Experimental validation of computational sub-aperture fusion wavefront sensing technology.

In this Letter, we introduce a computational sub-aperture fusion technique for Shack-Hartmann wavefront sensing. By incoherently fusing adjacent sub-apertures to emulate large-aperture imaging, this method significantly enhances the signal-to-noise ratio (SNR) within individual sub-apertures. Crucially, it decouples the target SNR from the sampling window size, thereby overcoming the conventional design trade-off between sub-aperture SNR and spatial sampling rate. Both online and offline adaptive optics (AO) experiments demonstrate that this approach substantially improves sub-aperture SNR for

Morphological analysis of posterior fossa in Apert and Crouzon syndromes before and after posterior cranial vault expansion.

BACKGROUND: Syndromic craniosynostosis, such as Apert and Crouzon syndromes, involve premature fusion of cranial sutures, leading to cranial deformities and potential neurological complications. Posterior cranial vault expansion (PCVE) is commonly used to address intracranial volume (ICV) anomalies in these cases, especially in the posterior fossa region. However, the morphological variation of this region between normal, Apert and Crouzon individuals remains understudied. This study aimed to carry out a detailed morphological analysis of the posterior fossa between the aforementioned groups b

Lateral Orbital Rim Advancement and Balanced Orbital Decompression for Exorbitism Regression in Crouzon Syndrome.

Crouzon syndrome is a syndromic craniosynostosis characterized by brachycephaly, midfacial hypoplasia, exorbitism, and malocclusion. Although current treatments can effectively address these deformities, they generally involve extensive staged procedures associated with prolonged treatment duration and substantial financial cost. For postpubescent and adult patients with relatively mild manifestations whose primary concern is noticeable exorbitism, less invasive options are limited. We describe a minimally invasive technique termed lateral orbital rim advancement combined with balanced orbital

En-bloc posterior vault distraction including the foramen magnum for Chiari I malformation in severe syndromic craniosynostosis: First cases and literature review.

This case series describes two patients with severe Crouzon syndrome and persistent Chiari I malformation. Initial treatment with standard posterior vault distraction osteogenesis and frontoorbital advancement failed to resolve the patient's symptoms. One patient developed severe syringomyelia and myelopathy refractory even to standard suboccipital decompression. Given the persistent posterior fossa constriction, an en-bloc posterior vault distraction incorporating the foramen magnum was performed over 21 days, achieving an expansion of 20 mm. This approach resulted in complete symptom resolut

Mechanical bone loading effects on morphology and mechanobiology in the coronal suture of Crouzon mice.

Craniosynostosis is a congenital condition characterized by the premature fusion of the craniofacial sutures. The Crouzon mouse (Fgfr2cC342Y/+) is a well-established model of this condition which shows premature fusion of the coronal suture. Our group has recently shown that postnatal, cyclic loading can potentially rescue the coronal suture and normalize skull morphology in Crouzon mice. This study aimed to investigate the underlying biological mechanism of the treatment. Wild-type (WT) and Crouzon (MUT) mice underwent in vivo loading sessions. Loading did not significantly affect skull shape

Use of electromagnetic Medtronic neuronavigation in monobloc frontofacial advancement for syndromic craniofacial synostoses: A preliminary case series.

Monobloc frontofacial advancement (MFFA) is a key procedure for treating functional and morphological complications of syndromic craniofacial synostoses. Critical osteotomies -particularly the frontonasal cut and the pterygomaxillary disjunction - are technically demanding and often performed under limited visibility. Conventional intra-operative fluoroscopy improves accuracy but prolongs surgery and exposes both patient and staff to radiation. We report our initial experience using an electromagnetic (EM) neuronavigation system (Medtronic StealthStation EM) in three pediatric patients undergo

Congenital Temporomandibular Joint Ankylosis: Investigating Potential Genetic Etiologies with Whole Exome Sequencing.

Background: Ankylosis of the temporomandibular joint (TMJ) is a rare developmental disorder that involves fibrous or bony fusion within the joint. It is a severe structural and functional disorder. Typically, the phenotype manifests as joint immobilization and results in facial deformity and trismus. To date, ankylosis is rarely diagnosed as congenital and its occurrence mechanism has not been thoroughly understood. We observed a female patient who as a newborn showed slight facial asymmetry and impaired mandibular retraction. In addition, non-uniform occlusal fissures were noted; the lower pa

Syndromic craniosynostosis: current concepts in intracranial hypertension, cranial base pathology, and surgical management.

