Surgical Approaches to Optic Nerve Decompression in Osteopetrosis: A Review of Endoscopic Endonasal and Transcaruncular Techniques.

Osteopetrosis is a rare genetic disorder characterized by impaired osteoclast function and excessive bone density, often leading to compressive optic neuropathy due to bony overgrowth of the optic canal. Timely surgical intervention is critical for preventing permanent vision loss in affected patients. This review summarizes the available literature on the surgical techniques of endoscopic endonasal optic nerve decompression (EEOND) and the transcaruncular approach in osteopetrosis patients. EEOND is a minimally invasive technique that offers excellent visualization, advanced instrumentation,

Sorting nexin 10 regulates lysosomal ionic homeostasis via ClC-7 by controlling PI(3,5)P2.

Mutations or ablation of Snx10 are associated with neurodegeneration, blindness, and osteopetrosis. The similarities between osteoclasts and macrophages prompted us to analyze the role of Snx10 in phagocytosis. Deletion of Snx10 impaired phagosome resolution. Defective resolution was caused by reduced Cl- accumulation within (phago)lysosomes, replicating the phenotype reported in macrophages lacking ClC-7, a lysosomal 2Cl-/H+ antiporter. Delivery of ClC-7 to (phago)lysosomes was unaffected by ablation of Snx10, but its activity was markedly depressed. Snx10 was found to regulate ClC-7 activity

Visualization and quantification of RANK-RANKL binding for application to disease investigations and drug discovery.

Receptor activator of NFκB (RANK)-receptor activator of NFκB ligand (RANKL) binding triggers the differentiation of osteoclasts, bone-resorbing cells. The imbalance between bone resorption by osteoclasts and bone formation by osteoblasts causes bone diseases. We herein report the real-time detection of RANK-RANKL binding using the NanoLuc method. Large-BiT-RANK and RANKL-Small-BiT fusion proteins were expressed in HeLa cells, and their co-culture exhibited chemiluminescence in the presence of luciferase substrates. This luminescence was inhibited by the treatment of cells with an anti-RANKL ne

Rare Cause of Bone Marrow Failure: Osteopetrosis, Case Series.

Osteopetrosis is a rare metabolic bone disease that can lead to progressive bone marrow failure if left untreated. Resulting cytopenia and extramedullary hematopoiesis are frequently encountered in autosomal recessive form of the disease (ARO) and may result in death. Recurrent bone fractures and skeletal deformities are mostly seen in autosomal dominant form osteopetrosis (ADO) and cause significant morbidity. In this report, clinical, laboratory, and radiological findings of 5 patients with osteopetrosis were presented. Three had cytopenias, typical peripheral smear, and bone marrow aspirati

SUGAR-seq reveals the transcriptome and N-linked glycosylation landscape of mononuclear phagocytes at single-cell resolution in a mouse model of autosomal dominant osteopetrosis type 2.

BACKGROUND: Heterozygous mutation of CLCN7 (R286W) is commonly found in patients with benign autosomal dominant osteopetrosis. However, there is no evidence from animal models to confirm that it is a disease mutation. And the characteristics of the bone marrow cell (BMC) landscape in osteopetrosis at the single-cell level are completely unknown till now. RESULTS: In this study, we generated the first autosomal dominant osteopetrosis type 2 (ADO2) mouse model with typical phenotypes carried a mutation Clcn7 (r284w) corresponding to CLCN7 (R286W) observed in human patients using gene editing te

How I do it: Optic nerve decompression in patient with osteopetrosis.

BACKGROUND: Osteopetrosis is a rare genetic disorder causing excessive skeletal density, with a predilection to affect the skull base. This commonly leads to optic canal stenosis, optic nerve compression, and atrophy and vision loss. Timely optic nerve decompression can be an effective surgical intervention to preserve vision. METHODS: Pterional approach combining both extra- and intradural optic nerve decompression was performed. Intradural optic nerve identification proved a helpful and safe way to facilitate adequate decompression while dissecting in the region of hypertrophic anterior cli

Identification of a novel mutation in the CLCN7 gene in pediatric osteopetrosis: case report.

Osteopetrosis, also known as osteosclerosis and marble-bone disease, is a rare genetic metabolic bone disorder caused by the dysplasia or dysfunction of osteoclasts, usually caused by variants of chloride voltage-gated channel 7 (CLCN7) gene. We retrospectively analyzed the clinical data of two children with osteopetrosis and their families. Whole-exome sequencing (WES) was used for genetic analysis, and Sanger sequencing confirmed possible pathogenic variants. In family 1, the proband harbored a novel mutation c.2351G>C (p.R784T) in CLCN7 gene. The initial symptom of proband 1 was a post-trau

Leucine-Rich Repeat Kinase 1 Signaling Targets Proteins Critical for Endosome/Lysosome Sorting and Trafficking in Osteoclasts.

