Endoscopic Endonasal Odontoidectomy and Occipitoaxial Fixation for Basilar Invagination With Platybasia and Atlas Occipitalization Associated With Trichorhinophalangeal Syndrome.

Basilar invagination is a craniovertebral junction deformity characterized by the upward migration of the odontoid process toward the foramen magnum. This condition is frequently associated with congenital anomalies such as platybasia and atlas occipitalization. On the other hand, trichorhinophalangeal syndrome is a rare autosomal dominant skeletal dysplasia characterized by distinctive craniofacial features, brachydactyly, and abnormalities in endochondral ossification. This study describes the surgical treatment performed in a 39-year-old man with genetically confirmed trichorhinophalangeal

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

CBCT features of florid cemento-osseous dysplasia with coexisting lesions at different maturation stages: a case report and literature review.

BACKGROUND: Florid cemento-osseous dysplasia (FLCOD) is a begin fibro-osseous lesion occurring in the tooth-bearing regions of the jaws and is characterized by replacement of normal bone with fibrous connective tissue and cementum-like mineralized material. Most patients are asymptomatic and are diagnosed incidentally during radiographic examinations, and the condition may be misdiagnosed because of overlapping imaging features with other jaw lesions. Because the disease is usually asymptomatic and exhibits imaging features that overlap with those of several odontogenic and inflammatory lesion

Evaluation of sleep-related breathing disorders and polysomnography findings in pediatric patients with achondroplasia.

PURPOSE: To describe sleep-disordered breathing in children with achondroplasia at their first polysomnography (PSG) assessment and examine associations with clinical characteristics. METHODS: This retrospective study included 49 children evaluated between March 2016 and February 2025. Obstructive sleep apnea syndrome (OSAS) was classified using the obstructive apnea-hypopnea index (oAHI). Total AHI was compared according to sex, upper airway surgery history, foramen magnum stenosis, and vosoritide treatment. Infant findings were also presented separately because severity thresholds require c

Achondroplasia: Current Disease-Modifying Therapies and Genome Editing Strategies.

Achondroplasia (ACH) is the most common non-lethal skeletal dysplasia and is caused in the vast majority of cases by a heterozygous gain-of-function variant in fibroblast growth factor receptor 3 (FGFR3, p.Gly380Arg). Hyperactive FGFR3 signaling constrains endochondral bone growth by suppressing growth-plate chondrocyte proliferation and hypertrophic differentiation. In the past decade, management has expanded beyond supportive care, and several mechanism-based interventions have shown disease-modifying effects in clinical studies. These include the activation of the NPR2/CNP axis with vosorit

Integrated stress signaling pathways promote adaptive reprogramming for chondrocyte survival in Metaphyseal Chondrodysplasia Type Schmid.

Collagen X, encoded by the Col10a1 gene, is expressed specifically by hypertrophic chondrocytes (HCs) in the cartilage. Mutations of the Col10a1 gene in humans, which increase the endoplasmic reticulum (ER) stress in HCs, lead to metaphyseal chondrodysplasia type Schmid (MCDS). However, the mechanism of how HCs respond to ER stress to affect bone development and growth is not entirely understood. In this study, we generated a new mouse model harboring a 13 bp deletion (13del) in the endogenous Col10a1 gene. These 13del-MCDS mice display all the known clinical characteristics of human MCDS, as

Expanding the Clinical and Molecular Spectrum of Cartilage-Hair Hypoplasia in the Mexican Population.

Cartilage-hair hypoplasia (CHH) is a rare autosomal recessive skeletal dysplasia caused by variants in RMRP (RNA component of mitochondrial RNA processing endoribonuclease), which encodes a non-coding RNA. Despite its well-established clinical and molecular spectrum in some populations, data from Mexico remain limited. We report eight patients from seven unrelated families with CHH. Clinical and radiographic findings were assessed, and RMRP was analyzed by polymerase chain reaction and Sanger sequencing. All patients exhibited disproportionate short stature; prenatal long-bone shortening and s

Types 1 diabetes mellitus and Graves' disease in Sanjad-Sakati syndrome: a case report in Saudi Arabia.

Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Characterization of familial short stature with or without skeletal dysplasia in a Pakistani cohort reveals founder mutations as well as novel variants.

Skeletal dysplasias are a diverse group of genetically inherited bone disorders which may manifest with or without short stature. Extreme short stature can also result from different growth hormone deficiencies. Our research aimed to identify the genetic causes of recessively inherited short stature, with or without skeletal abnormalities in Pakistani families. We recruited participants from thirty-eight pedigrees with a history of these disorders. Radiological examinations were performed for a few patients. Some samples were first screened for variants in GHRHR and RMRP by Sanger sequencing.

Fibrous dysplasia: clinical and pathologic characteristics, surgical approaches, and outcomes in a large Italian cohort.

