Five ways to find a parasite: comparison of five molecular assays for the detection of Strongyloides spp.

UNLABELLED: Strongyloidiasis, caused by nematodes of the genus Strongyloides, is a neglected tropical disease with significant public health implications. Accurate diagnostics are essential for disease surveillance and control, yet traditional stool-based and serological methods have limitations in sensitivity and specificity. Molecular assays offer an alternative diagnostic approach. In this study, we compared four probe-based real-time PCR (qPCR) assays and one loop-mediated isothermal amplification (LAMP) assay using human fecal DNA samples from Gabon and Papua New Guinea with both Strongyl

Bilateral giant pulmonary hydatid cysts in a toddler presenting as severe pneumonia: a risk-adapted staged surgical approach.

BACKGROUND: Pulmonary hydatid disease is common in endemic regions, yet bilateral giant cysts in toddlers are exceptionally rare and may mimic severe pneumonia. Their management is challenging, particularly in physiologically vulnerable children. CASE PRESENTATION: We report a 32-month-old girl presenting with fever, progressive cough, and severe respiratory distress. Imaging revealed four large pulmonary hydatid cysts-three in the left lung and one in the right-complicated by superimposed bacterial infection and hypoxemia. Albendazole therapy (15 mg/kg/day) was initiated, and a risk-adapted,

Clinical and Laboratory Dynamics of Bear Meat-Associated Trichinellosis in Hokkaido, Japan.

Trichinellosis is a rare infection associated with bear meat consumption found in Japan, where limited access to parasite-specific antibody testing makes definitive diagnosis challenging. In the present study, 19 patients, comprising 16 confirmed and three probable cases, treated at a tertiary care hospital in Hokkaido, Japan, during three independent outbreaks in 2018, 2019, and 2025, were retrospectively analyzed. Patients prepared brown bear meat as a roast (n = 18) or a cutlet (n = 1) and consumed it as a main dish (n = 7) or in smaller portions of a few slices (n = 12). Ten patients consu

Severe strongyloidiasis and Gram-negative bacterial superinfections: epidemiology, clinical implications, and therapeutic strategies.

PURPOSE OF REVIEW: Strongyloidiasis is a neglected tropical disease affecting over 600 million people worldwide. Its severe forms carry a fatality rate exceeding 60%, yet death is rarely attributable to the parasite alone: the terminal event is often bacterial sepsis or meningitis of enteric origin. We review recent evidence on Gram-negative and other bacterial superinfections in severe strongyloidiasis, and their diagnostic and therapeutic implications. RECENT FINDINGS: Cohort studies show that bacteraemia is the commonest complication of severe disease, affecting 38.3% of cases against 4.1%

Risk factors for local lymph node metastasis in hepatic alveolar echinococcosis: a 10-year retrospective cohort study.

BACKGROUND: Hepatic alveolar echinococcosis (HAE) is characterised by infiltrative growth and "cancer-like" biological behaviour. Although radical resection remains the standard treatment, the need for prophylactic lymph node dissection is still debated because preoperative identification of metastasis is difficult. This investigation assesses the risk factors associated with local lymph node metastasis in HAE. METHODS: Clinical data were retrospectively reviewed for 623 patients with HAE admitted to our hospital between January 2012 and January 2022. Based on strict screening criteria, 191 p

Emergency surgical management for complicated schistosomal massive splenomegaly with portomesenteric thrombosis and bowel ischemia: a case report.

INTRODUCTION: Schistosomal periportal fibrosis is a major cause of non-cirrhotic portal hypertension and massive splenomegaly. Rarely, superimposed portomesenteric venous thrombosis (PMVT) may precipitate bowel ischemia. Managing this combination in non-endemic settings presents significant diagnostic and therapeutic challenges. We report successful bowel salvage and definitive portal decompression during the same surgical course. CASE PRESENTATION: A 43-year-old man with schistosomal periportal fibrosis and massive splenomegaly presented with partial PMVT-associated bowel ischemia. Following

Complicated hepatic cyst with an incidental pelvic hydatid cyst: a case report.

INTRODUCTION: Hydatid disease, a zoonosis caused by Echinococcus granulosus, most commonly affects the liver and lungs. Primary pelvic localization is exceptionally rare (< 2% of cases) and poses significant diagnostic challenges, particularly in patients without classic epidemiological risk factors. This case report highlights the complexities of managing such an atypical presentation. CASE PRESENTATION: A 56-year-old Arab man from an urban area, with a history of mechanical aortic valve replacement and treatment with warfarin, presented with acute abdominal pain and fever. Imaging revealed

Human angiostrongyliasis: the past, present, and future.

Human angiostrongyliasis is an emerging foodborne zoonosis caused predominantly by Angiostrongylus cantonensis and Angiostrongylus costaricensis, which produce distinct neurological and abdominal forms of disease, respectively. Human infection occurs primarily through ingestion of raw or undercooked gastropod intermediate hosts, paratenic hosts such as freshwater prawns and crabs, or fresh produce contaminated with infective third-stage larvae (L3). A. cantonensis is the principal cause of neuroangiostrongyliasis and eosinophilic meningitis, whereas A. costaricensis causes abdominal angiostron

Human subcutaneous sparganosis caused by Spirometra erinaceieuropaei in Vietnam.

