Basilar invagination is a craniovertebral junction deformity characterized by the upward migration of the odontoid process toward the foramen magnum. This condition is frequently associated with congenital anomalies such as platybasia and atlas occipitalization. On the other hand, trichorhinophalangeal syndrome is a rare autosomal dominant skeletal dysplasia characterized by distinctive craniofacial features, brachydactyly, and abnormalities in endochondral ossification. This study describes the surgical treatment performed in a 39-year-old man with genetically confirmed trichorhinophalangeal

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

BACKGROUND: Florid cemento-osseous dysplasia (FLCOD) is a begin fibro-osseous lesion occurring in the tooth-bearing regions of the jaws and is characterized by replacement of normal bone with fibrous connective tissue and cementum-like mineralized material. Most patients are asymptomatic and are diagnosed incidentally during radiographic examinations, and the condition may be misdiagnosed because of overlapping imaging features with other jaw lesions. Because the disease is usually asymptomatic and exhibits imaging features that overlap with those of several odontogenic and inflammatory lesion

We respond to methodological questions raised in a "Letter to the Editor" regarding our retrospective case series on romosozumab treatment in osteogenesis imperfecta (OI). We clarify that the retrospective study design, the rarity of the disease, the small number of cases, and the exploratory nature of the analyses limit the methodological approaches as already acknowledged in our original manuscript. Thus, the primary concern is an inherent limitation of an exploratory study in a rare disease. Nevertheless, these data are helpful, as they generate new hypotheses given the currently very limit

PURPOSE: To describe sleep-disordered breathing in children with achondroplasia at their first polysomnography (PSG) assessment and examine associations with clinical characteristics.
METHODS: This retrospective study included 49 children evaluated between March 2016 and February 2025. Obstructive sleep apnea syndrome (OSAS) was classified using the obstructive apnea-hypopnea index (oAHI). Total AHI was compared according to sex, upper airway surgery history, foramen magnum stenosis, and vosoritide treatment. Infant findings were also presented separately because severity thresholds require c

Achondroplasia (ACH) is the most common non-lethal skeletal dysplasia and is caused in the vast majority of cases by a heterozygous gain-of-function variant in fibroblast growth factor receptor 3 (FGFR3, p.Gly380Arg). Hyperactive FGFR3 signaling constrains endochondral bone growth by suppressing growth-plate chondrocyte proliferation and hypertrophic differentiation. In the past decade, management has expanded beyond supportive care, and several mechanism-based interventions have shown disease-modifying effects in clinical studies. These include the activation of the NPR2/CNP axis with vosorit

Collagen X, encoded by the Col10a1 gene, is expressed specifically by hypertrophic chondrocytes (HCs) in the cartilage. Mutations of the Col10a1 gene in humans, which increase the endoplasmic reticulum (ER) stress in HCs, lead to metaphyseal chondrodysplasia type Schmid (MCDS). However, the mechanism of how HCs respond to ER stress to affect bone development and growth is not entirely understood. In this study, we generated a new mouse model harboring a 13 bp deletion (13del) in the endogenous Col10a1 gene. These 13del-MCDS mice display all the known clinical characteristics of human MCDS, as

Background McCune-Albright syndrome is a rare mosaic disorder caused by postzygotic activating mutations in GNAS , classically presenting with fibrous dysplasia (FD), hyperpigmented macules, and endocrinopathies. Craniofacial FD is common and typically follows a benign course. However, acute symptomatic changes with aggressive imaging features may mimic malignant transformation or secondary lesions such as aneurysmal bone cysts (ABCs), creating significant diagnostic uncertainty. Case Description We report a 20-year-old woman with known MAS and polyostotic FD who presented with a 4-week histor

Cartilage-hair hypoplasia (CHH) is a rare autosomal recessive skeletal dysplasia caused by variants in RMRP (RNA component of mitochondrial RNA processing endoribonuclease), which encodes a non-coding RNA. Despite its well-established clinical and molecular spectrum in some populations, data from Mexico remain limited. We report eight patients from seven unrelated families with CHH. Clinical and radiographic findings were assessed, and RMRP was analyzed by polymerase chain reaction and Sanger sequencing. All patients exhibited disproportionate short stature; prenatal long-bone shortening and s

Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Skeletal dysplasias are a diverse group of genetically inherited bone disorders which may manifest with or without short stature. Extreme short stature can also result from different growth hormone deficiencies. Our research aimed to identify the genetic causes of recessively inherited short stature, with or without skeletal abnormalities in Pakistani families. We recruited participants from thirty-eight pedigrees with a history of these disorders. Radiological examinations were performed for a few patients. Some samples were first screened for variants in GHRHR and RMRP by Sanger sequencing.

