Spinal arteriovenous malformation associated with congenital dermal sinus: a case report.

BACKGROUND: A spinal arteriovenous malformation is a rare condition caused by abnormal anastomoses in the spinal cord, in which blood flows from arteries to veins without capillary involvement. A congenital dermal sinus is a form of spinal dysraphism that results from aberrant development of ectodermal, mesodermal, and neuroectodermal tissues. We herein present the first reported case of an arteriovenous malformation with arteriovenous anastomosis adjacent to a congenital dermal sinus. CASE PRESENTATION: A 75-year-old Japanese man without underlying disease or trauma presented to a local hosp

Lateral congenital dermal sinuses extending into the spinal canal: case report and review of the literature.

Lateral congenital dermal sinus (CDS) typically blind-ends in the bone or soft tissue; meanwhile, medial CDS extends into the spinal canal through the spina bifida embryologically. We report the first case of lateral CDS extending into the spinal canal laterally without passing through the spina bifida. A 9-month-old infant presented with infected CDS refractory to partial sinus removal with a combination of surgical drainage and intravenous antibiotics. Eventually, a complete residual sinus removal was performed. Intraoperative findings revealed that the sinus extended into the spinal canal t

Closed spinal dysraphism: Think about it in the case of enuresis in children.

Closed dysraphism is a rare but complex neurological disorder that can manifest as neurological deterioration secondary to an inherently tethered spinal cord. Spina bifida occulta is a spectrum of abnormalities ranging from lack of fusion of the vertebral arch without other associated abnormalities sometimes. An uro-neurological assessment is essential as this malformation may present urinary symptoms as the only urinary symptoms without other obvious neurological abnormalities. Spinal cord MRI is useful for diagnosing and determining the morphology of the pathology in young children with this

Facial sinus tract mimicking cutaneous cyst or scar: unveiling a dental infection.

A 39-year-old man with an unrecognized facial sinus tract under his chin initially presented with an innocuous lesion resembling an acne pimple. The patient's journey began with dermatologic intervention, involving systemic and topical antibiotics. Upon treatment failure, referral to a general dentist revealed a carious lesion on the lower right lateral incisor, prompting further investigation. A subsequent referral to an endodontic specialist led to the diagnosis of a chronic periapical abscess, and an endodontic intervention was initiated. Despite the persistence of intracanal purulent exuda

Pterional dermal sinus: case report and review of the literature.

INTRODUCTION: Dermal sinus (DS) is a congenital condition of embryonic origin that occurs when the surface ectoderm fails to fully separate from the neuroectoderm, despite the neural tube remaining intact. Its most common location is the spinal region, followed by the occipital region. Off-midline presentation is extremely rare. The aim of this study is to present an atypical case of a dermal sinus located in the pterional region, outside the midline. CASE DESCRIPTION: A 4-year-old male patient presented with a mass in the tail region of the left eyebrow. Contrast-enhanced computed tomography

Composite type split cord malformation: a two-center study and analysis of a series of 25 cases.

INTRODUCTION: Split cord malformation (SCM) is a type of congenital spinal dysraphism characterized by the development of a split spinal cord separated by either a bony septum or a membranous/fibrous septum. The presence of two distinct SCMs in the same patient, with segments of normal cord interposed between them, is referred to as a composite-type SCM. This study aims to evaluate the variations of composite-type SCMs, which have been reported only at the case-report level in the literature, and to present the challenges encountered in surgical technique. RESULTS: The study included 25 patie

Radiological Insights Into Spina Bifida Occulta in Adults: Unveiling the hidden defect in the lumbosacral spine of the Northern Indian population at a tertiary care centre.

OBJECTIVES: Spina bifida occulta (SBO) is a congenital spinal defect characterised by incomplete closure of the posterior neural arch. Despite its clinical relevance, discrepancies remain in the published literature. This study aimed to determine the distribution pattern and prevalence of SBO in the Northern Indian population using three-dimensional reconstructed specimens of the lumbosacral spine. METHODS: This retrospective study was conducted between March 2023 and November 2024 at the All India Institute of Medical Sciences Patna, India. A total of 241 computed tomography (CT) scans of th

Spinal rhabdomyosarcoma in a child at the site of a lumbosacral lipoma: a rare entity.

Malignant tumours arising at the site of closed neural tube defects are extremely rare and have only been described three times in the literature. We report the case of a child with a congenital lumbosacral lipoma, a dermal sinus tract and low-lying spinal cord who was initially treated non-operatively due in part to non-attendance and who then presented at 2 years of age with recurrent urinary tract infections with intermittent pyrexia, back pain and difficulty walking. A craniospinal MRI was performed with suspicion of infection at the site of the closed neural tube defect (in view of the de

Concealed in Plain Sight: Revelation of Spina Bifida Occulta on 99mTc-MDP Whole-Body Bone Scan and SPECT/CT.

