Lethal Congenital Contracture Syndrome Type 3 in an Isolated Canadian Population.

The diagnosis of a genetic etiology for fetal akinesia deformation sequence (FADS) in the prenatal setting is challenging. It is especially difficult in patients from isolated populations in which genetic testing has been limited and the background allelic frequencies are unknown. Lethal congenital contracture syndromes type 3 (LCCS3; OMIM 611369) is a rare autosomal recessive condition caused by biallelic pathogenic variants in the PIP5K1C gene. There are only 13 cases of LCCS3 in the medical literature. We describe three cases sharing a novel variant in PIP5K1C in an isolated Canadian popula

Understanding Arthrogryposis Multiplex Congenita (AMC) Across the Lifespan: An Integrative Review of the Adult AMC Registry's Contributions With Lived Experience Partnerships.

This reflective essay explores how collaborative work between researchers and adults living with Arthrogryposis Multiplex Congenita (AMC) has deepened and broadened our understanding of what it means to live with this rare condition. By intentionally integrating the lived experiences of individuals with AMC into research processes, the resulting registry becomes far richer, more representative, and more meaningful. Including people with AMC as co-investigators not only strengthens the quality and relevance of the data collected but also offers participants a sense of agency, ownership, and emp

The effect of fatigue on kinetic and kinematic variables in individuals with flexible flatfoot during gait: a systematic review and meta-analysis.

BACKGROUND: Flexible flatfoot is a prevalent musculoskeletal deformity characterized by partial or complete loss of the medial longitudinal arch. It can modify lower-limb kinematic and kinetic parameters during walking. Fatigue may further impair gait patterns, potentially aggravating the deformity and increasing injury susceptibility. However, despite substantial research on fatigue and flatfoot separately, integrative evidence on fatigue-related gait biomechanics in individuals with flexible flatfoot remains limited. METHODS: This study was conducted as a systematic review and meta-analysis

Whole-genome resequencing with multidimensional annotation revealed pathogenic networks in sirenomelia.

BACKGROUND: Sirenomelia is a rare and lethal congenital malformation representing the most severe form of caudal dysgenesis, with a stillbirth rate of up to 53%. The genetic basis of sirenomelia remains poorly understood. Consequently, in this study, we aimed to comprehensively characterize genomic variants and explore potential pathogenic mechanisms in sirenomelia. METHODS: Whole-genome resequencing was performed on peripheral blood mononuclear cells from one patient with sirenomelia and one matched healthy newborn. Variants were identified and prioritized using the gnomAD and ClinVar popula

Association between video-based Pirani Böhm Sinclair score and treatment recommendations in recurrent clubfoot in walking-age children.

BACKGROUND: Management of recurrent clubfoot in walking-age children remains inconsistent, with substantial variation in treatment strategies. The role of clinical scoring systems in guiding treatment decisions in this group is unclear. This study investigated whether the video-based Pirani Böhm Sinclair (PBS)-score is associated with treatment recommendations. METHODS: Fifty-five children aged 4-15 years (85 clubfeet) were video-documented. Four international paediatric orthopaedic surgeons independently assessed each case using the PBS-score and recommended management. RESULTS: Higher tota

Breast reconstruction in supernumerary breast and ectopic breast tissue in Poland's syndrome.

BACKGROUND: Poland's syndrome is a rare congenital condition characterized by anterior chest wall malformations and, frequently, ipsilateral upper limb deformities. Treatment strategies vary depending on severity and may involve complex reconstructive approaches. CASE PRESENTATION: We present the case of a 20-year-old female with Poland's syndrome who exhibited left-sided supernumerary nipple and significant breast asymmetry. Clinical examination revealed two vertically aligned nipples and accessory breast tissue 6 cm lateral to the midclavicular line. The patient also presented with agenesis

Novel classification of tibial hemimelia and reconstruction-guiding principles.

BACKGROUND: Tibial hemimelia is an extremely rare congenital limb deficiency characterized by variable tibial absence, knee instability, and ankle deformity. Existing classification systems rely primarily on static radiographic morphology and provide limited guidance for surgical decision-making, particularly regarding limb reconstruction versus amputation. Determinants of limb salvage especially the integrity of the quadriceps extensor mechanism and knee stability are not systematically incorporated into current classifications. This study proposes a novel classification designed to integrate

Association Between Idiopathic Clubfoot and Developmental Dysplasia of the Hip: Do We Need Universal Screening for It?

