INTRODUCTION: Dysmorphic syndromes are characterised by congenital anomalies affecting craniofacial features, body proportions, and organ structure. They are frequently caused by complex genetic alterations, including deletions, duplications, and microdeletions, which impede optimal growth regulation.
AIM OF THE STUDY: The objective of this paper is to present a synopsis of selected dysmorphic syndromes associated with excessive growth, with particular emphasis on syndromes with confirmed or strongly suspected chromosomal aetiology or associated with DNA methylation disorders (epigenetic diso

Accurate measurement of fetal head circumference (HC) is critical for monitoring fetal growth and the diagnosis of microcephaly and macrocephaly. However, conventional manual 2D ultrasound measurements tend to vary because of factors such as the presence of amniotic fluid, acoustic shadowing, occlusions, and variations in fetal position. To address these problems, this study proposes a regression-based deep learning model using EfficientNet-B7 and the Adam optimizer to enhance the accuracy and robustness of HC measurement. The proposed model utilizes EfficientNet’s powerful feature extraction

BACKGROUND/OBJECTIVES: Malan syndrome (MALNS) is a rare overgrowth disorder caused by pathogenic Nuclear Factor I × (NFIX) gene variants, and characterized by postnatal overgrowth, macrocephaly, developmental delay, intellectual disability and distinctive facial features. Chiari type I malformation (CMI), a condition where the cerebellar tonsils extend below the foramen magnum, has been observed in some patients with MALNS, although the exact relationship between these disorders remains unclear. The objective of this case report is to describe a novel NFIX variant in a patient with MALNS and a

Background: Glutaric acidemia type 1 (GA1) is an autosomal recessive neurometabolic disorder caused by pathogenic variants in glutaryl-CoA dehydrogenase (GCDH), with variable clinical severity despite early biochemical detectability. Population-specific mutational spectra and genotype-phenotype correlations remain insufficiently defined in infantile-onset disease. Therefore, this study aimed to define the GCDH variant spectrum in GA1 patients with mild hepatopathy and assess genotype-phenotype correlations. Methods: We performed integrated clinical, biochemical, and molecular characterization

BACKGROUND: Benign enlargement of the subarachnoid space (BESS) is a condition characterized by widened subarachnoid spaces, typically associated with macrocephaly or increasing head circumference during infancy or early childhood. Often described as idiopathic and self-limiting, BESS has also been associated with developmental delays and spontaneous subdural collections. Yet, complete interrogation of these associations is limited by lack of normative subarachnoid space characterization and standardized diagnostic criteria.
OBJECTIVE: This project aimed to review and synthesize literature on

PURPOSE: Neurofibromatosis type 1 (NF1) is a multisystem disorder with wide-ranging clinical presentations. Patients with NF1 may manifest with macrocephaly, strokes, and cognitive deficits, abnormal neural development, and other neurologic symptoms. This study used an atlas-based approach to quantitatively examine structural and physiologic changes of the brain in children with NF1.
METHODS: Children evaluated for NF1 over a 9-year period at a children's hospital were retrospectively reviewed (n = 34). Children with intracranial tumors or prior strokes were excluded. Patients received diffus

Malan syndrome is a rare overgrowth disorder caused by deletions or pathogenic variants in the NFIX gene. Here, we present the case of a Japanese male infant with Malan syndrome, which was caused by a novel frameshift variant resulting from an eight-base insertion in Exon 2 of the NFIX gene. Echocardiography revealed dilatation of the pulmonary artery without hemodynamic abnormalities. Malan syndrome shares many clinical features with Sotos syndrome, including overgrowth, macrocephaly, developmental delay, and intellectual disability. Therefore, molecular genetic testing is required for diagno

INTRODUCTION: Gorlin syndrome (GS) or basal cell nevus syndrome type 1 (BCNS1) is a rare genetic disease belonging to the spectrum of genodermatoses. Here, we present a case with BCNS1 that was caused by haploinsufficiency of PTCH1 gene, which is associated with BCNS1.
METHODS: A diagnosis for BCNS1 was clinically set for a 15-year-old female patient who presented with four nevoid skin lesions, four radiolucent osteolytic lesions of the jaws, macrocephaly, unilateral congenital cataracts, strabismus, hypertelorism, and calcification of the falx cerebri. The patient's parents bore no clinical

