Background: DICER1 syndrome is an autosomal dominant disorder with reduced penetrance and variable expressivity, predisposing carriers to a broad spectrum of benign and malignant tumors as well as non-neoplastic features. Recent reports suggest an expanded phenotype overlapping with GLOW syndrome, characterized by developmental anomalies in addition to tumor risk. Case Presentation: We describe a 16-year-old male with overgrowth, macrocephaly, developmental delay, multinodular goiter, and a unilateral lung cyst. Genetic testing identified a germline heterozygous nonsense variant in DICER1 (c.1

Hydrocephalus is a disorder of cerebrospinal fluid circulation that leads to ventricular enlargement and increased intracranial pressure. Presentation and imaging choice vary with age: infants often develop progressive macrocephaly because of skull compliance, whereas older children more commonly present with headache, vomiting, and lethargy. In newly suspected hydrocephalus, imaging confirms ventricular dilation, helps assess acuity, may identify an underlying cause, and informs management. In patients with cerebrospinal fluid diversion, imaging primarily evaluates suspected shunt malfunction

BACKGROUND: Megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a rare inherited white matter disorder. Initially, a "classic" phenotype has been characterized, presenting early-onset macrocephaly, cerebellar ataxia, mild spasticity, and a distinctive neuroimaging pattern of diffuse white matter abnormalities with subcortical cysts. An "improving" phenotype has also been described, featuring milder or absent neurological signs and a remitting pattern on neuroimaging. Mutations in four genes, MLC1, HEPACAM, GPRC5B and AQP4 have been associated with MLC. We describe clinical and

Neurodevelopmental disorder, speech impairment, and dysmorphic facies (NEDSID) is an autosomal dominant condition primarily driven by SETD1A haploinsufficiency. While most documented cases are sporadic, genomic mechanisms underlying intrafamilial phenotypic heterogeneity remain poorly understood. This study presents a comprehensive clinical, neurophysiological, and molecular characterization of a two-generation Mexican family segregating a novel heterozygous missense SETD1A variant. Whole-exome sequencing (WES) identified a c.1604G>A (p.Gly535Glu) substitution localized within the critical Fun

INTRODUCTION: Primary fourth ventricle outlet obstruction (PFVOO) is a rare cause of obstructive hydrocephalus in children. Diagnosis often requires high clinical suspicion and advanced magnetic resonance imaging (MRI) sequences, and the optimal management remains a matter of debate.
CASE PRESENTATION: We report a 3-year-old girl with unexplained tetraventricular hydrocephalus who presented with progressive macrocephaly, intermittent headaches, and atypical stereotyped head movements. She had previously undergone ventriculoperitoneal shunting in infancy, which failed after 18 months. High-res

BACKGROUND: In this case report, the authors describe a rare case in which embolization of a pediatric choroid plexus papilloma (CPP) functioned as definitive management rather than a preoperative complement.
OBSERVATIONS: A 14-month-old boy underwent evaluation for macrocephaly and developmental delay and was found to have a 2.6-cm avidly enhancing intraventricular mass centered in the temporal horn of the left lateral ventricle with extension into the atrium, radiographically most consistent with CPP. Diagnostic cerebral angiography demonstrated predominant supply from the left anterior cho

Osteopetrosis is a rare genetic skeletal disorder caused by defective osteoclast-mediated bone resorption, leading to increased bone density and complications including hypocalcemia, pancytopenia, cranial nerve compression, and, more rarely, hydrocephalus. We report a rare case of intermediate osteopetrosis presenting with neonatal hypocalcemia and progressive macrocephaly. Brain magnetic resonance imaging confirmed obstructive hydrocephalus, which was treated with ventriculoperitoneal shunting. Clinical and radiological findings supported the diagnosis of osteopetrosis, although targeted gene

The CHD8 gene encodes a chromatin-remodeling protein critical for neural development and transcriptional regulation. Although CHD8 mutations are classically associated with autism spectrum disorder, macrocephaly, and intellectual disability, recent reports suggest that dystonia may also be part of this spectrum. Two unrelated female patients presenting with progressive dystonia were evaluated through detailed clinical, neurological, and neuroimaging assessments. Whole exome sequencing (WES) was performed using the Twist Human Core Exome capture kit and variants were classified following Americ

INTRODUCTION: Hydrocephalus is an etiologically heterogeneous disorder that may result from acquired insults or monogenic causes. More than 100 genes have been implicated in congenital and early-onset hydrocephalus, but MAP7D3 has not previously been associated with disturbances of cerebrospinal fluid (CSF) circulation. We report an adult patient with obstructive hydrocephalus in whom a novel frameshift variant in MAP7D3 was identified by exome sequencing.

BACKGROUND: Recent advances in sequencing technologies have enhanced patient diagnosis; however, causal pathogenic variants remain unidentified for a significant number of patients due to limited understanding of certain variants, regulatory sequences, or sequencing challenges, such as complex rearrangements. Investigating the epigenetic landscape has become essential to improve the diagnostic yield. Diseases caused by pathogenic variants in epigenetic regulators, often associated with growth abnormalities, intellectual disability, and facial dysmorphism, are prime models for studying episigna
