Mechanical compression causes lung hypoplasia in congenital diaphragmatic hernia with GATA4 genetic variants.

Congenital diaphragmatic hernia (CDH) is a common and severe structural malformation in which the high rate of morbidity and mortality is caused by lung hypoplasia and pulmonary hypertension. The severity of lung and pulmonary vascular defects in patients with CDH is heterogeneous with both intrinsic defects during development and mechanical compression playing important roles. Genetic variants have been identified in 30% of CDH patients and are associated with increased morbidity and mortality, but it is unclear how these variants impact lung and pulmonary vascular defect severity. Deletions

Intra-Diaphragmatic Extralobar Pulmonary Sequestration in an Adult With Laparoscopic Resection.

Intra-diaphragmatic extralobar pulmonary sequestration (IDEPS) is a rare congenital anomaly that is exceedingly uncommon in adults. Herein, we report an adult case of incidentally detected IDEPS. Computed tomography revealed a 49-mm mass beneath the left diaphragm with internal calcifications; however, preoperative diagnosis and precise localization were challenging. The differential diagnoses included extralobar pulmonary sequestration, bronchogenic cyst, and other tumors. Diagnostic surgical resection was planned with a laparoscopic approach based on imaging findings. Intra-operatively, the

A Rare Case of an Abdominal Pedunculated Bronchogenic Cyst.

Bronchogenic cysts (BC) are rare congenital malformations that result from abnormal budding of the primitive foregut. Most of these cysts are located in the mediastinum or the lungs. Rarely, abdominal BCs result from embryonic budding migrating below the diaphragm prior to the fusion of the pleuroperitoneal membranes. We present a case of a 34-year-old male patient undergoing evaluation for bronchopneumopathy. Preoperative imaging revealed a well-circumscribed cystic lesion adjacent to the colon, which failed to provide a definitive diagnosis due to its atypical location and nonspecific radiol

Intrapulmonary Bronchogenic Cyst Adjacent to the Inferior Pulmonary Ligament Mimicking a Paraesophageal Mediastinal Cyst: A Case Report.

Bronchogenic cysts are rare congenital foregut malformations that most commonly arise in the mediastinum. Intrapulmonary bronchogenic cysts are less frequent and may present diagnostic challenges when located adjacent to mediastinal structures. We report a case of an intrapulmonary bronchogenic cyst that closely mimicked a paraesophageal mediastinal cyst on preoperative imaging. A 72-year-old man was referred to our department after a cystic lesion was incidentally detected during a routine health examination. CT demonstrated a 37-mm well-circumscribed cystic lesion in the right lower thorax.

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Toward trustworthy water quality monitoring under sensor unreliability.

BACKGROUND: The influence of pre-pregnancy body mass index (BMI) and gestational weight gain (GWG) on perinatal outcomes of women with polycystic ovary syndrome (PCOS) remains unclear. Therefore, we explored how the above indicators influence pregnancy outcomes in women with PCOS. METHODS: A retrospective study was conducted involving the baseline characteristics, laboratory data, and pregnancy outcomes of 722 pregnant women with PCOS. Subjects were grouped in a way to find out risks in their pregnancy outcomes. Multivariable logistic regression analysis was performed to investigate how BMI a

Ultrasonographic diagnosis of secondary cardiac position alterations led by fetal thoraco-abdominal anomalies: a report of three cases.

BACKGROUND: Congenital diaphragmatic hernia (CDH) and congenital diaphragmatic eventration (CDE) are rare congenital anomalies. They occur in approximately one in 2,000-3,000 and 2,500-5,000 children respectively. It is difficult to distinguish between CDE and right diaphragmatic hernia in fetal period, which can cause changes in the position of the heart and lead to dysplasia of the right lung; absence of the right lung can lead to secondary dextrocardia. These are common in dextrocardia and lung agenesis, which are the main obstacles for diagnosis. CASE DESCRIPTION: This paper reports on th

Impaired myocardial deformation and aortic distensibility by cardiac MRI in girls with Turner syndrome.

