3: Two essential protein O-glucosyltransferases modifying EGF repeats in extracellular matrix proteins.

O-glycosylation of Notch epidermal growth factor-like (EGF) repeats has long been studied in the context of protein folding, secretion from cells, and protein function. Originally, the protein O-glucosyltransferase, POGLUT1, was thought to be the only enzyme adding an O-glucose modification to EGFs, specifically to a serine between cysteine one and two of a six cysteine containing EGF repeat. The POGLUT1 O-glucose can be elongated on Notch EGFs with xyloses by GXYLT1/2 and XXYLT1, forming a trisaccharide. Mouse knockouts of Poglut1 are embryonic lethal with Notch1-related phenotypes. Recently,

Loss of POGLUT2/3-mediated O-glucosylation produces lung and aortic phenotypes reminiscent of fibrillin1 mutants.

BACKGROUND: Fibrillins provide a scaffold for elastic fiber formation, which enables lung recoil and aortic compliance. Abnormal fibrillin microfibrils, as in Marfan syndrome, lead to enlarged alveoli, vascular stiffening, and aneurysms. Our earlier studies suggested that fibrillin function depends on O-glucosylation of its epidermal growth factor (EGF)-like repeats by protein O-glucosyltransferase 2 (POGLUT2) and 3 (POGLUT3). Mice lacking both enzymes (Poglut2/3 DKO) die soon after birth. Rare survivors are runted, have digit 2-3 syndactyly, and variable eye defects. Here, we wanted to determ

Timothy syndrome and CACNA1C-Related Disorder: first international language and management guidelines consensus statement.

Timothy syndrome is a multisystemic genetic disorder, classically characterised by prolonged QT interval and subsequent cardiac arrhythmias, neurodevelopmental disorders including developmental delay and autism and syndactyly or hip dysplasia. It is caused by variants in the CACNA1C gene, which encodes the widely expressed Cav1.2 voltage-gated calcium channel. Since its characterisation in 2004, the number of variants in CACNA1C associated with Timothy syndrome has expanded. With advances in sequencing and the inclusion of CACNA1C in genomic screening, further variants have been identified pre

Isolated Unilateral Heptadactyly With Combined Preaxial and Postaxial Polydactyly of the Foot in a 9-Month-Old Infant.

BACKGROUND: Polydactyly is a common congenital limb anomaly characterized by supernumerary digits, most frequently involving the hands or feet. Foot polydactyly is traditionally classified into preaxial, postaxial, and central types, with postaxial forms being the most common and central forms the rarest. Heptadactyly, defined as the presence of seven digits in a single limb, represents an exceptionally rare and severe expression within this spectrum, most often described as central duplication. Isolated unilateral heptadactyly of the foot with combined preaxial and postaxial duplication is ex

New Surgical Technique for Cleft Hand and Foot Using a Lateral Pantaloon Flap.

Various surgical techniques for cleft hand closure have been reported. Cleft closure aims to improve cosmetic appearance, and the form of interdigital spaces should be drawn closer to normal. Further innovations are required to reconstruct natural interdigital contours. The pantaloon flap used in syndactyly surgery provides a natural interdigital formation. We focused on this aspect and used the pantaloon flap as a lateral stem for cleft hand closure. This technique was applied in 3 cases of cleft hand (4 hands) and 2 cases of cleft foot (2 feet), classified as Manske types I, IIA, III, and IV

Apert Syndrome: Oral, Maxillofacial and Dental Management-A Narrative Clinical Review.

Apert syndrome is a rare genetic disorder characterized by premature fusion of the cranial sutures, syndactyly of the extremities, and distinct craniofacial deformities. The condition results from mutations in the FGFR2 gene, which disrupt normal craniofacial growth and lead to complex functional and morphological abnormalities. Patients with Apert syndrome commonly present with stomatognathic abnormalities, which significantly affect oral function and facial development. The management of Apert syndrome requires a multidisciplinary therapeutic approach. Surgical treatment strategies are typic

A Chinese infant with CACNA1C p.C1021R variant expands clinical phenotype of CACNA1C-related disorder.

CACNA1C-related disorder (CRD), a severe multisystem disorder caused by variants of CACNA1C gene, presents significant diagnostic and management challenges due to its rarity and variable expressivity. This study leverages a detailed longitudinal case report and comprehensive literature review to expand the phenotypic spectrum of CRD with Timothy syndrome (TS) features associated with the CACNA1C NM_001129830.1: c.3061 T > C (p.Cys1021Arg) variant. We describe an individual with CRD exhibiting a severe multisystem phenotype including classic features like syndactyly, profound neurological sympt

Web Creep Reoperation Following Simple Syndactyly Release Using Full-Thickness Skin Grafts and Skin Graft-Sparing Local Flaps.

BACKGROUND: Web creep following syndactyly release is a well-documented problem, yet studies focusing on simple syndactyly are limited. While numerous approaches, including local flaps and skin grafts, have been proposed for skin coverage, existing research lacks sufficient power or follow-up duration to accurately assess web creep rates. This study aimed to compare web creep outcomes between skin graft-sparing local flaps and full-thickness skin grafts after simple syndactyly release. METHODS: This is a single-institution, retrospective review encompassing patients identified using CPT codes

Complication Rates of Casts versus Bulky Dressings after Syndactyly Release.