Fibrous dysplasia (FD) is a rare benign bone disease caused by postzygotic activating mutations in the GNAS gene, leading to replacement of normal bone with fibrous tissue. It may be monostotic or polyostotic, often asymptomatic but sometimes associated with pain, deformity, or fractures. Management is mainly surgical in symptomatic or complicated cases. This retrospective study analyzes a large FD cohort to identify determinants of treatment and outcomes, with a focus on pain and its evolution. The primary aim was to assess factors influencing surgical indication, while the secondary aim was

Single-cell landscape of lymphoid and myeloid heterogeneity across type-stratified craniofacial lesions in polyostotic fibrous dysplasia.

BACKGROUND: Polyostotic fibrous dysplasia (POFD) is a mosaic skeletal disorder driven by somatic GNAS mutations that constitutively activate Gsα-cAMP signaling, producing fibro-osseous craniofacial deformities. POFD lesions are histologically classified into three subtypes-Type I (connective-tissue predominant), Type II (cancellous-bone predominant), and Type III (sclerotic-bone predominant)-yet the immune microenvironment across these categories remains undefined. This study aims to dissect lymphoid and myeloid heterogeneity and inter-cellular communication networks among distinct POFD histol

Florid Cemento-Osseous Dysplasia and Simple Bone Cyst Association: Co-Occurrence or Evolution of the Disease? Two Case Reports and Literature Review.

OBJECTIVES: Cemento-osseous dysplasia (COD) is the most common benign fibro-osseous lesion of the jaws, typically asymptomatic and identified through characteristic radiographic features. However, its rare association with simple bone cysts (SBCs) can significantly alter its presentation, mimicking more aggressive jaw lesions. This study aims to report additional cases of COD-associated SBCs and to review the literature to evaluate whether such lesions may represent a specific manifestation of the COD spectrum. MATERIALS AND METHODS: Two cases of florid COD with atypical radiolucencies were d

Bi-allelic EPHA2 mutations cause dysosteosclerosis by inhibiting osteoclastogenesis and enhancing osteoblastogenesis.

Erythropoietin-producing hepatocellular (EPH) receptors, the largest receptor tyrosine kinase family, have been shown to play a crucial role in normal physiological processes and pathologies. EPHA2, encoded by EPHA2 gene, a member of the EPH receptor family, has been implicated in cataracts and various bone tumors, including osteosarcoma, chondrosarcoma, and bone metastases of breast cancer. However, no association between the EPHA2 gene and genetic skeletal disorders has been reported to date. Here, we identified a Chinese family with a diagnosis of dysosteosclerosis, presenting with signific

Correlation Between Radiographic Parameters and Clinical Range of Motion in Slipped Capital Femoral Epiphysis.

BACKGROUND: Surgical procedures that improve motion for slipped capital femoral epiphysis (SCFE) deformity are typically guided by radiographic imaging. However, the specific aspects of the deformity that have the greatest impact on hip functional biomechanics remain unclear. This study investigated the relationship between radiographic parameters and clinical ROM measurements in patients with SCFE to better elucidate which elements of SCFE pathoanatomy have the greatest effect on joint mobility. METHODS: Patients who underwent in situ pinning for stable SCFE at a single institution between 2

Anifrolumab for refractory autoimmune hemolytic anemia in SPENCD-associated lupus: a case-based review.

Spondyloenchondrodysplasia (SPENCD) is a rare monogenic interferonopathy caused by biallelic ACP5 mutations, characterized by skeletal dysplasia, neurological involvement and immune dysregulation frequently manifesting as lupus-like disease. Autoimmune cytopenias, particularly autoimmune hemolytic anemia (AIHA), are among the most severe and treatment-refractory manifestations. Anifrolumab, a monoclonal antibody targeting interferon alpha receptor subunit 1 (IFNAR1), blocks signaling by all type I interferons and is approved for moderate-to-severe systemic lupus erythematosus. We report a 50-y

Understanding sexual esteem, sexual desire, and sexual pain in adults with Osteogenesis Imperfecta.

INTRODUCTION: Osteogenesis Imperfecta (OI) is a rare genetic condition caused by an impairment in type I collagen production, resulting in several symptoms, including bone fragility and frequent fractures, bone deformities, and growth abnormalities. While physical characteristics of those with OI are well described, sexual experiences and challenges encountered by individuals with OI remain largely understudied. This study analyzed whether sexual esteem, sexual pain, desire, and emotional distress stratify between different self-reported OI types and health severity perception (HSP) groups. Se

Anti-TGF-β improves bone architecture without detectable restoration of cortical bone lamellar nanomechanical organization in Col1a1+/- osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is characterized by impaired bone mechanical integrity and increased fracture risk. Although anti-transforming growth factor beta (anti-TGF-β) treatment improves bone architecture and whole-bone mechanical properties in Col1a1+/- OI mice, its effects on tissue-scale strain behavior and local cortical mechanics remain unclear. Here, digital volume correlation (DVC), atomic force microscopy (AFM), and Raman spectroscopy were used to assess tissue-scale strain distributions, local cortical mechanical organization, and matrix composition following 8 weeks of anti-TGF-β
