Temporal and spatial trends in isolated gastroschisis: a population-based study in a middle-income setting.

OBJECTIVE: To estimate the prevalence and neonatal mortality of isolated gastroschisis, assess temporal trends, identify risk factors for neonatal death, and describe its spatial distribution in a middle-income setting. METHODS: This population-based study included all live births in São Paulo State, Brazil, from 2004 to 2017. Cases of isolated gastroschisis were identified from live birth and death certificates. Temporal trends in prevalence and neonatal mortality (0-27 days) were analyzed using Prais-Winsten regression to estimate annual percent change (APC). Maternal and neonatal factors a

Sacrococcygeal teratoma associated with complex gastroschisis with favorable outcome. A case report.

INTRODUCTION: We present the case of a newborn with sacrococcygeal teratoma and complex gastroschisis, complicated with intestinal perforation. This is an unusual association, with only 1 reported case worldwide, and this is the first reported case that survived with a good outcome. CLINICAL CASE: The treatment was conducted in 4 stages: gastroschisis closure and enterostomy delaying the treatment of the atresia, teratoma resection, correction of the atresia using the Santulli technique and closure of the stoma. It took 6 months and 18 days to complete the whole treatment. The patient showed

Prenatal therapies to improve outcomes in gastroschisis: a systematic scoping review protocol.

INTRODUCTION: Gastroschisis is a congenital birth defect with a rising incidence. The herniation of intestines into the amniotic cavity leads to prenatal bowel injury, which contributes to significant postnatal morbidity and mortality. Current management focuses on postnatal interventions, though bowel injury begins in utero. Emerging interest in prenatal therapies aims to mitigate this injury and improve outcomes. However, none of these interventions has been widely adopted in clinical practice, highlighting the need for further research and validation. This protocol aims to outline the proce

Intra-abdominal Bowel Dilation in Experimental Gastroschisis Is Associated With a Modifiable Transcriptomic Program of Intestinal Dysfunction.

OBJECTIVE: To characterize intestinal transcriptional profiles in gastroschisis, their temporal evolution, and response to fetal intervention. SUMMARY BACKGROUND DATA: Gastroschisis causes significant intestinal dysfunction, with intra-abdominal bowel dilation clinically shown to correlate with worse outcomes. Although inflammation and neurovascular impairment have been implicated, genome-wide transcriptional characterization of disease severity remains lacking. METHODS: Using a fetal-ovine model of complex gastroschisis, in which all gastroschisis animals demonstrated significant intra-abdo

Development of a gastroschisis teaching model to enable surgical task sharing in Uganda.

INTRODUCTION: Gastroschisis mortality is less than 5% in high-income countries, but remains 60-95% in Africa, partly due to absence of prenatal diagnosis and delayed presentation. Midwives are usually the first point of contact and pediatricians often encounter these babies before surgeons. With training, these healthcare providers may be able to serve as task-sharers by initiating resuscitation, applying silos and directing babies to a pediatric surgeon. METHODS: A low-cost, non-perishable gastroschisis model was developed that is suitable for hands-on training. The model was piloted with nu

Epidemiology and Prognostic Factors of Three Major Neonatal Congenital Malformations in France: A 10-Year PMSI-Based Retrospective Study (2015-2024).

Congenital anomalies are a major cause of neonatal morbidity and mortality. In France, nationwide data describing surgical congenital malformations based on standardized hospital databases remain limited. To describe the epidemiology, management, and short-term outcomes of three major surgically relevant congenital malformations-gastroschisis (laparoschisis), omphalocele, and anorectal malformations-using the French national hospital discharge database from 2015 to 2024, and to identify prognostic factors for in-hospital mortality and length of stay. We conducted a retrospective PMSI-based stu

Accuracy of Preliminary Birth Defect Surveillance in Utah: A Validation Study Supporting Timely Family Referrals.

