Four patients (five limbs) were treated with preliminary soft tissue distraction using an Orthofix external distractor. The patient's ages ranged from 6 months to 14 years. Forearm deficiencies treated included partial absence of the radius (one patient), radial aplasia (two patients, three limbs), and partial absence of the ulna (one patient). Soft tissue distraction (mean, 55 days) was continued in radial deficiency until the hand could be passively centralized without residual radial deviation, allowing centralization to be accomplished through a single mid-dorsal incision. Mean measurement

BACKGROUND: Abnormalities of the upper and lower extremities, such as club feet, rocker-bottom feet, and abnormal hand posturing, are frequently detected prenatally in trisomy 18 fetuses. Aplasia of the radius, usually associated with the absence or hypoplasia of the first metacarpal and thumb, is the most prevalent reduction malformation in trisomy 18 infants. Because this anomaly is easily missed during prenatal ultrasonography, the role of prenatal detection of this particular preaxial upper limb reduction has not been emphasized.
CASES: Three cases of trisomy 18 in which preaxial upper li

Three men, aged 18, 21 and 27 years, underwent surgery for atrial septal defect of the ostium secundum. All had bilateral absence of the forearm and thumb, with the rest of the hand directly attached to the upper arm. The patients had been exposed to synthetic progestogens during early intrauterine development. The oldest patient is described, together with a 6-year-old boy with limb reduction and Fallot's tetralogy who was exposed to the contraceptive pill during the first 2 months of gestation. During surgery he was found to have atrial septal defect of the ostium secundum. The most plausibl

The skeleton of a 35- to 40-year-old male (A.D. 1250 to 1300) with distal phocomelia (intercalary transverse) of the right forearm and severe (90-100 degrees) right-sided scoliosis is examined. Congenital malformation of the the right forearm resulted in absence of the radius and ulna, and attachment of the upper arm and hand to the trunk. Lesser abnormalities consist of a-hypoplastic right scapula and misshapened ribs and sternum. This rare deformity reflects the variability and antiquity of congenital malformations of the upper limb.

Cubital or ulnar dimelia is a very rare congenital abnormality involving almost perfect duplication of the ulnar with absence of the radius, thumb and polydactyly. An eleven week old child was seen in the Department of Pediatric Surgery for a malformation of the left upper limb including deformity of the forearm with almost doubling of the hand and seven fingers. Radiological investigation revealed duplication of the ulna as well as of the metacarpals and fingers with absence of the radius and thumb. This type of abnormality remains controversial but clinical and radiological examination led t

Poland's syndrome is characterized by congenital aplasia of the sternocostal head of the pectoralis major muscle associated with ipsilateral hand deformities (most often synbrachydactylia). 20% of patients have rib cage abnormalities, some with herniation of the lung. In some cases other associated ipsilateral anomalies (hypoplasia or absence of the nipple and/or the breast, defects of the vertebrae, dextrocardia, renal aplasia of hypoplasia, undescended testes, Möbius syndrome) have been described. We describe a patient with right-sided Poland's syndrome, the first published case associated w

Most of the surgical cases were under the care of the Surgeon Robert William Parker, who was on the staff of the East London Hospital for Children, Shadwell. He was born in 1842 and qualified in 1869, and after working at the London Hospital he served with distinction as a volunteer with the Anglo-American Ambulance in the Franco-Prussian War 1870-1871. Then he became Resident Medical Officer at the Children's Hospital, 49 Great Ormond Street, and later in 1876 joined the staff of the Shadwell Hospital and was an active member of several London medical societies at which he presented surgical

Abnormal arterial patterns in the forearm were discovered in 12 of 77 patients (16%) with Down's syndrome. Eight patients had an enlarged anterior interosseous artery, the distal portion of which was palpable over the dorsum of the hand; in seven it was associated with absence of the radial artery and in one it coexisted with the radial and ulnar arteries. Three patients had a hypoplastic radial artery and a relatively dominant ulnar artery. One patient had a vestigial radial artery that ended as muscular branches in the forearm. The developmental aspects of these aberrations are discussed.

A case of partial sacral agenesis resulting in mixed upper and lower motor neurogenic bladder dysfunction is reported. Although the patient also has a concomitant congenital absence of one forearm she is being managed successfully on intermittent clean self-catheterization, illustrating the value of this modality in the management of neurogenic bladder dysfunction and that it can quite easily be managed by a young woman with only one functional hand. A review of the literature on this uncommon condition is also presented.

A newborn male with Möbius syndrome, Poland anomaly, and dextrocardia is described. This is the second case reported of Poland-Möbius syndrome associated with dextrocardia. The patient presented with strabismus, facial diplegia, difficulty in swallowing, hypoplasia of the left pectoralis major muscle, partial absence of the upper costal cartilages, absence of the left areola, hypoplasia of the left forearm and hand, and dextrocardia without murmurs.

Eight Chinese patients with congenital absence of the hand were treated with the Krukenberg procedure to provide a pinching function of the forearm bones. The rehabilitation program was simple and the functional results were satisfactory. We have selected three cases to illustrate the Krukenberg procedure and the indications for this operation, a procedure that is particularly suitable for the Chinese who are culturally adapted to chopstick functions.

Function is the most important consideration in the evaluation of patients with congenital ulnar deficiency. The upper extremity function of 8 patients with ulnar deficiency was evaluated. None of these patients had been treated with surgical procedures directed to the elbow, forearm, or wrist. The functional criteria included: (a) active ranges of elbow, forearm, and wrist motion, (b) power grip, (c) prehension, (d) dexterity, and (e) a patient's activities questionnaire. The averaged total active range of joint motion was 229 degrees or 46% of predicted normal active motion. Power grip avera

The Krukenberg procedure has been performed unilaterally on juvenile amputee patients with congenital bilateral absence of the hands and observed by long-term follow-up examinations. The opposite extremity was routinely fitted with a standard prosthetic prescription as an assistive hand. In all cases, the Krukenberg stump became the leading hand. In some of the cases, the prosthesis on the opposite hand has been discarded in favor of the sensibility available without. The prehension ability of the Krukenberg stump has provided the patient with all of his functional needs. The results have been

We report the anatomical variations of the limbs in eight infants with the trisomy-18 syndrome that were dissected and studied in detail. In each case, the upper limbs showed defects which further define the specific influence of this aneuploidy on the development of its preaxial (radial) component, and the tendency towards reduction defects. Abnormalities included muscle variations concentrated along the radial margin of the forearm and hand, the absence of the definitive musculocutaneous nerve in all of the limbs, and reductions of the radial artery in four of the bodies. Pathogenetic mechan

The Krukenberg operation involves surgical separation of the radius and ulna in a hand or forearm amputee. Thus the forearm is converted into two rays with good grasp and tactile sensation. Prerequisites for this surgery include an adequate forearm stump; indications include bilateral loss or congenital absence of the hands, loss of the hands in a blind person, or necessity for tactile sensation in an affected limb. Sixteen cases are described in which skin grafting was not required.

In a 16-year-old girl, congenital extensor deficiency of the hand with absence of true clasping of the thumb was transmitted by an autosomal dominant mechanism of inheritance. The age of presentation was more advanced than usual. This case is the basis for proposing an hypothesis that the condition is a manifestation of a failure of innervation to develop in the extensor muscles of the forearm.
