Anterior cervical decompression and fusion for Samartzis Type I Klippel-Feil syndrome complicated by cervical radiculopathy with radiographic spinal cord compression: a case report of sandwich-type interposed mobile segment.

BACKGROUND: Klippel-Feil syndrome (KFS) is a rare congenital cervical malformation caused by embryonic segmentation defects leading to vertebral fusion, which alters cervical biomechanics and predisposes to premature spinal degeneration. The "sandwich-type interposed mobile segment" refers to a solitary mobile cervical segment trapped between congenital bony fusion and iatrogenic surgical fusion. It bears concentrated mechanical stress and carries a high risk of early adjacent segment degeneration (ASD). Current epidemiological data for postoperative ASD are mostly derived from general spinal

Deciphering the Genetic Basis of Congenital Vertebral Malformations Through a Stepwise Diagnostic Approach.

Congenital vertebral malformations (CVMs), affecting approximately 0.5-1 per 1000 live births, occur either in an isolated form or as part of syndromic disorders. Despite the identification of numerous causative genes for CVMs, the molecular etiology of most cases remains unknown. In this study, we applied a three-tiered diagnostic approach (chromosomal microarray analysis, followed by custom gene panel analysis, and exome/genome sequencing) in a cohort of 34 patients with CVMs. We achieved a 12% diagnostic success rate, identifying a deletion upstream of SOX9 and pathogenic or likely pathogen

Bilateral Sprengel's Deformity Treated with Bilateral Modified Woodward Procedure: A Case Report.

INTRODUCTION: Sprengel's deformity (SD) is a rare congenital scapular deformity, characterized by a small, elevated, or malpositioned scapula. Although it is rare, it is considered one of the most common congenital abnormalities of the shoulder in children. The scapula will be highly raised triangularly shaped, anteriorly and medially rotated, with limited scapulothoracic motion. As a result, patients will present with functional impairment, such as limited abduction and forward flexion, and cosmetic concerns. Various surgical approaches aiming for correction of SD have been established, inclu

Robot- and navigation-assisted cervical fusion and decompression for complex vertebral artery anomalies in os odontoideum with Klippel-Feil syndrome: illustrative case.

BACKGROUND: Vertebral artery (VA) anomalies, particularly those associated with congenital atlantoaxial dislocations, are well documented in the literature. Typically, these anomalies are linked to persistent first intersegmental artery variants or occur in conjunction with occipitalization of the atlas. The case presented in this report is unique due to the presence of bilateral ectatic VAs with an anomalous course beneath the posterior arch of C1, in the absence of occipitalization of the atlas. This finding adds to the growing recognition of anatomical variability in patients with congenita

Anaesthesia for Caesarean Section in a Parturient with Klippel-Feil Syndrome: A Case Report.

Klippel-Feil syndrome (KFS) is a rare congenital condition defined by the fusion of one or more cervical vertebrae, often associated with a range of skeletal and extra-skeletal abnormalities. The presence of cervical vertebral fusion and spinal deformities can make both airway management and neuraxial anaesthesia technically challenging in this population. This report describes a primigravida with KFS who underwent an elective caesarean section under spinal anaesthesia. The preparation included consideration of a potentially difficult airway, even though general anaesthesia had not initially b

A Unique Presentation of Bilateral Sprengel's Deformity with a Horseshoe Configuration of Connected Omovertebral Bony Bars.

Background: Sprengel's deformity is the most common congenital anomaly of the scapula and is frequently associated with the presence of omovertebral bars and cervical spine anomalies such as Klippel-Feil syndrome. Multiple surgical techniques have been described aiming to correct Sprengel's deformities, with the Modified Woodward procedure being among the most widely utilized techniques. Bilateral Sprengel's deformities are relatively rare; when present, surgical correction is usually performed in a staged manner. Case presentation: We report a rare case of a three-year-old female with bilater

Every Emergency Department Should Stock Two Types of Supraglottic Airways: Laryngeal Tube Rescue After Failure of a Laryngeal Mask Airway: A Two Case Series.

BACKGROUND: Supraglottic airways (SGAs) are critical rescue devices in emergency airway management and provide effective oxygenation in most failed intubations. Current airway algorithms recommend availability of a supraglottic airway but do not distinguish between device classes. Laryngeal mask-style SGAs create a perilaryngeal seal over the glottic inlet, whereas laryngeal tube-style devices create a hypopharyngeal seal with dual cuffs. Because these devices seal in different anatomic locations, they may fail for different reasons. CASE REPORTS: We report two emergency department cases in w

A Rare Coexistence of Arteria Lusoria, Truncus Bicaroticus, and Klippel-Feil Syndrome: A Five-Year Follow-Up and Procedural Considerations.

