Apert syndrome (AS) is a rare autosomal dominant congenital disorder characterized by craniosynostosis, midfacial hypoplasia, and syndactyly. Most cases are caused by mutations in the fibroblast growth factor receptor 2 (FGFR2) gene, primarily S252W and P253R mutations, more than 98% resulting from de novo mutations. The FGFR2 gene encodes a receptor tyrosine kinase protein, which is essential for embryonic development and skeletal formation. The study reported a Chinese newborn diagnosed with AS, attributed to a novel FGFR2 gene mutation, c.514_515delinsCT (p. Ala172Leu), identified through w

Split-hand/foot malformation (SHFM), also known as ectrodactyly (ED), is a group of congenital limb malformations characterized by partial or complete aplasia of the central rays of the hand and/or foot, with variable fusion of the remaining digits. These conditions can severely impact the functions of the limbs, with an incidence ranging from 1/90 000 to 1/8 500, and account for 8% to 17% of all limb malformations. The typical clinical manifestation include median clefts of the hands/feet, syndactyly, and hypoplasia or aplasia of phalanges and metacarpal/metatarsal bones. SHFM are genetically

Patients with abbreviated cardiac repolarization are at increased risk of cardiac arrhythmias including ventricular and atrial fibrillation (AF). In this computational simulation study, we investigated pro-arrhythmic effects of loss-of-function missense mutations in CACNA1C (A39V and G490R Cav1.2) identified in patients with a phenotype combining Brugada syndrome with shorter-than-normal QT intervals. Biophysically-detailed computational models of human atrial cells were modified to incorporate the functional impact of the CACNA1C encoded A39V and G490R mutations on the reduction of the maxima

BACKGROUND AND OBJECTIVES: Digital deformities in pediatrics are one of the reasons why parents go to podiatry clinics with their children. There is a great diversity of digital alterations in the feet; the origin of the vast majority is genetic. Due to this diversity, different treatments are offered depending on deformity and age, ranging from monitoring progress and changing footwear to deformities that are directly evaluated for surgery. The main objective of this study is to describe and identify the different podiatric digital deformities present in pediatric patients, as well as the mos

OBJECTIVE: Prenatal detection of complex chromosomal rearrangements (CCR) is extremely rare, but is of great clinical importance, since CCR can be causative of different congenital disorders. We present an exceptionally rare case of prenatally diagnosed Saethre-Chotzen syndrome (SCS) rising as a consequence of chromothripsis involving chromosomes 5, 7 and 11 and deletion of TWIST1 gene.
CASE REPORT: Brachycephaly, hypertelorism, flat face, micrognathia, relative macroglossia and small posterior fossa were noted on ultrasound examination at 28th gestational week. Fetal karyotyping revealed de

Timothy Syndrome is a multisystemic genetic disorder, classically characterised by prolonged QT interval and subsequent cardiac arrhythmias, neurodevelopmental disorders including developmental delay and autism, and syndactyly or hip dysplasia. It is caused by variants in the CACNA1C gene, which encodes the widely expressed Cav1.2 voltage-gated calcium channel. Since it's characterisation in 2004, the spread of variants in CACNA1C associated with Timothy Syndrome has expanded. With advances in sequencing and the inclusion of CACNA1C in genomic screening, further variants have been identified p

Few studies have assessed the respiratory and craniofacial management of patients with Apert syndrome. The aim of the study was to report the clinical management at our national reference center over a 25-year-period. The multidisciplinary management of patients with Apert syndrome who had a respiratory polygraphy (PG) at our center was described. Twenty-eight patients with Apert syndrome had a first PG in our center at a mean age of 3.9 ± 5.4 (0.1-22.9) years old. Ten (36 %) patients had mild-to-severe obstructive sleep apnea (OSA), with 3 (11 %) having severe OSA. During the study period, 8

BACKGROUND: Apert syndrome is a rare congenital disorder characterized by craniosynostosis, midface hypoplasia, and syndactyly. The multisystem complexity and lack of standardized surgical guidelines complicate diagnosis and treatment. This study introduces a quantitative severity scale for Apert syndrome to support objective risk stratification and personalized care.
METHODS: The authors retrospectively reviewed children with Apert syndrome who were at least 11 years of age and treated from 1988 through 2024. A 5-domain severity scale based on literature review and clinical data was develope

BACKGROUND: Syndactyly demonstrates high genetic heterogeneity, with many cases lacking molecular diagnosis despite known HOXD13 involvement, suggesting conventional methods may miss a sub-class of variants.
RESULTS: Integrated whole-exome sequencing (WES) and whole-genome sequencing (WGS) analyses identified three novel HOXD13 variants: one 2-bp heterozygous deletion c.314_315del, p.(Lys105ArgfsTer131), and two heterozygous polyalanine expansions (PAE): c.186_212dup, p.(Ala63_Ala71dup) and c.203_204insAGCAGCGGCGGCTGCGGCGGCGGC, p.(Ala64_Ala71dup). BACKGROUND: Craniosynostosis, the premature fusion of cranial sutures, may occur in isolated or syndromic forms. Among syndromic craniosynostoses, Apert syndrome is a rare but significant condition, characterized by a triad of multisuture craniosynostosis, midface hypoplasia, and symmetric syndactyly of the hands and feet. Early prenatal diagnosis is challenging but essential for appropriate counseling and perinatal planning.
CASE DESCRIPTION: We report two cases of prenatal diagnosis of Apert syndrome, confirmed through molecular genetic testing. In the first case, a 32-year-old secundigravi

