Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

Response to the Letter to the Editor on "Interpreting romosozumab-associated BMD changes in osteogenesis imperfecta: design, densitometry, and multiplicity".

We respond to methodological questions raised in a "Letter to the Editor" regarding our retrospective case series on romosozumab treatment in osteogenesis imperfecta (OI). We clarify that the retrospective study design, the rarity of the disease, the small number of cases, and the exploratory nature of the analyses limit the methodological approaches as already acknowledged in our original manuscript. Thus, the primary concern is an inherent limitation of an exploratory study in a rare disease. Nevertheless, these data are helpful, as they generate new hypotheses given the currently very limit

Patient-specific zebrafish models reveal a complex phenotypic spectrum in NBAS-associated atypical osteogenesis imperfecta.

Neuroblastoma amplified sequence gene (NBAS) variants are associated with short stature, optic atrophy, and Pelger-Huët anomaly (SOPH) syndrome. We previously identified compound heterozygous variants in NBAS to cause atypical Osteogenesis Imperfecta (OI), with these patients presenting with short stature, developmental delay, and recurrent long-bone fractures. However, skeletal disease progression due to these variants and the disease mechanisms underlying NBAS-associated OI remain poorly understood. Here, we provide a clinical update on previously identified patients and investigate the role

Rare SERPINH1 and SERPINF1 Variants in Autosomal Recessive Osteogenesis Imperfecta Type X and Type VI.

Osteogenesis imperfecta (OI) is a group of connective tissue disorders characterized by bone fragility and low bone mineral density (BMD). Two severe autosomal recessive forms, OI type X and VI, can be caused by variants in the SERPINH1 and SERPINF1 genes, respectively. In this study, we reported two children from two unrelated families exhibiting low BMD (z-score <-2.2) and multiple fractures. Whole-genome sequencing revealed a homozygous variant in each family. In family 1, a novel 5' untranslated region (UTR) variant in SERPINH1 (c.-35G>C) was identified at a highly conserved site, predicte

Challenging Genetic Diagnosis in Siblings With Primordial Dwarfism, Muscle Wasting, Osteogenesis Imperfecta, Craniofacial Dysmorphism, and Ectodermal Anomalies.

Three siblings of consanguineous Pakistani parents have primordial dwarfism with microcephaly, failure to thrive, delayed developmental milestones, thin and lean physique, dysplastic limb joints, ectodermal anomalies, and craniofacial anomalies including dolichocephaly, micrognathia, and irregular dentition. Variable findings are muscular dystrophy and osteogenesis imperfecta. We hypothesized a novel syndrome and launched a disease gene search. At the gene locus identified by single-nucleotide polymorphism-based linkage mapping, genome sequencing detected a very rare intergenic variant g.13276

Understanding sexual esteem, sexual desire, and sexual pain in adults with Osteogenesis Imperfecta.

INTRODUCTION: Osteogenesis Imperfecta (OI) is a rare genetic condition caused by an impairment in type I collagen production, resulting in several symptoms, including bone fragility and frequent fractures, bone deformities, and growth abnormalities. While physical characteristics of those with OI are well described, sexual experiences and challenges encountered by individuals with OI remain largely understudied. This study analyzed whether sexual esteem, sexual pain, desire, and emotional distress stratify between different self-reported OI types and health severity perception (HSP) groups. Se

Anti-TGF-β improves bone architecture without detectable restoration of cortical bone lamellar nanomechanical organization in Col1a1+/- osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is characterized by impaired bone mechanical integrity and increased fracture risk. Although anti-transforming growth factor beta (anti-TGF-β) treatment improves bone architecture and whole-bone mechanical properties in Col1a1+/- OI mice, its effects on tissue-scale strain behavior and local cortical mechanics remain unclear. Here, digital volume correlation (DVC), atomic force microscopy (AFM), and Raman spectroscopy were used to assess tissue-scale strain distributions, local cortical mechanical organization, and matrix composition following 8 weeks of anti-TGF-β

Osteoporosis in Pediatric Orthopaedics: Congenital Diseases, Medical Management, and Perioperative Considerations.

PURPOSE OF REVIEW: Pediatric osteoporosis is a heterogeneous group of primary and secondary disorders that creates challenges for orthopaedic surgeons. This review summarizes evidence on conditions commonly encountered in pediatric orthopedic practice, medical management strategies, and perioperative care for children with bone fragility. RECENT FINDINGS: Diagnostic approaches have been evolving toward integrating fracture characteristics, skeletal risk factors, and bone mineral density (BMD). A single low-trauma vertebral compression fracture is now sufficient for diagnosis of osteoporosis i

Short-term side effects of first bisphosphonate infusion in children with different underlying bone pathologies.

BACKGROUND: Intravenous bisphosphonates are the primary pharmacological intervention for both primary and secondary paediatric osteoporosis. Following the successful use of bisphosphonates in children with osteogenesis imperfecta (OI), indications have expanded to a broad range of bone pathologies with variable underlying pathophysiology. While bisphosphonate use appears to be safe in children, data on risk factors for acute-phase reaction remain sparse. A prospective characterisation of adverse effects - including baseline musculoskeletal symptoms - has not previously been performed in childr

Pediatric osteoporosis: a comprehensive imaging-based review of diagnosis, differential diagnosis, and etiology.

Pediatric osteoporosis is a rare but increasingly recognized entity that differs from adult forms in its etiology, diagnostic criteria, and long-term skeletal implications. Unlike age-related osteoporosis in adults, bone fragility in children is categorized into primary forms, such as osteogenesis imperfecta and the monogenic causes of early-onset osteoporosis, and the more prevalent secondary forms arising from chronic systemic disease, endocrine and nutritional disorders, malignancy and its treatment, prolonged medication use, and immobilization. Early and accurate diagnosis is critical, as

Pathogenesis and precision management of osteogenesis imperfecta: from genetic mechanisms to novel therapies.

BACKGROUND: Osteogenesis imperfecta (OI) is a clinically and genetically heterogeneous skeletal dysplasia characterized by systemic bone fragility. Beyond skeletal manifestations, OI is a multisystem condition involving dentinogenesis imperfecta, hearing loss, joint hyperlaxity, and cardiopulmonary complications. The diagnostic paradigm for OI has undergone a revolutionary shift from traditional clinical phenotyping to a molecular etiology-based framework. Driven by advancements in high-throughput sequencing, mutations in over 20 implicated genes have expanded the current OI nosology to at lea

Minimally Invasive Transforaminal Lumbar Interbody Fusion for Osteogenesis Imperfecta: A Case Report.

This case report describes the use of minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) in a patient with osteogenesis imperfecta (OI). A 59-year-old woman with significant spinal stenosis and anterolisthesis at L4-L5 underwent MIS-TLIF due to a lack of improvement with conservative measures. Preoperative planning was key, as the patient's dual-energy X-ray absorptiometry scan revealed osteopenia. The procedure was completed without intraoperative or postoperative complications; nerve root decompression resulted in significant symptomatic improvement. At 1-year follow-up, th

Variable and fixed rate debt: Gambling on the future state of the economy after Covid-19.

Melatonin is a product of the amino acid tryptophan in the pineal gland. Once synthesized, the specific mechanisms governing the release of melatonin from the pineal gland and its functions are largely unknown. Besides its regulatory role in circadian rhythms in mammals, because of its widespread subcellular distribution, melatonin contributes to the reduction of oxidative damage in both the lipid and the aqueous environments of the cell. This postulate is widely supported by the experimental observations showing that melatonin protects lipids in membranes, proteins in the cytosol, and DNA in
