Prune-belly Syndrome: An Update.

The Prune-Belly (Eagle-Barrett) syndrome (PBS) is a congenital and genetically heterogeneous disease, more prevalent in males, defined by the clinical triad (1) deficiency of abdominal muscles, (2) bilateral cryptorchidism, and (3) urinary tract abnormalities. The abdomen of an infant with PBS has a typical appearance, similar to the aspect of a prune, which gives it its name. Although the etiology of this disorder is still unknown, numerous theories, mutations, and genetic disturbances have been proposed to explain the origin of PBS. Prognosis can differ a lot from one patient to another, sin

Abdominal and intra-abdominal fibromatoses: Outcomes over time.

OBJECTIVES: Abdominal wall and intra-abdominal fibromatoses are locally aggressive, nonmetastasizing neoplasms. Surgery has been the mainstay of local control, but new forms of therapy have been developed that may influence the clinical course and morbidity. We studied the clinical features and outcomes of patients with abdominal and intra-abdominal fibromatoses over time. METHODS: Ninety-one patients-46 with abdominal wall and 45 with intra-abdominal fibromatosis-treated in our hospital systems between 2009 and 2023 were included. The patients were allocated to 1 of 2 groups based on the yea

Failure of Prenatal Ultrasonography to Identify Classic Bladder Exstrophy in a Heavily Screened Population.

OBJECTIVE: To summarize the imaging characteristics of newborns with bladder exstrophy and to identify the pitfalls that led to prenatal misdiagnosis. METHODS: We evaluated prenatal ultrasonographic reports of all consecutive newborns with bladder exstrophy seen in our institution between 2017-2023. None had been diagnosed prenatally on early (14-16 weeks' gestation) and late (18-24 weeks' gestation) second trimester sonographic scans. RESULTS: The study cohort included 14 patients after exclusion of four with insufficient information and one who was correctly diagnosed prenatally. All 14 ha

Delayed presentation of mature teratoma with umbilical perforation in a 9-years old female without congenital abdominal wall defects: A rare case report of atypical teratoma location.

BACKGROUND: Mature teratomas are benign germ cell tumors that predominantly occur in gonadal regions. Extra-gonadal teratomas, especially in the umbilical region, are rare and often present significant diagnostic challenges. A delayed presentation with umbilical perforation in absence of congenital abdominal wall defects is uncommon. CASE PRESENTATION: A 9-year-old female presented with a four-year history of progressively enlarging umbilical mass with occasional discomfort. Examination revealed a firm, pedunculated mass protruding through a perforated umbilicus. A contrasted CT scan, reveale

A Rare Case of Chromosomal Abnormality: 19q13.11 Deletion in a Patient With Aplasia Cutis Congenita and Ambiguous Genitalia.

Aplasia cutis congenita (ACC) is described as the local or generalized absence of the skin, the most common site being the scalp. Approximately one-third to one-sixth of the cases are associated with underlying bone and scalp dura mater defects. ACC may have other congenital defects, some of which are known as cleft deformities, abdominal wall defects, limb anomalies, and fetus papyraceus. Some of the ACC cases involve lesser-known defects. Different treatment options are available without clear, definitive guidelines. This case involves a patient who was born without an antenatal diagnosis of

Causal relationship between gastroesophageal reflux disease and 7 types of hernias: A bidirectional Mendelian randomization study.

Gastroesophageal reflux disease (GERD) is linked to various esophageal and extra-esophageal complications. While GERD is theoretically a potential risk factor for abdominal hernias, current evidence is limited. Observational studies have suggested associations between GERD and both congenital diaphragmatic hernia and hiatal hernia. This study employed a bidirectional Mendelian randomization analysis to assess the causal relationship between GERD and 7 types of abdominal hernias. The inverse variance weighting method served as the primary statistical approach, supplemented by sensitivity analys

Postnatal outcomes of in utero repair of simple gastroschisis in an ovine model.

BACKGROUND: Gastroschisis is a congenital abdominal wall defect resulting in fetal bowel evisceration, causing significant postnatal morbidity, including prolonged bowel dysmotility and increased susceptibility to life-threatening infections. This study aims to optimize an ovine model of gastroschisis by evaluating defect size to improve fetal survival and assess the impact of in utero repair (IUR) on postnatal outcomes. METHODS: Ovine gastroschisis defect creation was achieved with a 2 or 3 cm abdominal wall defect at gestational age (GA) 76-82 days. Fetuses underwent repair either in utero

DISSECTING THORACIC AORTIC ANEURYSM WITH CLASSIC IMAGING FINDINGS AND REVIEW OF LITERATURE.

BACKGROUND: Thoracic aortic aneurysm is an increasingly recognized condition that is sometimes diagnosed incidentally on imaging examinations performed to evaluate other unrelated conditions. Symptomatic presentations of thoracic aortic aneurysms may be due to mass effect on the airway, esophagus, recurrent laryngeal nerve and the thoracic spine. Alternatively, they may present with the dreaded complication of aortic dissection or rupture.Occasionally abnormalities of the aortic contour or size can be detected on routine chest x-ray. However, it is difficult to confidently diagnose thoracic ao

Congenital Imprinting Diseases: Aetiology, Pre- and Perinatal Manifestations, Diagnosis and Care of Affected Families and Pregnancies.