Global knockout (KO) of the Lrrk1 gene in mice causes severe osteopetrosis because of the failure of osteoclasts to resorb bone. The molecular mechanism of LRRK1 regulation of osteoclast function is not fully understood. Here, we performed a 2D DIGE phosphor-proteomics analysis to identify potential LRRK1 targets in osteoclasts. Splenocytes from Lrrk1 KO and wild-type (WT) mice were differentiated into osteoclasts for protein extraction. Lysates from Lrrk1 KO and WT cells were labeled with Cy3- and Cy5-dye, respectively. Labeled proteins were mixed and analyzed on the same 2D SDS PAGE for prot

Wnt1's Differential Effects on Craniofacial Bone and Tooth Development.

The development of craniofacial bones and teeth relies heavily on the Wnt signaling pathway, yet the specific mechanisms and Wnt variants involved remain under continual investigation. Using publicly available single-cell sequencing data from the mouse incisor, we reveal Wnt1 expression across dental structures and investigate its role using a Col1a1-dependent Wnt1 transgenic mouse model. Inducing Wnt1 early on affects craniofacial bone without disturbing tooth development, but prolonged embryonic induction leads to postnatal mortality with osteopetrosis-like bone overgrowth and malformed teet

Adoptive Transfer of Lepr+ Bone Marrow Cells Attenuates the Osteopetrotic Phenotype of db/db Mice.

Leptin-deficient (ob/ob) and leptin receptor (Lepr)-deficient db/db mice develop a mild form of osteoclast-rich osteopetrosis, most evident in long bone epiphyses, implying leptin is important for normal replacement of cartilage during skeletal maturation. However, it is unclear whether leptin acts as a permissive or regulatory factor and whether its actions are mediated via peripheral pathways. Here we show the osteopetrotic phenotype is not evident in ob/+ or db/+ mice, suggesting that leptin acts as a critical but permissive factor for skeletal maturation. The importance of leptin is furthe

Treatment of subtrochantric femur fracture with intra-medullary nailing (IMN) in a patient with osteopetrosis: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Osteopetrosis is a rare hereditary bone disorder characterized by defective osteoclastic bone resorption, leading to dense but brittle bones. These structural abnormalities predispose patients to fractures and complicate orthopedic surgeries due to obliterated medullary canals and increased cortical density. Subtrochanteric femoral fractures in such patients are rare and challenging, with limited consensus on optimal surgical management. CASE PRESENTATION: We present a 45-year-old male with adult-onset autosomal dominant osteopetrosis who sustained bilateral subtr

Neuroaxonal Dystrophy With Osteopetrosis Associated With a Novel Biallelic Nonsense Homozygous Variant in BORCS5.

Neuroaxonal dystrophy (NAD) with osteopetrosis syndrome (OMIM # 600329) was first reported in a consanguineous Moroccan Jewish family. However, to date, no genetic variant has been linked to this disease. We report on sibs, born to consanguineous Pakistani parents identified prenatally with cerebral ventriculomegaly and agenesis of the corpus callosum, and autopsies done on both showed similar abnormalities, including facial dysmorphism, osteopetrosis, and neuropathologic findings consistent with NAD. Trio exome sequencing identified a homozygous c.283 C>T; p.(Arg95Ter) variant in exon 3 of th

Understanding rickets in osteopetrosis via a case: mechanisms and treatment implications.

OBJECTIVES: Osteopetrorickets is a rare autosomal recessive disease that affects many systems and is characterized by dense bone mass and paradoxical rickets due to insufficient resorption of calcified cartilage. Its rarity and presentation with nonspecific symptoms may cause delays in diagnosis. Early diagnosis and improvement of rickets accompanying bone marrow transplantation as a curative treatment option before the development of any irreversible complications are essential for prognosis. However, the etiopathogenesis and management of rickets treatment strategies are still controversial.

Re-fracture after Osteosynthesis of Femoral Shaft Fracture in Osteopetrosis.

This study presents a case report of a 54-year-old female patient with osteopetrosis who experienced re-fracture after undergoing osteosynthesis for a femoral shaft fracture. The patient visited our emergency department for a right distal femur fracture. The patient had undergone surgery for a right femoral shaft fracture six years prior to this visit, at which time osteopetrosis was diagnosed at another medical facility. Following three years of postoperative care, successful osteosynthesis was achieved, and the surgical implant was removed. The re-fracture occurred at the site of the previou

Elevated surface La promotes hyperfusion and contributes to impaired resorption in osteopetrosis.

The skeleton is a living, biological tissue responding to the biomechanical demands placed upon it throughout life. The individual bones creating this physiological system are each shaped by a multinucleated cell type - the osteoclast - that sculpts each bone in collaboration with local cellular partners, which offer chemical and even tactile feedback of many sorts. Unfortunately, the perturbation of osteoclast formation and function underpins a broad range of human skeletal pathologies, including osteopetrosis - a systemic pathology characterized by impaired osteoclast resorption leading to s
