Diarrhea in neonates is usually secondary to an infectious process or a food protein allergy. Once the infection is treated or the allergen is removed, the diarrhea resolves without lifelong sequelae. On the contrary, infants with anatomical disorders such as intestinal atresia, gastroschisis, malrotation with volvulus, or necrotizing enterocolitis can present with protracted diarrhea secondary to poor gut motility and short bowel syndrome. A small group of infants who present with severe diarrhea shortly after birth can have a congenital enteropathy resulting from a monogenic disorder. This g

OBJECTIVES: The prevalence of gastroschisis has been increasing for decades globally across all ages and races/ethnicities and is one of few birth defects with rising prevalence in both developing and developed countries. However, there are some indications that rates in the United States may be declining. We aim to investigate national gastroschisis prevalence with 3 independent large-scale databases. We hypothesize that from 2016-2022, the data will congruently reveal a decline in gastroschisis prevalence in the United States.
METHODS: This study is a retrospective cross-sectional database

BACKGROUND: Gastroschisis (GS) is the most common abdominal defect in infants, yet lack of consensus has led to variations in its management and outcomes. Length of stay (LOS) is an important measure of surgical quality and efficiency in GS infants. LOS depends on clinical and patient-related factors such as simple (sGS) or complex (cGS), type of surgery, associated complications, and factors such as the use of standardized care protocols.
OBJECTIVES: To determine the annual trends in LOS for sGS and identify the predictors leading to these trends.
METHODS/DESIGN: We conducted a retrospectiv

BackgroundFeeding practices for infants with gastroschisis are often based on institutional protocols, clinician judgement, and usual practice. The purpose of this study was to describe the range of feeding practices used in neonatal intensive care units across Canada.MethodsWe developed an 18-item survey with open-ended questions focused on feeding and nutrition among infants with gastroschisis. Clinicians from neonatal intensive care units that manage infants with gastroschisis were invited to participate. Interviews were completed via virtual meeting or email. Data were analyzed using descr

INTRODUCTION: We developed standardized electronic medical record templates (EMR-temp) for use in ambulatory prenatal surgical consultations for surgical fetal anomalies (SFAs). Our aim was to evaluate EMR-temp impact in provider documentation in prenatal care of SFA.
METHODS: Prenatal consultations for SFAs at a single institution were retrospectively reviewed (2019-2022). Disease-specific EMR-templates were developed. Note content and time to encounter closure (TEC) were collected. Descriptive statistics, chi-square, Fisher's Exact Test, and Wilcoxon rank sum tests were used.
RESULTS: Seve

Gastroschisis is a rare congenital condition that is increasing in incidence, possibly due to new lifestyles of women. Prenatal diagnosis is almost always possible in the second trimester, and color Doppler may be helpful. Fetuses with gastroschisis have higher rates of intrauterine growth restriction, but these rates may be overestimated by formulas that use abdominal circumference measurements in their calculations. Prenatal ultrasound markers of adverse perinatal outcomes have variable accuracy and should be interpreted with caution when suggesting early delivery. Intrauterine surgery is a

INTRODUCTION: This study was undertaken to explore if abdominal anthropometric measurements and pressure parameters measured before closure can predict the choice of surgical procedure in neonates with gastroschisis.
MATERIALS AND METHODS: Twenty-two neonates presenting to our tertiary center with gastroschisis over 18 months were included. Abdominal anthropometric measurements such as defect circumference (DC), abdominal circumference (AC), abdominal height (H), anteroposterior diameter (APD), and abdominal volume (AV) were measured soon after induction and again before final closure. Simult

BACKGROUND: In closed gastroschisis, the initial herniation of the intestines through the abdominal wall defect subsequently leads to spontaneous closure of the defect during fetal development, even before the migration of the intestines back to the abdominal domain.
MATERIALS AND METHODS: This study investigates the clinical presentation, diagnostic challenges, and management strategies of closed gastroschisis through a retrospective review of six cases presented at our clinic between 2009 and 2024. Data were prospectively collected and analyzed to gain insights into the varied presentations

This case report presents a preterm infant (36 weeks) with gastroschisis, successfully treated using a two-stage surgical approach. The newborn exhibited evisceration of the stomach and small intestine through a small umbilical defect, with significant edema but no additional anomalies. Due to the severity of the edema and limited abdominal capacity, a staged repair was planned. The first stage involved repositioning the stomach and duodenum into the abdominal cavity. In the second stage, the remaining bowel was placed in a sterile sac suspended above the incubator, allowing gradual reduction

