Compound heterozygous mutations in CC2D2A cause Meckel-Gruber syndrome: a case report and review of the literature.

BACKGROUND: Meckel-Gruber syndrome is a rare autosomal recessive ciliopathy characterized by the triad of occipital encephalocele, polycystic kidney dysplasia, and postaxial polydactyly, with an estimated incidence of 1:13,250 to 1:140,000 live births. Pathogenic variants in CC2D2A, encoding a ciliary transition zone protein, account for 5-15% of MGS cases. This report highlights the diagnostic utility of whole-exome sequencing (WES) in delineating molecular etiologies of MGS and reviews genotype-phenotype correlations associated with CC2D2A mutations. CASE PRESENTATION: A 30-year-old Chinese

Enhanced Precision for Deformity Correction Planning: Revision of Values for the Vitruvian Foot Quadrilateral with Demonstrative Case Report.

AIM AND BACKGROUND: The introduction of the 'Vitruvian' Foot concept in 2019 established initial parameter measurements of the 'normal' foot quadrilateral (FQ) in the sagittal plane. This effort had been aimed at increasing precision when analysing alignment abnormalities in post-traumatic and congenital foot deformities, so as to more accurately guide corrective treatment planning with osteotomies. The current study enhances the prior, existing data with an expanded normal cohort. We also offer a case report that describes the successful application of the revised reference angles for our ana

Aplasia Cutis Congenita and Congenital Heart Disease: A Case Report, Highlighting the Limitation of Antenatal Screening.

BACKGROUND Aplasia cutis congenita (ACC) is a rare congenital defect involving localized absence of skin, occasionally associated with deeper tissue anomalies and congenital malformations. CASE REPORT We report the case of a full-term newborn presenting with extensive ACC involving 40% of the scalp, alongside multiple congenital heart defects, ultimately consistent with tetralogy of Fallot (ToF). Initial management included conservative wound care, cardiologic and neurologic evaluations, and genetic testing. Despite clinical stability at discharge, the infant was readmitted 3 weeks later with

VCP variants cause Adams-Oliver syndrome with or without pulmonary hypertension.

PURPOSE: Adams-Oliver syndrome (AOS) is a genetically heterogeneous disorder with cardinal features of aplasia cutis congenita and terminal limb reduction defects. A minority of individuals with AOS develop potentially lethal pulmonary hypertension (PH) in infancy, a subgroup that has been refractory to genetic explanation. METHODS: We studied a cohort of individuals with AOS and no genetic diagnosis by genome and exome sequencing. We characterized rare, identified substitution variants in valosin-containing protein (VCP) in vitro using ATP hydrolysis, cryogenic-electron microscopy, thermal s

Differential Effects of Maternal Exposures on Clubfoot Laterality: A Case-Control Study.

BACKGROUND: Clubfoot is a common birth defect that affects one or both feet. Evidence suggests increased risks of clubfoot in offspring associated with maternal medication and cigarette smoking exposure in early pregnancy. We explored whether such associations differed by clubfoot laterality. METHODS: This case-control study was conducted in Massachusetts, North Carolina, and New York (2007-2011). State birth defects registries reported clubfoot diagnoses among infants aged < 11 months. Controls without birth defects were random samples of infants from the same catchment areas and born in the

Differential genetic analysis of ectrodactyly in a Fanconi anemia pedigree with FANCA mutations.

Fanconi anemia (FA; OMIM: 227650) is a rare genetic disorder characterized by bone marrow failure, congenital anomalies, and cancer predisposition. While FANCA mutations account for most FA cases, phenotypic overlap with other disorders complicates diagnosis. This study analyzes molecular diagnostic pathways for FANCA-related FA and establishes a hereditary differential diagnosis for ectrodactyly. A Chinese FA family clinical phenotype was collected. The proband and father underwent whole-genome sequencing. Copy number variations (CNVs) in FANCA were assessed by genomic qPCR. Functional charac

The Burden of Lower Limb Deformities in Low- and Lower-Middle-Income Countries: A Scoping Review.

UNLABELLED: Musculoskeletal (MSK) conditions like lower limb deformities significantly contribute to the global burden of disease. Low-income countries (LICs) and lower-middle-income countries (LMICs) are disproportionately affected, yet specifics surrounding MSK conditions in these regions remain largely unquantified. This scoping review seeks to (1) assess the epidemiologic information currently available for lower limb deformities in LICs and LMICs, including etiology and treatment, as well as (2) examine the individual, societal, and socioeconomic impact of these conditions. A scoping revi

Prenatal Diagnosis of a Feingold Syndrome Pregnancy Complicated with Severe Preeclampsia: A Report of a Challenging Case.

Feingold syndrome (FS) is a rare congenital disorder with an autosomal dominant inheritance pattern. Two distinct subtypes are recognized based on their molecular pathology: FS type 1 (FS1) and FS type 2 (FS2). Both types share skeletal anomalies such as microcephaly, brachymesophalangia, and clinodactyly; however, gastrointestinal atresia is unique to FS1. Herein, we report a rare prenatal diagnosis of FS1 in a female fetus. The second-trimester ultrasound revealed bilateral clinodactyly and fetal microcephaly, and the subsequent molecular karyotyping identified a ~342 kb deletion at 2p24.3 e

Breaking the Taboo: Four Cases of Thoracic Segmental Spinal Anesthesia in Modern Surgical Practice.

Thoracic segmental spinal anesthesia (TSSA) is an old neuraxial technique that has recently regained attention as a possible alternative to general anesthesia (GA) in selected surgical patients. Its segmental nature, low-dose local anesthetic requirement, and ability to preserve spontaneous ventilation may be particularly relevant in high-risk patients in whom airway manipulation, endotracheal intubation, positive-pressure ventilation, and systemic anesthetic exposure could represent an additional physiological burden. We report four selected high-risk patients undergoing procedures from diffe
