Clinical and Neurodevelopmental Course in a Case of EFNB1-Related Craniofrontonasal Syndrome With Unrepaired Craniosynostosis.

INTRODUCTION: Craniofrontonasal syndrome displays an unusual X-linked dominant inheritance pattern due to pathogenic variants in EFNB1, which encodes a membrane-bound ligand for the ephrin receptor. Females exhibit a more severe phenotype than males and can have craniosynostosis, hypertelorism, craniofacial asymmetry, thoracic skeleton abnormalities, and an assortment of other features. Males typically only develop a milder hypertelorism, or they have no discernable features at all. METHODS: Exome sequencing was used to identify a likely pathogenic EFNB1 c.129-2A>G splice site variant in an i

Is Endoscopic Treatment of Sagittal Craniosynostosis in Children Aged 6 to 9 Months an Effective Approach for Delayed Presentations?

BACKGROUND: Endoscopic approaches to sagittal craniosynostosis are typically used in children <6 months because of more predictable outcomes with postoperative helmet therapy. For children presenting at 6 months or older, surgeons usually opt for open repair for more predictable correction. PURPOSE: The study's purpose was to estimate the incidence of satisfactory results and identify associated factors for children 6 months and older undergoing endoscopic-assisted craniectomy for sagittal craniosynostosis. STUDY DESIGN, SETTING, SAMPLE: This retrospective cohort study included children who

Facial skeleton displacement in non-syndromic brachycephaly.

To analyse the deformations affecting the facial skeleton as a consequence of craniosynostosis in isolated brachycephaly, we performed a 3D vestibular orientation of a series of 41 CT scans acquired from 25 cases of brachycephaly using the 3DSlicer application. They were first superimposed using a single age-matched CT scan of patients free of craniofacial anomalies, oriented in the same way, and then superimposed onto oriented 3D templates to reduce bias. The Virchow laws were confirmed; more interestingly, in all cases, the facial skeleton was displaced upward. We hypothesised that the skele

Open surgery in lambdoid-related plagiocephaly: detailed techniques, risks, and long-term results.

Lambdoid craniosynostosis is the rarest form among monosutural ones, with a prevalence ranging between 1 and 4% of all craniosynostosis. Open-strip craniectomy used to be the common treatment method; however, a better understanding of the pathology as well as a more careful analysis of outcomes and complications led to a shift towards other techniques such as open cranial vault reconstruction, device-mediated distraction, and endoscopic procedures. The present paper will deal with open surgical options, namely open cranial vault reconstruction (OCVR) and device-assisted procedures (DAP). In pa

Evidence-based classification of genes implicated in craniosynostosis disorders using the ClinGen curation framework.

PURPOSE: The Clinical Genome Resource Craniofacial Malformations Gene Curation Expert Panel (Cranio GCEP) was formed in 2020 with an initial target of evaluating genes implicated in craniosynostosis and skull abnormalities. The current work summarizes the findings of the Cranio GCEP during its first round of curation and aims to provide expert guidance on the clinical validity of gene-disease relationships in the context of craniofacial malformations. METHODS: The curation scope of the GCEP was separated into multiple rounds based on the frequency of occurrence and the uniqueness of associate

What is the ideal age for surgery in single-suture craniosynostosis?

The optimal timing for surgical intervention in single-suture craniosynostosis (SSC) remains debated, despite advances in minimally invasive and open reconstructive approaches. This work integrates Children's National Medical Center (CNMC) institutional data with current literature to clarify age-related outcomes and guide timing recommendations. Published series from CMNC were analyzed and outcomes included perioperative morbidity, morphometric correction, intracranial pressure, and revision rates. Findings were contextualized with recent systematic reviews and multicenter data. Early endosco

Prevalence of motor development delay in children with craniosynostosis: a systematic review and meta-analysis.

UNLABELLED: Craniosynostosis (CS) may influence motor development in childhood through anatomical and functional alterations resulting from premature fusion of cranial sutures. Although several studies have addressed neuropsychomotor outcomes in CS, the lack of consolidated estimates of motor delay prevalence represents a relevant scientific gap, which motivated the present study. This systematic review and meta-analysis aimed to estimate the prevalence of motor development delay in children with CS. Seven electronic databases, gray literature sources, and reference lists of included studies w

TEG 6s thrombelastography versus conventional coagulation assays in infants and toddlers during craniosynostosis surgery: a prospective observational clinical study.

Craniosynostosis surgery is associated with severe diffuse bleeding, which can result in a significant coagulopathy. The aim of this study was to compare Thrombelastograph Coagulation Analyzer TEG 6s parameters and the time required for result availability with conventional coagulation parameters (CCP) during pediatric craniosynostosis surgery. In this prospective clinical observational study, 29 infants and toddlers (median age 6 months, median weight 8.5 kg) undergoing craniosynostosis surgery were enrolled. Blood was sampled preoperatively and perioperatively. Samples were analyzed by TEG 6

Is a standardised severity index needed in unicoronal craniosynostosis? Challenges in developing an objective metric.