Congenital Imprinting Disorders (ImpDis) are caused by abnormal expression of parentally imprinted genes. They are characterized by disturbances of imprinting marks which are a specific type of epigenetic signatures and occur either sporadic or familial. So far, twelve ImpDis have been identified, eight of them manifest prenatally or in the neonate period. With exception of abdominal wall defects, ImpDis are rarely associated with major malformations, but predominant early manifestations are disturbed pre- and/or postnatal growth, muscular hypotonia, neonatal feeding difficulties and metabolic

Chronic abdominal pain: Budd-Chiari syndrome with converging pathologies.

INTRODUCTION: Chronic abdominal pain can be diagnostically challenging, especially when vascular causes like Budd-Chiari Syndrome (BCS) are overlooked. BCS, often linked to prothrombotic conditions such as protein S deficiency, may be further complicated by anatomical anomalies like May-Thurner Syndrome (MTS), leading to significant hepatic congestion and morbidity. This case highlights the need to consider vascular etiologies in patients with hypercoagulable risk factors to ensure timely diagnosis and intervention. CASE STUDY: A 21-year-old Caucasian female with a complex history including t

Partial Resection of a Tailgut Cyst Attached to the Rectum via a Transcoccygeal Approach: A Case Report With a Favorable Outcome.

A tailgut cyst is a congenital cystic tumor that develops when the tailgut, derived from the hindgut in early embryonic development, fails to naturally regress and remains in this presacral space. Treatment typically involves surgical resection due to the possibility of cyst infection and neurological symptoms from compression. Various surgical approaches exist, including abdominal, transanal, and transsacral approaches. We report a case where partial resection of a tailgut cyst attached to the rectum was performed via a transcoccygeal approach, resulting in a favorable outcome. The patient wa

Imaging of the Urachus.

The urachus is a remnant fibrous cord of the allantois and the ventral aspect of the cloaca that connects the urinary bladder to the anterior abdominal wall at the level of the umbilicus. The authors explore the embryologic origins of the urachus and delineate its normal anatomy, followed by a discussion of urachal pathologic conditions seen with different imaging modalities-including US, CT, and MRI-using a case-based approach. Although it is a vestigial structure, the urachus can harbor significant disease, including congenital anomalies arising secondary to varying degrees of incomplete inv

Littre hernia in adults: imaging features and clinical implications.

Littre hernia is an inguinal or abdominal wall herniation that contains a Meckel's diverticulum alone or with other intestinal loops. The diagnosis is usually made at surgery, but its pre-operative radiological recognition has been a challenge due to inherent difficulties in detecting the Meckel's diverticulum within hernial content. The aim of this article is to present 8 adults in whom a Meckel's diverticulum protruding into their inguinal, umbilical or incisional hernia had been demonstrated by barium examination of the small bowel or colon, or on computed tomography and magnetic resonance

Optimizing prenatal diagnosis and referral of classic bladder exstrophy: Lessons from a single-institution experience.

INTRODUCTION: Classic bladder exstrophy (CBE) is a malformation of the genitourinary system that occurs due to failure of abdominal wall closure. Unlike other malformations of similar incidence, prenatal diagnosis of CBE relies on suggested, rather than formal, diagnostic criteria. OBJECTIVE: This report describes prenatal diagnosis of CBE in the largest single-institutional cohort to date and delineates key sonographic findings and protocols for specialist referral. MATERIALS AND METHODS: A single-institutional database was reviewed for CBE patients born since 2000. Data on screening ultras

Challenges and lessons learnt in the management of an HIV-exposed neonate with gastroschisis in a resource-limited setting: case report.

INTRODUCTION AND IMPORTANCE: The incidence of congenital abdominal wall defects is increasing, but few cases have been reported in the African population. CASE PRESENTATION: The authors report a case of gastroschisis in a term neonate who was delivered through spontaneous vaginal delivery (SVD) in a remote health facility before transfer to a tertiary hospital in Uganda. Although there was no environmental exposure to teratogens, the major risk factor of Gastroschisis, the neonate was low birth weight, HIV-exposed, and the mother had not received folic acid supplementation during the first tr

A Rare Case Report of Limb Body Wall Complex.

Limb body wall complex (LBWC), also known as body stalk anomaly, is a rare and lethal disorder of the anterior abdominal wall. It is characterized by a severe combination of congenital malformations in the fetus, including, abdomino- and/or thoracoschisis, exencephaly/encephalocele, limb deformities, and facial clefts. Short umbilical cord, abdominal placental attachment, and spinal anomalies are among other manifestations of this disorder. The cause of LBWC is still unknown. The main hypotheses include embryonic dysplasia, early amniotic rupture, and vascular accident during embryonic develop

Long-Term Neurodevelopmental Outcomes in Children with Gastroschisis: A Review of the Literature.

This study aimed to investigate and present a review of the literature on long-term neurodevelopmental outcomes in children with gastroschisis. Gastroschisis is the most common abdominal wall defect. Children with gastroschisis are at high risk for premature birth, intestinal failure, sepsis, and repeated anesthesia exposure, which collectively increase the risk for adverse long-term neurodevelopmental outcomes. The existing literature on neurodevelopmental outcomes is limited in number, quality, and generalizability, creating a gap in clinical knowledge and care. Five internet databases were

Frequency of gastroschisis and omphalocele and possible influence of maternal folic acid supplementation. A narrative review.

There is an increase in the worldwide prevalence of congenital abdominal wall defects (CAWD), with gastroschisis (GS) and omphalocele (OC) being the most common. It is widely accepted that folic acid supplementation (FAS) in the maternal diet decreases the incidence of anomalies such as neural tube defects, but there is controversy regarding the possible beneficial role for other congenital malformations. Several epidemiological studies raise controversy regarding a possible relationship between vitamin supplementation with the occurrence of abdominal wall malformations. The aim of the present
