Closure of gastroschisis by mesh skin grafts in problem cases.

Closure of gastroschisis can be achieved by primary adaptation of the abdominal wall, by implanting a dura patch or by covering the protruding gut with a silastic pouch. In the Pediatric Surgical Department of the University of Innsbruck this last method was used in 14 of 18 cases during the last 8 years. In three children multiple complications and infection resulted in the necessary removal of the silastic pouch from the still protruding gut. Lacking any other alternative, the defect was covered with mesh skin grafts, which took well and permanently closed the abdominal wall.

Intra-abdominal tissue expansion: an adjunct in the separation of conjoined twins.

Intra-abdominal tissue expansion routinely occurs physiologically with pregnancy and may be seen in a variety of pathologic states. There are a number of situations where additional abdominal wall would be helpful. We encountered such a situation in a set of ischiopagus conjoined twins. In effect, separation would result in an abdominal wall deficit of approximately 50%. As an alternative to intraperitoneal air, we inserted two 1,000 mL tissue expanders through a subcostal incision at 6 months of age. Over the ensuing 7 weeks, each expander was gradually inflated to greater than capacity (1,25

Congenital diverticulum of the ventricle.

Ten cases of ventricular diverticula were seen at the Texas Heart Institute between 1965 and 1984. Operations were performed on nine of the 10 patients with no operative mortality. The tenth patient, a 3-month-old infant with multiple abnormalities, was not returned after the initial evaluation. Most diverticular in our series were unsuspected and diagnosed during cardiac catheterization. Symptoms were related mainly to associated malformations, including intracardiac, midline thoracic, diaphragmatic, and abdominal wall defects. The surgical techniques employed in each case varied according to

Teratogenic effect of acetohydroxamic acid in clinically normal beagles.

Acetohydroxamic acid (AHA), a potent urease inhibitor used for treatment of infection-induced struvite urolithiasis, was teratogenic after administration of 25 mg of AHA/kg of body weight/day orally to 5 clinically normal Beagles from the onset of proestrus until parturition. Thirty pups exposed to AHA in utero developed anomalies of the skeletal system, heart, and ventral midline. Cardiac anomalies included atrial septal defects (20%), ventricular septal defects (3%), and atrial and ventricular septal defects (3%). Skeletal anomalies included coccygeal hemivertebrae and fused coccygeal verteb

Cloacal exstrophy and related abdominal wall defects in Manitoba: incidence and demographic factors.

A retrospective descriptive study of omphalocele and exstrophic defects of the abdominal wall was carried out in Manitoba to determine the prevalence of these defects, changes in birth incidence with time and their etiologic heterogeneity. The incidence of isolated omphalocele was similar to that reported in other series but the overall incidence of other types of abdominal wall defects was high. Cloacal exstrophy, bladder exstrophy and amnion disruption defects contributed significantly to the number of cases ascertained and emphasized the heterogeneity of abdominal wall defects. There were f

[Prenatal diagnosis: gastroschisis--pediatric surgical considerations and a report of 13 cases].

The gastroschisis is a congenital defect of the abdominal wall without other abnormalities and can be diagnosed by ultrasonography before 20 weeks of gestation. The management of these cases requires the cooperation of the obstetric and pediatric clinician in respect of an elective abortion. As a result of the advances in pediatric surgery and neonatal intensive care the treatment of the gastroschisis is possible and the prognosis of these children is very good. In the years 1978-1983 the primary closure of the abdominal wall defect was performed in 13 newborns. Whereas one child died due to i

[Magnetic resonance imaging of cardiovascular diseases: advantages of coronal and sagittal planes].

The usefulness of coronal and sagittal sections of the cardiovascular system by magnetic resonance imaging was evaluated. Coronal, sagittal and transverse spin echo scans using ECG-non-gating and gating during systole and diastole were performed for five normal volunteers, 91 with heart diseases (25 valvular disease, 28 ischemic heart disease, 14 cardiomyopathies, 14 congenital malformations, four pericardial diseases, and six others) and 32 patients with aortic abnormalities (17 aneurysms, 10 dissections and five others) using a 2.5 KGauss unit. Cardiac gating necessitated six to eight min pe

[Ventral abdominal wall defects--antenatal diagnosis, course of pregnancy and post partum therapy].

The article reports on antepartal sonographic diagnosis and the course and completion of 27 cases with ventral abdominal wall defects. There were 17 cases of omphalocele, 4 cases of gastroschisis and 6 cases of complex ventral abdominal wall defects with associated malformations. In 23 of the 26 women examined prenatally via sonography (89%), the diagnosis was ventral abdominal wall defect. In the 3 cases not diagnosed via sonography, there was one case of intrauterine foetal death. In another case, Potter's syndrome with anhydramnion was found besides a foetal abdominal wall defect, whereas t

[Prune belly syndrome].

