Prenatal and Postmortem Characterization of FGFR2-Related Fetal Craniosynostosis: Emphasizing Rare and Atypical Anomalies.

OBJECTIVE: Craniosynostosis (CS) is the second most common craniofacial birth defect after orofacial clefts. Genetic counseling is essential for reproductive planning in affected families. METHODS: Nine fetal CS cases-six Apert syndrome (AS) and three Pfeiffer syndrome (PS)-with established genetic causes were retrospectively analyzed for clinical, radiological, and molecular features. RESULTS: Mean gestational age at first examination was 26.2 weeks. Four AS and two PS cases were diagnosed during the prenatal stage by ultrasonography (USG), whereas two AS and one PS cases were identified du

Children With Synpolydactyly Type I Homozygous HOXD13 Mutations: Embryology and Surgical Correction of the "Cupping" of the Hands and Feet.

PURPOSE: Some patients with familial Synpolydactyly Type I have severe flexion deformities of the hands and feet at the metacarpophalangeal and the metatarsophalangeal joints, and the resulting deformity has been named "cupping" of the hands and feet. Severe "cupping" requiring surgical correction has not been previously reported. We explore the embryology and pathogenesis of the phenotype and present the surgical technique and results after the correction of the cupping deformity in a case series of 3 children. PATIENTS AND METHODS: Two children had severe deformity of the postaxial toes, an

Mapping Double-Bond Positional Isomers of Phosphatidylcholines in Zebrafish Using OzMALDI Mass Spectrometry Imaging.

There is good evidence from the medical literature that type I syndactyly, the most common form of the nonsyndromic syndactylies, is clinically heterogeneous. We therefore propose to group the condition into four subtypes, which are all autosomal dominantly inherited. Subtype 1, zygodactyly (cutaneous webbing of second and third toe without hand involvement) is the mildest and most common form. The phenotype varies from unilateral minor impression of webbing to bilateral complete webbing of second and third toe including a fusion of nails. Bony involvement is never observed. Subtype 2 is chara

Fractures of the base of the proximal phalanx: Palmar approach combined with palmar plating.

INTRODUCTION: Displaced fractures of the base of the proximal phalanx (PP) remain a challenging surgical problem. Regardless of the osteosynthesis material chosen (K-wires, screws, plates), when open reduction is required, the surgical approaches typically used are either dorsal or lateral. These approaches inevitably cause surgical trauma to the extensor apparatus which, via the intrinsic system, envelops the dorsal and lateral surfaces of PP at this level. To preserve the extensor apparatus, the authors propose a palmar approach combined with anterior plate fixation. PATIENTS AND METHODS: T

Combined Long-Read Genome and Transcriptome Sequencing Establishes Novel Variants in MEGF8 as the Cause for Carpenter Syndrome Type 2.

Carpenter syndrome type 2 (CRPT2) is a rare autosomal recessive disease mainly characterized by craniosynostosis and polysyndactyly. CRPT2 is the rarer subtype of Carpenter syndrome (CRPTS) and is caused by biallelic variants in the multiple epidermal growth factor-like domains 8 gene (MEGF8). Due to its rarity and phenotypic overlap with other craniosynostosis syndromes, definitive molecular diagnosis of CRPT2 can be challenging. Here, we describe a proband with CRPT2 carrying compound heterozygous variants in MEGF8: one de novo variant disrupting splicing and a maternally inherited missense

Predictors of early and late complications after syndactyly release: a retrospective review of 684 cases.

INTRODUCTION: This study aimed to identify predictors of early and late postoperative complications following syndactyly release. We analysed a series of 684 webspace releases at a paediatric tertiary care referral centre. We hypothesized that patient demographics and presentations would resemble the findings of previous reports. In addition, we hypothesized that complete or complex syndactyly would be predictive of early and late postoperative complications regardless of other demographic and presentation factors, as compared to simple or incomplete syndactyly. METHODS: Patients 0-17 years o

Digital Planning and Patient-Specific Implants Facilitate Accurate Conventional Sub-Cranial Le Fort III Advancement.

Syndromic craniosynostosis is characterized by multisuture fusion, midface hypoplasia, and, frequently, extracranial anomalies. In these patients, transcranial monobloc/facial bipartition or subcranial Le Fort III osteotomy is typically used to achieve total midface advancement. Presented here is a case of conventional subcranial Le Fort III osteotomy in a 15-year-old patient with Apert syndrome using digital planning and patient-specific implants. The accuracy of this procedure was assessed by digitally overlaying the postoperative computed tomography scan onto the preoperative surgical plan.

Engineering of genetically encoded programmable calcium channel inhibitory binders.

Store-operated Ca2+ release-activated Ca2+ (CRAC) channels, composed of STIM and ORAI, are essential for immune and developmental processes, and their dysregulation underlies channelopathies such as Stormorken syndrome. Here, we report the engineering of genetically encoded CRAC channel inhibitory binders (CRABs) derived from the ORAI C-terminal tail. Guided by deep mutational scanning, we optimize a membrane-anchored CRAB variant that potently inhibits Ca2+ influx and NFAT signaling, and rescues thrombocytopenia-like phenotypes in a zebrafish model of Stormorken syndrome. To enable tunable in

Long-term survival without high cancer risk in a cohort of 24 patients with Apert syndrome.