BACKGROUND: Public health surveillance is critical for understanding disease burden, yet rigorous case confirmation often introduces significant latency. The Utah Birth Defect Network (UBDN) utilizes a multistage active surveillance process that, while highly accurate, is a long and labor-intensive process. This delay creates a barrier to timely family referrals for early intervention services. This study evaluates whether preliminary surveillance data is sufficiently accurate to initiate public health actions before final clinician confirmation. METHODS: We conducted a retrospective analysis

A rare combination of gastroschisis, jejunal atresia, colonic atresia, and midgut malrotation in a newborn: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Gastroschisis is a congenital full-thickness anterior abdominal wall defect that allows bowel evisceration without a covering sac. While most cases are isolated and have good outcomes, complex gastroschisis involves associated intestinal pathology and carries significantly higher morbidity and mortality. The simultaneous occurrence of jejunal atresia, colonic atresia, and midgut malrotation in the setting of gastroschisis is extraordinarily rare. CASE PRESENTATION: A male infant was delivered at 36 weeks' gestation following an antenatal diagnosis of gastroschisis

Maternal Fish Intake in the Year Prior to Conception and Birth Defects, National Birth Defects Prevention Study, 1997-2011.

BACKGROUND: Epidemiologic data on the association between maternal fish intake and birth defects are sparse. Our objective was to assess associations between maternal fish intake and 52 different birth defects, most of which have not been assessed previously. METHODS: Using logistic regression, data was analyzed for 29,242 mothers of infants with birth defects and 10,973 mothers of control infants who participated in the National Birth Defects Prevention Study (NBDPS) and delivered between 1997 and 2011. We focused on associations between mothers who reported high fish intake of 2 or more ser

Adverse events and clinical risk factors during intrahospital transport of critically ill neonates: a prospective pilot study at a tertiary center in Vietnam.

BACKGROUND: Intrahospital transport is often unavoidable in neonatal intensive care, but it may expose critically ill neonates to physiological instability and adverse events. Data from low- and middle-income settings remain limited, and differing hypothermia thresholds across studies may underestimate the true burden of transport-related harm. METHODS: We conducted a prospective observational pilot study of intrahospital transport episodes from the neonatal intensive care unit (NICU) at a tertiary pediatric center in Ho Chi Minh City, Vietnam, between May and July 2024. Each transport episod

Maternal risk factors associated with complex gastroschisis: Cannabis exposure and recurrent urinary tract infections may be modifiable targets.

AIM OF THE STUDY: Maternal risk factors associated with gastroschisis occurrence have been identified, but predisposing factors to the complex variant are unknown. This study aimed to identify factors associated with complex gastroschisis by comparing the maternal risk factor profiles of simple and complex gastroschisis pregnancies. METHODS: All cases of gastroschisis managed in one tertiary care hospital between 1991 and 2022 were identified from a retrospective clinical database. Cases were classified as 'complex' if severe matting, necrosis, atresia or perforation were present; 'simple' ga

[Application of the Alexis O wound protector/retractor ring as an alternative silo system in the management of gastroschisis].

Gastroschisis is a congenital abdominal wall defect associated with extraabdominal herniation of the bowels and, rarely, other abdominal organs. In cases of severe visceroabdominal disproportion, primary abdominal wall closure may not be feasible, therefore staged reduction using silo systems is required. The use of the Alexis wound protector/retractor as an alternative silo system in gastroschisis was first described by Kusafuka et al. in 2005. We present the case of a restricted neonate born at 38 weeks of gestation with prenatally diagnosed gastroschisis. Due to the hypoplastic abdominal ca

Seasonal and geographic patterns of gastroschisis in Canada: protective effect of periconceptional sunlight exposure.

BACKGROUND: Gastroschisis is a right-sided, full-thickness defect with extra-amnionic herniation of abdominal contents directly into the amniotic cavity. The causes of gastroschisis remain obscure. We hypothesized that limited periconceptional sunlight exposure might lead to insufficient vitamin D during fetal development and play a role in gastroschisis etiology. METHODS: We conducted a population-based cohort study of infants conceived in Canada between April 2006 and March 2020. We used winter months (i.e., November and December) and northern geographic latitudes as indicators of insuffici

Fetal Superior Mesenteric Artery Peak Systolic Velocity as a Marker of Prenatal Bowel Inflammation in Gastroschisis: A Case-Control Study.

OBJECTIVE: This study aimed to evaluate whether peak systolic velocity (PSV) in the fetal extra-abdominal superior mesenteric artery (SMA) is a useful prenatal marker of bowel inflammation in gastroschisis (GS), which manifests postnatally as bowel matting. METHODS: This case-control study included 30 fetuses with gastroschisis who underwent standardized bowel assessment and Doppler measurement of the extra-abdominal SMA-PSV. Postnatal bowel status was classified as normal or inflamed. The primary analysis examined the association between fetal SMA-PSV and bowel inflammation, using linear mod