Apert syndrome is a recognizable craniofacial condition characterized by craniosynostosis, hypertelorism, exorbitism, midface hypoplasia, and complex symmetrical bony and cutaneous 'mitten' syndactyly of all four limbs. Around 98% of affected patients have one of two heterozygous missense variants in the FGFR2 gene, encoding either p.(Ser252Trp) (S252W) or p.(Pro253Arg) (P253R). We report a patient with unicoronal craniosynostosis and near normal limbs, in whom we unexpectedly identified a heterozygous FGFR2 S252W variant. Her mother, who had no history suggestive of craniosynostosis and only

Apert syndrome is one of the most challenging congenital hand disorders to treat due to the absence of skin and soft tissues and the complex osseous fusion morphology. In addition, hypoplasia of the digital arteries and abnormal bifurcations are also known, so it is not recommended to create all interdigital spaces in a single operation. Multiple surgeries are usually required. In this report, a two-stage interdigital reconstruction using adipose flaps was performed for syndactyly with Apert syndrome. The osseous fusion of the complex syndactyly was dissected and covered with adipose flaps sim

INTRODUCTION: Hydrocephalus is more common in patients with craniofacial syndromes (CS) relative to non-syndromic craniosynostosis, and the optimal management is challenging. This study examined the prevalence and treatment outcomes of hydrocephalus among children with CS.
MATERIALS AND METHODS: We performed a retrospective review of medical records for all children with established CS and registered in the Gothenburg Craniofacial Registry between 1975 and 2022. This review included analyses of data regarding patient demographics, radiological imaging, hydrocephalus treatment modalities, and

The craniofacial morphology in patients with syndromic craniosynostosis varies from one spectrum to another. Our study aims to explore the craniofacial morphology and growth pattern in patients with Apert and Crouzon syndrome. This study involved 39 computed tomographic scans of skull (Apert = 11, Crouzon = 10, Control = 18), divided into 2 age subgroups. The non-parametric Mann-Whitney U test was performed to investigate the morphological characteristics between Apert, Crouzon and Control groups. The length of the middle cranial fossa decreased in both syndromes, but clivus length increased s

Craniosynostosis is a congenital disorder marked by the early fusion of one or more cranial sutures, potentially leading to functional impairments, including airway obstruction. This is particularly common in syndromic craniosynostosis, such as in Apert, Crouzon, Pfeiffer, Saethre-Chotzen, and Muenke syndromes, where midface hypoplasia and retrusion are often present. Airway obstruction, including obstructive sleep apnea, affects up to 68% of individuals with syndromic craniosynostosis. Understanding the severity and variability of airway compromise across these syndromes is critical for guidi

Biallelic pathogenic variants in DHCR7 result in decreased activity of 7-dehydrocholesterol (7-DHC) reductase, which converts 7-DHC to cholesterol, and causes Smith-Lemli-Opitz syndrome (SLOS). Elevated serum 7-DHC levels are indicative of SLOS as are intellectual disability (ID), growth retardation, microcephaly, craniofacial anomalies, and 2-3 toe syndactyly. Additional congenital malformations may be present in SLOS, and broad clinical variability has been recognized in SLOS. Rarely, biallelic pathogenic DHCR7 variants were reported with low-normal and normal intelligence quotient (IQ) and

This study aimed to assess mean facial asymmetry (MFA) before and after midface surgery (Le Fort III, monobloc, or facial bipartition) in syndromic craniosynostosis patients and compare it to the general population. This retrospective study included 55 patients (22 Apert, 23 Crouzon, 10 craniofrontonasal dysplasia (CFNS)) with a mean age of 11.5 ± 5.5 at midface surgery, and 2304 general children from the Generation R study (ages 9 and 13 years). An automated algorithm quantified MFA from three-dimensional (3D) meshes created from preoperative CT-scans, registered to the postoperative scans, a

Craniosynostoses are congenital conditions characterized by premature suture fusions, altering skull growth and potentially impairing neurological function. Apert and Crouzon syndromes, both linked to FGFR2 mutations, share features but differ in their patterns of suture fusion and cranial deformation. This study quantitatively analyzes, with a high-resolution 3D morphometric analysis, global cranial morphology in syndromic craniosynostoses, comparing Crouzon and Apert syndromes to controls using standard medical CT scans; 72 unoperated patients with syndromic craniosynostoses (51 with Crouzon
