ADGRG6-related disorder: a novel mutation resulting in distal arthrogryposis and a patchy neuropathy.

Arthrogryposis multiplex congenita (AMC) is associated with >150 genes, including ADGRG6, which codes for an adhesion G protein-coupled receptor. Biallelic loss of function variants in ADGRG6 have been linked to lethal congenital contracture syndrome. Here we present an atypical, milder phenotype associated with a novel ADGRG6 variant. A 22-year-old female, born from non-consanguineous parents, presented with non-progressive, distal arthrogryposis of her limbs since birth. Clinical evaluation showed distal contractures of all four limbs, absent right biceps resulting in elbow flexion weakness

Content validity of mobility measures in arthrogryposis multiplex congenita: engaging clinicians and people with lived experience.

INTRODUCTION: Lower-extremity impairment is prevalent in children with Arthrogryposis multiplex congenita (AMC), frequently leading to mobility limitations. Without AMC-specific assessment tools, clinicians and researchers often employ tools that have not been formally validated for the AMC population. This study aims to establish the content validity of commonly used mobility measures in children with AMC following the COnsensus-based Standards for health Measurement INstruments (COSMIN) and the International Classification of Functioning, Disability, and Health (ICF) framework. METHODS: Ite

Biallelic CRELD1 variants cause severe muscle weakness and infantile epilepsy.

Nicotinic acetylcholine receptors are widely expressed in the peripheral and central nervous systems. Mutations in acetylcholine receptor-subunit genes have been associated with neuromuscular diseases, such as arthrogryposis multiplex congenita (AMC) and epilepsy. We report a patient with arthrogryposis, severe muscle weakness and neurodevelopmental delay. During his first year of life, he developed therapy-refractory epilepsy. Using whole-exome sequencing, we identified the compound pathogenic variants c. 875G>A (p. Cys292Tyr) and c. 959delA (p. Gln320Argfs*25) in the cysteine-rich with epide

Anesthetic management of children undergoing specialized orthopedic surgeries.

Children with musculoskeletal disorders frequently undergo surgical procedures requiring specialized need and care from an anesthetic perspective. In this review, we discuss anesthetic issues relating to some of the common orthopedic conditions amongst children that warrant knowledge, familiarity, and skills to provide safe anesthetic care for optimal outcome. We have included cerebral palsy, osteogenesis imperfecta, arthrogryposis multiplex congenita, and achondroplasia in this spectrum of review. The scope of this review encompasses existing medical literature and institutional experience ga

CRISPR Activation Reveals the Spliceogenicity of an Intronic NEB Variant in Fetuses With Arthrogryposis Multiplex Congenita 6.

Whole-genome sequencing identifies intronic variants whose pathogenicity can be predicted with tools like SpliceAI. However, an actionable classification of such variants may require RNA-based validation, which can be limited by low expression in clinically accessible tissues. We report two fetuses from one family with Arthrogryposis multiplex congenita 6 (AMC6 [OMIM # 619334]) and biallelic NEB variants: a paternally inherited likely pathogenic frameshift variant, Chr2(GRCh38):g.151579391del; NM_001164508.2:c.16653del; NP_001157980.2:p.(Asp5552ThrfsTer5), and a maternally inherited intronic v

Biallelic variants in the UTRN gene cause a novel form of multiple congenital arthrogryposis.

Arthrogryposis multiplex congenita (AMC) is a large group of congenital conditions characterized by joint contractures affecting two or more body areas. A part of AMC type is caused by heterozygous pathogenic variants in genes encoding sarcomeric components of skeletal muscle fibers. Here we report a 7-year-old boy with a phenotype including AMC with dysmorphic facial features, short stature, congenital malformations of brain, colon and lacrimal canal. Trio whole-genome sequencing identified compound heterozygosity in the UTRN gene, consisting of a splicing variant in intron 57 (c.8434 + 1G>A)

Gross motor functional classification for arthrogryposis multiplex congenita: protocol for co-development involving public with lived and professional experience.

BACKGROUND: Children with arthrogryposis multiplex congenita (AMC), a group of over 400 conditions characterized by congenital joint contractures, present with a wide range of gross motor functioning and mobility due to the heterogeneity of underlying diagnoses and physical involvement. Existing classification systems for AMC focus primarily on etiology and anatomical distribution but do not describe differences in gross motor functioning. In contrast, condition-specific functional classification systems have improved communication, treatment planning, and research by stratifying individuals b

Anesthetic Management of Inguinal Hernia Surgery in an Adult Patient With Arthrogryposis Multiplex Congenita: A Case Report.

