Art and Pediatric Orthopaedics: Bosch, Chronicler of Medieval Disability.

BACKGROUND: Art has historically reflected prevailing medical conditions and societal responses to disability. Hieronymus Bosch's The Procession of Cripples depicts numerous individuals with visible musculoskeletal deformities, providing a unique historical perspective on disability in late medieval Europe. METHODS: A visual and historical analysis of Bosch's ink-on-paper illustration was performed. Each depicted figure was evaluated for observable limb deformities, gait abnormalities, amputations, and use of assistive devices. Findings were interpreted using contemporary pediatric orthopaedi

Physical function, quality of life, musculoskeletal pain, and prosthesis use in adults with congenital reduction defects of the upper limb: A cross-sectional study.

BACKGROUND: Individuals with congenital reduction defects of the upper limb (CRDUL) are routinely offered prostheses to enhance function and prevent strain-related problems. Despite early interventions, long-term use is limited, and evidence of benefits in adulthood remains scarce. PURPOSE: To compare physical function, health-related quality of life (HRQoL), life satisfaction, and musculoskeletal pain in adults with CRDUL vs population controls, and to examine differences between current and former prosthesis users to inform individualized habilitation strategies. STUDY DESIGN: Cross-sectio

Measuring the Forefoot-to-Rearfoot Relationship in the Sagittal Plane-A Novel Measurement for Evaluating Pseudoequinus in the Lower Extremity.

Evaluation of the static angle of the forefoot-to-rearfoot relationship in the sagittal plane is essential in determining the degree of pseudoequinus in the lower extremity. The concept of an increased sagittal plane forefoot-to-rearfoot relationship is generally referred to as anterior equinus, which may result in a pseudoequinus syndrome. As its name implies, pseudoequinus is not a true lack of ankle joint dorsiflexion but rather a condition wherein the ankle joint functions as if it were restricted. Currently, the literature lacks specifications on how to obtain this measurement. Qualitativ

Contemporary national treatment patterns for clubfoot relapse-related interventions: a large multi-institutional cohort.

BACKGROUND: While the Ponseti method is the gold standard for treatment of congenital clubfoot, recurrence after initial correction remains common. Most published data describing management of recurrent deformity are derived from single center series, and national practice patterns are not well defined. The purpose of this study was to characterize contemporary utilization of interventions used as proxies for clubfoot recurrence in a large multi-institutional database. MATERIALS AND METHODS: We conducted a retrospective descriptive study using the TriNetX Research Network. Children diagnosed

Naviculectomy combined with 'à la carte' soft-tissue releases: is it a practical solution for congenital vertical talus in adolescence?

Neglected congenital vertical talus in adolescents often necessitates extensive soft-tissue dissection, leading to increased surgical trauma and worsening outcomes. We propose that excising the navicular bone can shorten the foot's medial column and loosen the ligaments around the talonavicular joint, thus improving reduction. This study aimed to evaluate whether naviculectomy and 'à la carte' soft-tissue releases could enhance the outcomes in these patients or not. A retrospective case series study was conducted on 15 adolescent patients (average age 10.67 years) with neglected congenital ver

Predictors of cast-related skin complications during Ponseti casting: analysis of 306 casting episodes.

Cast-related skin complications remain an under-recognised challenge during Ponseti casting for congenital talipes equinovarus (CTEV). These complications - ranging from erythema and skin peeling to deep sores - can prolong treatment, increase infection risk, and reduce caregiver compliance. This study aimed to determine the prevalence and predictors of skin complications during serial casting for idiopathic CTEV. An ambispective observational cohort study was conducted at a tertiary care centre between December 2023 and June 2025. Children with idiopathic CTEV undergoing Ponseti casting were

A New Case of PITX1-Related Mandibular-Pelvic-Patellar (MPP) Syndrome.

Background: The PITX1 gene encodes a transcription factor that plays a crucial role in the development of the lower limbs, pelvis, and structures derived from the first branchial arch. Pathogenic variants in PITX1 are associated with a limited spectrum of rare disorders, including congenital talipes equinovarus with or without long bone anomalies and/or mirror-image polydactyly, and Liebenberg syndrome. In 2020, a novel clinical phenotype, Mandibular-Pelvic-Patellar (MPP) syndrome, resulting PITX1 missense variants, was proposed. Case presentation: We report the fourth documented case of MPP s

Tibialis Anterior Tendon Transfer in an Arthrogrypotic Clubfoot: A Case Report.

CASE: A 30-month-old boy with arthrogryposis multiplex congenita presented with severe, recurrent teratological clubfoot refractory to serial casting, Achilles tenotomy, and plantar fascia release. Because of repeated cast slippage and patient distress, comprehensive posterior, plantar, and medial releases were performed, with cast exchange 2 weeks thereafter. A robust, taut tibialis anterior prompted addition of tibialis anterior tendon transfer to the lateral cuneiform. Postoperatively, plantigrade alignment was achieved both clinically and radiographically and maintained at the 2-year follo

Achilles Tendon Release in Clubfoot: Radiological Comparison of Needle and Scalpel Techniques.

BACKGROUND Achilles tenotomy is the final and most critical step of the Ponseti method for correcting equinus deformity in idiopathic congenital talipes equinovarus (CTEV). The procedure can be performed using either a needle or a scalpel; however, it remains uncertain whether these 2 percutaneous techniques provide different levels of tendon relaxation and radiological correction. MATERIAL AND METHODS This retrospective study included 130 feet of 90 patients with CTEV treated by the Ponseti method between January 2022 and December 2024. Patients underwent percutaneous Achilles tenotomy either

Effect of Insoles on Plantar Fascia Tension During Running in Individuals with Flatfoot.

Background: Plantar fasciitis is a common condition likely caused by abnormal foot alignment, such as flatfoot. Insoles are commonly used to treat flatfoot, and systematic reviews have shown that insoles improve pain; however, the underlying mechanism is unclear. This study aimed to investigate the effects of insoles on plantar fascial tension during running in individuals with flatfoot. Methods: The participants were 14 individuals with flatfoot. The task involved running under two conditions: with and without insoles. Insoles that absorbed the shock and supported the medial longitudinal arch

Expanding the MYCN Variant Spectrum in Feingold Syndrome Type 1: A Novel N-Terminal Missense Variant Segregating in an Affected Family.

This study reports a previously unreported heterozygous MYCN missense variant, c.454G>A (p.Ala152Thr), identified in a child and two affected relatives, with clinical findings consistent with Feingold syndrome type 1, an autosomal dominant developmental disorder most commonly caused by loss-of-function variants in MYCN. The proband presented with a cleft palate, craniofacial dysmorphism, feeding difficulties, hypotonia, and characteristic digital anomalies. Similar features were observed in the father and sibling. Clinical exome sequencing revealed the novel MYCN variant, which was confirmed b

Preemptive immunotherapy for fetal acetylcholine receptor antibody disorder (FARAD): from recurrent pregnancy losses to a healthy infant - a case report.

Maternal antibodies against the fetal acetylcholine receptor (fAChR) can cause fetal acetylcholine receptor antibody-related disorders (FARAD), a rare but important cause of arthrogryposis multiplex congenita (AMC) and Fetal Akinesia Deformation Sequence (FADS), with recurrence rates up to 100%. Unlike genetic causes, FARAD may respond to maternal immunotherapy, yet standardized treatment protocols are lacking. We report four pregnancies in an asymptomatic mother with confirmed fAChR antibodies. After three fetal losses due to FARAD and two unsuccessful treatment attempts initiated only after
