Breaking the Taboo: Four Cases of Thoracic Segmental Spinal Anesthesia in Modern Surgical Practice.

Thoracic segmental spinal anesthesia (TSSA) is an old neuraxial technique that has recently regained attention as a possible alternative to general anesthesia (GA) in selected surgical patients. Its segmental nature, low-dose local anesthetic requirement, and ability to preserve spontaneous ventilation may be particularly relevant in high-risk patients in whom airway manipulation, endotracheal intubation, positive-pressure ventilation, and systemic anesthetic exposure could represent an additional physiological burden. We report four selected high-risk patients undergoing procedures from diffe

Surgical timing and management patterns across joints in arthrogryposis: a systematic review.

This study systematically reviewed the literature on surgical management of arthrogryposis multiplex congenita (AMC) to identify trends in timing and outcomes of interventions across joints. A secondary objective was to provide practical recommendations and a clear research agenda to inform a forthcoming consensus process on timing and synchronization of care in AMC. A systematic review was conducted using MEDLINE, Embase, Web of Science, and Scopus databases from inception to June 2025. Studies reporting surgical management of AMC with data on age at intervention, recurrence, or functional ou

Laryngeal Mask Airway-Guided Fiberoptic Tracheal Intubation After Traumatic Failed Intubation in a Neonate With Severe Arthrogryposis Multiplex Congenita: A Case Report.

Airway management in neonates with arthrogryposis multiplex congenita (AMC) presents extreme challenges due to craniofacial abnormalities, cervical rigidity, limited mouth opening, and restrictive chest wall mechanics. These patients are at increased risk of failed intubation and perioperative respiratory complications, as airway trauma may rapidly develop, causing airway edema and compromising oxygenation. Early recognition and individualized airway planning are essential to avoid repeated traumatic airway manipulation and its associated complications. In our case, a full-term male neonate wi

Arthrogryposis as a neuromuscular phenotype: lessons from PIEZO2 loss-of-function.

Arthrogryposis multiplex congenita is a heterogeneous group of disorders characterised by multiple joint contractures resulting from impaired fetal movement. A 43-year-old woman presented with a long-standing congenital clubfoot, progressive scoliosis since childhood and distal joint contractures. She had pure sensory findings, with global areflexia, severe sensory ataxia and a positive Romberg sign, significantly affecting her balance and gait. Genetic analysis identified two likely pathogenic variants in the PIEZO2 gene, consistent with an autosomal recessive inheritance pattern. We discuss

Spinal Involvement in a Pediatric and Adult Cohort of Patients With Arthrogryposis Multiplex Congenita.

This was a single-center retrospective observational study with national recruitment from October 2007 to March 2022 at the AMC clinic of the University Hospital Grenoble Alpes (CHUGA). Participants underwent a clinical spinal assessment and spine radiography. Our main objective was to analyze the type of spinal involvement within each AMC subgroup. We hypothesized that there was a difference in the prevalence of scoliosis between subgroups. Arthrogryposis multiplex congenita (AMC) is classified into three subgroups (amyoplasia, distal arthrogryposis [DA], "other causes") according to their ph

Two Novel ACTC1 Variants Cause Arthrogryposis Multiplex Congenita.

Arthrogryposis multiplex congenita (AMC) is a group of conditions with congenital joint contractures in at least two body regions. ACTC1 variants cause heart disorders but have also been reported in six families with AMC. We report on two individuals with AMC who were heterozygous for ACTC1 missense variants (NM_005159.5; c.325G>A, p.Glu109Lys and c.650A>C, p.Lys217Thr). Individual 1 had scoliosis and no reported cardiac phenotype. Individual 2 had Shone's complex with coarctation of the aorta, which has not been reported in ACTC1-related AMC before. In vitro testing demonstrated that p.Glu109

Pediatric patients with arthrogryposis have increased early complications and long-term reoperation risk following posterior spinal fusion.

PURPOSE: Arthrogryposis multiplex congenita (AMC) is a rare syndrome characterized by congenital joint contractures involving at least two different body areas. AMC patients often develop scoliosis and may undergo posterior spinal fusion (PSF). However, such procedures can be challenging due to a spectrum of underlying differences in body structure and comorbidities. The present study aimed to investigate odds of postoperative adverse events and reoperation in patients with AMC following PSF relative to matched controls with idiopathic scoliosis (IS). METHODS: Pediatric AMC and IS patients un

Management of Lower-Extremity Deformity in Arthrogryposis Multiplex Congentia: A Narrative Review.

Arthrogryposis multiplex congenita (AMC) is a highly heterogeneous constellation of disorders defined by non-progressive congenital multiple joint contractures, typically manifesting across multiple limbs. The pathology results in significant functional impairment, including restricted range of motion, chronic arthralgia, and secondary musculoskeletal deformities like scoliosis. Given that AMC is an umbrella designation encompassing over 300 distinct etiologies, the profound clinical variability poses substantial diagnostic and therapeutic challenges. The relative rarity of AMC and the absence

The International Consortium for Arthrogryposis: A Collaborative Framework for Early Detection, Care, Research, and Education.

