Venous thoracic outlet syndrome in the presence of cervical rib: report of two cases.

Thoracic outlet syndrome is a rare condition caused by the compression of neurovascular structures in the region between the neck and chest. The venous form accounts for 2% to 3% of cases, with deep vein thrombosis most often caused by compression of the subclavian-axillary vein segment between the first rib and the clavicle. The occurrence of a cervical rib as a cause of venous deep vein thrombosis is even rarer. In this report, we present the diagnosis, treatment, and outcome of 2 cases of venous deep vein thrombosis resulting from 2 different mechanisms. The approach was based on a review o

Acute upper limb ischemia due to cervical rib-related thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) results from the compression of neurovascular structures at the thoracic outlet. The clinical presentation of arterial compression ranges from chronic ischemic symptoms, such as upper limb pallor, coldness, and exercise-induced claudication, to acute vascular complications including arterial thrombosis, which may progress to limb-threatening ischemia. We report the case of a 33-year-old patient with no significant medical history who presented with acute ischemia of the left upper limb. Radiological investigations revealed TOS with a cervical rib as the underlyin

Ultrasound-Guided Hydrodissection for Neurogenic Thoracic Outlet Syndrome Caused by a Cervical Rib: A Case Report.

Thoracic outlet syndrome (TOS) is an underdiagnosed condition that is frequently misattributed to cervical spine pathology. Neurogenic TOS (nTOS), the most common subtype, often presents nonspecific upper extremity pain and paresthesia, making diagnosis challenging. We report a case of nTOS associated with a cervical rib that was successfully treated with ultrasound-guided hydrodissection (HD). Dynamic ultrasound examination demonstrated brachial plexus tethering with symptom reproduction. Serial HD resulted in immediate and sustained symptom relief, suggesting that this minimally invasive tec

An original less-invasive surgical treatment for arterial thoracic outlet syndrome caused by cervical rib.

BACKGROUND: Thoracic outlet syndrome affects 0.3-8% of the population, with arterial thoracic outlet syndrome being particularly rare. The management of arterial thoracic outlet syndrome requires complete thoracic outlet decompression along with arterial reconstruction when necessary, a procedure that is typically associated with significant risks, complications, and symptom recurrence. Our study aims to introduce a less invasive surgical method for treating arterial thoracic outlet syndrome caused by a cervical rib, which involves only cervical rib resection and scalenotomy. METHODS: Patients

Operative management of vascular thoracic outlet syndrome (VTOS) over two decades: patency, complications, and functional outcome.

BACKGROUND: Vascular thoracic outlet syndrome (VTOS), encompassing both venous and arterial subtypes, arises from extrinsic compression of the subclavian vessels within the thoracic outlet, leading to significant morbidity. Surgical decompression remains the definitive treatment to restore vascular patency and relieve neurovascular compromise. OBJECTIVES: This study analyzed the long-term outcomes of surgical management of VTOS over a twenty-year period, focusing on vessel patency, recurrence, functional recovery, and procedure-related complications. METHODS: A retrospective review was conduct

Acute Upper Limb Ischemia as a Presentation of Arterial Thoracic Outlet Syndrome in a Young Patient: A Case Report.

Thoracic outlet syndrome (TOS) refers to a group of conditions caused by extrinsic compression of neurovascular structures at the thoracic outlet. Arterial TOS (aTOS) is the rarest form and may present with upper limb ischemic symptoms such as pain, coldness, pallor, and numbness, most often due to bony abnormalities such as a cervical rib.  We report the case of a 19-year-old woman on combined oral contraceptive therapy who presented with acute ischemia of the right upper limb. Duplex ultrasound and computed tomography angiography revealed thrombosis of the right subclavian artery in the sett

From compression to denervation: Multimodality imaging of cervical rib-related arterial thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) encompasses a spectrum of disorders caused by compression of neurovascular structures at the thoracic outlet. Arterial thoracic outlet syndrome (aTOS) is rare, accounting for less than 1% of cases, yet carries significant morbidity due to arterial injury, thrombosis, and distal embolization. We present a 34-year-old woman with progressive pain, paresthesia, and bluish discoloration of the left upper limb. Doppler ultrasound demonstrated reduced ulnar artery calibre with a tardus-parvus waveform, indicating distal arterial compromise. CT angiography revealed a fus

Blunt trauma precipitating arterial thoracic outlet syndrome: a case report.

