Ankle-arm index as a marker of atherosclerosis in the Cardiovascular Health Study. Cardiovascular Heart Study (CHS) Collaborative Research Group.

BACKGROUND: Peripheral arterial disease measured noninvasively by the ankle-arm index (AAI) is common in older adults, largely asymptomatic, and associated with clinically manifest cardiovascular disease (CVD). The criteria for an abnormal AAI have varied in previous studies. To determine whether there is an inverse dose-response relation between the AAI and clinical CVD, subclinical disease, and risk factors, we examined the relation of the AAI to cardiovascular risk factors, other noninvasive measures of subclinical atherosclerosis using carotid ultrasound, echocardiography and electrocardio

Congenital attenuation of the extensor tendon central slip.

A congenital attenuation of the central slip results in a flexion attitude of the affected digit that can be confused with more common conditions such as congenital trigger finger or camptodactyly. It is a mild form of congenital absence of the central slip. The diagnosis is discussed. Treatment consists of limited excision of the attenuated area and suturing of the normal components of the central slip. The author has treated five cases in the last 10 years.

[Flexional adduction contracture of the thumbs in distal arthrogryposis type I.].

The authors discuss diagnostic and therapeutic aspects of flexion adduction contractures of the thumb - so-called congenital clasped thumb - in patient with distal arthrogryposis type I. The diagnosis is based on classification of distal arthrogryposis elaborated by Hall et al. in 1982. For the prognosis of the deformity and strategy of treatment it is an advantage to divide congenital clasped thumb (CCT) into four groups, as recommended by Weckesser et al. in 1968. The demonstrated patient belongs according to the mentioned criteria into group 2. Associated congenital developmental defects we

Cleft hand, syndactyly and hypoplastic thumb.

The clinical features of 58 patients with typical cleft hand were examined and compared with 86 patients with syndactyly between the long and ring fingers, 27 patients with syndactyly between the ring, little and other fingers, 53 patients with hypoplastic thumb and 100 patients with symbrachydactyly. The clinical findings of the typical cleft hand resembled those of syndactyly. There were two unusual cases of typical cleft hand associated with hypoplastic thumb, congenital heart anomalies and absence of the axial triradius. One of these also had cleft lip and palate. The critical embryonic pe

Index finger pollicization for congenital aplasia or hypoplasia of the thumb.

Sensation, strength, dexterity, length, and range of motion were evaluated after index finger pollicization in 10 patients (14 hands). Diagnoses included congenital absence of the thumb (10 hands) and hypoplasia (4 hands). Average age at operation was 7 years, and follow-up averaged 9 years. Patients with unilateral pollicization averaged 67% grip strength, 60% lateral pinch, 56% palmar pinch, and 39% three-point pinch as compared with the normal contralateral hand. Manual dexterity averaged 70% efficiency as compared with normal standards defined according to age and sex. However, 55% of the

Aplasia cutis congenita, cleft palate, epidermolysis bullosa, and ectrodactyly: a new syndrome?

Aplasia cutis congenita (ACC) is a skin disorder in which localized or widespread areas of skin are absent at birth. It has been associated with numerous anomalies and recognizable syndromes. We report a newborn infant with ACC of the scalp, multiple facial abnormalities including cleft palate but not cleft lip, hypoplasia of the distal phalanges of the hands, ectrodactyly of the feet, and epidermolysis bullosa of the extremities and lower trunk. Although this patient had some features that overlapped with ectrodactyly-ectodermal dysplasia-clefting syndrome, the absence of cleft lip and presen

Digito-reno-cerebral syndrome: confirmation of Eronen syndrome.

In 1985, Eronen et al. described a new autosomal recessive syndrome with absence of the distal phalanges of the toes and fingers, renal defect and cerebral anomalies (dilated ventricles or seizures). Two unrelated children affected by this syndrome enable us to accept its autonomy and delineate its nosology. Variability of the expression of the renal and cerebral manifestations is emphasized.

Distal phocomelia of the forearm in a thirteenth-century skeleton from Poland.

The skeleton of a 35- to 40-year-old male (A.D. 1250 to 1300) with distal phocomelia (intercalary transverse) of the right forearm and severe (90-100 degrees) right-sided scoliosis is examined. Congenital malformation of the the right forearm resulted in absence of the radius and ulna, and attachment of the upper arm and hand to the trunk. Lesser abnormalities consist of a-hypoplastic right scapula and misshapened ribs and sternum. This rare deformity reflects the variability and antiquity of congenital malformations of the upper limb.

Combined inherited protein S and heparin co-factor II deficiency in a patient with upper limb thrombosis: a family study.

A 42-year-old Italian woman presenting with spontaneous deep vein thrombosis of the right arm, was found to have inherited a deficiency of both protein S (PS) and heparin co-factor II (HC II). The two defects seemed to segregate independently, since her son exhibited only a HC II deficiency while one of her sisters manifested only the PS defect. All affected patients appeared heterozygous for one or other or both deficiency states. The proposita and her sister exhibited a congenital PS deficiency consisting of normal or near normal levels of total PS antigen and C4b-binding protein (C4b-BP) bu

[Morphology and systematic aspects of the longitudinal radial defect].

