Thanatophoric dwarfism; drugs and antenatal diagnosis; a case report.

Thanatophoric dwarfism is not as rare as once thought. We present this case because of the possible association with drug administration and the probability of diagnosing this condition antenatally.

Cerebral abnormalities in thanatophoric dysplasia.

Neuropathologic evaluation of two infants with thanatophoric dysplasia displayed typical gross morphologic characteristics and a distinct pattern of brain malformations, including anomalies of the temporal lobe gyri and hippocampus, neuroglial heterotopias, fiber tract hypoplasia, and dysplasia of deep nuclei. Increased numbers of horizontal cells of Cajal-Retzius were striking in frequency and distribution. The pattern of abnormalities suggests arrest of cerebral cortical ontogeny late in development. As with the mucopolysaccharidoses, a shared common metabolic pathway is a potential mechanis

Histopathology of the temporal bones in thanatophoric dysplasia.

The pathological findings of temporal bone in two cases of thanatophoric dysplasia were reported. Thanatophoric dysplasia is classified into type 1 and type 2, each of which has been reported to show specific clinical and radiographic-findings. The present study revealed that each type also showed specific characteristic in the structure of the temporal bones. The developmental mechanisms of hearing impairment in this disease were also discussed on the basis of the pertinent literature.

Variation of quantitative and qualitative changes of enchondral ossification in heterozygous achondroplasia.

Whereas the radiologic features of achondroplasia--the most common type of skeletal dysplasia in adults--are clearly defined there is still some debate about the severity and the type of histologic changes. Earlier descriptions reported severe disturbance of enchondral ossification to be typical of achondroplasia: They are, however, misleading by dealing mostly with cases of lethal neonatal dwarfism (e.g. thanatophoric dysplasia). Newer findings confirmed that only minor, quantitative lesions are typical of heterozygous achondroplasia. But even in recent years some observers noted more severe

Prenatal sonographic diagnosis of thanatophoric dwarfism.

A case of thanatophoric dwarfism is reported in a 35-year-old woman. The diagnosis was made at 34 weeks gestation by ultrasound. Marked shortening of the extremities, macrocephaly, narrow thorax and polyhydramnios were observed. The importance of routine ultrasound screening in pregnancy is emphasized, since such malformations can be detected prenatally.

Holoprosencephaly-polydactyly ('pseudotrisomy 13') syndrome: a syndrome with features of hydrolethalus and Smith-Lemli-Opitz syndromes. A collaborative multicentre study.

A syndrome of holoprosencephaly and postaxial polydactyly, associated with hydrocephalus, heart defect, adrenal hypoplasia, and other visceral malformations, has been observed in five unrelated children with normal chromosomes. Clinical overlap with lethal acrodysgenital dwarfism (Smith-Lemli-Opitz syndrome type II) and hydrolethalus syndrome is discussed. Recessive inheritance seems likely.

A chromosome 17q de novo paracentric inversion in a patient with campomelic dysplasia; case report and etiologic hypothesis.

The campomelic syndrome is a skeletal dysplasia with a characteristic pattern of deformity involving the proximal and distal extremities, pelvic and shoulder girdles, thoracic cage and palate. Respiratory compromise often leads to death in early infancy. Etiology has not been determined although evidence suggests genetic heterogeneity in patients with campomelia. Cytogenetic analysis in the past have revealed an unexpectedly high incidence of a 46, XY karyotype in phenotypic females. We report here on a patient with a typical case of campomelic dysplasia in whom a de novo paracentric inversion

Atelosteogenesis I and boomerang dysplasia: a question of nosology.

We report a patient whose clinical, radiologic and histopathologic findings are compatible with severe atelosteogenesis (AT-I). The patient is compared with previously reported cases of AT-I, as well as with patients reported as having "boomerang" dysplasia. We conclude that it is reasonable to consider AT-I and boomerang dysplasia as part of a spectrum, probably reflecting a common etiology. More and detailed clinical, radiologic and histopathologic reports are needed to further clarify the relationship of AT-II and AT-III in this family of skeletal dysplasias.

