Identifying Risk Factors for Delayed Craniosynostosis Diagnosis and Barriers in Access to Care: A Retrospective Analysis at the University of Florida.

OBJECTIVE: Early diagnosis of craniosynostosis is crucial to providing minimally invasive surgery (MIS) options to families. Compared with open cranial vault reconstruction (OCVR), MIS is associated with improved patient outcomes, faster postoperative healing, improved cosmetic appearance, and reduced postoperative complications. This study aims to identify specific risk factors associated with delayed presentation for craniosynostosis repair at the University of Florida. METHODS: A retrospective chart review was conducted of patients under the age of 18 receiving OCVR, endoscopic strip crani

PTEN variant and genetic backgrounds combine to modify cerebellar neuronal differentiation in autism spectrum disorder.

Mutations in the PTEN gene have been implicated in autism spectrum disorders (ASD), particularly among individuals with comorbid macrocephaly. In our previous study, we demonstrated that the PTEN p.Ile135Leu variant, in an ASD-related genetic background dependent fashion, disrupts both cortical neurogenesis and gliogenesis. While abnormal cerebellar development is a recognized feature of ASD, the specific cellular targets and timing of disruptions during cerebellar differentiation and development remain poorly understood. To investigate these aspects, we applied our previously established cere

The Magnet-Actuated Craniofacial (MAC) Distraction System: Magnetic Coupling Modeling.

BACKGROUND: Craniofacial distraction osteogenesis (DO) has proven clinical effectiveness in the management of syndromic craniosynostosis and other complex craniofacial abnormalities. However, current craniofacial distraction systems rely on external activation ports which, in turn, increase the risk of infection, mechanical failure, premature removal of the distraction system and significant sociopsychological burden. To address these problems, we developed the Magnet-Actuated Craniofacial (MAC) distraction system, a fully-internalized distraction system to eliminate the need for external acti

MEGF8 controls osteogenic differentiation through post-transcriptional regulation of BMP-SMAD signaling in craniosynostosis.

Carpenter syndrome, caused by biallelic mutations in MEGF8 or RAB23, manifests with craniosynostosis through incompletely defined mechanisms. While both genes encode negative regulators of Hedgehog (Hh) signaling, we demonstrate that MEGF8 maintains cranial suture patency through a distinct, Hh-independent pathway. Loss of MEGF8 disrupts ubiquitination and lysosomal degradation of BMPR1A, leading to BMPR1A accumulation and hyperactivation of canonical BMP-SMAD1/5/9 signaling, which accelerates osteogenic differentiation of cranial mesenchyme. Using Megf8 mutant mice, we show tissue-specific sp

A Head of the Curve: Investigating Helmet Therapy Compliance in Patients With Craniosynostosis.

BACKGROUND: Adherence to helmet therapy is critical for achieving optimal cranial molding following endoscopic strip craniectomy (ESC). Limited data exist on factors influencing compliance, which may inform surgical planning and family counseling. This study evaluated clinical and sociodemographic factors associated with helmet therapy compliance in infants with sagittal craniosynostosis. METHODS: A retrospective review was performed of patients with nonsyndromic sagittal craniosynostosis who underwent ESC with postoperative helmet therapy between 2015 and 2024. Patients with open cranial vau

Generation of a P4hbY393C mouse model of cole-carpenter syndrome and therapeutic proof-of-concept.

Cole-Carpenter syndrome (CCS) is a rare autosomal dominant skeletal disorder characterized by severe bone fragility, recurrent fractures, craniosynostosis, and distinctive craniofacial abnormalities. Pathogenic variants in P4HB, encoding protein disulfide isomerase A1 (PDIA1), represent the most frequent cause of CCS. The recurrent missense mutation p.Tyr393Cys (Y393C) has been identified in unrelated patients, yet the in vivo consequences of this mutation on skeletal biology and its therapeutic implications remain poorly understood. Here, we generated a mouse model carrying the mouse homolog

CHD8 dysregulation in neurodevelopment: Emerging insights into autism pathophysiology.

