BACKGROUND: Congenital radioulnar synostosis (CRUS) is an uncommon developmental defect caused by failure of segmentation between the radius and ulna during early embryonic life. Despite its rarity, it is the most prevalent congenital functional abnormality affecting the elbow. Clinical presentation varies widely, ranging from subtle cosmetic concerns to significant restriction of forearm rotation with consequent limitations in activities of daily living.
CASE 1: A 4-year-old girl presented with inability to fully extend her left elbow since birth, accompanied by progressively worsening diffi

PURPOSE: Radial club hand is a congenital malformation characterized by varying degrees of radial deviation and radial bone hypoplasia or absence. However, the optimal timing for corrective surgery remains a subject of debate. This study aimed to assess the impact of the timing of radial club surgery on functional outcomes.
METHODS: Eligibility for inclusion was limited to patients exhibiting type IIIb or type IV deformities according to the Bayne and Klug classification, who began hand therapy and splinting postnatally, and underwent all surgical interventions before 10 months of age. Regula

Introduction The palmaris longus muscle (PLM), located in the forearm's anterior compartment, plays an essential role in wrist flexion. Its tendon is often used for grafting because of its accessibility and minimal function. However, its anatomy varies, with congenital absence being the most common variation. This study aims to detect the incidence of the palmaris longus tendon (PLT) and compare physical examination methods with standard ultrasonography, as previous studies have only utilized physical examinations. Methods In this study, 61 participants were examined bilaterally (122 wrists) u

Congenital anomalies of the hand and forearm present a complex challenge in plastic and pediatric surgery. We present the case of a 13-month-old child with a congenital left mirror deformity, characterized by the presence of eight digits and two ulnae exhibiting near-perfect bilateral symmetry. A novel palmar-based incision design was employed for the corrective and functional reconstruction of the left hand, which may serve as a reference for the surgical management of similar conditions. Preoperatively, the radial-side middle finger demonstrated palmar opposition against the ulnar aspect of

INTRODUCTION: Radial club hand is a rare congenital condition (1 in 100,000) involving varying deformities on the radial side of the forearm, ranging from thumb underdevelopment to absence of the radius and related structures. It can be isolated or part of syndromes, such as thrombocytopenia-absent radius, Holt-Oram, or vertebral, anal, cardiac, tracheo-esophageal, renal, limb, and is bilateral in 50-72% of cases.
CASE REPORT: A 12-year-old right-handed girl presented with an isolated, non-syndromic right radial club hand and flexion deformity at the wrist and elbow, without functional impair

INTRODUCTION: Radial aplasia is a rare congenital anomaly of the upper limbs, more common in females, affecting between one in 30,000 and one in 100,000 live births, and primarily impacting structures on the radial side of the forearm. While there have been numerous articles on bilateral radial aplasia and its management, this case is particularly significant as it represents the index case for our hospital. It is the first report to highlight how limited resources can lead to unfavorable outcomes in the management of this condition. In addition, it aims to stimulate the interest of the global

Children with a unilateral congenital below elbow deficiency (UCBED) have one typical upper limb and one that lacks a hand, ending below the elbow at the proximal/mid forearm. UCBED is an isolated condition, and affected children otherwise develop normal sensorimotor control. Unlike adults with upper limb absence, the majority of whom have an acquired loss, children with UCBED never developed a hand, so their residual muscles have never actuated an intact limb. Their ability to purposefully modulate affected muscle activity is often assumed to be limited, and this assumption has influenced pro

INTRODUCTION: Mirror hand is an extremely rare congenital abnormality characterized by polydactyly and duplication of the ulna, with the absence of the radius and thumb. Atypical presentations of mirror hand were described, including the presence of the radius in a few cases; here the authors report one of the atypical cases of mirror hand that underwent successful management.
CASE PRESENTATION: A 2-year-old and 7-month-old female child presented with 7 well-developed digits, with an absent thumb; the X-ray imaging of the forearm showed a well-formed ulna and radius with proximal fusion. The

We present a new indication of a three-dimensional statistical shape model (SSM): a patient with bilateral impaired forearm rotation due to a congenital variance in bone shape. A corrective osteotomy was planned and performed to best match the SSM created by computed tomography (CT) scans of 18 peers. Postoperatively, pronation increased by 70°, and the patient was pain-free. A CT scan showed accurate correction of the deformity and union of all osteotomies. This technique offers opportunities for patients with bilateral nontraumatic osseous forearm pathology.

