INTRODUCTION: Fetoscopic endoluminal tracheal occlusion (FETO) has demonstrated neonatal survival and pulmonary hypertension (PH) benefits in patients with severe congenital diaphragmatic hernia (CDH). This study describes early childhood outcomes of patients treated with FETO compared to a contemporary cohort of non-FETO patients.
METHODS: A single center, retrospective cohort study evaluated patients with a prenatal diagnosis of isolated severe left CDH at a single center from September 2016 through January 2023. Severe CDH was defined as liver herniation with an ultrasound observed/expecte

INTRODUCTION AND IMPORTANCE: Congenital diaphragmatic hernia (CDH) is a rare condition in adults, often presenting with nonspecific respiratory symptoms and is easily misdiagnosed. CDH results from pleuroperitoneal canal closure failure during gestation, with Bochdalek hernias comprising the majority and Morgagni hernias (MHs) being rare. This case underscores the importance of considering CDH in adults with respiratory symptoms, especially following trauma.
CASE PRESENTATION: A 50-year-old woman presented with a 4-month history of intermittent dyspnea worsened by physical exertion, abdominal

In this issue of Developmental Cell, Luna et al.1 identify myelin regulatory factor (MYRF) as a critical determinant of lung mesothelial specification and quiescence. Their study demonstrates that MYRF loss leads to congenital diaphragmatic hernia or a pleuroparenchymal fibroelastosis-like phenotype, revealing the mesothelium as an actively restrained progenitor population.

BACKGROUND: Infants with congenital diaphragmatic hernia (CDH) remain at high risk for ECMO and mortality. We previously reported improved outcomes after implementing new CDH care guidelines. This study reassesses ECMO rates, survival, and guideline adherence to determine if improvements persisted.
METHODS: Retrospective review of all neonatal CDH cases at a single center during pre-guideline (2003-2015, n = 229) and post-guideline (2016-2024, n = 160) periods, with post-guideline subdivided into two epochs: post-1 (2016-2019, n = 70) and post-2 (2020-2024, n = 90).
RESULTS: Survival without

We conducted a retrospective cohort study of 29 499 neonatal cases requiring extracorporeal life support (ECLS) at 237 centers from 2000 to 2023; there were 6747 infants with congenital diaphragmatic hernia (CDH) and 22 752 without CDH. Infants with CDH treated at ECLS centers with low total neonatal volumes (<28.5 total cases) had increased odds of inpatient mortality compared with those at high-volume centers (> 140 total cases, adjusted odds ratio [aOR]: 1.9; 95% confidence interval [CI]: 1.2-2.9). Similarly, centers with low non-CDH ECLS volumes were associated with higher odds of mortalit

OBJECTIVE: This study aimed to analyze the intraindividual development of lung perfusion in children with congenital diaphragmatic hernia (CDH) at the ages of 2 and 10 years, as well as to investigate prenatal and postnatal influencing factors.
MATERIALS AND METHODS: Fifty-nine children after CDH were examined as part of a monocentric follow-up program using dynamic contrast-enhanced MRI (DCE-MRI) at 2 years (hereafter referred to as Examination 1, E1) and again at 10 years of age (Examination 2, E2). Pulmonary blood flow (PBF) and pulmonary blood volume (PBV) were quantified separately for e

PURPOSE: Circular RNAs (circRNAs), generated by backsplicing, are stable closed-loop RNAs. CircRNAs in amniotic fluid (AF) may serve as diagnostic and prognostic markers for Congenital Diaphragmatic Hernia (CDH). This study profiled AF circRNAs from CDH pregnancies to explore associations with CDH pathways and survival. METHODS: AF was collected from CDH pregnancies undergoing fetoscopic endoluminal tracheal occlusion (FETO) (n = 8; 4 survivors, 4 non-survivors). CircRNAs were profiled using the Arraystar Human circRNA Array V2. Differential expression was analyzed with limma in R and compared

