Combined Long-Read Genome and Transcriptome Sequencing Establishes Novel Variants in MEGF8 as the Cause for Carpenter Syndrome Type 2.

Carpenter syndrome type 2 (CRPT2) is a rare autosomal recessive disease mainly characterized by craniosynostosis and polysyndactyly. CRPT2 is the rarer subtype of Carpenter syndrome (CRPTS) and is caused by biallelic variants in the multiple epidermal growth factor-like domains 8 gene (MEGF8). Due to its rarity and phenotypic overlap with other craniosynostosis syndromes, definitive molecular diagnosis of CRPT2 can be challenging. Here, we describe a proband with CRPT2 carrying compound heterozygous variants in MEGF8: one de novo variant disrupting splicing and a maternally inherited missense

Predictors of early and late complications after syndactyly release: a retrospective review of 684 cases.

INTRODUCTION: This study aimed to identify predictors of early and late postoperative complications following syndactyly release. We analysed a series of 684 webspace releases at a paediatric tertiary care referral centre. We hypothesized that patient demographics and presentations would resemble the findings of previous reports. In addition, we hypothesized that complete or complex syndactyly would be predictive of early and late postoperative complications regardless of other demographic and presentation factors, as compared to simple or incomplete syndactyly. METHODS: Patients 0-17 years o

Digital Planning and Patient-Specific Implants Facilitate Accurate Conventional Sub-Cranial Le Fort III Advancement.

Syndromic craniosynostosis is characterized by multisuture fusion, midface hypoplasia, and, frequently, extracranial anomalies. In these patients, transcranial monobloc/facial bipartition or subcranial Le Fort III osteotomy is typically used to achieve total midface advancement. Presented here is a case of conventional subcranial Le Fort III osteotomy in a 15-year-old patient with Apert syndrome using digital planning and patient-specific implants. The accuracy of this procedure was assessed by digitally overlaying the postoperative computed tomography scan onto the preoperative surgical plan.

Engineering of genetically encoded programmable calcium channel inhibitory binders.

Store-operated Ca2+ release-activated Ca2+ (CRAC) channels, composed of STIM and ORAI, are essential for immune and developmental processes, and their dysregulation underlies channelopathies such as Stormorken syndrome. Here, we report the engineering of genetically encoded CRAC channel inhibitory binders (CRABs) derived from the ORAI C-terminal tail. Guided by deep mutational scanning, we optimize a membrane-anchored CRAB variant that potently inhibits Ca2+ influx and NFAT signaling, and rescues thrombocytopenia-like phenotypes in a zebrafish model of Stormorken syndrome. To enable tunable in

A Case of Short-Rib Thoracic Dysplasia: Diagnostic Clues in Early Pregnancy and Genetic Analysis.

To investigate the diagnostic clues and genetic etiology of short-rib thoracic dysplasia (SRTD) in early pregnancy, we report a case of a fetus suspected of having SRTD based on ultrasound findings at 13 weeks of gestation. Prenatal genetic testing revealed a pathogenic variant in the DYNC2H1 gene, and post-abortion examination of the specimen along with X-ray findings confirmed the diagnosis as short-rib thoracic dysplasia 3 (SRTD3; OMIM #613091). Such cases are rarely reported in the early gestational period. The detection and subsequent confirmation in this case at 13 weeks demonstrate that

Long-term survival without high cancer risk in a cohort of 24 patients with Apert syndrome.

The classification of congenital malformations has been transformed over recent decades by advances in genetic analysis, so that the natural history of many disorders during childhood is well described. However, implications for adult prognosis and survival are often poorly documented. In Apert syndrome, caused by heterozygous germline mutations in the fibroblast growth factor receptor type 2 gene (FGFR2), the question of prognosis is particularly pertinent because FGFR2 is a known cancer driver gene (oncogene) and the identical mutations, when arising somatically, are enriched in specific tum

Cohesin variants associated with human reproductive and developmental disorders.

The cohesin complex is an evolutionarily conserved multi-subunit protein assembly essential for sister chromatid cohesion, meiotic recombination, DNA double-strand break repair, and transcriptional regulation. Pathogenic variants in its subunits are implicated in a spectrum of reproductive and developmental disorders, including non-obstructive azoospermia, premature ovarian insufficiency, reproductive aging, aneuploidy, Cornelia de Lange syndrome, Roberts syndrome, cancer, and neuropsychiatric disease. Consequently, identifying cohesin mutations is a priority for precision diagnostics and pers

Targeted Genetic Testing Uncovers a Novel LRP4 Variant in Cenani-Lenz Syndactyly Syndrome.

