Complexity of genomic diagnosis: Lessons learnt from the UK Biobank and Generation study newborn genome sequencing analyses.

BACKGROUND: Pathogenic variants in COL1A1 and COL1A2 account for 85-90% of osteogenesis imperfecta (OI). Glycine substitutions within the triple-helical domain are traditionally considered highly pathogenic. The COL1A2 c.2827G>A p.(Gly943Arg) variant was previously classified as pathogenic under ACGS 2019/2020 criteria. METHODS: One proband with this heterozygous variant was identified through national newborn genomic screening prompting further evaluation from the UK National OI Reference Laboratory. Four additional patients with the same variant were identified through referral to tertiary

A de novo TRPV4 variant c.2479C>G (p.Pro827Ala) in Spondylometaphyseal dysplasia Kozlowski type: identification and functional analysis.

Spondylometaphyseal dysplasia, Kozlowski type (SMDK), is an autosomal dominant skeletal disorder characterized by abnormalities of the spine, metaphyses and epiphyses. It is associated with variants in TRPV4, although the underlying molecular mechanisms remain unclear. A de novo heterozygous TRPV4 variant (c.2479C>G, p.Pro827Ala) was detected in a patient with SMDK by whole-exome sequencing, and transcriptome sequencing was performed in available family members. Expression analyses showed reduced TRPV4 transcript and protein levels associated with the p.Pro827Ala variant. Cellular functional a

Ultrasound-Guided Third Occipital Nerve Block for Painful C1-C2 Fibrous Dysplasia: A Case Report of Nonsurgical Success.

BACKGROUND: Fibrous dysplasia (FD) affecting the upper cervical spine is exceptionally rare and results in severe cervico-occipital pain and limited neck mobility due to altered biomechanics and irritation of the third occipital nerve (TON). Surgical management is challenging because of the proximity to critical neurovascular structures. In the absence of neurological deficits, targeted TON blocks have shown promise as part of conservative management. CASE REPORT: A 33-year-old woman presented with a 7-year history of neck and suboccipital pain. Imaging revealed expansile FD at C1-C2 with par

New lens on congenital mild bone fragility: a novel Col1a1 knockout mouse model for osteogenesis imperfecta type 1.

Osteogenesis imperfecta (OI) is a genetic disorder characterized by bone fragility. It is one of the most prevalent rare skeletal dysplasias. The mildest form, OI type 1, predominantly results from collagen type I haploinsufficiency due to pathogenic variants in the COL1A1 gene, leading to reduced collagen type I. Despite OI type 1 representing approximately half of the OI population, the lack of an effective mouse model has hindered research and therapy development. To address this gap, we developed a genetically engineered mouse model harboring a heterozygous deletion of the Col1a1 allele us

Atypical Femoral Fractures in Adult Patients with Classical Osteogenesis Imperfecta.

Atypical femoral fractures (AFF) are rare fractures with characteristic radiographic features, most commonly associated with long-term bisphosphonate use. Their occurrence in patients with osteogenesis imperfecta (OI), a hereditary bone disease leading to bone fragility, is not well understood. In this study, 138 adults with genetically confirmed classical OI were screened for AFF. Five patients with AFF were identified and compared to an age- and treatment-matched adult OI cohort without AFF (n = 23). Demographical parameters, biochemical markers, bone mineral density (DXA), bone microarchite

Magnetically Controlled Intramedullary Compression Nailing for Femoral Nonunion in Osteogenesis Imperfecta: A Case Report.

CASE: A 51-year-old nonambulatory woman with type III osteogenesis imperfecta sustained a diaphyseal femur fracture during a transfer, distal to a childhood-placed intramedullary pin. At the time of presentation, the fracture was a painful atrophic nonunion, preventing effective transfer. Retrograde nailing with a motorized magnetic lengthening nail, inserted with preset distraction, allowed gradual compression, solid union, and pain relief. CONCLUSION: This report describes the method of off-label lengthening nail modification used for gradual compression of a femoral nonunion site, achievin

High-coverage paleo-genome research identifies a pathogenic mutation on EXT1 in a Qing-period Chinese individual with multiple osteochondromas.

Multiple osteochondromas (MO) is a rare bone disease with variable manifestations that make it difficult to distinguish from phenotypically similar diseases, both morphologically and radiologically. As a monogenic disorder associated with mutations in EXT1 or EXT2, ancient DNA analysis can provide genetically confirmed diagnoses and insights into the pathogenesis of suspected cases. To further investigate a previously reported suspected MO case from Qing-period Shandong, China, we generated ancient whole-exome sequencing data from the individual's petrous bone, yielding a mean sequencing cover

Progenitors from distinct perichondrium layers initiate tumor formation in hereditary multiple osteochondromas as revealed in a mouse model.

Hereditary Multiple Osteochondromas (HMO) is a rare, pediatric disease characterized by osteochondromas that form next to the growth plates, accumulate over time and cause major health problems. Most HMO cases are linked to loss-of-function mutations in Golgi-resident proteins EXT1 or EXT2, but tumor development remains unclear. In particular, the source of tumor-forming progenitors has long been debated, with recent studies pointing to a perichondrial origin. Perichondrium normally flanks the growth plates, has essential roles in skeletal growth, and is composed of an inner cuboidal cell laye

Gαs variant detection discerns skeletal cell types involved in fibrous dysplasia pathogenesis.

