Illustrative case series of eosinophilia in UK Armed Forces personnel and recommended management for non-specialists.

Peripheral eosinophilia (>0.5×109/L) in the full blood count is caused by a variety of infectious and non-infectious aetiologies. Of particular importance to the UK military are parasitic infections, especially in individuals recruited from overseas and those who have deployed to areas that are highly endemic for schistosomiasis, strongyloidiasis and soil-transmitted helminths. These infections may persist for decades without causing symptoms. UK Armed Forces (UKAF) personnel have recently presented with more severe forms of strongyloidiasis, which can be fatal, especially following immunosupp

Gallbladder volvulus in an elderly patient presenting as acute calculous cholecystitis: a case report.

INTRODUCTION AND IMPORTANCE: Gallbladder volvulus is a rare, life-threatening emergency resulting from torsion of the gallbladder on its vascular pedicle. Its rarity and clinical similarity to acute cholecystitis make diagnosis difficult. This is the first documented case in the Philippines, highlighting its significance for local surgical practice. PRESENTATION OF CASE: A 74-year-old woman presented with acute right upper quadrant pain, nausea, vomiting, and bloating. Laboratory workup revealed leukocytosis with neutrophilia, and imaging suggested acute calculous cholecystitis. At surgery, c

Patient trust in community-based interventions versus health-facility care: an economic experiment in Cameroon.

Trust in patient-provider relationships is central to healthcare utilisation in health facilities, but also in community-based interventions, particularly in contexts embedded within weak health systems. Neglected tropical disease (NTD) control programmes rely heavily on community-based mass drug administration (MDA) to reach over one billion individuals worldwide each year. However, empirical evidence on behavioural trust in any community-based interventions remains limited, especially in sub-Saharan Africa. We measured trust across two healthcare settings in Cameroon: health facilities and c

Research Priority Setting Exercises in Neglected Tropical Diseases: A Systematic Mapping Review.

BACKGROUND: Neglected tropical diseases (NTDs) affect more than 1.7 billion people worldwide, primarily in low- and middle-income countries. Despite their high burden, NTDs remain underfunded and receive fragmented research attention. Research priority-setting provides a systematic approach to align limited resources with the most pressing needs. While several priority setting exercises have been conducted for NTDs, their findings have not been synthesized. This study aimed to systematically map the published research priority-setting exercises for NTDs, synthesize their methodologies, stakeho

Complexity of Clathrin Coat Dynamics in Endocytosis.

Resource availability can alter infection outcomes through its impact on host immunity and on parasite reproduction. On one hand, enhanced nutrition could favor immunity, limiting the parasite, and on the other hand, it could favor establishment and reproduction of the parasite. Our study aimed to determine the effect of diet on (1) host susceptibility to infection and (2) parasite production in a snail-trematode system. We fed Biomphalaria sudanica, a snail vector of Schistosoma mansoni, either a strict lettuce (low nutrient) or pellet (high nutrient) diet for two generations before exposing

Metabolomic Biomarkers for the Human Diagnosis of Neglected Tropical Diseases: A Systematic Review.

Neglected tropical diseases (NTDs) affect more than 1.6 billion people worldwide and remain major contributors to morbidity, disability, stigma, and poverty. Metabolomics has emerged as a promising approach for investigating disease-associated biochemical alterations and identifying biomarkers for diagnosis, prognosis, and treatment monitoring. This systematic review evaluated the available evidence on metabolomic profiles in WHO-defined NTDs and their potential clinical applications. MEDLINE/PubMed, Web of Science, and Scopus were searched through March 2026. Observational studies investigati

Molluscicidal Activity of Camellia Sinensis (Green Tea) and Camellia sinensis var. Assamica (Purple Tea) Extracts Against Biomphalaria pfeifferi, the Major Vector Snail of Human Schistosomiasis in Sub-Saharan Africa.

Freshwater pulmonate snails are prevalent across Kenya and act as intermediate hosts for trematode parasites, some of which are snail vectors for human schistosomiasis. Chemical molluscicides have rarely been used routinely in Kenya to control snails due to high costs of manufacture and the subsequent environmental concerns associated with their use. This study tested extracts from green tea and purple tea plants, Camellia sinensis, which are widely grown in Kenyan highland areas, against Biomphalaria pfeifferi, the obligate intermediate host for Schistosoma mansoni. Snails were exposed to fiv

Temporal variability and flooding influence the ecological niche of Biomphalaria intermediate hosts for Schistosoma mansoni in rural Uganda.

Understanding the niches of intermediate hosts and vectors for environmentally transmitted pathogens is crucial for identifying endemic areas, assessing habitat suitability and targeting interventions. This study focuses on intermediate hosts of intestinal schistosomes, with over 700 million people at risk of lifelong infection. We compared habitat suitability and species interactions across 674 sites in 52 villages in rural Uganda between 2022 and 2024, capturing a severe flooding event. Spatiotemporal models incorporating a polygon-based method to account for space with time as a fixed effec

Long-term persistence of Schistosoma mansoni hotspots in western Kenya despite ongoing mass drug administration.

