Repeated Surgeries and Clinical Assessment Using 3D Gait Analysis in Unilateral Congenital Short Femur: A Case Report.

INTRODUCTION: There are numerous etiologies relating to limb length discrepancy (LLD). Clinical manifestations of LLD may include gait abnormality and compensatory scoliosis. Congenital short femur (CSF) is a rare congenital skeletal abnormality with several manifestations, including coxa vara, abnormalities of the cruciate ligaments, hypoplasia of the lateral femoral condyle, fibular hemimelia, ball-in-socket ankle, and tarsal coalition. CASE REPORT: A 4-month-old female was referred to a primary care hospital following findings of LLD and significant thigh circumference discrepancy. She was

Changes in Lower Limb Axial Alignment, Gait Biomechanics, and Plantar Force in Crowe Type IV Hip Dysplasia After Total Hip Arthroplasty: A Mean Ten-Year Follow-Up Retrospective Cohort Study.

OBJECTIVES: Total hip arthroplasty (THA) is the gold standard for treating Crowe IV developmental dysplasia of the hip (DDH). However, its long-term effects on lower limb alignment, gait biomechanics, and plantar force in these patients remain underexplored, which is discussed in this article. METHODS: We conducted a retrospective cohort study that included 43 DDH Crowe IV patients who underwent THA between February 2008 and October 2019 and a control group of 43 matched healthy volunteers. Postoperative functional outcomes and quality of life were assessed using the Harris Hip Score, KOOS, A

Horns, nails, and leaky kidneys: A rare case of congenital nephrotic syndrome.

Nail-patella syndrome (NPS) is an uncommon autosomal dominant condition marked by nail dysplasia, skeletal abnormalities, and variable renal manifestations, resulting from mutations in the LMX1B gene. We report a rare case of a 23-year-old male presenting with nephrotic-range proteinuria, characteristic skeletal manifestations of NPS, and a family history of renal failure. Genetic testing identified a previously unreported heterozygous missense variant in the homeodomain of LMX1B (c.791A>C; p.Gln264Pro), supporting its pathogenicity. The absence of patellar hypoplasia in our patient highlights

Congenital Genu Recurvatum: Report of Two Cases.

BACKGROUND: Congenital hyperextension of the knee joint is a rare malformation with a variety of names, principally genu recurvatum of knee, backward bending of the knee, and dislocation of knee. Abnormal anterior hyperextension position and limitation of flexion of tibiofemoral joint are the classical clinical presentation in an infant at birth. The accurate etiology of the malformation is undetermined. Congenital genu recurvatum (CGR) can occur as an isolated entity or can present in association with other congenital abnormalities like talipes equinovarus, developmental dysplasia of the hip,

Positional Hemodynamic Instability in a Patient With Pectus Excavatum Undergoing Lumbar Discectomy.

Pectus excavatum (PE) is the most common congenital chest wall deformity, accounting for 90% of cases, and may predispose patients to hemodynamic compromise due to cardiac compression. Such events during spinal surgery, however, are rarely reported. In the widely used genupectoral prone position for spine surgeries, the patient lies prone with the torso supported and the legs flexed, thereby reducing abdominal compression, improving venous drainage, and reducing bleeding. However, the support provided by the thorax in this position may cause significant mediastinal compression in the presence

Bilateral anterior cruciate ligament agenesis in a 12-year-old boy: A rare case report.

Bilateral anterior cruciate ligament (ACL) agenesis is an exceptionally rare condition. Only a limited number of bilateral ACL agenesis cases have been reported in the literature, and these patients typically present with additional congenital anomalies or other pathologies involving the knee joint. Here, a 12-year-old male patient was reported with bilateral absence of the ACL, who had no comorbidities other than a history of casting and Achilles tenoplasty performed for pes equinovarus. The patient presented to the clinic with bilateral knee laxity and a sense of instability during daily act

Extreme trochlear morphology in pediatric patients with fixed lateral patellar dislocation: a radiographic study.

BACKGROUND: Fixed lateral patellar dislocations (FLPD) are a rare subtype of chronic patellofemoral instability (PFI) where the patella remains dislocated in flexion and extension. The aim of this study was to describe radiographic morphological features in pediatric patients with FLPD and determine how they differ from those with acute patellar dislocation (APD) and unaffected controls. METHODS: Pediatric patients who underwent medial patellofemoral ligament reconstruction for FLPD or APD were identified and age- and sex-matched to controls without PFI. On axial magnetic resonance imaging, b

Management of Lower-Extremity Deformity in Arthrogryposis Multiplex Congentia: A Narrative Review.

Arthrogryposis multiplex congenita (AMC) is a highly heterogeneous constellation of disorders defined by non-progressive congenital multiple joint contractures, typically manifesting across multiple limbs. The pathology results in significant functional impairment, including restricted range of motion, chronic arthralgia, and secondary musculoskeletal deformities like scoliosis. Given that AMC is an umbrella designation encompassing over 300 distinct etiologies, the profound clinical variability poses substantial diagnostic and therapeutic challenges. The relative rarity of AMC and the absence

Short foot exercises for flatfoot therapy: Status and prospects.

Flatfoot, a disorder defined by the flattening or loss of the medial longitudinal arch (MLA), affects approximately 15.6% of the child population. It may result from congenital or acquired dysfunction of muscles, tendons, or ligaments in the foot and ankle, as well as skeletal abnormalities. Clinically, this disorder manifests as foot pain, gait abnormalities, and compensatory injuries in lower limb joints (e.g., knee and hip), thereby affecting motor function and quality of life. Short foot exercises (SFE) as a noninvasive rehabilitation strategy enhances arch height and improves dynamic foot

Lateral discoid meniscus in the pediatric knee: imaging features and tear patterns.

