The gastrointestinal tract in Down syndrome.

Down Syndrome is recognized as one of the most common predisposing conditions for a group of serious gastrointestinal (GI) anomalies. Tracheo-esophageal fistula, duodenal obstruction with or without pyloric stenosis, annular pancreas, imperforate anus and Hirschsprung's disease are the most prevalent lesions. Understanding of the morphogenetic mechanisms responsible for this range of abnormalities is far from clear, as none of the lesions is specific to the trisomic state and the underlying defects (i.e., failure of foregut canalization, failure of neural crest cell migration into the myenteri

Antenatal fetal diagnosis and maternal transport gastroschisis. A maternal-infant case report.

Gastroschisis is a rare congenital birth defect of the abdominal wall that invariably is fatal if not managed appropriately. Early in utero diagnosis is now possible using maternal serum alpha-fetoprotein and ultrasound. As with other serious congenital disorders, antenatal diagnosis allows therapeutic options and time for preparation. Even if the diagnosis is made late, however, efforts to achieve prenatal maternal transport are still reasonable. The neonatal outcome of an unplanned delivery of an anomalous child requiring immediate attention is likely to be better if the delivery occurs in a

Mesenteric fibromatosis. A follow-up study.

Eighty-two fibromatoses of the mesentery and other peritoneal sites were studied, with emphasis on features associated with recurrence. Twelve patients had Gardner's syndrome. Seventy-two tumors were completely excised at the time of surgery; 56 patients had no evidence of tumor recurrence (mean, 91 months), and in 16 patients tumors recurred (mean, 117 months of follow-up). Patients with Gardner's syndrome had a much higher risk of recurrence and death related to fibromatosis or surgery for fibromatosis than sporadic cases. There was a weaker association between recurrence and multiplicity. A

[Gastroschisis and omphalocele. Treatment and results].

Between 1976 and 1988, 28 neonates with gastroschisis and 44 with omphalocele were treated. In 44 patients primary closure was achieved, while silastic prostheses were used in 23 patients, and dura implant in three patients. Two patients were not treated. The mortality rate was 8% (4/52) among the patients without serious congenital defects and birth weights over 1,800 g. This mortality was mainly caused by bowel infarction and, in infants with birth weights below 1,800 g, by the respiratory distress syndrome. Our experience suggests that ventilatory assistance with total paralysis is mandator

Risk factors for gastroschisis.

The prevalence at birth of gastroschisis, a rare abnormality of the abdominal wall, appears to have increased over the past decade. To characterize risk factors that might explain this increase, birth certificates for Washington State residents were compared for 62 infants born with gastroschisis during the years 1984 to 1987 and 617 randomly selected unaffected infants matched for birth year. After simultaneously adjusting for 14 potential risk factors, 4 factors stood out. Infants born during January, February, or March were at greater risk than infants born in any other months (odds ratio 2

Pars sternalis diaphragmatic hernia with omphalocele: a report of two cases.

A rare type of congenital diaphragmatic hernia occurs in which there is a large opening in the anterior diaphragm between the pericardial and peritoneal cavities without a sac. This hernia is invariably associated with defects in the anterior abdominal wall and with sternal defects. More commonly, it is associated with cardiac anomalies as in the Pentalogy of Cantrell. The etiology of this hernia is undoubtedly different from the more common hernia of Morgangni, which has a sac and few associated anomalies. The etiology may be failure of fusion of the pars sternis area of the septum transversu

[Initial measures in newborn infants with surgically correctable abnormalities].

In newborns with surgically correctable malformations the quality of primary care has considerable influence on the results of the eventual surgical repair. This is particularly true for extensive diaphragmatic and abdominal wall defects as well as for esophageal and intestinal atresia, exstrophy of the bladder and urogenital malformations. The neonatal management comprises measures generally valid for all diseased newborns and measures specific for the particular malformation. The general care concerns heat regulation, fluid balance and other vital functions. Specific regimens are necessary a

Prenatal diagnosis of sirenomelia.

Prenatal sonographic findings of sirenomelia (or mermaid fetus) were retrospectively reviewed in eleven proven cases. Sonography showed oligohydramnios in all the cases, five (45%) of which had severe oligohydramnios that limited prenatal diagnosis by poor visibility. In five cases (45%), sirenomelia was correctly diagnosed by ultrasound; in the remainder, only bilateral renal agenesis was identified. All eleven fetuses had other associated malformations: congenital heart defects (4), skeletal deformities (10), and abdominal wall defects (4). Death resulted from termination of pregnancy in six

Chromosomal abnormalities in fetuses with omphalocele. Significance of omphalocele contents.

Twenty-six consecutive fetuses with a sonographically detectable omphalocele and known karyotype were reviewed to identify risk factors that might be associated with chromosomal abnormalities. Risk factors that were analyzed included contents of the omphalocele sac, maternal age, fetal sex, sonographically detectable concurrent anomalies, and any major concurrent anomaly. Chromosomal abnormalities were found in 10 cases (38%) from trisomy 18 (n = 4), trisomy 13 (n = 4), trisomy 21 (n = 1), or 45, X (n = 1). The absence of liver from the omphalocele sac (intracorporeal liver) was strongly assoc

Anterior abdominal wall defects.