Syndromic craniosynostosis is characterized by premature fusion of multiple cranial sutures accompanied by progressive craniofacial dysostosis and neurological complications. Although reduction of intracranial volume (ICV) was historically regarded as the principal mechanism underlying elevated intracranial pressure (ICP), contemporary evidence suggests that intracranial hypertension in syndromic craniosynostosis is multifactorial. Abnormal cerebrospinal fluid circulation, venous hypertension, upper airway obstruction, and premature cranial base fusion appear to contribute substantially to dis

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Highlighting the importance of X-ray diagnostics for targeted molecular genetic analysis in the diagnosis of rare autosomal dominant craniometaphyseal dysplasia.

Autosomal dominant craniometaphyseal dysplasia (AD-CMD) is a rare condition defined by the occurrence of progressive diffuse hyperostosis of cranial bones and abnormal metaphyseal widening of the tubular bones. ANKH is known to be the only gene associated with AD-CMD. We present a case of a toddler boy with macrodolichocephaly, asymmetry of the skull, wide bulging forehead, gingival hypertrophy and irregular teeth. Physical examination, X-ray and DNA analysis were performed. All exons and flanking intron regions of ANKH were amplified by PCR and directly sequenced using the Sanger method. X-ra

Correction of Secondary Exophthalmos Caused by Lateral Orbital Rim Recession: Add-on Implants Through a 3D Approach.

Improving exophthalmos is a crucial aspect of treatment for patients with syndromic craniosynostosis. However, in patients who have undergone previous treatments including fronto-orbital advancement and Le Fort III distraction osteogenesis and who have reached skeletal maturity, the recurrence of exophthalmos is not uncommon. The severity of the exophthalmos may vary. Whether secondary correction involving osteotomy should be performed or symptomatic treatment strategies should be adopted is a dilemma, and the patient's symptoms and background and the invasiveness of the surgery should also be

Postnatal Progressive Craniosynostosis: An Unusual Case Presentation Leading to Cascade Diagnosis for Multiple Generations.

NM_000141.5: FGFR2 c.1032G>A is a pathogenic variant that causes Crouzon syndrome through activation of a new donor splice site. This clinical report highlights the intrafamilial variability that can exist with this specific variant. The proband is a 4-year-old boy who initially presented with concern for seizures. Computed tomography and magnetic resonance imaging revealed pancraniosynostosis and Chiari 1 malformation. Papilledema on fundoscopic exam and copper beaten appearance of the skull on imaging both suggested the presence of elevated intracranial pressure. Genetic testing identified c

The cranial base and midface characteristics in apert and Crouzon syndrome: A 3-dimensional analysis of morphological variations.

The craniofacial morphology in patients with syndromic craniosynostosis varies from one spectrum to another. Our study aims to explore the craniofacial morphology and growth pattern in patients with Apert and Crouzon syndrome. This study involved 39 computed tomographic scans of skull (Apert = 11, Crouzon = 10, Control = 18), divided into 2 age subgroups. The non-parametric Mann-Whitney U test was performed to investigate the morphological characteristics between Apert, Crouzon and Control groups. The length of the middle cranial fossa decreased in both syndromes, but clivus length increased s

Strategies for Improving Case Reports Involving Patients With Rare Diseases.

For those treating patients with rare diseases, there may be a disproportionate clinical reliance on the literature, compared with those treating patients with common problems. Moreover, the rare disease literature consists of a preponderance of case reports. Together, these factors place a higher burden for accuracy on authors of case reports of patients with rare diseases. Our decades of experience with the rare congenital craniofacial myopathy, Freeman-Sheldon syndrome-now, Freeman-Burian syndrome, and other rare diseases suggests that accurate and current information may not efficiently pr

Upper Airway Obstruction Trends in Craniofacial Syndromes: A Comparative Study.

Craniosynostosis is a congenital disorder marked by the early fusion of one or more cranial sutures, potentially leading to functional impairments, including airway obstruction. This is particularly common in syndromic craniosynostosis, such as in Apert, Crouzon, Pfeiffer, Saethre-Chotzen, and Muenke syndromes, where midface hypoplasia and retrusion are often present. Airway obstruction, including obstructive sleep apnea, affects up to 68% of individuals with syndromic craniosynostosis. Understanding the severity and variability of airway compromise across these syndromes is critical for guidi

Fathoming the scientific paradox of intangibles: protocol reappraisal for optimizing cognitive outcomes in faciocraniosynostosis-an institutional experience.

BACKGROUND: The critical nature of normal intracranial pressure (ICP) and proper airway function in early brain development, particularly in the first 5 years, are well established. These elements are compromised in faciocraniosynostoses and potentially affect neurological growth. The impact of variations in ICP and airway on cognitive development remains underappreciated and challenging to quantify. OBJECTIVE: The primary aim of this study was to assess the efficacy and safety of a comprehensive early surgical intervention institutional protocol in children (0-5 years) with syndromic faciocr