Autism is considered a multifaceted neurodevelopmental condition, marked by impairments in social interaction, communication, and repetitive behaviours. Recent genetic research has highlighted mutations in the CHD8 (Chromodomain Helicase DNA-binding Protein 8) gene as one of the most significant risk factors associated with ASD. CHD8 encodes a chromatin remodeler protein that regulates gene expression by altering chromatin accessibility. This review aims to comprehensively review current findings on CHD8 mutations, with a focus on the gene's role in brain development, synaptic function, and AS

Long-Term Outcomes of a Vascularized Pericranial Flap to Prevent Persistent Skull Defects After Craniectomy for Sagittal Craniosynostosis.

Sagittal craniectomy for craniosynostosis creates an open calvarial defect, which generally closes within a year. However, some defects may persist and need eventual corrective cranioplasty. We previously demonstrated a significant reduction in the incidence of persistent defects 1 year postoperatively after suturing a vascularized pericranial flap to the dura exposed by sagittal craniectomy. This follow-up study evaluates the long-term postoperative outcomes of this technique. We conducted a retrospective review of a prospectively acquired single-site database of children who underwent primar

Brain organoid models of SZT2-related disease reveal an overproduction of outer radial glial cells through mTORC1 activation.

Biallelic loss-of-function variants of Seizure Threshold 2 (SZT2) cause neurodevelopmental diseases with developmental delay, epilepsy, and macrocephaly. SZT2 forms the KICSTOR complex, which represses the mechanistic target of rapamycin complex 1 (mTORC1) amino acid-sensitive pathway. SZT2 dysfunction is thought to cause abnormal activation of the mTOR pathway, underlying the pathogenesis of SZT2-related diseases. We previously reported constitutive activation of mTORC1 in lymphoblastoid cell lines derived from patients with SZT2-related disease. However, the impact of SZT2 dysfunction on hum

Patient risk factors for 30-day unplanned readmission after pediatric neurosurgery: are we improving?

OBJECTIVE: Thirty-day unplanned readmission is a key quality metric in pediatric neurosurgery, yet contemporary drivers of readmission in pediatric neurosurgery and whether outcomes are improving remain unknown. Thus, the authors assessed temporal readmission trends and evaluated patient- and hospital-level predictors of 30-day unplanned readmissions across major pediatric neurosurgical procedures. METHODS: A retrospective cohort study was conducted using the American College of Surgeons National Surgical Quality Improvement Program Pediatric data (2012-2023). Children who underwent CSF shunt

Prenatal Diagnosis of Malformations of Cortical Development: A Review of Genetic and Imaging Advances.

Malformations of cortical development (MCD) are a group of neurodevelopmental disorders caused by abnormalities in cerebral cortex development, leading to conditions such as intellectual disability and refractory epilepsy. The prenatal phenotypes of MCD are complex and non-specific, complicating accurate diagnosis and prognosis assessment. Genetic testing, particularly chromosomal microarray analysis (CMA) and whole-exome sequencing (WES), has become an important tool for prenatal diagnosis. This review synthesizes current research on prenatal MCD, focusing on the integration of imaging and ge

Auricular Cartilage Grafting for Lower-Eyelid Entropion in Syndromic Craniosynostosis: A Case Series.

Patients with syndromic craniosynostosis, including Crouzon and Pfeiffer syndromes, may develop lower-eyelid malposition, including retraction and secondary entropion, associated with complex midfacial and orbital anatomical abnormalities. Progressive midface hypoplasia, shallow orbits, and postoperative anatomical changes following craniofacial surgery may contribute to imbalance between the anterior and posterior lamellae, relative posterior lamellar insufficiency, and persistent eyelash-corneal contact. We report a retrospective case series of 3 patients who underwent posterior lamellar len

AKT is necessary for neuronal hypertrophy and epilepsy caused by Pten knockout.

Phosphatase and tensin homolog on chromosome 10 (PTEN) is a key negative regulator of the AKT/mTOR signaling pathway. Mutations in PTEN are highly implicated in Autism Spectrum Disorder (ASD), epilepsy, congenital hydrocephaly, and macrocephaly. While the conditional genetic knockout of Pten in murine neurons results in hypertrophy, increased migration, excitatory synaptogenesis, hyperexcitability, and epileptiform activity, the specific downstream signalling mediators of these pathologies remain to be fully elucidated. Using retroviral-mediated genetic manipulation of individual neurons withi