[Prevalence of bone dysplasias in newborns at the Ruíz y Páez Hospital in Bolívar City. Venezuela. 1978-1990].

A congenital malformations surveillance program in effect at the Ruiz y Páez Hospital in Ciudad Bolívar since April 1978, has allowed us to detect 25 patients with osteochondrodysplasias (OCD) in a total of 70,152 newborns, up to August 1990, for a prevalence of one case of OCD every 2,806 newborns. The clinical entities found were: Achondroplasia, Thanatophoric Dysplasia, Osteogenesis Imperfecta II-A, Camptomelic Dysplasia, Kniest Dysplasia, Conradi-Hunnerman syndrome, Parenti-Fracaro type Achondrogenesis and Jeune Asphyxiating Thoracic Dysplasia. The results presented in this paper indicate

[Fatal nanism: 3 different entities].

Three cases of congenital dwarfism are presented. All of them are lethal and represent the three better known nonviable nosologic entities: Achondrogenesis I, Achondrogesis II and Thanatophoric dwarfism. According to clinical features and radiologic data it is possible to approach the diagnosis accurately. We comment genetic, clinic, radiologic and histologic aspects of these processes. It is important to establish a differential diagnosis as these entities have different genetic basis, what influences genetic counsel.

Neural arch stenosis and spinal cord injury in thanatophoric dysplasia.

Bony abnormalities caused by thanatophoric dysplasia affect the base of the skull and the vertebrae as well as the ribs and appendicular long bones. We present our findings in a full-term infant with thanatophoric dysplasia in whom the posterior fossa, the rostral vertebral column, and the neuraxis at and adjoining the craniovertebral junction were studied by dissection, roentgenography, and histologic examination. In this infant, malformations of the vertebral laminae, most prominent in the basiocciput and atlas vertebra, led to compression of the rostral cervical spinal cord, causing gliosis

Simple autopsy method for analysis of complex fetal cranial malformations.

A new radiography technique for examining second trimester abortions permits an accurate analysis of growth and maturation of the fetal skull. Reference material for normal development has been previously published. The potential of the method is demonstrated with three examples: thanatophoric dysplasia, Arnold-Chiari malformation, and anencephaly.

Short-limb skeletal dysplasias: evaluation of the fetal spine with sonography and radiography.

Sonograms and radiographs of the lumbar spine in 51 fetuses and neonates with thanatophoric dwarfism, achondroplasia, and osteogenesis imperfecta type II were retrospectively evaluated. Study data included 27 prenatal and 27 neonatal sonograms and radiographs in 40 fetuses and neonates and 11 cases described in the radiology literature. To precisely compare the sonographic and radiographic appearances of platyspondyly in dysplasia, a simple vertebral ratio was obtained from measurement of vertebral interspace and vertebral body height. Normal ranges were obtained from sonograms in 125 normal f

Prenatal detection of simian crease.

We present the prenatal detection of a simian crease (transverse palmar line) in seven fetuses. Three fetuses had trisomy 21, four had normal chromosomes but another anomaly (meningomyelocele, thanatophoric dysplasia, achondroplasia [2]). The technique used and normal anatomy are described.

[Evidence of heterogeneity in dyssegmental dysplasia].

The dyssegmental dysplasia is a lethal form of neonatal short-limbed dwarfism in which unusual facies, short neck, narrow thorax, cleft palate, and reduced joint mobility are the characteristic commonly seen. Radiologically, vertebral segmentation defects and short, thick, bowed long bones are the prominent features. To date, 30 cases have been reported. Clinical, radiographic, and histologic examination of these cases of the literature demonstrates the presence of two distinct forms of dyssegmental dysplasia, the milder form (type Rolland-Desbuquois), characterized clinically by frequent surv