KMT2D, the histone methyltransferase and core component of the COMPASS/MLL4 complex, has been implicated in developmental diseases such as Kabuki Syndrome, interstitial lung disease, and congenital diaphragmatic hernia, with clear links to pediatric pulmonary disorders. Despite this, the mechanism by which KMT2D governs lung development remains unclear. Knock-in mouse models rendering, KMT2D catalytically deactivated (KMT2D KI ) and reducing H3K4 methylation, have demonstrated potential in defining KMT2D's role in pulmonary development. Our examination of the lungs of KMT2D KI mice revealed in

BACKGROUND: Previous studies suggest discrepancy between North American and European centres in use of extracorporeal life support (ECLS) in infants with congenital diaphragmatic hernia (CDH). The impact of this on outcomes is unknown. We aimed to compare indications, management and outcomes of infants with CDH receiving ECLS between North American and European centres.
METHODS: ECLS organisation (ELSO) prospective registry study including infants with CDH receiving ECLS over 11 years starting January 2012. Outcomes were mortality, ECLS-related complications and length of inpatient stay. Prop

BACKGROUND: In congenital diaphragmatic hernia, postoperative right-left asymmetry of pulmonary perfusion affects outcome. Although pulmonary perfusion scintigraphy (PPS) is widely used to assess pulmonary perfusion, it requires the use of radionuclides and peripheral venous access. In contrast, dynamic digital radiography (DDR) visualizes and analyzes pulmonary perfusion by extracting subtle changes in X-ray attenuation from sequential images acquired using a pulsed X-ray generator and a flat-panel detector.
OBJECTIVE: This study aimed to evaluate the accuracy of DDR for assessing pulmonary

There is a strong genetic contribution to the etiology of congenital diaphragmatic hernia (CDH). This study evaluated genetic testing results and diagnostic yield for fetuses and children with CDH. This was a retrospective cohort study of exome sequencing (ES) performed at GeneDx for fetuses and children ≤ 18 years of age with CDH compared with genome sequencing (GS) from an institutional cohort. ES was available for 560 individuals (62% [n = 349] complex/syndromic; 38% [n = 211] isolated/non-syndromic) with a reportable finding identified in 61% (n = 342) overall, including 64% in complex/syn

Retrospective studies comparing NAVA to assist control ventilation (ACV) in neonates with congenital diaphragmatic hernia (CDH) have shown that ventilatory mode may improve respiratory parameters. The aim of this study is to determine if infants with CDH studied post-operatively had a lower oxygenation index (OI) on NAVA compared to ACV. This dual-centre randomised cross-over trial compared post-operative NAVA with ACV in infants with CDH. Infants were randomised to receive either NAVA or ACV first in a 1:1 ratio for a 4-h period. At the end of each 4-h period, blood gas analysis was performed

BACKGROUND AND OBJECTIVE: Pediatric post-operative congenital diaphragmatic hernia (CDH) patients have been shown to encounter reduced pulmonary function tests (PFT) potentially leading to respiratory symptoms. Strategies involving exercise have been used to improve PFT. This meta-analysis aims to determine the effectiveness of exercise in improving lung function in pediatric post-operative CDH patients.
METHODS: An electronic search was done on May 2023 in MEDLINE via Pubmed, Cochrane Library, Embase, ClinicalKey, Scopus, Google Scholar, and Herdin Plus, using the search terms "exercise" and

BACKGROUND: LONP1 encodes an ATP-dependent protease essential for maintaining mitochondrial homeostasis. LONP1 variants have been associated with cerebral-ocular-dental-auricular-skeletal anomalies syndrome, pediatric cataract, congenital diaphragmatic hernia, and neurodevelopmental disorders; moreover, these variants can be inherited in both autosomal recessive and autosomal dominant modes.
METHODS: We conducted a retrospective analysis of the clinical data and genetic test results of a Chinese boy diagnosed as having mitochondrial encephalopathy. Subsequently, we evaluated the pathogenicity

Children with congenital diaphragmatic hernia (CDH) may require cardiac catheterization for evaluation of pulmonary hypertension. The extent of abnormal systemic venous return complicating access to the right heart in this population has not been well described. We identified children with CDH who underwent cardiac catheterization at the University of California, San Francisco, at less than or equal to 2 years from 2006 to 2025. Non-invasive imaging, catheterization reports, and angiography were reviewed. Nine children with right CDH (three with hepatopulmonary fusion) and twenty-one children