We present a case of an 11-year-old boy who presented with a 2-month history of low back pain, with no preceding medical conditions or underlying diseases reported. Initial diagnostic evaluation with a 3-phase bone scan revealed a focal area of uptake posterior to the L5 vertebra, which was confirmed by subsequent SPECT/CT imaging to be a reactive change associated with a narrow defect in the spinous process of the L5 vertebra, consistent with spina bifida occulta (SBO). This case highlights the ability of bone scans to uncover missed diagnoses of reactive changes at a typically nonsymptomatic

A case of congenital fibular hemimelia associated with skeletal and non-skeletal malformations.

Fibular hemimelia (FH) is a rare congenital abnormality where the fibula is either totally or partially absent. It can occur alone or alongside other skeletal malformations, and in very few cases, it may occur along with non-skeletal anomalies. A 4-year-old female was diagnosed with unilateral right-sided FH, accompanied by limb shortening, a right-side ankle deformity, valgus foot, and 3 lateral rays that had been totally absent since the first week of birth. And she was incidentally diagnosed with spina bifida occulta at a 3-year-old age. FH is most commonly unilateral and mostly affects the

Prevalence of occult spina bifida in isthmic versus degenerative spondylolisthesis: retrospective radiographic review of consecutive surgical patients.

BACKGROUND: Spondylolisthesis is defined as the anterior or posterior translation of one vertebral body over another. Spina bifida occulta (SBO) is a developmental anomaly characterized by the incomplete fusion of the laminae of one or more vertebral arches along the midline. Currently, there is no documented prevalence of SBO in patients with degenerative spondylolisthesis (DS) undergoing spinal surgery. This study aims to estimate the prevalence of SBO in patients with DS undergoing spine surgery and compare it with that in patients with isthmic spondylolisthesis (IS) undergoing surgery. ME

Novel variants in STAG2 and PKD1 associate with multiple congenital malformations and autosomal dominant polycystic kidney disease in a Chinese family: A case report and literature review.

Cohesinopathies are rare multisystem disorders caused by defects in the cohesin complex, which is critical for chromosome segregation, DNA repair, replication, heterochromatin formation and gene transcription regulation. Stromal antigen 2 (STAG2), a key cohesin component, is linked to neurodevelopmental disorders such as X-linked holoprosencephaly 13 and Mullegama-Klein-Martinez syndrome (MKMS). Polycystic kidney disease (PKD), particularly autosomal dominant PKD (ADPKD), is characterized by renal cysts and is commonly associated with variants in the PKD1 gene. In the present study, a Chinese

Characteristics of upper lumbar spondylolysis in children.

BACKGROUND: Pediatric lumbar spondylolysis (LS) is common in junior and senior high school athletes. Lower LS (L4-L5 level) is more common in children, and upper LS (L1-L3 level) is relatively rare; therefore, the pathogenesis of upper LS remains unclear. PURPOSE: To elucidate the mechanisms of upper LS by identifying and comparing characteristics between upper and lower LS cases. STUDY DESIGN/SETTING: Retrospective cross-sectional study. PATIENT SAMPLE: This study included 842 pediatric patients aged ˂18 years diagnosed with single-level acute LS at a single outpatient clinic between 2015

Large L5-S1 Lordosis is an Independent Risk Factor for Recurrence After Bone Union of Pediatric Lumbar Spondylolysis at L5: A Retrospective Case-control Study.

STUDY DESIGN: Retrospective case-control study. OBJECTIVE: To determine risk factors for recurrence of pediatric lumbar spondylolysis at L5 after return to sport in patients who achieved bone union with conservative treatment. SUMMARY OF BACKGROUND DATA: Pediatric lumbar spondylolysis is a stress fracture commonly observed in adolescent athletes, particularly at the L5 vertebra. Because some patients experience a recurrence of spondylolysis after bone fusion with conservative treatment, identifying risk factors for recurrence may help athletes continue sports activities, maintain performance

Evaluation of MRI technologies for surgical spine planning and navigation.

PURPOSE: CT is considered the gold standard to assess bone morphology, whereas MRI is the imaging modality of choice to assess soft tissue. To reduce the ionising radiation exposure of the patient from CT, new MRI technologies have been developed to obtain images of bone. Two promising methods include MRI-based synthetic CT (sCT) and T1-weighted isotropic MRI. METHODS: A prospective study including twenty-four adult patients receiving lumbar or sacral spine stabilisation surgery was performed. For each patient, two scans were acquired: a 3D dual echo T1-weighted gradient image, from which a s

ZFHX3-associated neural tube defect.

The zinc finger homeobox 3 (ZFHX3) gene encodes a transcription factor involved in neurodevelopment and organogenesis. ZFHX3 haploinsufficiency is linked to intellectual disability, epilepsy and neurodevelopmental defects, but its potential involvement in neural tube defects (NTDs) and related structural anomalies is unknown. We report a female proband with a de novo heterozygous ZFHX3 variant (NM_006885.3: c.5876 A>C, p.(Gln1959Pro), identified via exome sequencing. She presented with spina bifida occulta, segmental spinal dysgenesis, bilateral clubfeet, bicuspid aortic valve and genital anom
