BACKGROUND: Tibial hemimelia is an extremely rare congenital limb deficiency characterized by variable tibial absence, knee instability, and ankle deformity. Existing classification systems rely primarily on static radiographic morphology and provide limited guidance for surgical decision-making, particularly regarding limb reconstruction versus amputation. Determinants of limb salvage especially the integrity of the quadriceps extensor mechanism and knee stability are not systematically incorporated into current classifications. This study proposes a novel classification designed to integrate

BACKGROUND: The association between congenital talipes equinovarus (CTEV) and developmental dysplasia of the hip (DDH) remains contested. Some guidelines consider idiopathic clubfoot a DDH risk factor warranting routine hip ultrasonography, while others do not. This study aimed to determine the prevalence of DDH in a large cohort of infants with idiopathic CTEV and assess whether routine screening is justified.
METHODS: A retrospective review of prospectively collected data was conducted at a single tertiary pediatric orthopaedic center. Children treated for idiopathic CTEV between July 2000

This study reports a case of multiple congenital malformations in a burrowing owl (Athene cunicularia) rescued in southern Brazil. Clinical assessment revealed partial agenesis of the forelimbs, bilateral asymmetric syndactyly, and selective anonychia of the second digit in both pelvic limbs. Despite severe locomotor limitations and the inability to fly, the individual maintained a satisfactory body condition score, demonstrating the species' adaptive and behavioural capacity. The integrity of the epithelial tissues and the symmetry of the digital alterations support a teratogenic etiology, di

Objectives: Flat feet is a very common deformity in children. Often, radiologic evaluation for children with flat feet is made. Although the measurement reliability of some angles in flat feet has been evaluated in the literature, none of them have been performed in the age range of 3-7 years, when the deformity is most common. Our aim is to evaluate the interobserver and intraobserver reliability of the 7 angles that are used for assessing flat feet. Methods: Radiologic angles were measured by four independent observers (two specialists and two residents) in two separate sessions on the foot

BACKGROUND/OBJECTIVES: Diseases affecting the craniofacial skeleton are normally associated with disturbances in the regulation of cellular differentiation, the development of bone structures, and changes in bone density and ossification. Thus, the objective of this integrative review is to evaluate the published scientific literature from the last 8 years concerning the impact of genetics on some craniofacial dysplasias. Our aim covers the identification of oral cavity alterations to those dysplasias, through the most common orofacial manifestations. Three dysplasias were selected to be part

Myhre syndrome is a rare genetic disorder characterized by progressive multisystem involvement. Gain-of-function missense heterozygous variants affecting the Ile500 residue and Arg496 residue of the SMAD4 gene are implicated in this condition. In this article, we aim to understand the spectrum of congenital anomalies in Myhre syndrome by studying a cohort of previously unreported patients alongside published literature. Our analysis revealed that the musculoskeletal system was the most common system to be affected, followed by the cardiovascular system. Intrauterine growth restriction was the

Heterozygosity for the pathogenic missense TBX5 (T-box transcription factor 5) variant p.G125R, identified in members of a family presenting with atypical Holt-Oram syndrome, is associated with supraventricular arrhythmias, including atrial fibrillation and disrupted sinus rhythm. Tbx5+/G125R mice recapitulated major aspects of the patient phenotype, including increased susceptibility for atrial arrhythmias, and revealed deregulation of the adult atrial cardiomyocyte transcriptional regulatory network and epigenetic state. Because TBX5 has also been implicated in the development and function o

BACKGROUND: Tethered cord syndrome (TCS) is most commonly associated with congenital spinal dysraphism, thickened filum terminale, lipomas, or split cord malformations (SCM ). Bony causes of tethering are typically described in association with diastematomyelia. Isolated intradural bony spurs without cord splitting or extradural communication are exceptionally rare.
CASE DESCRIPTION: We report a young adult female presenting with long-standing progressive lower limb sensory disturbances, gait tightness, and urinary dysfunction. A delayed neurological diagnosis resulted in multiple orthopedic

BACKGROUND: Meckel-Gruber syndrome is a rare autosomal recessive ciliopathy characterized by the triad of occipital encephalocele, polycystic kidney dysplasia, and postaxial polydactyly, with an estimated incidence of 1:13,250 to 1:140,000 live births. Pathogenic variants in CC2D2A, encoding a ciliary transition zone protein, account for 5-15% of MGS cases. This report highlights the diagnostic utility of whole-exome sequencing (WES) in delineating molecular etiologies of MGS and reviews genotype-phenotype correlations associated with CC2D2A mutations.

AIM AND BACKGROUND: The introduction of the 'Vitruvian' Foot concept in 2019 established initial parameter measurements of the 'normal' foot quadrilateral (FQ) in the sagittal plane. This effort had been aimed at increasing precision when analysing alignment abnormalities in post-traumatic and congenital foot deformities, so as to more accurately guide corrective treatment planning with osteotomies. The current study enhances the prior, existing data with an expanded normal cohort. We also offer a case report that describes the successful application of the revised reference angles for our ana

PURPOSE: Adams-Oliver syndrome (AOS) is a genetically heterogeneous disorder with cardinal features of aplasia cutis congenita and terminal limb reduction defects. A minority of individuals with AOS develop potentially lethal pulmonary hypertension (PH) in infancy, a subgroup that has been refractory to genetic explanation.
METHODS: We studied a cohort of individuals with AOS and no genetic diagnosis by genome and exome sequencing. We characterized rare, identified substitution variants in valosin-containing protein (VCP) in vitro using ATP hydrolysis, cryogenic-electron microscopy, thermal s

BACKGROUND: Clubfoot is a common birth defect that affects one or both feet. Evidence suggests increased risks of clubfoot in offspring associated with maternal medication and cigarette smoking exposure in early pregnancy. We explored whether such associations differed by clubfoot laterality.
METHODS: This case-control study was conducted in Massachusetts, North Carolina, and New York (2007-2011). State birth defects registries reported clubfoot diagnoses among infants aged < 11 months. Controls without birth defects were random samples of infants from the same catchment areas and born in the

Fanconi anemia (FA; OMIM: 227650) is a rare genetic disorder characterized by bone marrow failure, congenital anomalies, and cancer predisposition. While FANCA mutations account for most FA cases, phenotypic overlap with other disorders complicates diagnosis. This study analyzes molecular diagnostic pathways for FANCA-related FA and establishes a hereditary differential diagnosis for ectrodactyly. A Chinese FA family clinical phenotype was collected. The proband and father underwent whole-genome sequencing. Copy number variations (CNVs) in FANCA were assessed by genomic qPCR. Functional charac

BACKGROUND: Developmental dysplasia of the hip (DDH) presents a diagnostic challenge due to varying risk factors (RFs) and ultrasonographic (USG) classifications. This study aimed to identify independent RFs for DDH and evaluate their association with USG hip types in a large pediatric cohort. METHODS: A retrospective review was conducted on infants under six months of age from a tertiary DDH screening program (2008–2022). The study included 227 cases with DDH and 924 controls with normal hips. Univariate and multivariate logistic regression analyses were performed to assess the association of