Vancomycin-resistant enterococci (VRE) are a major cause of healthcare-associated infections (HAIs). However, the clinical significance of VRE colonization and the subsequent risk of VRE infection in hospitalized patients are not fully established. Prolonged hospital stays have been observed in neonates colonized by VRE. The mortality rate in pediatric patients with VRE infections ranges from 0% to 42% in both endemic and outbreak settings, often occurring in VRE-colonized neonates. Host and bacterial factors associated with a worse outcome are not fully understood yet. We describe an outbreak

BACKGROUND: A well accepted approach to the management of gastroschisis is gradual reduction of the herniated viscera using preformed silos followed by surgical closure of the abdominal wall defect. However, if the abdominal wall closure is delayed for a longer duration than necessary, it may increase morbidities. We sought to compare the outcomes of infants undergoing silo reduction whose abdominal wall defect was closed ≤ 5 days versus > 5 days after birth.
METHODS: Retrospective cohort study (January-2010 to December-2020).
RESULTS: One-hundred-and-nine infants who were managed using prim

BACKGROUND: Survival rates for infants born with gastroschisis are extremely poor in low and middle income countries. While reasons for mortality are multifactorial, commercial silo bags have been shown to improve outcomes; however, high cost limits access to these bags. We aimed to develop a low-cost silo bag alternative using locally available materials to meet this need.
METHODS: To make the SimpleSilo, a flexible ring is tailored to the size of the fascial defect by forming a loop of oxygen tubing. This tubing is folded into a silicone bag and sealed using a commercially available heat se

The neurodevelopment of children with gastroschisis may be affected by intrinsic (exposure of the abdominal viscera to the amniotic fluid, causing an inflammatory reaction) and extrinsic (pain, mobility restriction, hospitalization) factors. Thus, the objective of this study was to analyze the information available in the literature on the neurodevelopment of children born with this pathology. A scoping review was done, considering as outcomes the neurodevelopment in the motor, cognitive and language domains. A search was performed in the main databases using the term "gastroschisis", associat

Background: Gene variants of ICAM1 (rs1799969), ADD1 (rs4961), NPPA (rs5065), and NOS3 (rs1799983) have been proposed as possible genetic risk factors for vascular compromise in gastroschisis. However, this interpretation is questionable because the vascular disruption theory is now considered unlikely. According to current knowledge, gastroschisis is caused by a separation of the umbilical ring. Aim: This study reevaluated the gene variants of ICAM1 (rs1799969), ADD1 (rs4961), NPPA (rs5065), and NOS3 (rs1799983) as potential genetic risk factors for gastroschisis. The chicken is a valuable model to study embryonic development, genetic manipulation, regenerative medicine, tumorigenesis, vaccine development, toxicology, and teratology research. Different methods have been described for incubating chicken embryos outside the eggshell (ex-ovo or shell-less incubation). To date, no studies have focused on using ex-ovo incubation as a model to study the effects of hyperthermia as a teratogen. In this work, a total of 350 fertile chicken eggs were used to study the development of congenital malformations in an ex-ovo and in-ovo incubation system exposed to nor

BACKGROUND: Gastroschisis is a congenital abdominal wall defect that has increased in prevalence at birth globally over the past three decades. Despite advancements in neonatal care, gaps remain in understanding the long-term outcomes and factors driving differences between simple (SG) and complex gastroschisis (CG). Previous studies have highlighted disparities in morbidity and mortality between these two groups but lack consistent data over extended periods. This study aims to address these gaps by evaluating outcomes over a 30-year period, hypothesizing that differences in clinical outcomes

OBJECTIVE: We evaluated associations between prenatal cannabis use and major structural birth defects of the child.
METHODS: This population-based retrospective cohort study comprised singleton births (January 2011-July 2020) universally screened for substance use at entrance to prenatal care. Prenatal cannabis use was defined as self-reported use or a positive toxicology test during pregnancy. Electronic health record and birth certificate data were used to identify 38 specific major structural birth defects within 8 organ systems (i.e., central nervous, eye, ear, cardiac, orofacial/respirat
