Pulmonary hypertension (PH) in congenital diaphragmatic hernia (CDH) is nearly universal yet incredibly complex. A patent ductus arteriosus (PDA) is increasingly utilized to both monitor echocardiographically and manage CDH-PH. However, as pulmonary pressures are driven by both vascular resistance and blood flow, there is a risk that as the former improves, PH may in fact worsen from pathologic left-to-right (LtR) PDA shunting. Herein, we describe two cases of neonatal CDH whose PH was complicated by pathologic LtR PDA shunting and pulmonary overcirculation and improved following resolution of

BACKGROUND: Acute kidney injury (AKI) is a frequent complication in critically ill neonates and is associated with adverse outcomes. Infants with congenital diaphragmatic hernia (CDH) are particularly vulnerable due to pulmonary hypoplasia, hemodynamic instability, and exposure to nephrotoxic agents.
METHODS: We retrospectively analyzed 193 neonates with CDH treated at a tertiary referral center (2012-2021). AKI was graded using modified pediatric RIFLE (pRIFLE), neonatal KDIGO (nKDIGO), and CysC-based neonatal AKI (CyNA) criteria, disregarding the urine output criteria. Clinical variables in

INTRODUCTION: The mediastinal shift angle (MSA) estimates the lateral displacement of the heart in the fetal thorax. This study aimed to evaluate the MSA's performance in predicting neonatal mortality in fetuses with left isolated congenital diaphragmatic hernia (CDH) as an isolated marker and in combination with other ultrasound and MRI predictors.
METHODS: MSA was obtained by ultrasound in a cross-sectional four-chamber view of the thorax in 96 fetuses with left isolated CDH. MSA was calculated by drawing two lines from a common point from the skin edge posterior to the spinous process of t

Background: Congenital diaphragmatic hernia (CDH) is a rare but serious congenital anomaly linked to high mortality rates and significant long-term morbidity. Although numerous prognostic factors for short-term outcomes have been identified through hospital-based studies, data on long-term mortality at the population level are limited. Specifically, nationwide assessments of long-term outcomes for infants with CDH are scarce. This study aimed to estimate the national 5-year all-cause mortality for CDH and to create a population-level risk stratification nomogram utilizing nationwide health ins

INTRODUCTION: The objectives of this study were to evaluate the feasibility of slice-to-volume registration (SVR) of the fetal thorax in congenital diaphragmatic hernia (CDH) and to compare observed-to-expected total fetal lung volume (O/E TFLV) obtained from original and SVR MRI volumes for prediction of postnatal survival.
METHODS: We retrospectively analyzed 35 fetuses with CDH undergoing MRI before and after fetoscopic endoluminal tracheal occlusion (FETO). High-resolution, motion-corrected SVR volumes were reconstructed using an automated pipeline. O/E TFLV was measured on both original

PURPOSE: Provide a quantitative summary of parent- and child-reported quality of life (QoL) outcomes in children with congenital diaphragmatic hernia (CDH).
METHODS: A systematic search was conducted in MEDLINE, Cochrane, and Embase databases from inception to October 25, 2024. Studies that examined child and parent-reported quality of life outcomes in pediatric patients with CDH were included. Screening and data extraction was conducted by two reviewers. The primary outcome was the reported overall QoL, and the secondary outcomes included QoL of physical, emotional, social, psychosocial, and

BACKGROUND: Severe and persistent pulmonary hypertension (PH) predicts mortality and short-term pulmonary morbidity in infants with congenital diaphragmatic hernia (CDH). Intrathoracic liver herniation (liver-up) is a predictor of survival and increased need for extracorporeal membrane oxygenation. There is a paucity of data on the timeline of PH resolution in infants with and without liver herniation. We aim to determine the time to PH resolution in infants with liver-up and liver-down CDH, and to describe the resource utilization among these infants.
METHODS: We conducted a retrospective st

INTRODUCTION: The impact of hernia sac (HS+) on outcomes in congenital diaphragmatic hernia (CDH) when accounting for defect size is unknown. In a multicenter analysis, considering defect size, we aim to delineate the association of HS+ and morbidity and mortality.
METHODS: A retrospective analysis of CDH Study Group (2007-2024) data was performed. Demographics, defect characteristics, morbidity, and mortality were abstracted. Multilevel regression and parallel Bayesian analysis were performed on 80 % of the data, tested on 20 % hold-out data, and reported in area under the curve (AUC).

INTRODUCTION AND IMPORTANCE: Transdiaphragmatic intercostal hernias (TDIHs) are rare and typically posttraumatic. The coexistence of a TDIH with congenital anomalies, such as a Morgagni hernia (MH) and chest wall defect, is exceptionally uncommon. This report details a unique case of MH associated with an ipsilateral intercostal hernia in an adult, underscoring key diagnostic and surgical challenges.
CASE PRESENTATION: A 71-year-old man with multiple comorbidities presented with acute abdominal pain and a right-sided chest wall bulge. Computed tomography revealed a large right lateral diaphra

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a severe neonatal condition frequently requiring patch repair, yet the optimal prosthetic material remains contested. Synthetic patches offer structural durability but are associated with foreign body reactions, while biologic patches may promote tissue integration but raise concerns regarding long-term recurrence.
METHODS: We conducted a systematic review and meta-analysis of observational studies comparing biologic versus synthetic patch repair in neonates with CDH. Databases were searched through May 2025 per PRISMA guidelines. The prima

Congenital diaphragmatic hernia (CDH) is a rare and potentially life-threatening condition caused by incomplete diaphragm formation, allowing abdominal organs to herniate into the thoracic cavity. This typically results in pulmonary hypoplasia and, rarely, cardiac compression with subsequent cardiopulmonary collapse. We report the emergency management of a three-week-old term neonate with a previously undiagnosed left-sided Bochdalek hernia who presented at an external hospital with cardiac arrest and mediastinal shift due to massive obstructing enterothorax. Following stabilization and transf

OBJECTIVES: To define clinically the initial (<24 hour) hemodynamic phenotypes (no/mild pulmonary hypertension [PH], precapillary PH, and postcapillary PH) in neonates with congenital diaphragmatic hernia (CDH) by echocardiography, and to assess their relationship with clinical outcomes.
STUDY DESIGN: This was a retrospective, single-center, observational study that included all neonates with CDH with an echocardiogram in the first 24 hours from 2018 to 2025. Phenotypes were categorized via an a priori-defined algorithm, characterized by clinical and echocardiography indices and were evaluate

Despite ample research in acute pulmonary hypertension in neonates with congenital diaphragmatic hernia (CDH) leading to improved survival, there is a paucity of literature on the physiology of chronic pulmonary hypertension in this population. Serial hemodynamic assessments and physiology-centric care have been shown to be safe and effective in the acute transitional and peri-operative care of these neonates and will likely be informative in the discussion of chronic pulmonary hypertension as well. Herein, we discuss the epidemiology, contributors, and proposed physiology-based phenotypes of

Congenital diaphragmatic hernia (CDH) is a rare condition, with many affected patients remaining asymptomatic, while others may present with non-specific respiratory manifestations. The coexistence of a CDH and a giant hiatal hernia is particularly uncommon. We report the case of a pediatric patient presenting with both a giant hiatal hernia and a CDH, accompanied by distinctive facial features. The initial presentation included poor oral intake and abnormal chest sounds, as observed by the patient's mother. A diagnostic evaluation using chest radiography and computed tomography confirmed the
