Craniometaphyseal dysplasia leading to hydrocephalus and Chiari I malformation.

We describe a case of craniometaphyseal dysplasia (CMD) that was initially misdiagnosed as craniodiaphyseal dysplasia (CDD). CMD and CDD are both rare genetic disorders affecting the craniofacial and tubular bones, due to ANKH and SOST gene mutations, respectively, causing similar defects in the control of bone mineralisation. The patient is male, who has been followed longitudinally from birth to his mid-teens, touching on important milestones concerning misdiagnosis and management of CMD. We discuss relevant investigations, diagnosis of ANKH mutation on genetic testing and neurosurgical manag

Identifying New Susceptibility Gene of Nonsyndromic Orofacial Cleft Based on Syndromes Accompanied With Hypertelorism.

ObjectivesGenetic studies of nonsyndromic orofacial clefts (NSOC) focus on identifying susceptibility genes and elucidating underlying pathogenesis. A correlation between orofacial clefts and hypertelorism has been established by previous studies, hinting at shared genetic risk factors.Materials and MethodsSyndromes characterized by hypertelorism and orofacial clefts were screened, followed by analysis and selection of their causative genes, resulting in 35 candidate genes. After genotyping quality control, 340 single nucleotide polymorphisms (SNPs) were analyzed. Allelic and genotypic associa

Prenatal diagnosis of chromothripsis causing complex chromosomal rearrangement involving chromosomes 5, 7 and 11 leading to TWIST1 deletion and Saethre-Chotzen syndrome.

OBJECTIVE: Prenatal detection of complex chromosomal rearrangements (CCR) is extremely rare, but is of great clinical importance, since CCR can be causative of different congenital disorders. We present an exceptionally rare case of prenatally diagnosed Saethre-Chotzen syndrome (SCS) rising as a consequence of chromothripsis involving chromosomes 5, 7 and 11 and deletion of TWIST1 gene. CASE REPORT: Brachycephaly, hypertelorism, flat face, micrognathia, relative macroglossia and small posterior fossa were noted on ultrasound examination at 28th gestational week. Fetal karyotyping revealed de

Clinical Features and PTCH1 Expression in Gorlin-Goltz Syndrome: A Case Report.

Background and Clinical Significance: Basal cell nevoid carcinoma syndrome, or Gorlin-Goltz Syndrome (GGS), is a genetic disease caused by germline mutations in genes involved in the Sonic HedgeHog (SHH) signaling pathway, mainly in the PTCH1 gene. PTCH1 is a receptor for SHH, and the activation of SHH signaling exerts a direct effect on the proliferation and maintenance of stem cells; alteration of its signaling could promote a favorable microenvironment for the maintenance of tumor viability. The main clinical manifestations of patients with GGS include multiple basal cell carcinomas, odonto

miR-383-3p and miR-6951-3p activate cell proliferation through the regulation of genes related to hypertelorism.

Hypertelorism, characterized by an abnormal increase in the distance between the eyes, is often associated with various congenital birth defects. While there is increasing evidence suggesting common underlying mechanisms for hypertelorism, the role of microRNAs (miRNAs)-short noncoding RNAs that suppress target genes by inhibiting translation and degrading mRNA-in the condition's pathogenesis remains unclear. This study aimed to identify the miRNAs associated with hypertelorism in mice. By searching the Mouse Genome Informatics (MGI) database and reviewing full-text references, we identified a

Prenatal diagnosis of Apert syndrome with continuation of pregnancy-a report of two cases.

BACKGROUND: Craniosynostosis, the premature fusion of cranial sutures, may occur in isolated or syndromic forms. Among syndromic craniosynostoses, Apert syndrome is a rare but significant condition, characterized by a triad of multisuture craniosynostosis, midface hypoplasia, and symmetric syndactyly of the hands and feet. Early prenatal diagnosis is challenging but essential for appropriate counseling and perinatal planning. CASE DESCRIPTION: We report two cases of prenatal diagnosis of Apert syndrome, confirmed through molecular genetic testing. In the first case, a 32-year-old secundigravi

Absence of Syndactyly Associated With the Common Apert FGFR2 S252W Mutation: A Clinical Report and Likely Molecular Explanation.

Apert syndrome is a recognizable craniofacial condition characterized by craniosynostosis, hypertelorism, exorbitism, midface hypoplasia, and complex symmetrical bony and cutaneous 'mitten' syndactyly of all four limbs. Around 98% of affected patients have one of two heterozygous missense variants in the FGFR2 gene, encoding either p.(Ser252Trp) (S252W) or p.(Pro253Arg) (P253R). We report a patient with unicoronal craniosynostosis and near normal limbs, in whom we unexpectedly identified a heterozygous FGFR2 S252W variant. Her mother, who had no history suggestive of craniosynostosis and only

Clinical insights of the TBX19 C.856 C>T variant: a case report and literature review on neonatal isolated ACTH deficiency.

OBJECTIVES: Congenital isolated adrenocorticotropic hormone deficiency (IAD) is a rare condition often caused by variants in the TBX19 gene, leading to significant adrenal insufficiency and metabolic disturbances in neonates. Early recognition and treatment are significant for improving outcomes. Although the c.856 C>T (p.Arg286Ter) variant in TBX19 has been identified as pathogenic, little is known about the genotype-phenotype correlation due to the limited number of cases. CASE PRESENTATION: We present a male infant diagnosed with neonatal IAD, presenting with hypoglycemic seizures, hypokal

Recurrent RAC3 related neuro-rachopathy in a pair of Indian siblings with novel findings: expanding the spectrum of brain anomalies.

OBJECTIVE: The Rho GTPase gene family plays a crucial role in key cellular functions. RAC3, one of the three genes in this family, along with RAC1 and RAC2, is highly expressed in the brain. It is specifically involved in neuronal differentiation, maturation, and migration. Therefore, dysregulation of RAC3 can lead to neurodevelopmental abnormalities. METHODS: Here we report two siblings, born to a nonconsanguineous couple, with global developmental delay, intellectual disability, and facial dysmorphism. RESULTS: Whole exome sequencing revealed a pathogenic heterozygous variant, c.184G>A (p.

Reviving the Past, Moving on to the Present, and Planning for the Future of Craniofacial Malformations.

The malformed newborn with craniofacial anomalies was variously regarded in different civilizations. Some populations considered it a symbol of the presence of God, whereas others brutally eliminated it. Understanding of craniofacial malformations started in the 19th century, with the development of embryological, experimental studies. Management of congenital facial deformities was not mentioned in medical literature until the middle of the 14th century. Ambroise Paré (1510-1590), published the first image of a cleft lip closure in 1561. Priority for cleft palate suture is shared between the
