Total management of short gut secondary to midgut volvulus without prolonged total parenteral alimentation.

Absorption studies in rats have shown that intestinal adaptation after catastrophic injury can be stimulated by early enteral feeding. Using this concept, we have devised a technique of early initiation and advancement of oral feedings that begins with Cho-Free and Polycose and gradually adds sucrose and MCT in increasing proportions. The increasing complexity and caloric density of this diet provide sufficient nutrition to allow weaning from total parenteral alimentation within 2--3 wk. Our preliminary experience in babies with midgut volvulus, necrotizing enterocolitis, and gastroschisis has

[Cholestatic jaundice and hypophosphataemia in parenterally-fed premature infants--coincidence or causal connection? (author's transl)].

A report is presented of the chemical pathological findings in 14 premature and one full-term infant receiving almost exclusively parenteral nutrition during the first two weeks of life. Six infants developed cholestatic jaundice. The underlying diseases were the idiopathic respiratory distress syndrome in 10, gastroschisis in 3 and renal insufficiency in 1, while one was an otherwise healthy small for dates infant. After parenteral nutrition had been discontinued obstructive jaundice cleared by the third month of life except in one infant. Apart from the higher glucose intake during the secon

A pneumatic reduction device for gastroschisis and omphalocele.

A new preformed pneumatic reduction device is available in two sizes, and it is recommended that a set be available for emergency use in any hospital which deals with the definitive emergency therapy of the newborn. The devices can be washed and flash sterilized as needed and, if properly cared for, can be reused repeatedly. The bag has facilitated the early care of newborns with omphalocele and gastroschisis and has allowed the rapid and effective reduction of eviscerated intestines. These bags can also be used for the controlled ventral hernias necessary in the treatment of infants born with

The role of the silastic prosthesis in the management of gastroschisis.

We reviewed data on 43 patients with gastroschisis treated at The Hospital for Sick Children, London, between 1959 and 1975. In the past five years, the overall mortality has been reduced from 76% to 33%. The introduction of the Silastic silo has been the most important single factor in this improvement. Mortality may be further reduced by achieving all-layer closure following thorough bowel decompression and stretching of the abdominal wall.

Gastroschisis treated with lyophilized dura.

At the Royal Alexandra Hospital for Children, Sydney, we have found that primary closure in gastroschisis is usually successful. However, in a small number of cases such closure is found to be impossible. In such patients we have not been satisfied with silastic sheeting and have on two occasions successfully used lyophilized dura.

Gastroschisis complicated by intestinal atresia.

Gastroschisis complicated by intestinal atresia is a complex problem. Six cases are presented. A review of the literature and of our own cases shows a high mortality rate. Success or failure is related more to the pathology present than to any specific method of operative management. Resection and primary anastomosis is the favored method of treatment, as the intestine heals well in spite of its appearance. Primary closure of the abdominal wall musculature and skin is done whenever possible. A gastrostomy is used uniformly. Intravenous hyperalimentation is critical to survival of these babies

[First aid measures in emergency pediatric surgery (author's transl)].

The type of first aid given to patients requiring emergency pediatric surgery is decisive for the prognosis in many cases. With this aspect in mind, individual disease pictures from the group of connatal deformities (esophageal atresia, intestinal atresia, gastroschisis, diaphragmatic hernia and defect, myelomeningocele), from emergency surgical situations beyond the neonatal stage (acute abdomen and ileus, esophageal varices, pneumothorax), and accident injuries (blunt abdominal trauma, cranio-cerebral trauma, burns) are selected and the most important first aid measures described. But for al

Perinatal aspects of omphalocele and gastroschisis.

Forty-one infants treated for abdominal wall defects were studied retrospectively. Using established anatomic criteria the infants were identified as having either an omphalocele or gastroschisis. They were compared for maternal and infant characteristics to determine if distinct perinatal characteristics exist between the two groups. Gastroschisis patients had lower mean maternal age, were more likely to be first born, and to be of lower birth weight. Major malformations apart from those associated with the gastrointestinal tract and the anterior abdominal wall occurred 12 times more often in

Simplified treatment of large congenital ventral wall defects.

A preformed silicone polymer silo has been used in eleven infants with omphalocele and gastroschisis. This prosthesis has significant advantages over other forms of silo construction. Among these advantages are: (1) easy and rapid primary application; (2) a totally nonreactive and closed system which prevents peritoneum fluid loss and bacterial contamination; (3) a uniform constant stretching force applied to the abdominal wall which promotes expansion of the peritoneal cavity; and (4) the transparent material of the silo which permits constant evaluation of the contained viscera. These factor

Test strip meconium screening for cystic fibrosis.

The Boehringer-Mannheim Corporation (BMC) strip test is extremely reliable in indicating an albumin content above 20 mg/gm of dried meconium. All infants born during one year in 14 Milwaukee area hospitals were tested. Of 16,224 newborns, two were diagnosed correctly as suffering from cystic fibrosis and two were missed. False-positive tests were obtained in 0.9% of infants (prematurity, melena, gastroschisis, and intrauterine infection). The strip test is, at present, the best available but not the perfect screening method for cystic fibrosis.

Atresia of the colon.

Eleven infants with colon atresia have been managed by staged procedure with survival and good health in ten. Complications have been relatively few. At the initial operation the proximal atretic segment is exteriorized as an end colostomy. Such a procedure is simple and safe, results in rapid relief of the obstruction, and permits normal feeding by mouth within a few days. At a second procedure several weeks or months later, intestinal continuity is established by an end-to-end or end-to-side anastomosis. Gastroschisis has been an associated anomaly in four of these cases. This association gi

[Problems of anesthesia and recovery in neonatal surgery. Personal experience during ther period 1970-1975].

On the basis of personal experience during the past five years, the anaesthesiological and resuscitatory problems of neonatal surgery are considered, with special regard to the following pathological conditions: oesophageal atresia, diaphragmatic hernia, omphalocele and gastroschisis, neonatal peritonitis, anal atresia, intestinal atresia and stenosis. It is stated that the new post-operative care tecniques have not only improved survival percentages, but, in the case of oesophageal atresia, have made it possible to operate (radically) even in seriously immature babies with ongoing complicatio

[Infusion and nutrition problems in pediatric surgery].

The considerable differences in the body composition of infants, children and adults is responsible for special problems of infusion therapy in various age groups. Because of the narrow range of tolerance in the neonatal period special knowledge and skill is required to manage postoperative fluid therapy. Subsequently the difficulties of feeding in pediatric surgery are discussed with special reference to gastroschisis, partial resection of the small intestine, short bowel syndrome and Hirschsprung's disease. Nowadays new balanced diets and progress in parenteral therapy have improved the outc

Omphalocele.

From a historical curiosity at the turn of the century, today omphalocele and gastroschisis can be corrected with good results, thanks to innovative techniques and better understanding of the newborn. Associated anomalies consist mainly of malrotation, Meckel's diverticulum, intestinal atresia, congenital heart defects, and genitourinary abnormalities. Nonoperative management in certain centers has been sucessful for the very large omphaloceles with an intact sac; however, a secondary procedure is necessary. In the past 10 years, the use of Silastic sheeting and total parenteral nutrition has
