Congenital diaphragmatic hernia is a rare malformation with a prevalence of approximately 1 in 3,000 births. Pulmonary development during fetal life is variably impaired, and newborns typically present pulmonary hypoplasia, often associated with pulmonary hypertension. In France, in 2008, the creation of the Rare Diseases Reference Center (CRMR) for Congenital Diaphragmatic Hernia, affiliated with the Rare Disease Network FIMATHO (Program for Rare Abdominothoracic Diseases), enabled the establishment of a territorial network ensuring relatively homogeneous expertise across the country. Congenital diaphragmatic hernias (CDHs) are developmental defects often found in infancy; however, they may be rarely found as a delayed diagnosis in adults. Although often an incidental finding, adult patients with preexisting CDH who present with symptoms of respiratory distress pose unique challenges in airway management. Complications include gastric insufflation, mechanical compression of the surrounding viscera, exacerbation of respiratory distress, and failed ventilatory weaning. We present the case of a 70-year-old female with acute hypoxic respiratory failure requiring mechanical vent

PURPOSE: Early prognostic markers for pediatric patients receiving extracorporeal membrane oxygenation (ECMO), particularly those with congenital diaphragmatic hernia (CDH), remain poorly defined. This study aimed to evaluate pulse pressure (PP) as an early hemodynamic indicator of mortality in neonates with CDH undergoing veno-arterial (VA) ECMO.
METHODS: We retrospectively analyzed newborns who underwent VA ECMO as primary rescue therapy for isolated CDH. Perinatal characteristics, arterial blood gas parameters, and blood pressure (BP) during the first 48 h post-ECMO were evaluated and comp

INTRODUCTION: The detrimental effect of congenital heart disease (CHD) on congenital diaphragmatic hernia (CDH) survival is well known. However, estimates of survival and of extracorporeal membrane oxygenation (ECMO) utilization in CDH associated with critical congenital heart defects (CCHD) have not been described. Accurate data will enable prognostication for prenatal counseling and possibly expansion of the indications for fetal intervention.
METHODS: The Pediatric Health Information System (PHIS) was queried for all infants born with CDH between 2016 and 2024 using ICD-10 diagnostic codes

BACKGROUND: Congenital diaphragmatic hernia (CDH) remains a challenging neonatal surgical condition associated with significant morbidity and mortality. Thoracoscopic repair has emerged as a minimally invasive alternative to conventional open repair in selected patients.
AIM: The aim of this study is to share our experience in the surgical management of CDH and to compare postoperative outcomes between thoracoscopic repair and open laparotomy.
MATERIALS AND METHODS: A retrospective analysis of six patients with CDH managed surgically at a tertiary care center was performed. Demographic data,

Congenital diaphragmatic hernia (CDH) is a rare anomaly with an incidence of 1 in 3000 live births and characterized by defective closure of the diaphragm, lung hypoplasia, and pulmonary vascular remodeling. Although several genes associated with the occurrence of CDH, no clear genetic component has been identified. Previously, we showed that CDH patients have vascular abnormalities already early in development, and that perivascular cells are affected in CDH. In the current study, we focused on early abnormalities in the nitrofen-induced CDH mouse model. Transcriptome analysis of FACS-sorted

OBJECTIVE: Identify how a core congenital diaphragmatic hernia (CDH) team coupled with early recognition and treatment of cardiac dysfunction changed outcomes for infants with CDH.
METHODS: A retrospective chart review compared a historical cohort from 2015-2019 (n = 51) with a post-guideline cohort from 2020-2025 (n = 53). Statistical analysis was performed with Fisher's exact, unpaired t-test, and Mann Whitney U-test.
RESULTS: Survival improved from 68.6% to 88.7% (p = 0.016) and ECMO utilization decreased, 51.0% to 26.4% (p = 0.015). Infants with cardiac dysfunction had increased survival

The diaphragm is a physical barrier separating the thoracic and abdominal cavities. Its physiological function is fundamental to pulmonary ventilation. Congenital diaphragmatic hernia (CDH) is a malformation that leaves a hole in the diaphragm during fetal development. Synthetic nondegradable membranes are currently used for the repair of diaphragm holes. However, these membranes do not help cells to adhere and proliferate. There is a need for biodegradable membranes composed of muscular and fibroblast cells to replicate a simplified diaphragmatic tissue. We thus developed a biocompatible and