Girls and women with Turner syndrome (TS) suffer from increased risk of cardiovascular diseases. We hypothesized that left ventricular (LV) myocardial strain and aortic elasticity will be impaired in girls with TS. Cardiac MRI of 45 girls with TS and 14 healthy control girls was performed. Tissue tracking was used to assess LV Global Longitudinal (GLS), circumferential (GCS), radial short and long axes (GRS SAX and GRS LAX) in patients compared to controls. Maximal and minimal aortic areas were measured in ascending aorta, proximal descending, and aorta at diaphragm. Regional strain and disten

Epithelial Dysfunction in Congenital Diaphragmatic Hernia: Mechanisms, Models and Emerging Therapies.

Congenital diaphragmatic hernia (CDH) is a complex disorder whereby improper formation of the diaphragm allows herniation of the internal organs into the thoracic cavity, resulting in pulmonary hypoplasia among other complications. Although epithelial dysfunction is central to CDH pathology, relatively little attention has been paid to the underlying mechanisms orchestrating epithelial malfunction. Proinflammatory signaling downstream of impaired mechanotransduction due to in utero lung compression has been elucidated to drive epithelial cell phenotypes. This has been illustrated by a reductio

Three-dimensional MRI reconstruction of diaphragmatic defects in fetuses with congenital diaphragmatic hernia and mortality prediction.

OBJECTIVES: This study aimed to evaluate diaphragmatic defects in fetuses with congenital diaphragmatic hernia (CDH) using three-dimensional (3D) MRI reconstruction for visualization and quantification, and to investigate its correlation with neonatal mortality. MATERIALS AND METHODS: In this retrospective diagnostic accuracy study, fetuses with prenatally diagnosed left-sided CDH (LCDH) underwent 3D reconstruction of the diaphragm and defect using ITK-SNAP software. The defect-to-diaphragm ratio (DDR) was calculated from measured sizes of the diaphragm and defect. We assessed the correlation

DISSECTING THORACIC AORTIC ANEURYSM WITH CLASSIC IMAGING FINDINGS AND REVIEW OF LITERATURE.

BACKGROUND: Thoracic aortic aneurysm is an increasingly recognized condition that is sometimes diagnosed incidentally on imaging examinations performed to evaluate other unrelated conditions. Symptomatic presentations of thoracic aortic aneurysms may be due to mass effect on the airway, esophagus, recurrent laryngeal nerve and the thoracic spine. Alternatively, they may present with the dreaded complication of aortic dissection or rupture.Occasionally abnormalities of the aortic contour or size can be detected on routine chest x-ray. However, it is difficult to confidently diagnose thoracic ao

Peritoneo pericardial hernioplasty in a 2-month-old Shih Tzu.

BACKGROUND: Peritoneopericardial hernias (PPHs) are congenital malformations characterized by continuity defects between the diaphragm and the peritoneum that allow the transposition of abdominal organs into the thoracic cavity. These malformations can lead to clinical, gastroenterological, and respiratory problems, such as vomiting, dyspnea, and tachypnea. They can be congenital, and patients develop clinical signs at a young age or can be diagnosed as incidental when no complications occur. Furthermore, other malformations, such as cryptorchidism and cardiac interventricular communication, c

Incidental finding and surgical management of a diaphragmatic macrocystic lymphatic malformation in a child.

Macrocystic lymphatic malformation, previously known as cystic hygroma, is a congenital anomaly and a subtype of lymphatic malformations. The most common location for macrocystic lymphangiomas is the neck, followed by the axilla. On rare occasions, it can be found in other organs and tissues. Our case presented with a large diaphragmatic macrocystic lymphangioma. Our case is about an incidentally discovered right subdiaphragmatic abdominal mass in a normally healthy middle childhood patient. The patient was treated surgically by resection of the mass. Even though the diaphragm is a rare locati