Arthrogryposis multiplex congenita (AMC) is a group of rare congenital conditions, characterized by multiple joint contractures but may involve any body system including central nervous system. AMC is etiologically heterogeneous, with over 400 genetic and many non-genetic causes implicated in its prenatal development. AMC requires lifelong, multidisciplinary management, a challenge compounded by their rarity and phenotypic heterogeneity. Our understanding of AMC is hampered by diagnostic delays, scarce trained specialists, and variable clinician expertise. Research networks are paramount to ha

Disagreement About Withholding Treatment on the Basis of Disability.

The parents of an infant born with arthrogryposis multiplex congenita (AMC) insist that she will not have a life worth living and ask the clinicians not to pursue life-preserving interventions and to discontinue feeding her. The neonatology team disagrees and worries that the parents are not making care choices in the patient's best interests. This challenging case is analyzed from 3 unique perspectives. First, a scholar living with AMC shares her experience navigating this diagnosis from childhood to adulthood alongside her family. A neonatologist-ethicist argues for neonatologists to inform

Child Neurology: Arthrogryposis and Antenatal-Onset Hyperkinetic Movements in a Newborn.

The evolving field of fetal neurology can offer insights into early nervous system development through antenatal dynamic ultrasonography and maternal perception of movements. The spectrum of abnormal fetal movements ranges from fetal akinesia, often associated with arthrogryposis multiplex congenita (AMC), to increased repetitive movements, typically indicating fetal seizures. Nonepileptic hyperkinetic movement disorders of the fetus are rarely documented. Here, we report a female infant who had clonus-like movements in utero and postnatally. Antenatal ultrasonography revealed polyhydramnios,

Integrated Advanced Monitoring and Target-Controlled Infusion Anesthesia in a Child With Arthrogryposis Multiplex Congenita.

Arthrogryposis multiplex congenita (AMC) is associated with significant anesthetic challenges due to multisystem involvement, craniofacial abnormalities leading to potential difficult airway, thermoregulatory instability, and concerns regarding susceptibility to malignant hyperthermia. Current perioperative practice emphasizes advanced airway planning, quantitative neuromuscular monitoring, tailored hypnotic delivery, and reliable neuromuscular reversal. We describe the perioperative management of a child with AMC using an integrated, technology-guided anesthetic strategy. A 10-year-old, 33.5

Upper Extremity Entrapment Neuropathies in Adults With Arthrogryposis Multiplex Congenita: A National Database Study.

Adults with arthrogryposis multiplex congenita (AMC) experience lifelong disability from joint contractures, yet susceptibility to upper extremity compressive neuropathy remains unevaluated. Using the TriNetX US Collaborative Network, we identified 2772 adults with AMC and 100,903,909 controls. Prevalence of upper extremity entrapment neuropathies was compared using prevalence ratios (PR) and 95% confidence intervals, with age-stratified analyses across three strata (18-39, 40-59, and 60-69 years). Adults with AMC demonstrated elevated entrapment neuropathy rates across all distributions. Carp

Genetic Bases of Arthrogryposis Multiplex Congenita.

Arthrogryposis multiplex congenita (AMC) is characterized by congenital joint contractures in two or more body areas resulting from reduced or absent fetal movements. AMC exhibits marked phenotypic and genetic heterogeneity, as it is a symptom rather than a disease and may be part of a large number of unrelated conditions. Despite advances in genomic approaches and the increasing number of newly identified genes, disease-gene identification was achieved in fewer than half of cases in several reports, including in a French cohort of 367 AMC patients. The most frequent cause of AMC in these repo

Pathogenic variants in BORCS5 cause a spectrum of neurodevelopmental and neurodegenerative disorders with lysosomal dysfunction.

BORCS5 encodes a subunit of the BLOC-One-Related Complex (BORC), which is known to promote anterograde movement and fusion of lysosomes. We identified 16 individuals from 9 families with bi-allelic BORCS5 variants, revealing a spectrum of neurodevelopmental and neurodegenerative phenotypes. Carriers of homozygous protein-truncating variants (PTVs), resulting in complete loss of BORCS5, presented with prenatally lethal arthrogryposis multiplex congenita, brain malformations, and neuropathological evidence of neuroaxonal dystrophy. Individuals with missense or splice-site variants presented diff

Arthrogryposis With CNS Involvement in AFF3-Related KINSSHIP Syndrome: A Detailed Phenotypic Characterization From a Mexican Patient and Review of the Literature.

Arthrogryposis Multiplex Congenita (AMC) is a heterogeneous group of disorders characterized by multiple contractures, often associated with central nervous system (CNS) involvement. To date, advances in Next Generation Sequencing (NGS) have enabled the identification of more than 400 genes implicated in AMC. Genetic causes of AMC and CNS involvement are especially challenging due to their heterogeneous clinical presentation, overlapping phenotypes, and often uncertain etiology. Pathogenic variants in AFF3 degron motif were identified as a cause of KINSSHIP syndrome (MIM #619297) which is clin