PURPOSE: Morphological severity of unicoronal synostosis (UCS) is highly variable. Yet, there is no consensus on an objective and standardised metric to quantify 3D severity to evaluate preoperative severity and surgical outcomes. This study aimed to investigate previously described severity indices and compared these to a statistical shape model (SSM) of the cranial vault and expert rating. METHODS: Computed tomography of 77 patients with non-syndromic UCS and 75 healthy controls were included. Principal component analysis (PCA) and partial least squares discriminant analysis (PLS-DA) were u

Postoperative raised ICP after minimally invasive surgery for monosutural craniosynostosis.

INTRODUCTION: Minimally invasive craniectomies (MIC) for monosutural craniosynostosis are more and more used. Their results are usually investigated in terms of surgical, esthetic, and neuropsychological outcomes but not specifically about the risk of postoperative raised intracranial pressure (ICP). The goal of this article is to review the literature and the personal experience looking for the possible risk of postoperative raised ICP after MIC. METHODS: A systematic review adhering to PRISMA guidelines was realized. In parallel, the personal experience concerning scaphocephalic children co

Comparative Outcomes of Fronto-Orbital Advancement and Strip Craniectomy in the Management of Metopic Craniosynostosis.

OBJECTIVE: Metopic craniosynostosis results in characteristic trigonocephaly and may impair cranial growth. Surgical correction is traditionally performed using fronto-orbital advancement (FOAR), while strip craniectomy with postoperative helmet therapy (SC) has emerged as a minimally invasive alternative. This study compares perioperative outcomes and postoperative cranial shape between FOAR and SC. METHODS: A retrospective cohort study was conducted at a tertiary pediatric craniofacial unit. Patients with isolated metopic craniosynostosis treated with FOAR or SC between 2015 and 2024 were i

Surgery for isolated sagittal synostosis: a neuro-developmental necessity or an isolated cosmetic challenge? Mini-survey and literature review.

INTRODUCTION: While surgery of single-suture sagittal craniosynostosis (SS) is effective for cranial shape correction, its role in preventing neurocognitive impairment or intracranial pressure (ICP)-related sequelae is uncertain. We conducted a mini-survey of experts and a narrative literature review to evaluate current perspectives and evidence guiding treatment indications for SS. METHODS: A mini-survey was distributed to 25 pediatric neurosurgery experts following a debate at the 2025 Varna meeting. Respondents were presented with clinical scenarios of infants with moderate or severe SS an

Does Socioeconomic Disadvantage Delay Access to Care for Craniosynostosis? A Study of 200 Children in Australia's Universal Healthcare Context.

INTRODUCTION: Craniosynostosis, the premature fusion of calvarial bones, requires surgical intervention within a critical time-period. Early referral potentiates the application of less invasive treatments. Previous studies from the United States have demonstrated that socioeconomically disadvantaged groups experience delayed referrals, older age at surgery and require more complex interventions. This study investigated the relationship between socioeconomic status and management for non-syndromic single suture craniosynostosis within Australia's universal healthcare system. METHODS: A retros

Prenatal diagnosis of Apert syndrome caused by a de novo FGFR2 mutation in the second trimester: a case report.

BACKGROUND: Apert syndrome is a syndromic craniosynostosis primarily caused by pathogenic mutations in the fibroblast growth factor receptor 2 (FGFR2) gene. Current prenatal diagnosis of Apert syndrome predominantly relies on ultrasound imaging in the third trimester (28 weeks of gestation to delivery), which is not conducive to early clinical decision-making. CASE PRESENTATION: In this case, prenatal ultrasound at the 20 weeks of gestation revealed craniosynostosis, oligohydramnios, and syndactyly of hands and feet in the fetus. Whole-exome sequencing (WES) followed by Sanger sequencing was

Severe Plagiocephaly Due to Isolated Left Frontosphenoidal Craniosynostosis: An Exceptional Presentation.

Craniosynostosis is a condition characterized by premature fusion of cranial sutures, leading to abnormal head shapes and potential developmental and neurological complications. Although craniosynostosis typically involves major cranial sutures, isolated frontosphenoidal suture fusion represents an exceptionally rare entity. In this case report, we present a 2-year-old girl with a rare occurrence of early closure of the left frontosphenoidal suture, resulting in significant left-sided plagiocephaly. After frontal-orbital advancement surgery with barrel-stave osteotomies, the patient experience

The ten test for facial sensory assessment: A scoping review.

Practical and reliable assessment of facial sensation is critical in plastic and reconstructive surgery given its role in mastication, speech, and communication. The Ten Test (TT), a sensory assessment based on comparison between normal and affected sensation, emerged as an alternative to calibrated monofilaments and has been validated extensively in the upper extremity. The goal of this scoping review was to map the existing evidence regarding TT assessment of facial sensation and identify gaps that warrant further investigation. Seven relevant publications were identified, comprising five cl
