Complex presentation of neonatal arthrogryposis multiplex congenita: insights into a multifactorial disorder.

This case report details a neonate diagnosed with arthrogryposis multiplex congenita (AMC), specifically the amyoplasia subtype, presenting with bilateral talipes equinovarus, elbow contractures, ulnar deviation of the wrists and clinodactyly. Additional clinical findings included mild developmental dysplasia of the hips and a small atrial septal defect. Genetic analysis revealed heterozygous variants of uncertain significance in the SYNE1 and CHD4 genes, both of which were inherited from asymptomatic parents. While SYNE1 is associated with autosomal recessive AMC type 3 and CHD4 with Sifrim-H

Management of Upper Limb Deformities in Arthrogryposis Multiplex Congenita: A Narrative Review.

Arthrogryposis multiplex congenita (AMC) presents complex therapeutic challenges in upper limb reconstruction, in which establishing an optimal positioning-reaching-grasping axis is critical for patient independence. However, clinical evidence is often limited by low-level data and significant phenotypic heterogeneity, leaving management strategies fragmented. This review synthesizes current evidence to provide a strategic framework for managing complex upper extremity deformities in AMC. A selective literature search was conducted via the MEDLINE database in April 2026, focusing on advancemen

A Novel BORCS5 Loss-Of-Function Variant in a Patient With a Rare Neurodevelopmental Disorder.

BACKGROUND: The BORCS5 is an important and critical component of the BLOC-one-related complex (BORC), involved in kinesin-dependent anterograde lysosomal transport. Aberrations in the octameric protein complex encompass lethal neurological manifestations, such as arthrogryposis multiplex congenita, brain malformations and neuropathological evidence of diffuse neuroaxonal dystrophy. Loss-of-function variants in BORCS5 are associated with extremely rapid and progressive neurodegeneration. METHODS: Herein, we present a family consisting of a proband with intellectual disability, microcephaly, de

Acute flexion contracture correction for failed distal femoral extension osteotomy in arthrogryposis: early results and complications.

Distal femoral extension osteotomy (DFEO) is a surgical option for knee flexion contractures in arthrogryposis multiplex congenita (AMC); however, recurrence rates are high in skeletally immature patients. This study evaluated posterior knee release with femoral shortening osteotomy as a revision procedure following failed DFEO in AMC. A retrospective review was performed on patients with AMC with failed prior DFEO who underwent revision surgery between 2021 and 2023. Six patients, ten knees, were identified. Radiographic and clinical measurements were compared using paired t tests. The primar

Tibialis Anterior Tendon Transfer in an Arthrogrypotic Clubfoot: A Case Report.

CASE: A 30-month-old boy with arthrogryposis multiplex congenita presented with severe, recurrent teratological clubfoot refractory to serial casting, Achilles tenotomy, and plantar fascia release. Because of repeated cast slippage and patient distress, comprehensive posterior, plantar, and medial releases were performed, with cast exchange 2 weeks thereafter. A robust, taut tibialis anterior prompted addition of tibialis anterior tendon transfer to the lateral cuneiform. Postoperatively, plantigrade alignment was achieved both clinically and radiographically and maintained at the 2-year follo

Preemptive immunotherapy for fetal acetylcholine receptor antibody disorder (FARAD): from recurrent pregnancy losses to a healthy infant - a case report.

Maternal antibodies against the fetal acetylcholine receptor (fAChR) can cause fetal acetylcholine receptor antibody-related disorders (FARAD), a rare but important cause of arthrogryposis multiplex congenita (AMC) and Fetal Akinesia Deformation Sequence (FADS), with recurrence rates up to 100%. Unlike genetic causes, FARAD may respond to maternal immunotherapy, yet standardized treatment protocols are lacking. We report four pregnancies in an asymptomatic mother with confirmed fAChR antibodies. After three fetal losses due to FARAD and two unsuccessful treatment attempts initiated only after

Milestone Attainment in Young Children With Arthrogryposis Multiplex Congenita: Developmental Profile and Associated Factors.

Evidence on developmental milestones in children with arthrogryposis multiplex congenita (AMC) under the age of five is scarce. This multisite cross-sectional study described developmental status and examined factors associated with milestone attainment in 143 children aged 0-66 months from a pediatric AMC Registry. Development was assessed using the Ages and Stages Questionnaire, Third Edition (ASQ-3), across Communication, Gross Motor, Fine Motor, Problem-Solving, and Personal-Social domains. Independent variables included demographic, perinatal, clinical, and parental factors. Gross Motor f

Growth arrest following posterior knee release in children with arthrogryposis multiplex congenita: a previously unreported complication.

Knee flexion contractures in children with arthrogryposis multiplex congenita (AMC) significantly impair function and ambulation. Posterior knee soft tissue release is a widely used surgical intervention when conservative management fails. However, literature describing complications following this procedure remains limited. This study aims to report distal femoral and proximal tibial physeal arrest as a previously undocumented complication following posterior knee release in children with AMC. Following institutional review board approval, a retrospective case series was performed. Patients w

Initial psychometric evaluation of an upper extremity arthrogryposis multiplex congenita-specific measure using Rasch analysis.

BACKGROUND: In the absence of an upper extremity outcome measure specific to children with arthrogryposis multiplex congenita, a new outcome measure, the SHAPE-UP, was developed to help guide clinicians in their evaluation and decision making for treatment. It is designed for children with arthrogryposis multiplex congenita aged 1-21, consists of six descriptive questions and a video-recorded evaluation, divided into Task Completion and Analysis of Joint Motion and Position scales. PURPOSE: The study aimed to evaluate the psychometric properties of the SHAPE-UP Task Completion scale using Ras

A Novel Splice Variant in ERGIC1 Causes Arthrogryposis Multiplex Congenita-Characterization Using Urine-Derived Cells.