INTRODUCTION: Human sparganosis is a rare zoonotic disease caused by Spirometra spp. To our knowledge, this is the second documented case of human subcutaneous sparganosis in Vietnam, following the first case reported in 1995. CASE PRESENTATION: A rare case of human subcutaneous sparganosis caused by Spirometra erinaceieuropaei was detected in Vietnam on April 27, 2023, using morphological and molecular methods. The source of the infection is presumed to be associated with the consumption of raw vegetables and frog hotpot. CONCLUSIONS: This case highlights the diagnostic value of molecular t

Epidemiology of helminth infections in adults in rural rainforest communities of the Nkam-Wouri River Basin, Littoral Region of Cameroon: Implications for NTDs elimination using preventive chemotherapies.

BACKGROUND: Chronic helminth infections cause serious health problems; however, limited data on prevalence and distribution hinder effective control interventions. This study assessed the prevalence, distribution, and risk factors of helminth infections in adults across four health districts (HD) in the Littoral Region of Cameroon, while hypothesizing that helminth infection prevalences are significantly higher in densely forested and wetland forest plantation districts compared to degraded and mosaic plantation forest districts despite over more than two decades of preventive chemotherapy. Over-dispersion in malaria-schistosoma co-infection: insights from a meta-analytical approach and systematic review.

BACKGROUND: Malaria and schistosomiasis are two major parasitic diseases that are co-endemic in many regions of sub-Saharan Africa. Despite their frequent geographical overlap, the potential epidemiological patterns and potential interactions associated with Plasmodium--Schistosoma co-infection remain poorly characterized. This study aimed to systematically assess the epidemiology of malaria-schistosomiasis co-infection and to quantify whether co-infections occur more frequently than expected under an assumption of independence between the two infections. METHODS: We conducted a systematic re

Parasite-Derived IL-33 Inhibitors Prevent Bleomycin-Induced Lung Fibrosis.

Idiopathic pulmonary fibrosis (IPF) is a progressive lung-scarring disease in which epithelial injury, consequent release of alarmins such as interleukin (IL)-33, and immune activation contribute to extracellular matrix deposition. The intestinal helminth Heligmosomoides polygyrus bakeri (Hpb) secretes immunomodulators that suppress type 2 immunity, including HpARI2, which binds IL-33, and HpBARI_Hom2, which blocks the IL-33 receptor ST2. Here, we test this parasite-derived dual blockade strategy in the bleomycin (BLM) mouse model, moving from live Hpb infection to administration of total Hpb

Heterologous antibody-based immunity in cattle naturally infected with Ostertagia ostertagi and Haemonchus sp.

Heterologous antibody‑mediated immunity was evaluated in cattle naturally infected with Ostertagia ostertagi (Oo) and Haemonchus sp. (Hp). Antibody responses were assessed using ovalbumin (OVA) as unrelated model antigen in three groups of eight heifers, matched for eggs per gram of feces (EPG) and allocated to independent, nematode‑free pastures. Anti‑OVA titers were determined in serum by ELISA. EPG counts and nematode genera were determined in feces using the modified McMaster technique and pooled cultures, respectively. All variables were analyzed using mixed models. One group was previous

Molecular characterization of Echinococcus species causing human cystic echinococcosis in Algeria: predominance of Echinococcus granulosus sensu stricto.

Genotyping of cystic echinococcosis (CE) specimens enables the identification of predominant species and genotypes, yielding critical insights into disease epidemiology. Species identification is essential for the development of effective prevention and control strategies. In this context, we assessed the molecular diversity of E. granulosus sensu lato among surgically confirmed human CE cases recruited from Algerian referral hospitals. Hydatid cysts were collected from 31 patients undergoing surgery for pulmonary and/or hepatic CE in Algeria between 2022 and 2024. Hydatid fluid, cyst wall, an

Novel EG95-based mRNA vaccines induce long-term protective immunity against echinococcosis.

Cystic echinococcosis (CE), a neglected zoonotic disease caused by Echinococcus granulosus sensu lato, remains a significant public health and veterinary concern worldwide. Despite efforts to develop recombinant protein-based vaccines against EG95, a key protective antigen, existing platforms suffer from limited immunogenicity, short-lived immunity, and complex production processes. Here, we designed and evaluated three novel mRNA-LNP vaccines, including EG95 mRNA-LNP vaccine, IL-4-EG95 mRNA-LNP vaccine encoding EG95 fused with IL-4 via a flexible linker to enhance immune stimulation, and EG95

Beyond IgE: Expanding Perspectives on Multiclass Immunoglobulin Responses in Anisakis Allergy.

Anisakis allergy may involve antibody responses beyond IgE alone. Considering both mucosal and systemic immunoglobulin responses may improve our understanding of sensitization, immune regulation, and host-parasite interactions; although further studies are required before these observations can be translated into clinical practice.