A 39-year-old male who sustained polytraumatic injuries in a motor vehicle collision, including a neck seatbelt sign and acute thrombosis of the left subclavian artery adjacent to an undiagnosed hypoplastic cervical rib, presented with a threatened left upper extremity. He underwent emergent surgical exploration through a supraclavicular incision, with cervical rib resection, resection of the contused subclavian artery segment, and end-to-end anastomosis. Through an infraclavicular incision, he had a thrombectomy of the axillary and brachial arteries. Postoperatively, he developed progressive

Imaging Spectrum of Thoracic Outlet Syndrome: A Case Series Highlighting Arterial, Venous, and Neurogenic Subtypes.

INTRODUCTION: Thoracic outlet syndrome (TOS) encompasses a spectrum of neurovascular compression disorders involving the brachial plexus, subclavian artery, and subclavian vein. Diagnosis remains challenging due to variable clinical presentations and limited specificity of imaging findings, particularly in neurogenic TOS. This case series aims to demonstrate the multimodality imaging spectrum of TOS across arterial, venous, and neurogenic subtypes, compare imaging appearances among subtypes, and emphasise the importance of clinicoradiological correlation in diagnosis. METHODS: This retrospect

Neurogenic thoracic outlet syndrome secondary to an unilateral cervical rib in a poodle mix.

A 2-year-old spayed female poodle mix was evaluated for persistent intermittent right thoracic limb lameness and pain refractory to oral analgesic and anti-inflammatory medications. Orthopedic and neurologic physical examinations localized discomfort to the right shoulder and cervical region. Orthogonal right shoulder radiographs did not detect any abnormalities. Magnetic resonance imaging of the right brachial plexus revealed an anomalous bone structure at the level of the right thoracic inlet. Orthogonal cervical radiographs confirmed this finding. Surgical exploration of the right thoracic

Bilateral cervical ribs forming pseudoarthrosis with the first ribs co-occurring with an aberrant right subclavian artery.

While anatomical variations in both aortic arch branching and rib cage development are considered relatively common, compared to various other anomalies, their coexistence seems to be underreported. We present a case report of a 70-year-old Polish woman with bilateral complete cervical ribs forming a pseudoarthrosis with the superior surface of the first ribs, and an aberrant right subclavian artery (arteria lusoria) with a retroesophageal course. We believe that the frequency of their co-occurrence may be higher than would be expected based on statistical probability derived from their preval

Anatomical Variants of the Cervical Spine and Sports-Related Injury in Athletes: A Narrative Review.

Congenital and developmental cervical spine variants, as well as acquired or sport-adapted cervical morphologies, may alter canal reserve, segmental stability, foraminal dimensions, and the cervicothoracic outlet in athletes. This narrative review aimed to integrate the anatomically and methodologically heterogeneous literature linking these findings with sports-related injury and return-to-play decisions. PubMed/MEDLINE was searched from database inception through 6 July 2026 using controlled vocabulary and free-text terms for cervical anatomy, specific variants, athletic exposure, neurologic

Imaging of cervicothoracic junction anatomical variation in neurogenic thoracic outlet syndrome: A scoping review protocol.

OBJECTIVE: The objective of this scoping review is to systematically map the extent and nature of the literature on the use of imaging for assessing cervicothoracic junction anatomical variation in patients with neurogenic thoracic outlet syndrome. INTRODUCTION: Neurogenic thoracic outlet syndrome is a complex, disputed diagnosis that lacks a definitive gold-standard test, despite being the most common thoracic outlet syndrome subtype. Imaging is essential for identifying anatomical variations, such as cervical ribs, and excluding competing diagnoses. This review aims to clarify how imaging m

Magnetic Resonance Neurography Detection of the Wedge-Sickle Sign Fibromuscular Bands in Neurogenic Thoracic Outlet Syndrome: A Case Report.

CASE: A 17-year-old girl presented with chronic right arm numbness, cramping, burning pain, and weakness. Initial nerve studies showed slowing in the median and ulnar nerves, but an outside magnetic resonance imaging was negative. Repeat magnetic resonance neurography (MRN) revealed right C8 nerve root and lower trunk enlargement, compressed by a fibrous band from a small cervical rib. The lower trunk appeared indented in a "sickle" shape-termed the wedge-sickle sign. Surgery confirmed the MRN findings. CONCLUSION: MRN can depict the wedge-sickle sign previously seen only on ultrasound, poten

High-definition point-of-care ultrasound in neurogenic thoracic outlet syndrome: An evidence-informed clinical perspective.

Neurogenic thoracic outlet syndrome (nTOS) remains diagnostically challenging because symptoms are common, objective neurological signs are often absent, and conventional investigations may be normal or non-specific. Contemporary diagnostic frameworks remain predominantly clinical, leaving clinicians to infer the anatomical site and mechanism of compression before committing patients to injection, rehabilitation, or decompressive surgery. This opinion article argues that high-frequency point-of-care ultrasound (HF-POCUS) should be incorporated into specialist nTOS assessment as an adjunct to,
