Gait and posturographic parameters of children with achondroplasia - a long-term pilot study.

Walking and maintaining balance are essential for participating in daily life. For children with achondroplasia, reaching these milestones is with a delay and during growth, challenges persist for those skills. However, the impact of developmental changes on gait and balance in this population has not yet been investigated longitudinally. Therefore, this study aimed to examine the influence of growth on gait pattern and postural control in children with achondroplasia over a two-year period. 6 children with achondroplasia (whereas 5 are treated with vosoritide, a growth promoting peptide) and

Once-Weekly Navepegritide in Children With Achondroplasia: The APPROACH Randomized Clinical Trial.

IMPORTANCE: Historically considered a skeletal dysplasia characterized by disproportionate short stature, achondroplasia is a condition with multisystemic effects due to the widespread expression of the fibroblast growth factor receptor 3 variant throughout the body, impacting muscle, neurological function, cardiorespiratory health, and health-related quality of life. OBJECTIVE: To evaluate the efficacy, safety, and tolerability of once-weekly navepegritide, an investigational prodrug of C-type natriuretic peptide, while assessing benefits beyond growth that may have important implications fo

Spatiotemporal Regulation and Lineage Specification in Embryonic Endochondral Ossification.

Long bone formation in vertebrates proceeds via endochondral ossification, a sequential process that begins with mesenchymal condensation, advances through cartilage anlage formation, and culminates in its replacement by mineralized bone. Recent advances in inducible lineage tracing and single-cell genomics have revealed that, rather than being a uniform event, mesenchymal condensation rapidly segregates into progenitor pools with distinct fates. Centrally located Sox9+/Fgfr3+ chondroprogenitors expand into the growth plate and metaphyseal stroma, peripheral Hes1+ boundary cells refine condens

Development of Muscle Function in Children with Achondroplasia Under Vosoritide Treatment: A Retrospective Single-Centre Observational Study.

OBJECTIVES: Vosoritide, a C-type natriuretic peptide analog, is the first approved pharmacological treatment for achondroplasia that targets the overactive FGFR3 pathway and promotes longitudinal bone growth. As increased body height may improve biomechanical leverage and influence muscle function, this retrospective, monocentric, observational study aimed to evaluate changes in muscle performance parameters during long-term vosoritide treatment. METHODS: Nineteen (19) children with achondroplasia treated with vosoritide for at least 12 months were included. Muscle function was assessed via t

Genetic Bone Diseases: A Scoping Review of Pathology, Symptoms, Diagnosis, Treatment, and New Horizons.

Genetic bone diseases are a rare group of afflictions suffered by the general population. However, their rarity should not diminish research efforts to help patients understand and treat their diseases. This review summarizes the pathology, symptoms, diagnosis, and treatment insight into six well-known genetic bone diseases. Only six bone diseases are included due to the relatively low prevalence of them as whole limiting our scope to ensure accurate information and attention is provided for each disease individually. A literature search of PubMed is conducted, including studies published with

UK consensus guidelines for multidisciplinary care of children and young people with achondroplasia: a modified Delphi process.

BACKGROUND: Achondroplasia (ACH), the most common skeletal dysplasia, arises from gain-of-function variants in the fibroblast growth factor receptor 3 gene. Children with ACH experience lifelong medical, functional and psychosocial challenges requiring coordinated and anticipatory care. Although international guidance exists, the UK lacks national clinical care recommendations specific to its healthcare systems. OBJECTIVE: To develop UK-specific, multidisciplinary clinical recommendations for the care of children and young people (CYP) with ACH. METHODS: The UK Achondroplasia Network develop

Excess FGFR3 signaling in achondroplasia disrupts turnover of resting zone chondrocytes via CREB signaling.

Achondroplasia, associated with gain-of-function mutations in FGFR3, causes growth plate cartilage dysfunction, resulting in short-limb dwarfism. However, its precise molecular and cellular mechanisms remain unclear. To address this, we aimed to generate knock-in mice (Fgfr3Ach) harboring the achondroplasia mutation (p.Gly380Arg). In addition to previously reported abnormalities, we observe an expansion of the resting zone. EdU labeling and lineage tracing analyses indicate that disruption of turnover and impairment of stem cell-like behavior of resting zone chondrocytes results in accumulatio

Pathways to Facilitate Early Recognition and Diagnosis of Hypochondroplasia.

INTRODUCTION: Hypochondroplasia (HCH) is a disproportionate short-statured skeletal dysplasia condition caused by gain-of-function pathogenic variants in the fibroblast growth receptor 3 gene (FGFR3). Although HCH typically becomes clinically apparent after the first year of life, when height discrepancy compared with the general population becomes more pronounced, diagnosis is often delayed by several years. Early recognition of HCH is challenging because of wide phenotypic variability and subtle clinical and radiographic features, leading to delayed or missed diagnosis. Furthermore, wide var

Hip Dislocation With Femoral Short Neck in Spondyloepiphyseal Dysplasia Treated by Open Reduction and Valgus Osteotomy: A Case Report.

Spondyloepiphyseal dysplasia congenita (SEDC) often involves hip deformity and dislocation. We present a case of an eight-year-old boy with bilateral femoral head deformity and hip dislocation treated by open reduction and femoral valgus osteotomy. Surgery was performed via an anterior approach. After removal of obstructive factors, reduction was only maintained at 30° adduction, necessitating valgus osteotomy to stabilize the hip. Postoperative fixation was achieved with hip spica casting. This case suggests that femoral valgus osteotomy may be essential to maintain reduction in SEDC-associat

[Key points of the International consensus guidelines on the implementation and monitoring of vosoritide therapy in individuals with Achondroplasia].

Achondroplasia (ACH) is a common inherited skeletal dysplasia (inherited dwarfism) that compromises quality of life across the lifespan. In 2021, vosoritide became the first approved precision therapy for ACH and is now available in more than 40 countries. Compared with prior symptomatic measures, vosoritide has demonstrated favorable efficacy and a reassuring safety profile. Nevertheless, existing international ACH guidelines largely emphasize complication management and symptomatic care, and there is no unified consensus on pharmacologic therapy. To address this gap, an international expert

Development of a Step-By-Step Clinical Decision Tool for Difficult Spinal Anesthetics: A Case Report of a Pregnant Patient With Achondroplasia, Previous Lumbar Fusion, and Class III Obesity.

Achondroplasia is the most common bone dysplasia. Many patients have concomitant challenging neuraxial and airway anatomies. We describe a case of a G1P0 who presented for cesarean delivery due to achondroplasia, with comorbidities of Class III obesity and previous spinal fusion of unknown level. No preoperative anesthesia consultation had been requested, nor was previous neuraxial imaging available. After several failed spinal attempts with a 25-G Whitacre needle, a 22-G spinal needle was used with success. This case catalyzed our development of a clinical decision tool to algorithmically tro

Somatic Mosaicism Modulating Clinical Severity in COL2A1-Related Skeletal Dysplasia: A Case of Profound Skeletal Asymmetry.

Type II collagenopathies represent a broad spectrum of skeletal dysplasias caused by pathogenic variants in the COL2A1 gene, ranging from perinatally lethal achondrogenesis type II to milder phenotypes such as spondyloepiphyseal dysplasia congenita and Stickler syndrome. Although specific mutations may suggest certain phenotypic trends, the genotype-phenotype relationship is not fully clarified. We describe a patient with marked skeletal asymmetry due to somatic mosaicism for a pathogenic COL2A1 variant (c.G1537A; p.G513S) previously associated with lethal phenotypes. The mutation, identified
