The role of nonvascularized toe phalanx transplantation.

Stabilization of boneless finger stumps or lengthening of partially aplastic digits in congenital malformations is difficult, because the bone graft, placed on top of the existing bone, will undergo resorption. Only the periosteum-covered proximal toe phalanx will withstand resorption and show a complete take. With the toe phalanx, including the plantar plate and the collateral ligaments of the metatarsophalangeal joint as a half-joint, it is possible to construct a new joint with the cartilage-covered distal end of the recipient bone (metacarpal or proximal phalanx) as the proximal half-joint

Limb reduction defects in Emilia Romagna, Italy: epidemiological and genetic study in 173,109 consecutive births.

Epidemiological and genetic variables in limb reduction defects (LRD) were analysed during the years 1978 to 1987 in a case control study in Emilia Romagna, northern Italy. During the observation period, 83 neonates out of 173,109 consecutive births had LRD (4.8 per 10,000). Cases were divided into five subgroups: transverse, intercalary, longitudinal, split, and multiple types of LRD. Of all cases, 64% were upper limb, 21% lower limb, and 15% both. Coexisting non-limb malformations were found in 10 cases (12%), five with recognised syndromes and five with other associated defects. About 7.2%

Amelia: incidence and associated defects in a large population.

Amelia, or complete absence of a limb, is a very rare congenital anomaly. The incidence of amelia in a population of 1,213,913 consecutive livebirths in British Columbia during the period 1952-1984 was studied using the records of a population-based registry with multiple sources of ascertainment. There were 18 cases of amelia, giving a minimal incidence rate of 0.15 per 10,000 livebirths for this birth defect. Amelia occurred equally frequently in upper and lower limbs, and 11 of 18 (61%) liveborn cases also had malformations of other organ systems. In the group with lower limb amelia a speci

Proximal toe phalanx transplantation for bony stabilization and lengthening of partially aplastic digits.

In congenital malformations the lengthening of partially aplastic digits, by on-top plasty of a bone graft or the bony stabilization of finger stumps consisting of soft tissues without skeletal elements, is extremely difficult. The usual bone graft, taken from the iliac crest or the fibula, will undergo resorption and will have disappeared a few months postoperatively. Experience has shown that the proximal toe phalanx will with-stand any resorption if it is taken whole, covered by its periosteum. Another advantage is the possibility of the construction of a new joint between the metatarsophal

Limb reduction defects in over one million consecutive livebirths.

Limb reduction defects occurring among 1,213,913 consecutive livebirths in British Columbia during the period 1952-1984 inclusive were reviewed. A total of 659 cases of limb reduction defects were identified, 393 of them involving the long bones and 190 of them more than one limb. The time period 1966-1984, during which ascertainment was consistent, was evaluated, and an incidence of 5.97 per 10,000 livebirths (1 in 1,692 live births) was found. The data were evaluated for trends over time, sex ratio, and regional and ethnic distribution. Associated anomalies of other organ systems in these ca

Reciprocal inhibition between forearm muscles in patients with writer's cramp and other occupational cramps, symptomatic hemidystonia and hemiparesis due to stroke.

Reciprocal inhibition of H reflexes in the forearm flexor muscles was examined in a group of 16 patients with writer's and other occupational cramps. The early disynaptic phase of reciprocal inhibition was normal. However, there was a reduction in the amount of later, presynaptic inhibition, when compared with age-matched normal subjects. Similar findings were seen in 2 patients with symptomatic hemidystonia in whom structural brain lesions were present. However, this reduction in presynaptic inhibition was not specific to patients with dystonia. In a further group of 13 patients with hemipare

[The Beals-Hecht syndrome (congenital contractural arachnodactyly) revealed in a neonate].

A new case of congenital contracture arachnodactyly (CCA) revealed in the neonatal period is reported. CCA is a dominantly inherited syndrome associating arachnodactyly, kyphoscoliosis, multiple congenital joint contractures and crumpled ears. This condition differs from Marfan's syndrome by the usual absence of visceral involvement, although cardiac complications are possible. The neonatal forms result from new mutations are are generally severe.

Cleft hand without absence of a finger.