Arteria lusoria, an aberrant right subclavian artery (ARSA) arising as the most distal branch of the aortic arch, is the most common congenital aortic arch anomaly, yet it is clinically silent in over 90% of individuals and is usually discovered incidentally. We report the incidental identification of ARSA together with a truncus bicaroticus and Klippel-Feil syndrome (KFS) in a 32-year-old man investigated for intermittent pretracheal pressure. Contrast-enhanced MRI of the neck demonstrated a retro-oesophageal ARSA with mild oesophageal compression and revealed congenital fusion of the C2-C3 a

Secondary Chiari-like formation caused by grafted bone overgrowth following occipitocervical fixation: a rare cause of pediatric quadriplegia.

Secondary Chiari-like formation (SCLF) is an acquired herniation of the cerebellar tonsils through the foramen magnum, typically resulting from a craniospinal pressure gradient or reduced posterior fossa volume. We report a rare case of SCLF caused by massive bone overgrowth after occipitocervical fixation (OCF) in a child. An 8-year-old boy with Klippel-Feil syndrome and basilar invagination developed quadriparesis after OCF. His symptoms worsened following repeat OCF performed due to misdiagnosis. Four months later, the quadriparesis progressed further. Serial imaging demonstrated foramen ma

Does congenital cervical fusion predispose to adjacent segment degeneration? A retrospective cohort study.

BACKGROUND: Congenital cervical fusion is associated with adjacent segment degeneration (ASD). The debate continues over whether ASD is a result of cervical fusion or a natural process of cervical aging. OBJECTIVE: This study aims to determine if congenital cervical fusion is associated with an increased risk of ASD. METHODS: This research utilized a retrospective cohort study with a 1:2 case-control design to investigate 70 individuals diagnosed with Type I Klippel-Feil Syndrome (KFS) fused at C5-6. These subjects were compared against a control group comprising 140 age- and gender-matched in

Klippel-Feil Syndrome: A Rare Case Report.

Klippel-Feil syndrome (KFS) is a rare, complex syndrome characterized by abnormal fusion of cervical vertebrae. This case highlights the effectiveness of conservative management in KFS patients without neurological deficits. We reported the case of an 18-year-old Iraqi male presenting a short neck, low back hairline, winged right scapula, and scoliosis after physical examination and deformities of cervical spine fusion, kyphoscoliosis and Sprengel's deformity after imaging tests. Clinical and radiological diagnosis shows KFS. After conservative treatment-including a corrective collar, cervical

A case of Klippel-Feil syndrome with adult-onset neurological symptoms.

Klippel-Feil syndrome is a rare congenital anomaly characterized by fusion of cervical vertebrae. We describe the case of a 45-year-old female presenting with nonspecific neurological symptoms which was discovered to be due to undiagnosed Klippel-Feil syndrome. This case is unique due to presentation in adulthood as this condition is usually diagnosed in childhood due to visual asymmetries. Additionally, we highlight the value of multimodal imaging and diagnostics when attempting to delineate overlapping pathologies related to the cervical spine.

Recurrent left internal carotid artery stenosis in a patient with intrathoracic carotid bifurcation and right aortic arch.

Intrathoracic carotid bifurcation (ITCB) is a rare vascular anomaly often associated with Klippel-Feil syndrome (KFS); however, its coexistence with tetralogy of Fallot (TOF) has not been previously reported. A male patient in his 30s with childhood TOF repair presented with mild right-sided hemiparesis and motor aphasia due to an ischemic infarction. Cerebral angiography revealed ITCB at the T2 level with severe left internal carotid artery stenosis, a right aortic arch, and aberrant left subclavian artery. Percutaneous transluminal angioplasty initially resolved the stenosis, but progressive

Anatomical Variants of the Cervical Spine and Sports-Related Injury in Athletes: A Narrative Review.

Congenital and developmental cervical spine variants, as well as acquired or sport-adapted cervical morphologies, may alter canal reserve, segmental stability, foraminal dimensions, and the cervicothoracic outlet in athletes. This narrative review aimed to integrate the anatomically and methodologically heterogeneous literature linking these findings with sports-related injury and return-to-play decisions. PubMed/MEDLINE was searched from database inception through 6 July 2026 using controlled vocabulary and free-text terms for cervical anatomy, specific variants, athletic exposure, neurologic

Revision alloplastic temporomandibular joint reconstruction for re-advancement in syndromic ankylosis: a case report involving simultaneous Klippel-Feil syndrome and Pierre Robin sequence.

PURPOSE: Temporomandibular joint (TMJ) ankylosis impairs jaw function, limiting mouth opening, speech, and mastication, and rendering airway access high-risk. In patients with syndromic conditions such as Pierre Robin Sequence (PRS) and Klippel-Feil Syndrome (KFS), ankylosis presents additional challenges due to airway compromise, restricted mandibular growth, and complex skeletal abnormalities. In addition, predicting the ability to advance the mandible using a prosthesis may be challenging due to anatomic factors. METHODS: We report the case of an 18-year-old female with TMJ ankylosis, PRS,