Arthrogryposis multiplex congenita (AMC) poses major anesthetic challenges due to severe joint contractures, airway restrictions, and a debated risk of malignant hyperthermia. Despite these complexities, anesthetic reports in adult AMC patients remain extremely limited, leaving perioperative management largely unstandardized. We present the anesthetic approach for a 35-year-old male patient with AMC undergoing open inguinal hernia repair. Given the high risk of airway compromise and joint immobility, we opted for ultrasound-guided ilioinguinal and iliohypogastric nerve blocks with dexmedetomid

Arthrogryposis Multiplex Congenita (AMC) and counselling before and during pregnancy: a questionnaire study.

BACKGROUND: Pre-pregnancy counselling in women with Arthrogryposis Multiplex Congenita (AMC) is not implemented as standard care. Prior surveys revealed that the majority of adults with AMC live independently with or without support, are working, and engage in social actifities. Although many of them underwent pregnancies, literature is scarce concerning women with AMC and pregnancies. This study enquires information of women with AMC and their preferences to optimize counselling and guidance concerning aspects related to (pre-)pregnancy, childbirth and parenthood. Women with AMC, being member

Fracture Risk Following Hardware Removal in Children With Arthrogryposis.

BACKGROUND: Children with arthrogryposis often undergo lower extremity long bone osteotomy for improved biomechanical alignment with subsequent hardware removal. The incidence of fracture and infection is unknown in this population. Therefore, the aim of this study was to identify the incidence of these complications following hardware removal and compare them to a matched cohort of patients with cerebral palsy. METHODS: Patients with arthrogryposis aged 0 to 20 years who underwent lower extremity osteotomy with subsequent hardware removal were included. All hardware removal events were recor

Maternal, fetal and neonatal outcomes among pregnant women with arthrogryposis multiplex congenita: a scoping review.

BACKGROUND: The rarity of pregnancies in women with arthrogryposis multiplex congenita (AMC) could lead to healthcare providers having limited exposure to these cases. Consequently, they may be less familiar with the possibilities and challenges associated with pregnancies in women affected by AMC. AMC is an umbrella term for a disorder with multiple contractures at birth, having a broad spectrum of causes, onset and severity of expression. A clinical classification describing the phenotype is Group 1 with primary limb involvement, Group 2 with musculoskeletal involvement plus other system ano

Arthrogryposis Multiplex Congenita Discovered at Birth: A Case Report.

Arthrogryposis multiplex congenita (AMC) is a rare condition characterized by multiple joint contractures present at birth. It results from fetal akinesia, which disrupts joint development and is accompanied by muscle weakness and fibrosis. Its etiology is heterogeneous. We report the case of a female newborn. The mother, a 27-year-old woman (G2P2) with no significant medical history, carried the pregnancy to term with a cephalic fetal presentation. Delivery, performed via cesarean section, resulted in a neonate in a state of apparent stillbirth, requiring resuscitation measures. The Apgar sco

Does Open Reduction of Arthrogrypotic Hips Cause Stiffness?

BACKGROUND: Concerns of potential hip stiffness following open reduction for congenitally dislocated hips in arthrogryposis multiplex congenita (arthrogryposis) persist, despite published accounts of positive outcomes. We retrospectively compared preoperative and postoperative hip motion and evaluated the ambulatory abilities of patients who underwent the procedure. METHODS: Over a 10-year period, 52 patients with arthrogryposis had 75 open reductions for congenitally dislocated hips with a minimum follow-up of 2 years. The mean patient age at surgery was 23 months. Hip range of motion data w

[Expert consensus on the clinical diagnosis and management of Arthrogryposis multiplex congenita (2025 Edition)].

Arthrogryposis multiplex congenita (AMC) is a clinical syndrome with complex etiology. It is a non-progressive syndrome characterized by joint deformity and stiffness, muscle atrophy, reduction of skin folds and subcutaneous tissue, and contracture of periarticular tissue, and can affect children in whom there are at least 2 or more joint contractures in multiple areas. AMC may be divided into amyoplasia, distal arthrogryposis, central nervous system and neuromuscular diseases which can be diagnosed by prenatal ultrasound and genetic testing, as well as by clinical evaluation, genetic examinat