Cleft hand implies a central longitudinal defect. In this paper, six cases of cleft hand with a normal number of digits are reported. Such clefts are less severe than those in which fingers are missing. In our cases, associated anomalies of the upper extremities were cutaneous syndactyly and a more severe cleft hand with an absent finger. Those of the lower extremities were polydactyly, syndactyly, and cleft foot. The clinical features of cleft hand with a normal digital count were similar to those with a missing finger. It is supposed that the teratogenic mechanism of such cleft hands are the

Cleft hand involving only the ring and small fingers.

The clinical features of congenital clefts between the ring and small fingers are described. After examining patients I find it difficult to decide whether cases with a cleft between the ring and small fingers, without absence of any digits, should be categorized as failure of formation or failure of differentiation. There was no functional disability justifying an operation, but the parents requested the cleft be closed for cosmetic reasons.

Microsurgical transfer of the second toe for congenital deficiency of the thumb.

Twelve second-toe transfers have been performed to substitute for thumbs congenitally deficient through constriction ring syndrome, symbrachydactyly, and true transverse arrest. The children were on average 3 years of age, and the youngest was undertaken at 10 months. Anatomic variations were the rule in the six cases of transverse absence and the three cases of symbrachydactyly, requiring nerves, tendons, and vessels in the toe be connected to whatever appropriate structure could be located. All transfers survived, and only one required exploration. Sensation appeared good in the 11 seen in l

Clinical and experimental studies on ulnar ray deficiency.

Thirteen cases with ulnar ray deficiency and thirty-three cases with radial ray deficiency were analysed. The differences of these two anomalies are not only affected parts, but also clinical features. The severity of absence of the fingers in ulnar deficiency was classified into five types. The degree of ulnar arrest was closely related to the severity of deficiency of the fingers and also to abnormalities of the elbow joint. Ulnar deficiency was induced by myleran in rat fetuses. The features of ulnar ray deficiency in rats were the same as in humans. The critical period of ulnar deficiency

[Cubital dimelia. Apropos of a case with review of the literature].

Cubital or ulnar dimelia is a very rare congenital abnormality involving almost perfect duplication of the ulnar with absence of the radius, thumb and polydactyly. An eleven week old child was seen in the Department of Pediatric Surgery for a malformation of the left upper limb including deformity of the forearm with almost doubling of the hand and seven fingers. Radiological investigation revealed duplication of the ulna as well as of the metacarpals and fingers with absence of the radius and thumb. This type of abnormality remains controversial but clinical and radiological examination led t

[Hereditary Poland syndrome with megacalycosis of the right kidney].

Poland's syndrome is characterized by congenital aplasia of the sternocostal head of the pectoralis major muscle associated with ipsilateral hand deformities (most often synbrachydactylia). 20% of patients have rib cage abnormalities, some with herniation of the lung. In some cases other associated ipsilateral anomalies (hypoplasia or absence of the nipple and/or the breast, defects of the vertebrae, dextrocardia, renal aplasia of hypoplasia, undescended testes, Möbius syndrome) have been described. We describe a patient with right-sided Poland's syndrome, the first published case associated w

Thumb polydactyly as a part of the range of genetic expression for thenar hypoplasia.

Attempts to study the genetics of human thumb polydactyly have been hampered by lack of awareness of the extremely varied expression of upper limb preaxial anomalies. It has been appreciated that thumb polydactyly could range from a broadened distal phalanx to complete duplication of the entire thumb. Most cases are sporadic and unilateral, but rare familial cases with wide variability and occasional nonpenetrance have been described. Four unrelated families are described who have thumb polydactyly as part of the range of expression for a dominant gene that is frequently associated with absenc

Congenital constriction band syndrome and transverse deficiency.

To distinguish the clinical features of amputation due to congenital constriction band syndrome from those of transverse deficiency, 42 cases of constriction band syndrome and 27 cases of transverse deficiency were analysed. All the transverse deficiencies were unilateral. Two cases of transverse deficiency were associated with pectoral muscle absence, whereas amputation from constriction band syndrome was often associated with similar anomalies in other parts of the body. The level of amputation of transverse deficiency was more proximal than that of constriction band syndrome. Rudimentary fi