PURPOSE: Transverse deficiency (TD) and symbrachydactyly may be difficult to distinguish due to shared phenotypes and a lack of pathognomonic features. The 2020 Oberg-Manske-Tonkin classification update modified these anomalies to include "with ectodermal elements" for symbrachydactyly and "without ectodermal elements" for TD as a defining differentiating characteristic. The purpose of this investigation was to characterize ectodermal elements and the level of deficiency and to examine whether ectodermal elements versus the level of deficiency was a greater determining factor for Congenital Up

Congenital hand and forearm anomalies pose a unique challenge in plastic and pediatric surgery. We present a case report of an 8-months-old girl with a congenital left sided hand and forearm anomaly, provisionally diagnosed as atypical left mirror hand anomaly. Classically there is absence of radius and duplication of ulna; however, our case had normal radius and ulna and a hand with seven digits arranged in two groups. We did a surgery which involved a ray amputation of the finger ulnar to the most radial digit, aiming to preserve an adequate first web space to reconstruct the thumb. The resu

Bilateral cleft hand syndrome is a rare congenital malformation with complex anatomy. Previous reports have mainly focused on the description of bone and soft tissue abnormalities, but information about innervation is scarce. Knowledge of the peripheral nerve anatomy is helpful for surgical treatment, optimizing the reconstruction, and preventing iatrogenic damage. Following clinical assessment and conventional radiologic imaging, we used high-resolution ultrasound of both hands and forearms to image the peripheral nerves in a patient with severe bilateral cleft hand syndrome. The patient pres

Ulnar dimelia is an infrequent congenital anomaly of the upper limb characterized by the duplication of the ulna, absence of the radial ray, and polydactyly. We report on 2 cases of young girls with upper limb abnormalities who were brought to our Advanced Imaging Department for specialized imaging evaluation. The first case is a 15-month-old girl with a typical manifestation of ulnar dimelia. X-ray images of her right forearm and hand showed 2 ulnar-like bones, absence of the radial ray, and post-axial polydactyly. Our second case is a 7-year-old girl with evident limited pronosupination and

OBJECTIVE: The purpose of this study is to explore the feasibility of reconstructing the APB with a pedicled PQ flap, and to report results in a child presenting with bilateral radial deficiency.
METHODS: Twenty-one injected cadaver upper extremities were dissected. The muscle was pedicled distally on the transverse carpal artery, and reinnervated with the flexor digiti minimi (FDM) motor branch. The transfer was evaluated on 3 parameters: surgical feasibility, length of the distal pedicle and distance from the coaptation site to the muscle entry of recipient nerve. A bilateral PQ pedicled tr

Ultrasound-based sensing of muscle deformation, known as sonomyography, has shown promise for accurately classifying the intended hand grasps of individuals with upper limb loss in offline settings. Building upon this previous work, we present the first demonstration of real-time prosthetic hand control using sonomyography to perform functional tasks. An individual with congenital bilateral limb absence was fitted with sockets containing a low-profile ultrasound transducer placed over forearm muscle tissue in the residual limbs. A classifier was trained using linear discriminant analysis to re

BACKGROUND: A terminal transverse limb defect with absence of the forearm and hand or just the hand is an uncommon limb deformity in an otherwise healthy newborn. Most of the affected infants also have tiny digit-like nubbins on the stump of the affected limb, a finding that could represent an attempt at regeneration following vascular obstruction in early limb development.
METHODS: One hundred ninety-four newborn infants with a limb deficiency were identified among 289,365 births in an active malformations surveillance program at Brigham and Women's Hospital in Boston, MA from 1972 to 2012.

Facilitated communication (FC) belongs to augmentative and alternative methods of communication. Currently, FC is very rarely and unofficially used with people suffering from verbal/communicative disorders or neurodevelopmental disorders such as intellectual deficiency or autism spectrum disorder (ASD). FC consists of physical support exerted by a facilitator at the hand/wrist/forearm/elbow of a patient/participant, aimed at helping him/her to point at pictures/words, and sometimes to type letters/words on a keyboard. Given most of (but not all) validation studies using control procedures fail

Understanding anatomical variations as well as normal anatomy of the muscles and tendons of the hand is vital for successful clinical evaluation and surgery. A number of extensor muscle and tendon variations have been reported in the literature, including duplication, triplication, and absence. We report a rare anatomical variation that includes bilateral absence of the extensor indicis (EI) muscles and bilateral duplication of the extensor digitorum (ED) tendon to the second digit in the forearm of an 83-year-old male cadaver during routine upper limbs dissection. In the present case, only th