OBJECTIVE: To systematically map and synthesize the available evidence on the use of vasopressin for hemodynamic support in neonates.
STUDY DESIGN: We conducted a scoping review to synthesize available data on indications, dosing, clinical effects, and safety.
RESULTS: Twelve studies were included (one randomized trial, eleven observational), encompassing heterogeneous neonatal populations. Vasopressin was primarily used as rescue therapy in refractory shock, including sepsis, persistent pulmonary hypertension of the newborn, and congenital diaphragmatic hernia. Dosing regimens varied widely

Acquired diaphragmatic hernia (ADH) is a rare postoperative complication in pediatric patients and may cause abrupt respiratory and hemodynamic deterioration due to intrathoracic herniation of abdominal organs. We report the anesthetic management of a 17-month-old child who developed early postoperative gastric herniation after thoracoscopic resection of pulmonary sequestration. The patient presented with acute respiratory distress, hypoxemia, and mediastinal shift. Anesthetic management included rapid-sequence induction, invasive arterial monitoring, and lung-protective pressure-controlled ve

The morphogenesis of the diaphragm requires a coordinated development of different components with potential of malformation. Diaphragmatic herniation has rarely been reported in large animals and has not been described in a rhinoceros. An 8-day-old- female southern white rhinoceros calf was diagnosed with congenital diaphragmatic hernia with large parts of the intestine translocated in the thoracic cavity. Gross necropsy revealed bilateral Bochdalek hernia, severe damage of the intestinal tract and congestion of both lungs. Histologic findings demonstrated lung hypoplasia and severe pneumonia

INTRODUCTION: Mouse models of congenital diaphragmatic hernia (CDH) are confounded by the teratogenic effects of nitrofen. Large animal surgical models are created later in lung development, making them less suitable to study the pathogenesis of pulmonary malformation. Therefore, we developed a surgical murine model of CDH that does not have the limitations of other models. Here we describe the feasibility of this model.
METHODS: The fetuses of E16.5 pregnant C57BL/6 mice underwent thoracotomy and creation of a defect in the left hemidiaphragm. Littermates served as controls. A sham group und

First-trimester fetal abdominal cysts at the right thoracoabdominal border are typically regarded as benign findings. We report three cases in which such cysts, initially attributed to hepatic or pulmonary origin, subsequently evolved into right-sided congenital diaphragmatic hernia (CDH), providing additional sonographic evidence for a previously under-recognized pattern. Notably, cyst regression in the second trimester coincided with-rather than preceded-the development of intrathoracic hepatic herniation. Recognition of this pattern may facilitate earlier CDH diagnosis and improve perinatal

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a complex anomaly with high rates of morbidity and mortality. Successful management depends on interprofessional and multidisciplinary collaboration. Multiple previous publications have underscored the effect that center-specific factors can have on CDH outcomes. However, beyond institutional differences, optimal outcomes also rely on a well-composed team with both surgical and critical care expertise. Understanding the center effect requires identifying key drivers, with team structure being a major factor. While surgeons and neonatologists

Intracardiac thrombus is a rare but difficult-to-treat complication in premature infants, with limited therapeutic options. If an urgent neonatal surgical procedure is needed, rescue systemic thrombolysis may be considered as an option to hasten thrombus resolution. In the present case, we illustrate the successful lysis of a right ventricle intracardiac thrombus in a premature child prior to a neonatal emergent surgery for a severe congenital diaphragmatic hernia. The 1-day-old premature child with right-sided diaphragmatic hernia contracted preoperatively a right ventricle thrombus in the co

BACKGROUND: Research models of congenital diaphragmatic hernia (CDH) have evolved over time. We aimed to evaluate global research activities in CDH models using bibliometric analysis over the past three decades.
METHODS: Studies were retrieved from the Web of Science Core Collection (WoSCC) database between 1995 and 2024. Microsoft Excel, GraphPad Prism, and VOSviewer were applied to collect and analyze the bibliometric variables of this field.
RESULTS: A total of 302 original publications from 21 countries were identified. The nitrofen rat model (n = 187) was used in most studies, followed

Congenital oral synechiae are rare fibrous bands connecting the maxillary and mandibular structures. We report a male neonate referred to our oral and maxillofacial surgery service at 16 days of age with bilateral anterior and posterior oral synechiae, an atypical submucous cleft palate, and a concurrent left-sided congenital diaphragmatic hernia. The synechiae were undetected prenatally and obstructed neonatal intubation in the delivery suite, compounding the immediate airway demands of the diaphragmatic hernia. This triad has not been previously described, and comprehensive genetic testing i