BACKGROUND: The association between congenital talipes equinovarus (CTEV) and developmental dysplasia of the hip (DDH) remains contested. Some guidelines consider idiopathic clubfoot a DDH risk factor warranting routine hip ultrasonography, while others do not. This study aimed to determine the prevalence of DDH in a large cohort of infants with idiopathic CTEV and assess whether routine screening is justified. METHODS: A retrospective review of prospectively collected data was conducted at a single tertiary pediatric orthopaedic center. Children treated for idiopathic CTEV between July 2000

Multiple congenital malformations in a burrowing owl (Athene cunicularia) from Southern Brazil.

This study reports a case of multiple congenital malformations in a burrowing owl (Athene cunicularia) rescued in southern Brazil. Clinical assessment revealed partial agenesis of the forelimbs, bilateral asymmetric syndactyly, and selective anonychia of the second digit in both pelvic limbs. Despite severe locomotor limitations and the inability to fly, the individual maintained a satisfactory body condition score, demonstrating the species' adaptive and behavioural capacity. The integrity of the epithelial tissues and the symmetry of the digital alterations support a teratogenic etiology, di

Reliability of Radiologic Measurements in Children with Flexible Flat Feet.

Objectives: Flat feet is a very common deformity in children. Often, radiologic evaluation for children with flat feet is made. Although the measurement reliability of some angles in flat feet has been evaluated in the literature, none of them have been performed in the age range of 3-7 years, when the deformity is most common. Our aim is to evaluate the interobserver and intraobserver reliability of the 7 angles that are used for assessing flat feet. Methods: Radiologic angles were measured by four independent observers (two specialists and two residents) in two separate sessions on the foot

Gait in childhood and adulthood in persons with myelomeningocele - a retrospective analysis.

BACKGROUND: Gait patterns in children with myelomeningocele (MMC) at various neurological levels have been described, both with and without orthotic support. Although the neurological level of the lesion serves as an important predictor of ambulatory potential, the expected walking ability is not always achieved, as additional factors such as spasticity may influence gait negatively. The aim of this study was to retrospectively compare gait patterns as assessed in childhood with those observed in adulthood. METHODS: Of 59 individuals with MMC aged 18 years or older, 29 had undergone three-dim

The Impact of Genetics on Craniofacial Dysplasias and Consequent Oral Malformations-Integrative Review.

BACKGROUND/OBJECTIVES: Diseases affecting the craniofacial skeleton are normally associated with disturbances in the regulation of cellular differentiation, the development of bone structures, and changes in bone density and ossification. Thus, the objective of this integrative review is to evaluate the published scientific literature from the last 8 years concerning the impact of genetics on some craniofacial dysplasias. Our aim covers the identification of oral cavity alterations to those dysplasias, through the most common orofacial manifestations. Three dysplasias were selected to be part

Spectrum of Congenital Anomalies in Myhre Syndrome-Insights Into Effects Brought by Altered TGF-β Signaling via Gain-of-Function Variants in SMAD4.

Myhre syndrome is a rare genetic disorder characterized by progressive multisystem involvement. Gain-of-function missense heterozygous variants affecting the Ile500 residue and Arg496 residue of the SMAD4 gene are implicated in this condition. In this article, we aim to understand the spectrum of congenital anomalies in Myhre syndrome by studying a cohort of previously unreported patients alongside published literature. Our analysis revealed that the musculoskeletal system was the most common system to be affected, followed by the cardiovascular system. Intrauterine growth restriction was the

TBX5-p.G125R postnatally deregulates pacemaker cardiomyocyte state and function.

Heterozygosity for the pathogenic missense TBX5 (T-box transcription factor 5) variant p.G125R, identified in members of a family presenting with atypical Holt-Oram syndrome, is associated with supraventricular arrhythmias, including atrial fibrillation and disrupted sinus rhythm. Tbx5+/G125R mice recapitulated major aspects of the patient phenotype, including increased susceptibility for atrial arrhythmias, and revealed deregulation of the adult atrial cardiomyocyte transcriptional regulatory network and epigenetic state. Because TBX5 has also been implicated in the development and function o