Autism spectrum disorder (ASD) is a neurodevelopmental condition with a 4:1 male bias, yet the biological basis for this sex difference remains poorly understood. Heterozygous mutations in the chromatin remodeler CHD8 confer high risk for ASD, with phenotypes influenced by sex and genetic background. Here, we generated Chd8+/R2219* mice carrying a CRISPR-Cas9-generated mutation orthologous to a human variant. These mice recapitulated core ASD features, including macrocephaly and autistic-like behaviors. Notably, social deficits showed a male preponderance, directly mirroring the human sex bias

Pathogenic variants in GNB2 have been associated with a neurodevelopmental disorder that includes global developmental delays and intellectual disability, hypotonia, increased risk for seizures, heart and renal anomalies, and characteristic facial features. Thus far, all reported patients with GNB2-related disorder have had de novo missense variants. Many of these single nucleotide substitutions have been recurrent. Exome sequencing identified a heterozygous GNB2 variant, c.217G>A, predicting p.(Ala73Thr) (NM_005273.3) in a family, including two brothers with developmental delays and their mor

PPP2R5D-related neurodevelopmental disorder (Houge-Janssens syndrome type 1) is a rare autosomal dominant condition characterized by macrocephaly, intellectual disability, and epilepsy. Progressive parkinsonism is an emerging adult phenotype that neurologists should be aware of since timely genetic diagnosis opens the door to disease-modifying therapies currently in clinical trials. We describe a 33-year-old woman with this condition who developed progressive mixed-tremor parkinsonism in early adulthood. Dopamine transporter imaging confirmed presynaptic nigrostriatal dysfunction. Whole-genome

Pathogenic CHD8 variants cause autosomal dominant 'Intellectual developmental disorder with autism and macrocephaly' (IDDAM) and are amongst the most common monogenic causes of autism. The clinical significance of CHD8 missense variants (MVs) frequently remains uncertain. Systematically applying ACGS/ACMG guidelines to 36 CHD8 MVs in 39 affected patients, only two variants were classified likely pathogenic (LP), with the remaining 34 classified variants of uncertain significance (VUS). We subclassified the variants according to posterior probability of pathogenicity (PPP), with 14 being at lea

BACKGROUND: Trio Rho guanine nucleotide exchange factor (TRIO) encodes the guanine nucleotide exchange factor (GEF) for RHOA and RAC1 GTPases, which plays a critical role in neurodevelopment. Pathogenic variants in the TRIO gene are predominantly associated with two autosomal dominant neurodevelopmental disorders: intellectual developmental disorder 44 with microcephaly (MRD44) and intellectual developmental disorder 63 with macrocephaly (MRD63). However, the genotype-phenotype correlation of TRIO-related disorders and the impacts of TRIO variants on early-life disease progression remain uncle

BACKGROUND: Phenotypic variability in PTEN hamartoma tumor syndrome (PHTS) remains incompletely understood, and the contribution of additional rare genetic variants has rarely been functionally investigated.
CASE PRESENTATION: We report a 9-year-old girl with macrocephaly, gastrointestinal manifestations, hypogammaglobulinemia, and lymphocyte abnormalities. Thyroid function and biochemical screening for SDHB-associated tumors were normal. Trio-based whole-exome sequencing identified a de novo pathogenic PTEN variant, c.464A > G (p.Tyr155Cys), together with a de novo SDHB variant, c.719T > C (

Next-generation sequencing has transformed the diagnostic approach to rare neurogenetic disorders. However, interpretation of molecular findings remains challenging when identified variants are discordant with the clinical phenotype. We report the case of a preschool-aged girl presenting with seizures, infantile-onset macrocephaly, and the characteristic magnetic resonance imaging (MRI) pattern of megalencephalic leukoencephalopathy with subcortical cysts (MLC). Whole-exome sequencing identified no pathogenic variants in the currently recognized MLC-associated genes. Instead, it detected a het