Neuroblastoma amplified sequence gene (NBAS) variants are associated with short stature, optic atrophy, and Pelger-Huët anomaly (SOPH) syndrome. We previously identified compound heterozygous variants in NBAS to cause atypical Osteogenesis Imperfecta (OI), with these patients presenting with short stature, developmental delay, and recurrent long-bone fractures. However, skeletal disease progression due to these variants and the disease mechanisms underlying NBAS-associated OI remain poorly understood. Here, we provide a clinical update on previously identified patients and investigate the role

Fibrous dysplasia (FD) is a rare benign bone disease caused by postzygotic activating mutations in the GNAS gene, leading to replacement of normal bone with fibrous tissue. It may be monostotic or polyostotic, often asymptomatic but sometimes associated with pain, deformity, or fractures. Management is mainly surgical in symptomatic or complicated cases. This retrospective study analyzes a large FD cohort to identify determinants of treatment and outcomes, with a focus on pain and its evolution. The primary aim was to assess factors influencing surgical indication, while the secondary aim was

BACKGROUND: Polyostotic fibrous dysplasia (POFD) is a mosaic skeletal disorder driven by somatic GNAS mutations that constitutively activate Gsα-cAMP signaling, producing fibro-osseous craniofacial deformities. POFD lesions are histologically classified into three subtypes-Type I (connective-tissue predominant), Type II (cancellous-bone predominant), and Type III (sclerotic-bone predominant)-yet the immune microenvironment across these categories remains undefined. This study aims to dissect lymphoid and myeloid heterogeneity and inter-cellular communication networks among distinct POFD histol

OBJECTIVES: Cemento-osseous dysplasia (COD) is the most common benign fibro-osseous lesion of the jaws, typically asymptomatic and identified through characteristic radiographic features. However, its rare association with simple bone cysts (SBCs) can significantly alter its presentation, mimicking more aggressive jaw lesions. This study aims to report additional cases of COD-associated SBCs and to review the literature to evaluate whether such lesions may represent a specific manifestation of the COD spectrum.
MATERIALS AND METHODS: Two cases of florid COD with atypical radiolucencies were d

Erythropoietin-producing hepatocellular (EPH) receptors, the largest receptor tyrosine kinase family, have been shown to play a crucial role in normal physiological processes and pathologies. EPHA2, encoded by EPHA2 gene, a member of the EPH receptor family, has been implicated in cataracts and various bone tumors, including osteosarcoma, chondrosarcoma, and bone metastases of breast cancer. However, no association between the EPHA2 gene and genetic skeletal disorders has been reported to date. Here, we identified a Chinese family with a diagnosis of dysosteosclerosis, presenting with signific

BACKGROUND: Surgical procedures that improve motion for slipped capital femoral epiphysis (SCFE) deformity are typically guided by radiographic imaging. However, the specific aspects of the deformity that have the greatest impact on hip functional biomechanics remain unclear. This study investigated the relationship between radiographic parameters and clinical ROM measurements in patients with SCFE to better elucidate which elements of SCFE pathoanatomy have the greatest effect on joint mobility.
METHODS: Patients who underwent in situ pinning for stable SCFE at a single institution between 2

AIMS: Comparative assessment of both hip joints in patients with slipped capital femoral epiphysis (SCFE) was performed relative to the control group.
METHODS: We created 3D CT reconstructions of the acetabulum in 27 hips with SCFE, 21 unaffected contralateral hips, and 108 hips of the age- and sex-matched non-SCFE control group. Following measurements performed using true axial CT scans were assessed: acetabular version angle (AVA°), posterior sector acetabular angle (PASA°), posterior wall angle (PWA°), and acetabular roofing percentage.
RESULTS: Based on the morphology of the posterior ac
