Hidden Hollows of the Fetal Cranium-Lacunar Skull in Isolation.

INTRODUCTION: Lacunar skull, also known as Lückenschädel or craniolacunae, is a rare congenital calvarial ossification abnormality characterized by multiple lucent defects of the skull. It is most associated with neural tube defects, Chiari II malformation, craniosynostosis, and other congenital anomalies. Isolated lacunar skull, without associated structural abnormalities, has been infrequently reported, and its natural history, prognosis, and optimal clinical management are poorly defined. CASE PRESENTATION: We report the case of a female infant born at 38 weeks and 5 days' gestation to a 3

Olfactory Deficits in Fragile X Syndrome.

Fragile X syndrome (FXS) is the most common inherited form of intellectual disability and a major genetic cause of autism spectrum disorders (ASDs). It results from the loss of fragile X mental retardation protein (FMRP), an mRNA-binding protein critical for synaptic development and plasticity. Although sensory processing abnormalities are well recognized in FXS, the olfactory system remains relatively underexplored in both human and animal studies. Evidence from rodent and drosophila models reveals that FMRP loss profoundly alters olfactory circuitry and function. In Fmr1 knockout mice, aberr

Psilocybin improves novel object recognition in a rat model of Fragile X Syndrome through the modulation of the BDNF/TrkB signaling pathway.

Fragile X Syndrome (FXS) is the most common inherited intellectual disability and a leading monogenic cause of autism spectrum disorder (ASD). As a synaptic disorder, FXS involves the loss of Fragile X messenger ribonucleoprotein 1 (FMRP), leading to abnormal dendrite development and immature dendritic spines. Serotonergic signaling, essential for neuronal development and circuit remodeling, has been implicated in ASD and related conditions, including FXS, raising the possibility that serotonergic modulation could ameliorate neurodevelopmental impairments. This study investigated the therapeut

Neuraxial Anesthesia in a Parturient With Newly Diagnosed Klippel-Trenaunay Syndrome: A Case Report.

Klippel-Trenaunay syndrome (KTS) is a complex, congenital vascular disorder characterized by three primary manifestations: cutaneous capillary malformation (most commonly port-wine stain), congenital varicose veins, and soft tissue and bone hypertrophy. In the obstetric population, the evaluation of these vascular malformations is essential to mitigate bleeding risk with regional, neuraxial, and general anesthesia, particularly if there is the involvement of the spinal column or oropharyngeal airway. We report a unique case highlighting the successful administration of neuraxial anesthesia in

Radiological analysis of congenital spinal deformities in patients associated with anorectal malformations: a cross-sectional study of 86 patients.

OBJECTIVE: To propose a morphologic-phenotypic stratification and investigate the radiological characteristics for the patients with anorectal malformation (ARM) associated with congenital spinal deformities. METHODS: A retrospective review was conducted on ARM patients who had received treatment in the neonatal surgery department of our hospital from January 2008 to December 2022. A total of 86 patients diagnosed with spinal deformities were enrolled, including 49 males and 37 females. We classified the patients included into three groups according to the range vertebral anomalies involving:

Multiple midline brain anomalies in a CMV-exposed fetus with successful surgical management: a case report and literature review.

INTRODUCTION: Congenital arachnoid cysts (ACs) are cerebrospinal fluid collections resulting from abnormal splitting of the arachnoid membrane during embryogenesis. They may occur as isolated lesions or in association with other central nervous system (CNS) malformations. Congenital cytomegalovirus (cCMV) infection is a leading infectious cause of neurodevelopmental impairment and may contribute to complex CNS anomalies. PRESENTATION OF CASE: A term male infant was diagnosed prenatally at 30 weeks with midline cystic lesions, ventriculomegaly, and absent corpus callosum. Postnatal magnetic re

It's not about the dent: Anteroposterior insufficiency in pectus excavatum.

BACKGROUND: Pectus Excavatum (PEx) is an anterior chest wall deformity associated with sternal depression. Minimal invasive repair of PEx (MIRPE) and open pectus repair depend on anterior ribs to elevate the sternal defect. Many PEx patients, however, have inadequate anteroposterior (AP) distance to support a complete repair. The quantity and frequency of AP insufficiency (API) in a PEx surgical population is reported. METHODS: A retrospective analysis of 100 pectus excavatum (PEx) patients with a CT or MRI Haller Index >3.2 was performed. Demographics, Haller Index (HI), Correction Index (CI

Pseudo-osteoporosis on bone densitometry due to congenital spinal dysraphism.

A patient in his late adolescence was referred for evaluation of low and apparently declining spinal bone density on dual-energy X-ray absorptiometry (DXA). Review of the DXA images revealed normal bone mineral density at other body sites but exceedingly low lumbar spine Z-scores which appeared to get progressively lower over time. The DXA result was explained by congenital spinal dysraphism with absent posterior spinous elements, rather than true low bone mineral content. Recognition of this structural artefact prevented unnecessary interventions. The case underscores the importance of correl

The Baraitser-Winter Cerebrofrontofacial Syndrome Recurrent R196H Variant in Cytoplasmic β-Actin Impairs Its Cellular Polymerization and Stability.