A total of 48 infants with abdominal wall defects referred to the South West Regional Neonatal Surgical Centre over a period of six years were reviewed. There were 27 (56%) infants with gastroschisis and 21 (44%) infants with exomphalos. At the regional centre, four pregnancies with gastroschisis were terminated following a prenatal diagnosis, one child was stillborn, and one with exomphalos and trisomy 13 died soon after birth. Of the remaining 42 live births, the one year survival for babies born with a gastroschisis was 95% (21/22) and with an exomphalos was 81% (17/20). All deaths, except

[Bullous amyloidosis].

The patient, a 75-year old man, was admitted in May, 1986 for separation of the epidermis and extensive ecchymotic patches. Physical examination showed numerous haemorrhagic erosions on the extensor aspect of the limbs, feet and hands, and wide patches of epidermal separation in the axillary and dorsal regions. Ecchymotic purpura was present on the limbs, abdominal wall, neck and right orbital region. Nikolsky's sign was positive at the periphery of the lesions. Epidermal cysts, 1 to 5 mm in diameter, were visible on the back of the hands and on the upper part of the neck. There was no macrogl

Cardiac and noncardiac malformations: observations in a population-based study.

A regional case-control study of congenital cardiovascular malformations (CCVMs) searches for all live-born infants in the community in whom the cardiac diagnosis has been confirmed by echocardiography, cardiac catheterization, surgery, or autopsy. Their families are studied in comparison to those of a representative sample of resident live-born infants. Detailed descriptions of noncardiac abnormalities are obtained from physician reports and maternal interviews expanded by medical record and death certificate data. Among 1,494 cases and 1,572 controls, chromosomal abnormalities, syndromes, he

[Unusual increase in amniotic fluid alpha fetoprotein and trisomy 13].

From 10 observations of trisomy 13, 3 presented an elevated amniotic fluid alpha-fetoprotein level considered as unusual in 2 cases, superior to cut-off level in the other case. Macroscopic examination of the three fetus could not reveal a cause of AFP elevation, neural tube defect or abdominal wall defect. The authors discuss the role of an undetected abnormality such as minor scalp defect with very thin membrane and for one case false-negative result of Kleihauer test.

Congenital soft tissue necrosis of the right anterior abdominal wall.

A rare case of congenital soft tissue necrosis of the right anterior abdominal wall in a newborn baby is described. The defect, which measured 10 X 8 cm at birth, appeared spontaneously and healed completely within 8 weeks with conservative management. Only one other similar case is known to have been reported in the literature hitherto, but in that patient the "ulcer" was presumed to have been caused by faulty technique in fetoamniography: the fetus was traumatized by the needle used to inject the contrast material. In the present case, the etiology was unknown.

[Cantrell's syndrome].

Congenital diaphragmatic hernia is associated with a high mortality, especially when combined with other defects. The unusual repeated combination of the same congenital abnormalities by a number of patients and the fact that during embryonic life these organs develop from the mesoderm implies the existence of a specific syndrome. This article summarises the available clinical data and reports on three cases with Cantrell's syndrome, showing different pattern of congenital defects such as: 1) supraumbilical abdominal wall defects; 2) a defect of the lower sternum; 3) a deficiency of the anteri

Limb body wall complex: I. Pathogenesis.

Twenty-five fetuses with limb body wall complex (LBW complex) were evaluated. The diagnosis was based on two out of three of the following: exencephaly/encephalocele with facial clefts; thoraco- and/or abdominoschisis; and limb defect. Ninety-five percent (24/25) of the fetuses had associated internal structural defects. In 72% (18/25) the internal defects have been recognized as being secondary to vascular disruption. Concordance was not found between the side and location of the body wall defect versus the limb, internal, and cranial defects. In 85% there was evidence for persistence of the

Justification of maternal serum alphafetoprotein screening in a population with low incidence of neural tube defects.

A prospective study of maternal serum alphafetoprotein (alpha-FP) screening of 9838 women in an area with low prevalence of neural tube defects and predominance of anencephalics revealed that an intervention point of single serum alpha-FP level above 2.8 times the median was appropriate for this population. Ninety per cent of anencephalics and all fetuses with anterior abdominal wall defects were detected. There was no spina bifida among the population screened. Two per cent of the population screened had serum alpha-FP level above this cut-off level. Thirty-two per cent of twin pregnancies, 7

Limb-body wall complex: in utero sonographic diagnosis of a complicated fetal malformation.

Limb-Body Wall Complex is a complicated fetal malformation with the essential features of neural-tube defects, body-wall disruption, and limb abnormalities. This complex should be distinguished from other body-wall defects including omphalocele and gastroschisis since the prognosis for limb-body wall complex is uniformly poor. For 13 cases of this unusual fetal anomaly, sonographic, radiologic, and pathologic findings were correlated. Thoracic or abdominal body-wall defects, neural-tube abnormalities, severe scoliosis, positional deformities, and abnormalities of fetal membranes were consisten

The prenatal diagnosis of ventral abdominal wall defects.

Ventral abdominal defects are frequently diagnosed by prenatal ultrasonography. This review of 47 infants with ventral defects was designed to determine the effect of prenatal diagnosis on neonatal outcome. All nine infants with prenatal diagnosis were delivered by cesarean section at tertiary level hospitals. Only 13 (34%) of 38 infants with unsuspected defects were delivered by cesarean section and 15 (39%) were delivered at tertiary level hospitals. There was no difference in mortality between infants with prenatal diagnosis (44%) and those with unsuspected defects (37%). Increased mortalit