The classification of congenital malformations has been transformed over recent decades by advances in genetic analysis, so that the natural history of many disorders during childhood is well described. However, implications for adult prognosis and survival are often poorly documented. In Apert syndrome, caused by heterozygous germline mutations in the fibroblast growth factor receptor type 2 gene (FGFR2), the question of prognosis is particularly pertinent because FGFR2 is a known cancer driver gene (oncogene) and the identical mutations, when arising somatically, are enriched in specific tum

Measured Flap Planning for Adult Complex Syndactyly Release: A Technical Case Report.

INTRODUCTION: Adult untreated complex syndactyly is a rare clinical presentation and may pose a unique set of reconstructive challenges due to long-standing tissue adaptation. Various flap designs for syndactyly release are described, but pre-operative planning of the flaps is often based on visual estimation and surgeon experience. We report a systematic measurement-based flap planning technique (MFPT) in the reconstruction of complex syndactyly in an adult patient. CASE REPORT: A 23-year-old male presented with an untreated complex congenital syndactyly of the middle and ring fingers with f

Targeted Genetic Testing Uncovers a Novel LRP4 Variant in Cenani-Lenz Syndactyly Syndrome.

Cenani-Lenz syndactyly syndrome (CLSS, OMIM 212780) is a rare autosomal recessive disorder characterized by syndactyly and oligodactyly of the digits, fusion of carpal and tarsal bones, and radioulnar synostosis. Craniofacial anomalies and renal malformations have been reported in approximately half of cases, with variable expression. The condition results from homozygous or compound heterozygous mutations in the LRP4 gene. We identified a consanguineous family with four individuals affected by CLSS, comprising two brothers and their two maternal male cousins. All patients presented with bilat

Prenatal diagnosis of Apert syndrome caused by a de novo FGFR2 mutation in the second trimester: a case report.

BACKGROUND: Apert syndrome is a syndromic craniosynostosis primarily caused by pathogenic mutations in the fibroblast growth factor receptor 2 (FGFR2) gene. Current prenatal diagnosis of Apert syndrome predominantly relies on ultrasound imaging in the third trimester (28 weeks of gestation to delivery), which is not conducive to early clinical decision-making. CASE PRESENTATION: In this case, prenatal ultrasound at the 20 weeks of gestation revealed craniosynostosis, oligohydramnios, and syndactyly of hands and feet in the fetus. Whole-exome sequencing (WES) followed by Sanger sequencing was

Symbrachydactyly and Sports Participation.

BACKGROUND: While sports have demonstrated psychosocial and physical health benefits in children, there is little specific research regarding children with symbrachydactyly or congenital hand differences. The aim of this study is to assess sports participation, incidence of injury, and perceived abilities among children with symbrachydactyly, partial, or complete absence of a hand. METHODS: In this single-center, cross-sectional survey study, patients between the ages of 6 and 18 years with a symbrachydactyly diagnosis were recruited at a pediatric tertiary hospital to complete a survey inclu

Chest surgery for patients with Poland's syndrome: Findings from co-produced group discussions.

Poland's syndrome (PS) is a rare congenital condition with unilateral impacts on the chest. In addressing chest differences, surgery is often offered but not consistently performed on patients with PS. The present study sought to identify areas of importance around decision-making and outcomes in surgeries performed to address reduced or missing pectoral muscles. A qualitative approach was employed and co-produced, semi-structured group discussions were conducted online primarily involving UK-based participants, who (1) have PS and (2) have been offered chest surgery for their affected side. [Research progress of genetic study on Filippi syndrome].

Filippi syndrome is a rare autosomal recessive genetic disorder characterized primarily by syndactyly, microcephaly, distinctive facial features, and intellectual disability. It may also involve multiple systemic manifestations including growth retardation, language disorders, visual impairment, cryptorchidism, abnormal dental and skeletal development, and so forth. The pathogenic gene associated with Filippi syndrome has been identified as CKAP2L, which encodes a mitotic spindle protein localized in the spindle, centrosome, and neural progenitor cells. Deficiency of CKAP2L may disrupt the div

Syndactyly in pigs.

BACKGROUND: Syndactyly, also known as mulefoot, is one of the most common physical extremity malformations. In pigs, syndactyly has been associated with genetic or environmental factors and occurs as an isolated defect or in association with other congenital anomalies. This case report aimed to describe a radiographically confirmed case of syndactyly in a piglet. CASE DESCRIPTION: A 17-day-old 6.2 kg piglet showed syndactyly in all four limbs. Although this anomaly did not affect the patient's daily activities, it was a congenital malformation of clinical and zootechnical relevance. A complete

Bi-allelic RNU6ATAC variants cause a minor spliceopathy characterized by transcriptome-wide minor intron retention and multisystem manifestations.

We report three individuals with bi-allelic variants in RNU6ATAC, which encodes the U6atac minor spliceosomal small nuclear RNA (snRNA), causing a multisystem minor spliceopathy. Through RNA sequencing analysis, we identified a distinctive excess of minor intron retention (MIR) in two unrelated individuals, which guided the identification of bi-allelic RNU6ATAC variants. The discovery cohort presented with variable multisystem manifestations. One individual presented with refractory epilepsy, microcephaly, developmental delay, ataxia, bilateral toe syndactyly, hypereosinophilia, and short stat