Variants in cytoskeletal actin encoding genes are associated with a broad spectrum of disorders, called non-muscle actinopathies. Among them, the Baraitser-Winter cerebrofrontofacial syndrome (BWCFF) displays the most severe symptoms, such as intellectual disability and epilepsy. We found that the BWCFF-associated R196H mutation results in reduced proliferation and migration of patient-derived fibroblast cells, and the latter is likely related to decreased fibronectin expression. The mutation causes a 50% drop in filamentous (F-) actin content, which is correlated with an approximately fourfol

Thoracic Spine Fracture-Dislocation After Minor Trauma in a Neurologically Intact Patient With Congenital Absence of Posterior Spinal Elements: A Case Report.

Thoracic spine fracture-dislocations are typically the product of high-energy injuries, and their neurological sequelae are usually devastating. For a patient to experience such an injury from a minor trauma and preserve all neurological function is rare and is often credited to the paradoxical decompression effect that follows a traumatic fracture of the posterior elements of the spine. This is a case of a 31-year-old female patient who suffered a severe thoracic spine fracture-dislocation from a low-energy fall from a manually operated "merry-go-round" in a kids' park. Despite the apparent i

Intraneural Hemangioma: A Report of Two Cases.

Spinal hemangiomas are congenital vascular malformations. Most reports of spinal hemangiomas describe resection via traditional open approaches; however, advances in minimally invasive surgery have led to comparable results. We report on two cases, namely, one nerve root intraneural hemangioma and one nerve root extraforaminal hemangioma, both removed utilizing a minimally invasive technique. Case 1 is about a 61-year-old woman who presented with low back pain with left lower extremity radiculopathy refractory to maximum medical management. This was secondary to an enhancing mass at the right

Positional Hemodynamic Instability in a Patient With Pectus Excavatum Undergoing Lumbar Discectomy.

Pectus excavatum (PE) is the most common congenital chest wall deformity, accounting for 90% of cases, and may predispose patients to hemodynamic compromise due to cardiac compression. Such events during spinal surgery, however, are rarely reported. In the widely used genupectoral prone position for spine surgeries, the patient lies prone with the torso supported and the legs flexed, thereby reducing abdominal compression, improving venous drainage, and reducing bleeding. However, the support provided by the thorax in this position may cause significant mediastinal compression in the presence

ACAN-related disorder, antenatal presentation and phenotypic variability: a case series.

BACKGROUND: Pathogenic ACAN variants ('aggrecanopathies') are increasingly recognised as a non-syndromic cause of skeletal dysplasias and short stature. Unlike many other aetiologies, ACAN-related disorder is reportedly associated with advanced bone age due to early epiphysial fusion, although presentation can vary. This study aimed to further characterise skeletal findings in paediatric and adult patients. METHODS: A retrospective study of confirmed ACAN-related disorder cases from a tertiary genetics centre in England was performed. Data collected included patient demographics, antenatal ul

Outcomes of Spinal Deformity Treatment in Asphyxiating Thoracic Dystrophy.

BACKGROUND: Asphyxiating thoracic dystrophy (ATD), or Jeune syndrome, is a rare skeletal dysplasia marked by a severe hypoplastic chest wall and restrictive lung disease, leading to an often-lethal thoracic insufficiency syndrome. Early-onset scoliosis (EOS) can complicate ATD, but details of operative management remain limited. This study aims to analyze the peri- and postoperative course of children with ATD who underwent posterior spinal fusion (PSF). METHODS: A retrospective review was conducted for all patients with ATD treated for EOS who underwent PSF at our institution. Preoperative s

Choroid plexus cyst of the spinal cord presenting with signs of spinal cord compression in a 55-year-old male: a case report.

BACKGROUND: Choroid plexus cysts are benign, typically asymptomatic cystic lesions found almost exclusively in the brain's lateral ventricles, often noted during second-trimester fetal ultrasounds and usually resolving spontaneously before the 28th week of gestation. Spinal intramedullary choroid plexus cysts remain extremely rare, with only few cases documented in  the literature to date. This case describes a 55-year-old man with thoracic spinal cord compression due to a choroid plexus cyst, highlighting a rare differential diagnosis for adult spinal cord lesions. CASE PRESENTATION: A 55-ye

Co-Occurring Non-Cardiac Congenital Anomalies Among Cases With Congenital Heart Defects.

Cases with congenital heart defects (CHD) often have other associated anomalies. The aim of this investigation was to assess the prevalence and the types of co-occurring anomalies in CHD in a well-defined population. The anomalies co-occurring with CHD were ascertained in all live births, stillbirths and terminations of pregnancy for fetal anomaly during 29 years in 387,067 consecutive pregnancies of known outcome in the area covered by our population-based registry of congenital anomalies. Of the 4640 cases with CHD ascertained during this period (prevalence of 119.9 per 10,000), 1228 (26.5%)

Management of two variants of the OEIS complex (Omphalocele, Exstrophy, Imperforate anus, Spinal defects) : case reports and literature review".

BACKGROUND: There is a wide spectrum of anorectal malformations (ARMs). The majority of ARM types can be diagnosed thorough perineal examination and radiological investigations such as MRI and distal loopogram. The most important aim of ARM repair is keeping the rectum in the muscle complex to protect continence, but still there are important factors that influence continence such as spine anomalies and associated urological anomalies. CASE PRESENTATION: In our report we describe two patients with ARM and abnormal anal sphincteric locations. The first case was a two-year-old female patient who