The present study includes a total of 117 observations in 74 patients with longitudinal distal and combined radial defects. This is the largest patient population with radial deficiencies not due to thalidomide reported in the literature. The objective of the present paper is to appraise the principles and typical patterns in reduction as well as characteristic radiological morphology. A seasonally higher incidence of longitudinal radial deficiency can be observed. Overall, there is only a slight preponderance of female patients. The bilateral occurrence of the malformation is more frequent th

Motor reorganization after upper limb amputation in man. A study with focal magnetic stimulation.

To evaluate reorganization in motor pathways following amputation, we studied motor evoked potentials (MEPs) to transcranial magnetic stimulation in 7 patients with unilateral upper limb amputations, a patient with congenital absence of a hand, and 10 normal subjects. Electromyographic recordings were made from biceps and deltoid muscles immediately proximal to the stump and the same contralateral muscles. Magnetic stimulation was delivered by a Cadwell MES-10 magnetic stimulator through a 'figure eight' magnetic coil over scalp positions separated by 1-2.5 cm. Maximal M responses were elicite

[Restricted mouth opening as a leading symptom of trismus-pseudocamptodactyly syndrome].

In the absence of pain or dental findings, incapacity to open the mouth fully, concomitant with a typical restriction of movement of the fingers is defined as the trismus-pseudocamptodactyly syndrome. This autosomal dominantly inherited feature is quite rare. So far only affections of families in the USA, the Netherlands, Canada and, recently, Japan, have been reported in the literature. Now, a German case affecting six members of the same family has been found for the first time. An important factor to be considered in the differential diagnosis of painless chronic trismus, this syndrome is d

[Aplasia of the radius of the hand].

In the article is described the congenital pathology of the hand, characterized by the absence of one or several fingers with the corresponding metacarpal bones and bones of the wrist. There is given the clinical picture of 104 patients with the hand radius aplasia. There are singled out main forms of pathology, presented the data of the additional investigation methods (angiography, rheovasography, histologic structure of the affected hand tissues). There are presented the main principles of the hand radius aplasia treatment.

Microsurgical toenail transfer to the hand.

Free nonvascularized toenail grafts have been used to reconstruct congenital or traumatic nail defects of the thumb or finger. Unfortunately, these transfers often result in deformity or atrophy. To avoid these undesirable results, microsurgical free vascularized toenail transfer was performed in 10 patients, 3 for congenital nail absence and 7 for traumatic nail defects. Patient age averaged 17 years (range 2 to 32 years). In contrast with previous reports, the whole big or second toenail complex without pulp was used in reconstruction. All 10 nails were successfully transferred with complete

The treatment of longitudinal radial deficiency.

Absence of the radius is the commonest of the longitudinal deficiencies but is itself quite an unusual congenital anomaly. No one surgeon is likely to encounter the condition more than once or twice in a career and there is a strong case for congenital upper limb conditions to be collected into special clinics. The pathology and methods of surgical treatment are reviewed. It is a difficult condition to treat but there is no doubt that function and appearance can be improved (Pulvertaft, 1973). Pollicisation of the index finger should be considered and can often greatly improve the function.

Transplantation of an osteoarthrotendinous allograft with autogenous soft-tissue coverage for thumb reconstruction.

A fresh-frozen thumb osteoarthrotendinous allograft and autogenous coverage were used to reconstruct a thumb. Immunosuppressants were not used. The components of the composite allograft are present and functioning 1 year post-operatively. Host cells have replaced and are replacing bone and tendinous structures. The "survival" of this osteoarthrotendinous allograft may have important implications in the treatment of patients with previous digital amputations, congenital absence of digits, and amputated digits that have failed replantation or are not replantable because of severely damaged vesse

Topographic maps of human motor cortex in normal and pathological conditions: mirror movements, amputations and spinal cord injuries.

We studied motor evoked potentials to transcranial magnetic stimulation in patients with unilateral upper limb amputations, complete T10-T12 spinal cord transection, and congenital mirror movements and in controls. Different muscles in the trunk and upper and lower extremities were evaluated at rest. In controls, muscles could be activated with stimulation of regions several centimeters wide. These areas overlapped extensively when muscles studied were from the same limb and shifted positions abruptly when muscles were from different limbs. Distal muscles were easier to activate than proximal

Absence of one atrioventricular connection associated with straddling atrioventricular valve: distinction of a solitary from a common valve and further considerations on the diagnosis of ventricular topology.

Hearts with absence of the right atrioventricular connection (tricuspid atresia) are rarely associated with straddling of the left atrioventricular valve. We describe two hearts in which the left atrium is connected mostly to a dominant left ventricle but, in part, to a rudimentary right ventricle. In these particular hearts, this arrangement caused major problems in determining ventricular topology. This feature, the so-called handedness of the ventricular mass, is determined in hearts with two complete ventricles by attempting to place the palm of one's hand on the right ventricular septal s

[Krukenberg-plasty in young patients].

The Krukenberg procedure has been performed unilaterally on juvenile amputee patients with congenital bilateral absence of the hands and observed by long-term follow-up examinations. The opposite extremity was routinely fitted with a standard prosthetic prescription as an assistive hand. In all cases, the Krukenberg stump became the leading hand. In some of the cases, the prosthesis on the opposite hand has been discarded in favour of the sensibility available without it. The prehension ability of the Krukenberg stump has provided the patient with all of his functional needs. The results have
