Several studies have found no increase in the overall frequency of birth defects in association with the use of spermicides, but the possibility of an increase in specific defects remains. We evaluated this possibility in a large case-control study. Infants with certain malformations (265 with Down's syndrome, 396 with hypospadias, 146 with limb reduction defects, 116 with neoplasms, and 215 with neural-tube defects) were compared with 3442 control infants with a wide variety of other defects. Exposure to spermicides was assessed for three periods: use during the periconceptional period (one m

Report of a male with bilateral aplasia of the tibiae and polydactyly. The autosomal dominant pattern of inheritance is emphasized through the study of this family and literature review.

The authors report the second case in the English language literature of a rare abnormality of the fingers and nails associated with Pierre Robin syndrome. The patient was born at term to non-consanguineous parents. The only potentially toxic drug taken by the mother during the first months of pregnancy was Sympathyl, a combined preparation of phenobarbitone, methenamine and plant extracts (boldo, crataegus) used as sedative. The child presented at birth with typical Pierre Robin syndrome responsible for respiratory distress, associated with hypotonia, dysplasia of the right hip and cardiac mu

Most previous cases of unilateral terminal transverse defects of the hand have not been familial. Several previously reported cases of apparent autosomal dominant inheritance of such defects have subsequently been reclassified as type B brachydactyly. We report a pair of adult twin women with unilateral terminal transverse defects affecting the left hand in one woman and the right hand in the other woman. The latter woman has one daughter with a unilateral terminal transverse defect affecting the left hand. The hand anomaly is characterized by absence of the terminal portions of digits 2 to 5

A 7-year-old girl presented with symptoms of constriction ring syndrome, syndactyly, and brachytelophalangy in the presence of proximal unilateral symphalangism. Physical findings revealed previous surgical correction of constriction rings of the right index and long fingers, a short right thumb with a hypoplastic distal phalanx, a shortened long finger with a stiff proximal interphalangeal joint and characteristic absence of skin creases, and incomplete syndactyly of the right first, second, and third web spaces. A brief review of symphalangism and a surgical treatment plan are presented. Thi

Most of the surgical cases were under the care of the Surgeon Robert William Parker, who was on the staff of the East London Hospital for Children, Shadwell. He was born in 1842 and qualified in 1869, and after working at the London Hospital he served with distinction as a volunteer with the Anglo-American Ambulance in the Franco-Prussian War 1870-1871. Then he became Resident Medical Officer at the Children's Hospital, 49 Great Ormond Street, and later in 1876 joined the staff of the Shadwell Hospital and was an active member of several London medical societies at which he presented surgical

Abnormal arterial patterns in the forearm were discovered in 12 of 77 patients (16%) with Down's syndrome. Eight patients had an enlarged anterior interosseous artery, the distal portion of which was palpable over the dorsum of the hand; in seven it was associated with absence of the radial artery and in one it coexisted with the radial and ulnar arteries. Three patients had a hypoplastic radial artery and a relatively dominant ulnar artery. One patient had a vestigial radial artery that ended as muscular branches in the forearm. The developmental aspects of these aberrations are discussed.

A 7-year-old girl who was unable to flex her left thumb at the interphalangeal joint proved to have an extremely hypoplastic flexor pollicis longus with normal thenar muscles, which is very rare. The flexor digitorum superficialis of the ring finger was transferred to the inserting portion of the flexor pollicis longus tendon with good results. The patient's cooperation seems to be a factor determining the prognosis.

Classification of central defect of the hand includes three general categories--Type I (typical), Type II (atypical), and Type III (two, three, and four digit hand). These three types of central defect have one common denominator--central metacarpal deficiency or absence. Otherwise, these three distinct types differ completely in inheritance pattern, characteristic features, bilaterality, and functional management. Functional management of the Type I central defect combines release of the tethered thumb metacarpal from its adduction contracture and simultaneous closure of the cleft using that

Microsurgical transfers are discussed for pterygium syndrome, congenital pseudarthrosis, aplasia cutis congenita, and transverse absence of the digits. Also reviewed are general considerations for the performance of microsurgical procedures in infants.

Three children aged 13 to 15 months had been pulled by the wrist. Subsequent pain and limitation of motion were typical of nursemaid's elbow. On radiographs, a line drawn through the longitudinal axis of the radius failed to bisect the humeral capitellum as is normally seen. Both the symptoms and the radiographic abnormalities resolved after successful manipulative reduction.

A case of Lobster-claw hand presenting for surgery in an adult is described. Surgical correction, although not perfect, was worthwhile and not difficult.

In the absence of grasp, either traumatic or congenital, free transfer of the second toe to the hand is one of the reconstructive possibilities. An essential prerequisite for the acceptance of the transferred toe is a guaranteed blood supply through the vascular pedicle. The blood supply of the second toe can be either by dorsal or plantar arteries, a fact which is due to anatomic variations. Through preoperative angiographic examination the arterial blood supply of the second toe can be defined. The location and length of the vascular pedicle have to be considered in the design of the anastom

This paper describes a prosthesis for patients who have had a partial amputation of the hand, or who have congenital absence of all or part of the hand. The prosthesis incorporates a new concept whereby the grip is operated by flexion and extension of the wrist. A covering for the prosthesis has been developed using a silicone polymer which produces a lifelike flexible glove. Thirteen patients have so far been fitted with this type of prosthesis, which can give good function and cosmesis.

A case of partial sacral agenesis resulting in mixed upper and lower motor neurogenic bladder dysfunction is reported. Although the patient also has a concomitant congenital absence of one forearm she is being managed successfully on intermittent clean self-catheterization, illustrating the value of this modality in the management of neurogenic bladder dysfunction and that it can quite easily be managed by a young woman with only one functional hand. A review of the literature on this uncommon condition is also presented.

A newborn male with Möbius syndrome, Poland anomaly, and dextrocardia is described. This is the second case reported of Poland-Möbius syndrome associated with dextrocardia. The patient presented with strabismus, facial diplegia, difficulty in swallowing, hypoplasia of the left pectoralis major muscle, partial absence of the upper costal cartilages, absence of the left areola, hypoplasia of the left forearm and hand, and dextrocardia without murmurs.

Eight Chinese patients with congenital absence of the hand were treated with the Krukenberg procedure to provide a pinching function of the forearm bones. The rehabilitation program was simple and the functional results were satisfactory. We have selected three cases to illustrate the Krukenberg procedure and the indications for this operation, a procedure that is particularly suitable for the Chinese who are culturally adapted to chopstick functions.

A kindred of 15 affected individuals in five generations is described with autosomal dominant inheritance of bilateral five-fingered hand. Some of them had additional pre-axial polydactyly of the fingers or toes and some had partial or complete absence of the tibia. The range of expression of the gene is variable and genetic advice to these families must take account of the whole spectrum of defects. The function of both upper and lower limbs was improved by surgery. A distinction is drawn between the five-fingered hand shown in this family and the different deformity of a four-fingered hand w

Ulnar dimelia ("mirror hand") is a rare congenital anomaly of the upper extremity characterized by duplication of the ulna, absence of the radial ray, and polydactyly. A case report is presented in which the patient had seven triphalangeal digits arranged in a radial cluster of three and an ulnar cluster of four. Emphasis is placed on the technique of one-stage thumb reconstruction with the use of the principles described by Buck-Gramcko.