Previous studies have reported the existence of 'hotspots', where schistosomiasis prevalence and infection intensity remain high despite regular preventive chemotherapy. Western Kenya is one region where such hotspots were reported. We investigated whether hotspots remain following eight years of school-based deworming programme by comparing the prevalence and infection intensity of Schistosoma mansoni between five purposively selected hotspots and non-hotspots in Siaya County, western Kenya. We conducted a cross-sectional study involving 500 participants (250 from hotspots and 250 from non-ho

Association between Schistosoma mansoni infection and fecal occult blood in schoolchildren in Mbita, Suba North sub-county, western Kenya.

BACKGROUND: Schistosoma mansoni infection is highly prevalent in sub-Saharan Africa and is associated with significant intestinal morbidity in children. Current monitoring tools primarily assess infection status and intensity, which may underestimate the disease burden. Fecal occult blood (FOB) is a reliable indicator of bowel morbidity; however, its utility in intestinal schistosomiasis remains inadequately characterized. This study aimed to evaluate FOB as a surrogate marker of S. mansoni-induced intestinal morbidity among children in endemic areas of Kenya. METHODS: A pre-post intervention

The DEAD-box RNA helicase eIF4A is a crucial factor for stem-cell activity and reproduction of the parasite Schistosoma mansoni.

INTRODUCTION: The parasite Schistosoma mansoni has a unique reproductive biology, because female maturation depends on constant pairing with a male. Paired females produce each up to 300 eggs per day, which are the pathogenic factors of schistosomiasis, a neglected tropical disease that affects > 240 million people worldwide. Due to the importance of egg production for life-cycle maintenance and pathology, molecular mechanisms controlling schistosome reproduction are in the focus of research. Among the candidates involved in regulating the reproductive biology of this parasite are DEAD-box RNA

Antischistosomal activity of imidazolidine-2,4-dione derivatives.

Schistosomiasis is a neglected tropical disease with limited treatment options and growing concerns over praziquantel resistance. We evaluated a series of 12 synthetic compounds for antiparasitic activity against Schistosoma mansoni adult worms in vitro. Isoindole-1,3-dione derivatives were inactive, whereas four imidazolidine-2,4-dione derivatives displayed selective antischistosomal activity with low cytotoxicity in mammalian cells. These findings highlight imidazolidine-2,4-dione as a promising scaffold for the development of new therapeutic agents against schistosomiasis.

Snail 3G: genomics, genetics, and gene-editing of Biomphalaria glabrata.

This review highlights recent advances and ongoing challenges in the genomics, genetics, and gene-editing (3G) of the freshwater snail Biomphalaria glabrata, based on insights gained from a novel model system we initiated two decades ago. B. glabrata is an intermediate host of the human blood fluke Schistosoma mansoni and serves as the principal model organism for schistosomiasis research. We developed two homozygous lines of B. glabrata, the iM line and iBS90, through 81 and 41 generations of selfing the commonly used M line and BS90, respectively. These lines display contrasting infection ph

Heat shock protein 90 as a vital signalling hub in Schistosoma mansoni: a master regulator of stem cell proliferation, development and survival.

Heat shock protein 90s (HSP90s) are molecular chaperones often produced by cells in response to hostile conditions. Here, we characterized the importance of HSP90 to growth/development and survival of the blood fluke Schistosoma mansoni, focusing on life stages that parasitize the human host. Western blotting revealed striking HSP90 upregulation in developing schistosomula in vitro, and pharmacological assays revealed that the HSP90 inhibitors gedunin, 17-N-allylamino-17-demethoxygeldanamycin (17-AAG) and 5-(2-amino-4-chloro-7-((4-methoxy-3,5-dimethylpyridin-2-yl)methyl)-7H-pyrrolo[2,3-d]pyrim

Extracellular Vesicle Properties and Functions Are Defined by the Originating Cell's Fitness Status.

Recent studies have focused on the role of dying cells in modulating immune function and shaping the local microenvironment. Apoptotic cells release various extracellular vesicles (EVs), such as exosomes and microvesicles, and form apoptotic bodies through membrane blebbing. Despite growing interest in EVs, the effects of EVs derived from apoptotic cells versus those from live cell remain poorly understood. Here, via transmission electron microscopy, nanoparticle tracking analysis, proteomics, and flow cytometry, we characterise the distinct features of EVs from live versus apoptotic cells. Us

Ecological influences on host-parasite dynamics among Biomphalaria snails in two schistosomiasis endemic regions of Kenya.

Schistosomiasis is a neglected tropical disease affecting over 240 million people globally, with sub-Saharan Africa bearing the highest burden. In Kenya, transmission of Schistosoma mansoni, the causative agent of intestinal schistosomiasis, remains prevalent in western, coastal, and central regions, particularly in the Mwea Irrigation Scheme and the Lake Victoria basin. The parasite depends on Biomphalaria snails as intermediate hosts, yet ecological determinants influencing infection dynamics remain underexplored. This study examined S. mansoni infection in Biomphalaria snails across two con

Schistosoma mansoni soluble egg antigen and its key proteins differentially affect dextran sodium sulphate-induced inflammatory bowel disease.

BACKGROUND: Inflammatory bowel disease (IBD) is an increasingly prevalent disease, affecting over seven million people worldwide and imposes a heavy burden on public health. The rising prevalence of IBD may be attributed to the hygiene hypothesis, which suggests that reduced exposure to parasites and microbes may weaken the immune system, thereby increasing susceptibility to developing IBD. Studies suggest helminths and their secretory products can modulate the host immunity and attenuate IBD. Our previous research also demonstrated that intestinal schistosomiasis can mitigate chronic IBD symp
