Cartilage-hair hypoplasia (CHH) is a rare autosomal recessive skeletal dysplasia caused by variants in RMRP (RNA component of mitochondrial RNA processing endoribonuclease), which encodes a non-coding RNA. Despite its well-established clinical and molecular spectrum in some populations, data from Mexico remain limited. We report eight patients from seven unrelated families with CHH. Clinical and radiographic findings were assessed, and RMRP was analyzed by polymerase chain reaction and Sanger sequencing. All patients exhibited disproportionate short stature; prenatal long-bone shortening and s

SOX9 and RUNX2 are lineage defining transcription factors that drive differentiation of chondrocyte and osteoblast lineages respectively from osteochondral progenitors. In limb development, these progenitors are specified first by SOX9 expression required for mesenchymal condensation prior to RUNX2 activation and osteochondral differentiation to chondrocyte and osteoblast lineages. Unlike limb development, the anterior craniofacial skeleton arises from multipotent cranial neural crest cells (cNCCs). To examine the temporal activation of SOX9 and RUNX2 within cNCCs, we utilized a combination of

BACKGROUND: Pyle disease is a rare metaphyseal dysplasia caused by loss-of-function variants in SFRP4, leading to abnormal cortical modeling with relative preservation of trabecular bone. The mechanisms involved in severe and persistent bone pain in adult patients remain incompletely understood.
CASE PRESENTATION: We report an adult woman with genetically confirmed homozygous SFRP4 p.Asn53Tyr-related skeletal disease presenting with marked and persistent bone pain. Dual-energy X-ray absorptiometry showed preserved lumbar bone mineral density but markedly reduced femoral neck bone mass, consis

After injury, bone tissue initiates a reparative response to restore its structure and function. The failure to initiate or delay this response could result in fracture nonunion. The molecular mechanisms underlying the occurrence of fracture nonunion are not yet established. We propose that hypoxia-triggered signaling pathways, mediated by reactive oxygen species (ROS) homeostasis, control Bmp2 expression and fracture healing initiation. The excessive ROS leads to oxidative stress and, ultimately, fracture nonunion. In this study, we silenced Apex1, the final ROS signaling transducer that medi

Recent studies have revealed de novo or germline-derived GNAS-Gsα variants with constitutive ligand-independent gain-of-function (GOF) effects on specific G-protein-coupled receptor signalings in patients with nephrogenic syndrome of inappropriate antidiuresis (NSIAD), osteolytic bone disorder with metaphyseal dysplasia, and peripheral precocious puberty. We encountered a Japanese girl with NSIAD and osteolytic bone disorder with metaphyseal dysplasia. Whole genome sequencing identified a de novo ″likely pathogenic″ heterozygous GNAS-Gsα missense variant (NM_000516.7:c.163 A > G:p.(Thr55Ala))

OBJECTIVES: Dyggve-Melchior-Clausen (DMC) syndrome is a rare autosomal recessive skeletal dysplasia caused by mutations in the DYM gene. It is characterized by progressive spondyloepimetaphyseal dysplasia, short stature, coarse facial features, microcephaly and intellectual disability. While it clinically resembles Morquio syndrome (mucopolysaccharidosis type IV, MPS IV), DMC is distinguished by cognitive impairment, absence of corneal clouding, normal urinary glycosaminoglycans and distinctive radiological features.
CASE PRESENTATION: We reported three siblings with DMC syndrome. Two 4-year-

Cartilage hair hypoplasia (CHH) syndrome (OMIM #250250) is a rare autosomal recessive metaphyseal dysplasia, characterized by disproportionate short stature, hypotrichosis and variable extra-skeletal manifestations, including immunodeficiency, anemia, intestinal diseases, and predisposition to malignancies. CHH results from homozygous or compound heterozygous mutations in the RMRP gene on chromosome 9p13, which encodes an untranslated RNA component of mitochondrial RNA-processing endoribonuclease. RMRP pathogenic variants can also lead to Omenn Syndrome (OS) (OMIM #603554), a systemic inflamma

Recent classification of skeletal dysplasias has reclassified disorders according to their underlying pathophysiologic mechanisms. The latest nosology of skeletal dysplasias introduced a new pathogenic category termed "skeletal disorders of the parathyroid hormone (PTH) signaling cascade." The group encompasses genetic defects associated with abnormal signaling of PTH, PTH-related protein (PTHrP) encoded by the PTHLH (PTH-like hormone) gene, and their common receptor termed parathyroid hormone 1 receptor (PTH1R). PTH1R is a guanine nucleotide-binding protein (G protein)-coupled receptor; and t

INTRODUCTION AND BACKGROUND: Invasive home mechanical ventilation is used for patients with chronic respiratory insufficiency. This elaborate and technology-dependent ventilation is carried out via an artificial airway (tracheal cannula) to the trachea. Exact numbers about the incidence of home mechanical ventilation are not available. Patients with neuromuscular diseases represent a large portion of it.
RESEARCH QUESTIONS: Specific research questions are formulated and answered concerning the dimensions of medicine/nursing, economics, social, ethical and legal aspects. Beyond the technical a

The authors present a case of a 22-year-old female with a history of intrauterine growth restriction, metaphyseal dysplasia, adrenal hypoplasia congenita, and genital anomalies syndrome (IMAGe) secondary to pathogenic variants in the polymerase epsilon (POLE) gene who was newly diagnosed with type 2 diabetes. The patient was noted on routine lab work to have elevated blood glucose, and subsequent laboratory analysis revealed a hemoglobin A1c of 8.9% with normal serum insulin concentrations and negative autoimmune evaluation. She did not have physical exam findings or stigmata of insulin resist

OBJECTIVES: MMP13-related metaphyseal dysplasia Spahr type, is an extremely rare skeletal disorder, and only a dozen patients with a confirmed molecular diagnosis have been reported. It is characterized by mild short stature, genu varum, and metaphyseal irregularities including fraying, splaying, and cupping of the long bones. The disorder is in the differential diagnosis of rickets, a relatively common disorder in childhood that shares similar clinical and radiological features with metaphyseal dysplasia.
CASE PRESENTATION: Herein, we present a 39-month-old girl patient who was initially eva

Paediatric maxillofacial surgeons infrequently encounter primary life-limiting conditions with escalating symptoms, contributing to the underutilization of palliative care services. This paper highlights two rare life-limiting oral and maxillofacial conditions - infantile osteopetrosis and osteosclerotic metaphyseal dysplasia - and proposes a collaborative framework integrating palliative care. The cases illustrate the multidisciplinary approach needed for complex symptom management, including end-of-life care in one case. The collaboration involved anaesthesia, surgery, pharmacy, dietetics, en

Pyle's disease (PD), also known as metaphyseal dysplasia, is a rare genetic skeletal disorder characterized by a specific radiologic feature known as the Erlenmeyer-flask deformity, the expansion of trabecular metaphyses, specifically in the distal aspects of long bones. The main pathophysiology of this disease is caused by mutations in the Secreted Frizzled-Related Protein 4 (SFRP4) gene. This case report aims to discuss the electrodiagnostic findings of an 8-year-old girl diagnosed with PD. This evaluation revealed normal sensory nerve action potentials (SNAP); however, compound muscle actio