BACKGROUND: The arms are commonly immobilized in a cast after syndactyly release. Despite this practice, there is little research examining complications from cast immobilization. This study compares the complication rates of congenital syndactyly release using casts and bulky soft dressings. METHODS: A retrospective review was performed of patients undergoing syndactyly release. Patients were included if they underwent a primary release of congenital syndactyly and excluded if they had a noncongenital cause. Data regarding surgical procedures, medical complications, and unplanned caregiver p

Morphological analysis of posterior fossa in Apert and Crouzon syndromes before and after posterior cranial vault expansion.

BACKGROUND: Syndromic craniosynostosis, such as Apert and Crouzon syndromes, involve premature fusion of cranial sutures, leading to cranial deformities and potential neurological complications. Posterior cranial vault expansion (PCVE) is commonly used to address intracranial volume (ICV) anomalies in these cases, especially in the posterior fossa region. However, the morphological variation of this region between normal, Apert and Crouzon individuals remains understudied. This study aimed to carry out a detailed morphological analysis of the posterior fossa between the aforementioned groups b

Toward in vivo assessment of skin mechanical properties for flap design: challenges, methods, and technical pathways.

There is a strong need to quantify skin mechanical properties for flap design in plastic surgery, such as syndactyly reconstruction. Quantitative measurement of skin mechanical properties enables more accurate surgical planning, thereby improving surgical outcomes and reducing complications. Although a large number of methods and devices have been proposed, nearly none of them have been applied in surgical practice. We therefore conducted a comprehensive narrative review of the literature published from 1969 to the present on in vivo skin mechanical property measurement. To identify the factor

Expanding the clinicopathological spectrum of nevoid basal cell carcinoma syndrome associated with odontogenic keratocyst in the Indian population: an institutional experience of 39 cases and review of the literature.

Nevoid basal cell carcinoma syndrome (NBCCS) is a rare autosomal dominant hereditary condition associated with developmental abnormalities and tumourigenesis with a prevalence rate of 1:60,000. The objective of this study was to identify NBCCS in patients diagnosed with odontogenic keratocysts (OKC) at Nair Hospital Dental College in Mumbai, India, and to compare the findings with those from Indian and international studies. Archived institutional data on OKC cases from the years 2009-2024 were analysed for NBCCS based on the criteria established by Kimonis et al. in 1997. Of the 231 cases of

Fraser Syndrome in a Newborn: Diagnostic Challenges Based on Clinical Criteria.

Fraser syndrome is a rare autosomal recessive congenital malformation syndrome characterized by a wide clinical spectrum, including ocular, limb, genital, and visceral anomalies. Diagnosis is suggested by the association of multiple characteristic malformations. We report the case of a newborn admitted to the Neonatology and Neonatal Intensive Care Unit of Mohammed VI University Hospital in Tangier, born to consanguineous parents, who presented with a polymalformative syndrome associating bilateral anophthalmia with ankyloblepharon, syndactyly, genital anomaly, anal imperforation, and low-set

Multiple congenital malformations in a burrowing owl (Athene cunicularia) from Southern Brazil.

This study reports a case of multiple congenital malformations in a burrowing owl (Athene cunicularia) rescued in southern Brazil. Clinical assessment revealed partial agenesis of the forelimbs, bilateral asymmetric syndactyly, and selective anonychia of the second digit in both pelvic limbs. Despite severe locomotor limitations and the inability to fly, the individual maintained a satisfactory body condition score, demonstrating the species' adaptive and behavioural capacity. The integrity of the epithelial tissues and the symmetry of the digital alterations support a teratogenic etiology, di

The Impact of Genetics on Craniofacial Dysplasias and Consequent Oral Malformations-Integrative Review.

BACKGROUND/OBJECTIVES: Diseases affecting the craniofacial skeleton are normally associated with disturbances in the regulation of cellular differentiation, the development of bone structures, and changes in bone density and ossification. Thus, the objective of this integrative review is to evaluate the published scientific literature from the last 8 years concerning the impact of genetics on some craniofacial dysplasias. Our aim covers the identification of oral cavity alterations to those dysplasias, through the most common orofacial manifestations. Three dysplasias were selected to be part

Parental Aesthetic Anxiety, Disability Aesthetics, and the Ethics of Surgical Normalization in Congenital Hand Differences.

Children born with congenital hand differences (syndactyly, fusion of adjacent digits; polydactyly, duplication of digits; radial club hand, partial or complete absence of the radius; and symbrachydactyly, short, webbed, or absent fingers) occupy a unique and underexamined position at the intersection of pediatric surgery and disability studies. While surgical literature has meticulously documented operative techniques, functional outcomes, and complication profiles, it has almost entirely neglected the aesthetic and cultural dimensions of the decision to operate. This narrative review address