Arthrogryposis multiplex congenita (AMC) is defined as the presence of joint contractures affecting at least two body regions at birth. Three different ERGIC1 variants have been reported in individuals with AMC. Here, we report on a 16-year-old male with a homozygous ERGIC1 c.250+1G>A variant that was classified as a variant of uncertain significance on trio genome analysis. The proband's phenotype resembled that of previously reported individuals with ERGIC1 variants, but also included an absent patella and advanced bone age, which have not been reported in ERGIC1-related AMC. Embodied language and early motor restriction: evidence from children with obstetric brachial plexus palsy and arthrogryposis.

Embodied and embedded cognition (EEC) theory proposes that language and cognitive development emerge from bodily interactions with the environment, yet empirical tests of this claim in clinical developmental populations remain rare. This mini-review synthesizes behavioral, electrophysiological, and structural neuroimaging evidence from children with serious early motor disorders-obstetric brachial plexus palsy and arthrogryposis multiplex congenita-which restrict upper limb movement from birth or before, providing a unique opportunity to test EEC predictions in a motor-restricted population. Caring for Pediatric Clients With Arthrogryposis Multiplex Congenita: The Development of an Occupational Therapy Continuing Education Course.

Occupational therapy practitioners (OTPs) are vital members of the multidisciplinary team involved in caring for pediatric clients diagnosed with arthrogryposis multiplex congenita (AMC). However, a shortage of educational resources on AMC has led to gaps in knowledge and OTPs feeling inadequately prepared to provide comprehensive care. A 4-h continuing education (CE) course on caring for pediatric clients with AMC was designed using evidence-based literature and structured using the person-environment-occupation model. The occupation-based model guided learners to understand how therapy can i

Apical Ectodermal Ridge Disruption and Hypoplastic Digits in Amyoplasia.

The cause of Amyoplasia remains controversial, with the predominant theory being loss of anterior horn cells secondary to vascular disruption as the primary insult. Disruption of the apical ectodermal ridge (AER) is extremely rare, occurring in 1 in 2 million live births. Amyoplasia has an incidence of 1 in 10,000 births, making the probability of both conditions occurring simultaneously by chance 1 out of 20 billion. Muscle precursor cells do not differentiate until they reach the limb and contact signaling factors that are AER dependent. Without sufficient AER function, muscle cell line deve

Femoral anterior distal hemiepiphysiodesis for correction of knee flexion contractures in arthrogryposis.

UNLABELLED: Knee flexion contractures in children with arthrogryposis limit or prevent efficient ambulation. This study assesses the effectiveness of femoral anterior distal hemiepiphysiodesis (FADHE) to treat knee contractures. Over an 8-year period, 57 children with arthrogryposis underwent 90 FADHEs for knee flexion contractures. The mean age at surgery was 7.1 years. Knee range of motion data was recorded preoperatively, postoperatively, and at clinical follow-ups, as was ambulatory ability. The average follow-up was 4.2 years; 65 knees had concomitant posterior releases; eight knees were

Characteristics of intellectual development in children diagnosed with arthrogryposis multiplex congenita.

This study investigates how congenital motor disorders affect cognition by analyzing intelligence in children with arthrogryposis multiplex congenita (AMC), a condition featuring joint contractures without CNS involvement. Unlike other motor disorders, AMC offers a unique model to study movement restriction's isolated cognitive effects. The study included 32 children diagnosed with AMC and a control group of 33 healthy children. To assess cognitive development, we administered the Culture-Free Intelligence Test (CFIT; GFT 2) to measure fluid intelligence, along with the Block Design, Similarit

Pain and Health: Sex-Specific Differences From a Survey Study of Adults With Arthrogryposis Multiplex Congenita.

Arthrogryposis Multiplex Congenita (AMC), a rare congenital condition characterized by joint contractures in two or more body regions, is often accompanied by pain. Understanding pain and its relationship to health outcomes among adults with AMC is critical for effective pain management. The objectives of this survey study were to characterize the pain experience in adults with AMC and determine the extent to which pain intensity relates to health, while considering potential sex differences. Participants (n = 73, 67.3% female) reported average pain intensity, severity, and extent per body dia

Lethal Congenital Contracture Syndrome Type 3 in an Isolated Canadian Population.

The diagnosis of a genetic etiology for fetal akinesia deformation sequence (FADS) in the prenatal setting is challenging. It is especially difficult in patients from isolated populations in which genetic testing has been limited and the background allelic frequencies are unknown. Lethal congenital contracture syndromes type 3 (LCCS3; OMIM 611369) is a rare autosomal recessive condition caused by biallelic pathogenic variants in the PIP5K1C gene. There are only 13 cases of LCCS3 in the medical literature. We describe three cases sharing a novel variant in PIP5K1C in an isolated Canadian popula

Understanding Arthrogryposis Multiplex Congenita (AMC) Across the Lifespan: An Integrative Review of the Adult AMC Registry's Contributions With Lived Experience Partnerships.

This reflective essay explores how collaborative work between researchers and adults living with Arthrogryposis Multiplex Congenita (AMC) has deepened and broadened our understanding of what it means to live with this rare condition. By intentionally integrating the lived experiences of individuals with AMC into research processes, the resulting registry becomes far richer, more representative, and more meaningful. Including people with AMC as co-investigators not only strengthens the quality and relevance of the data collected but also offers participants a sense of agency, ownership, and emp