Cenani-Lenz syndactyly syndrome (CLSS, OMIM 212780) is a rare autosomal recessive disorder characterized by syndactyly and oligodactyly of the digits, fusion of carpal and tarsal bones, and radioulnar synostosis. Craniofacial anomalies and renal malformations have been reported in approximately half of cases, with variable expression. The condition results from homozygous or compound heterozygous mutations in the LRP4 gene. We identified a consanguineous family with four individuals affected by CLSS, comprising two brothers and their two maternal male cousins. All patients presented with bilat

Prenatal diagnosis of Apert syndrome caused by a de novo FGFR2 mutation in the second trimester: a case report.

BACKGROUND: Apert syndrome is a syndromic craniosynostosis primarily caused by pathogenic mutations in the fibroblast growth factor receptor 2 (FGFR2) gene. Current prenatal diagnosis of Apert syndrome predominantly relies on ultrasound imaging in the third trimester (28 weeks of gestation to delivery), which is not conducive to early clinical decision-making. CASE PRESENTATION: In this case, prenatal ultrasound at the 20 weeks of gestation revealed craniosynostosis, oligohydramnios, and syndactyly of hands and feet in the fetus. Whole-exome sequencing (WES) followed by Sanger sequencing was

Symbrachydactyly and Sports Participation.

BACKGROUND: While sports have demonstrated psychosocial and physical health benefits in children, there is little specific research regarding children with symbrachydactyly or congenital hand differences. The aim of this study is to assess sports participation, incidence of injury, and perceived abilities among children with symbrachydactyly, partial, or complete absence of a hand. METHODS: In this single-center, cross-sectional survey study, patients between the ages of 6 and 18 years with a symbrachydactyly diagnosis were recruited at a pediatric tertiary hospital to complete a survey inclu

Clinical outcomes after calcaneonavicular coalition resection in adults: A systematic review and meta-analysis of preoperative to postoperative changes in pain and function.

Calcaneonavicular coalition is traditionally considered a pediatric condition; however, some patients remain symptomatic into adulthood, and evidence regarding surgical outcomes in this population remains limited. This study aimed to evaluate clinical improvement following calcaneonavicular coalition resection in adults through a systematic review and single-arm meta-analysis of preoperative to postoperative changes in pain and function. A systematic search of PubMed, EMBASE, and Scopus was performed to identify studies published between January 2016 and December 2025. Eligible studies include

Chest surgery for patients with Poland's syndrome: Findings from co-produced group discussions.

Poland's syndrome (PS) is a rare congenital condition with unilateral impacts on the chest. In addressing chest differences, surgery is often offered but not consistently performed on patients with PS. The present study sought to identify areas of importance around decision-making and outcomes in surgeries performed to address reduced or missing pectoral muscles. A qualitative approach was employed and co-produced, semi-structured group discussions were conducted online primarily involving UK-based participants, who (1) have PS and (2) have been offered chest surgery for their affected side. Laryngeal Mask Airway-Guided Fiberoptic Tracheal Intubation After Traumatic Failed Intubation in a Neonate With Severe Arthrogryposis Multiplex Congenita: A Case Report.

Airway management in neonates with arthrogryposis multiplex congenita (AMC) presents extreme challenges due to craniofacial abnormalities, cervical rigidity, limited mouth opening, and restrictive chest wall mechanics. These patients are at increased risk of failed intubation and perioperative respiratory complications, as airway trauma may rapidly develop, causing airway edema and compromising oxygenation. Early recognition and individualized airway planning are essential to avoid repeated traumatic airway manipulation and its associated complications. In our case, a full-term male neonate wi

A RUNX2 reporter is expressed prior to osteochondral differentiation and models metaphyseal dysplasia with maxillary hypoplasia and brachydactyly.

SOX9 and RUNX2 are lineage defining transcription factors that drive differentiation of chondrocyte and osteoblast lineages respectively from osteochondral progenitors. In limb development, these progenitors are specified first by SOX9 expression required for mesenchymal condensation prior to RUNX2 activation and osteochondral differentiation to chondrocyte and osteoblast lineages. Unlike limb development, the anterior craniofacial skeleton arises from multipotent cranial neural crest cells (cNCCs). To examine the temporal activation of SOX9 and RUNX2 within cNCCs, we utilized a combination of

Arthrogryposis as a neuromuscular phenotype: lessons from PIEZO2 loss-of-function.

Arthrogryposis multiplex congenita is a heterogeneous group of disorders characterised by multiple joint contractures resulting from impaired fetal movement. A 43-year-old woman presented with a long-standing congenital clubfoot, progressive scoliosis since childhood and distal joint contractures. She had pure sensory findings, with global areflexia, severe sensory ataxia and a positive Romberg sign, significantly affecting her balance and gait. Genetic analysis identified two likely pathogenic variants in the PIEZO2 gene, consistent with an autosomal recessive inheritance pattern. We discuss