Fibrous dysplasia (FD) is a mosaic disorder caused by activating Gαs variants. Skeletal stem cells give rise to abnormal osteoprogenitors, resulting in fibro-osseous lesions with prominent osteoclastogenesis. Gαs variants are reliably demonstrated in osteoprogenitors, however involvement in other skeletal cell types is not well-characterized. We evaluated human specimens to investigate Gαs variant expression in chondrocytes and osteoclasts, two prominent cell types in FD lesions. Seven of 20 bone specimens had cartilage present in addition to typical fibrous tissue. Droplet digital polymerase

Missense mutation of BMP1 may cause feline osteogenesis imperfecta without bone deformity.

A 3-mo-old domestic shorthair cat was presented with multiple fractures. Bone morphology was normal radiographically, with no long bone deformity or increased bone translucency. A bone biopsy from the ilium was examined histologically, revealing that bone matrix in the trabeculae extended from the growth plate, but cartilage remained in the distal trabeculae. Osteoblasts were observed at the bone surface via immunohistochemical detection with an anti-RUNX2 antibody. Whole-genome sequencing identified a homozygous missense mutation (valine to methionine) in the zinc-dependent metalloprotease do

Small molecule inhibition rescues the skeletal dysplasia phenotype of Trpv4 mutant mice.

The TRPV4 skeletal dysplasias are characterized by short stature, short limbs with prominent large joints, and progressive scoliosis. They result from dominant missense mutations that activate the TRPV4 calcium permeable ion channel. As a platform to understand the mechanism of disease and to test the hypothesis that channel inhibition could treat these disorders, we developed a knock-in mouse that conditionally expresses the p.R594H Trpv4 mutation. Embryonic, chondrocyte-specific induction of the mutation using Col2a1-Cre resulted in a skeletal dysplasia affecting the long bones, spine, and c

Pyle disease - Functional validation of disease-causing missense variants in SFRP4/sFRP4.

Pyle disease is a rare autosomal recessive bone dysplasia characterized by a modeling defect of the tubular bones resulting in metaphyseal widening and cortical thinning, and bone fragility with fractures. Bi-allelic truncating variants in SFRP4 have been described as the primary molecular cause for Pyle disease. One report described a family with Pyle disease with compound heterozygous missense variants in SFRP4, yet no functional studies were performed. SFRP4 encodes the secreted Frizzled related protein 4 (sFRP4), which acts as a negative regulator of WNT signaling through the binding with

Why was the concordance rate of imaging and clinical diagnosis in cemento-osseous dysplasia low? A retrospective study of 55 cases.

OBJECTIVE: The World Health Organization (WHO) recommends that cemento-osseous dysplasia (COD) be diagnosed based on imaging findings, avoiding biopsy when possible. This study aimed to evaluate the concordance between imaging and clinical diagnoses in 55 COD patients, and to identify key clinical and radiographic features that may help prevent unnecessary biopsy or inappropriate treatment. METHODS: Between September 1, 2017, and August 31, 2018, 55 patients diagnosed with COD were randomly selected from the imaging database of West China Hospital of Stomatology. Two radiologists reviewed all

The PERK inhibitor GSK2606414 evokes developmental defects in zebrafish consistent with Wolcott-Rallison syndrome phenotypes.

BACKGROUND: Protein kinase RNA-like endoplasmic reticulum kinase (PERK) is an endoplasmic reticulum stress kinase whose loss of function disturbs human development, leading to skeletal dysplasia and permanent neonatal diabetes, as in the Wolcott-Rallison Syndrome (WRS). The lack of effective, less invasive therapies for developmental diseases highlights the need for animal models that replicate complex pathological phenotypes, while allowing scalable drug screening. Zebrafish high fecundity and rapid development enable efficient in vivo drug testing. We assessed zebrafish’s potential for study

Deep Learning on Histology Images for Differentiating Fibro-osseous Lesions of the Jaw.

Fibrous dysplasia (FD), cemento-ossifying fibroma (COF), and cemento-osseous dysplasia (COD) are fibro-osseous lesions of the jaw that share histologic features, making their differentiation in routine pathology practice challenging. Distinguishing these conditions is crucial for clinical management due to their varying prognoses. To address these limitations, we developed and validated a deep learning model using 1,218 hematoxylin and eosin whole slide images from 338 cases (FD, n = 130; COF, n = 146; COD, n = 62) across 3 institutions. After digital scanning and image preprocessing, we compa

UK consensus guidelines for multidisciplinary care of children and young people with achondroplasia: a modified Delphi process.

BACKGROUND: Achondroplasia (ACH), the most common skeletal dysplasia, arises from gain-of-function variants in the fibroblast growth factor receptor 3 gene. Children with ACH experience lifelong medical, functional and psychosocial challenges requiring coordinated and anticipatory care. Although international guidance exists, the UK lacks national clinical care recommendations specific to its healthcare systems. OBJECTIVE: To develop UK-specific, multidisciplinary clinical recommendations for the care of children and young people (CYP) with ACH. METHODS: The UK Achondroplasia Network develop