Lateral discoid meniscus is the most common meniscal congenital variant. A lateral discoid meniscus manifests with a spectrum of findings, including abnormalities in size and shape, disorganized collagen architecture, and/or hypermobility, predisposing the meniscus to early degeneration and tearing. The purpose of this narrative review is to discuss the anatomy of the lateral discoid meniscus and to highlight MRI findings that should be included in radiology reports to guide management.

Favorable Long-term Outcomes Following Arthroscopic Preservation Surgery for Pediatric and Adolescent Discoid Lateral Meniscus.

BACKGROUND: Discoid lateral meniscus (DLM) is a congenital anomaly associated with increased risk of tearing and peripheral rim instability due to abnormal morphology and deficient capsular attachments. While total meniscectomy was historically performed, long-term complications have prompted a shift toward arthroscopic saucerization with or without repair. Though midterm outcomes are favorable, long-term data remain limited. This study aimed to evaluate long-term clinical outcomes following arthroscopic preservation surgery for symptomatic DLM and compare results between saucerization alone a

Gait analysis in children with bladder exstrophy shows increased hip adduction, knee valgus and external foot progression in comparison with control participants.

BACKGROUND: Bladder exstrophy is a congenital malformation occurring in fetal development characterized by an open pelvic ring and inferiorly rotated pelvis, impacting typical gait and development. Little is known about the gait patterns in children with bladder exstrophy, especially if they undergo an osteotomy surgery for pelvic reconstruction. OBJECTIVE: This study aimed to investigate gait patterns of children with and without bladder exstrophy, as well as children with bladder exstrophy with and without osteotomies. METHODS: Fifteen participants with bladder exstrophy (mean age 10.6 y;

Preoperative physiotherapy rehabilitation of lower limb deformities secondary to meningomyelocele in mid-childhood.

Meningomyelocele is a congenital defect resulting from the incomplete closure of the neural tube during embryonic development, making it the most common structural birth anomaly of the central nervous system. Over time, these deformities can progress to the knee and ankle joints due to the development of contractures. In addition, rotational deformities of the lower extremities are also seen. This study outlines a mid-childhood female with a complaint of fixed knee flexion and a spastic equinovarus deformity that has been affecting her walking and performing activities of daily living for the

Crowe severity and femoral stem geometry are associated with lower-limb alignment after total hip arthroplasty for unilateral developmental dysplasia of the hip-related secondary osteoarthritis: valgus alignment trajectories.

BACKGROUND: Total hip arthroplasty (THA) for secondary osteoarthritis related to developmental dysplasia of the hip (DDH) restores hip mechanics. We aimed to quantify pre-to-postoperative changes in ipsilateral knee and global lower-limb alignment after unilateral THA to determine whether alignment changes differ by Crowe classification and femoral stem geometry. METHODS: This retrospective cohort study included 74 patients who underwent unilateral primary THA for DDH-related secondary osteoarthritis, with a mean follow-up of 45.4 months. The predefined primary radiographic outcome set compri

An infant with equino-varus deformity of her right foot and flexion deformity of the knee presented to our centre. Based on clinical and radiological examination findings, she was diagnosed with Paley Va (Jones Ia) axial longitudinal intercalary deficiency of the right leg as the entire tibia was absent. Consequent to cultural non-acceptance of amputation by the parents, her management involved staged procedures. The initial surgery aimed to achieve fusion between the talus and the distal end of the fibula after soft tissue release and fibular shortening. In the subsequent surgery, Weber patel

Building the cancer research workforce through implementation of the RACE21 undergraduate cancer research and education program.

BACKGROUND: Cancer remains the second leading cause of death in the state of Alabama and disproportionately affects communities that are rural, of lower socioeconomic status, and individuals with high-risk, predisposing conditions such as obesity, cardiovascular disease, advanced age, and diabetes. One method to address the increased burden of cancer is to expand the cancer research and clinical workforce. The Roadmap for America's Cancer Explorers for the 21st Century (RACE21) is a National Cancer Institute-funded Youth Enjoy Science program at the University of Alabama at Birmingham (UAB) an

Deep vein thrombosis in a 14-year-old boy with a combination of May-Thurner syndrome, protein S deficiency and lupus anticoagulant: a case report.

BACKGROUND: Here, we present the first reported case of a child presenting with unprovoked deep vein thrombosis (DVT) due to the unique combination of May-Thurner syndrome, protein S deficiency and lupus anticoagulant. Of interest to the clinician and scientific reader alike is that this one-of-a-kind interplay between mechanical obstruction and hypercoagulability caused a very extensive blood clot which extended from the left popliteal to the femoral and iliac veins. A 12-month course of warfarin with a target INR value of 2.5 was used to treat the DVT which would be of use to note for any fu

Efficacy and safety of one-stop Angiojet mechanical thrombectomy combined with iliac vein stenting in May-Thurner syndrome complicated with acute iliofemoral deep vein thrombosis.

May-Thurner Syndrome (MTS) with acute iliofemoral deep vein thrombosis (IFDVT) carries high risks of limb ischemia and post-thrombotic syndrome (PTS). Traditional treatments like catheter-directed thrombolysis have limitations, including high major bleeding risk and prolonged hospital stays. A retrospective cohort study included 52 patients with MTS-related acute IFDVT (one-stop group: 27, Angiojet mechanical thrombectomy + simultaneous iliac vein stenting; traditional group: 25, catheter-directed thrombolysis alone or with delayed stenting). Main outcomes were 24-hour limb circumference edema