Referring to a patient operated on the third day of life for pyeloureteral stenosis the 'prune belly' syndrome is described. The syndrome is delineated by urinary tract anomalies, undescended testicles and a prune like abdominal wall. Several other anomalies are commonly seen. In this patient the megaureters, the vesicoureteral reflux and megacystis are considered part of the syndrome. The pyeloureteral stenosis was only seen once before in a succumbed newborn.

[Prenatal diagnosis of neural tube and abdominal wall defects in the 2d trimester].

Combined alpha-fetoprotein screening in serum and specified sonographic examination has proved useful in prenatal diagnosis of neural tube and abdominal wall malformations. 207 amniocenteses were performed within a period of 3 years due to pathologic serum alpha-fetoprotein values and/or abnormal sonographic findings. 101 amniotic fluid specimens showed an increase of alpha-fetoprotein of more than 2.5 times the average value. 60 malformed neural tubes, 17 foetuses with abdominal wall defects, as well as 2 other malformations, were diagnosed. "Open" defects, as well as 17 false positive AFP va

Fetus-in-fetu: report of a case.

A 5 month-old female was brought to our clinic because of diarrhea and abdominal distension. A plain radiograph demonstrated a mass with a vertebral column in the right upper quadrant of the abdomen. At operation a mass was found to be retroperitoneal, well encapsulated, and connected to the abdominal aorta of the host by two small vessels; no other connections and adhesions were seen between the mass and the host. The ovaries, uterus, and other pelvic and abdominal viscera of the host were normal. The mass was diagnosed as a fetus-in-fetu. The fetus-in-fetu, encapsulated with an amniotic caps

Varicose veins of the abdominal wall in a child. A case report.

A case of a venous malformation of the abdominal wall in a 13-year-old boy is hereby presented. This malformation, manifesting as varicosities which were present from infancy caused no disturbances other than cosmetic. Clinical, laboratory and radiological examinations revealed no underlying pathology linked with this malformation. Considering the patient's history, we assumed the varicosities to be sequelae of a deep thrombophlebitis of the right leg in infancy, although a congenital etiology was not excluded. The varicose veins were surgically excised with excellent postoperative results. To

During the decade from July 1970 through June 1980, 57 patients with omphalocele and 64 with gastroschisis were treated at the Childrens Hospital of Los Angeles. Among the patients with omphalocele, the mortality was not significantly different between those with an abdominal wall defect smaller than 4 cm (5 of 24 patients) and those with a larger defect (6 of 33 patients); between those with a birth weight of less than 2,500 g (3 of 13 patients) and those with a higher birth weight (8 of 44 patients); between patients who had part of their liver in the omphalocele sac (6 of 29 patients) and t

Computed tomography of the heart and great vessels: present and future.

Computed tomography (CT) has emerged as a new imaging method for the diagnosis and evaluation of cardiovascular disease. With CT body scanners and contrast enhancement, evaluation of aortic dissections and aneurysms, coronary bypass graft patency, cardiovascular thrombus, cardiac tumors, and pericardial disease is possible. On occasion, this technique provides clinically useful information that is not available with other imaging methods. Electrocardiographic gating retrospectively or prospectively improves the image resolution of CT scans, but a new ultrafast CT scanner with a scan time of 30

Prenatal assessment of anterior abdominal wall defects and their prognosis.

Eighty-eight cases of abdominal wall defect with and without other lesions were ascertained by reviewing all labour room records in the West of Scotland, all surgical admissions to the Royal Hospital for Sick Children, Yorkhill, and all post mortems at Royal Hospital for Sick Children between 1978 and 1981. The affected cases comprised 39 terminations of pregnancy (17 of anterior abdominal wall defect without neural tube defect), 20 spontaneous stillbirths and 29 livebirths. All 10 cases of body stalk anomaly, 37.5 per cent of 16 gastroschisis cases and 53 per cent of 62 exomphalos cases had a

Malformations associated with congenital absence of the gall bladder.

Thirty-four cases (29 children and five adults) of congenital absence of the gall bladder were found in a retrospective necropsy study. When the distribution of associated malformations in these patients was analysed, the cases were found to fall into several groups. The largest group (13) had multiple anomalies involving the genitourinary (83% reproductive tract, 42% renal), gastrointestinal (46% imperforate anus, 23% tracheo-oesophageal fistula), cardiovascular (54% cardiac defects, 23% single umbilical artery), and skeletal (31%) systems. Eight other patients had predominantly cardiac anoma

Fetal anterior abdominal wall herniations: ultrasonographic diagnosis.

Gastroschisis and omphalocele are congenital abdominal wall herniations. Both are characterized by evisceratin of bowel loops, and additionally, especially in omphalocele, liver and occasionally other organs. The bowel loops present ultrasonographically as cystic masses in relation to the fetal abdomen. Sometimes the walls are thickened due to the inflammation from exposure to amniotic fluid. Ultrasonographic identification of herniated liver (omphalocele) indicates potential complex abnormalities of other organ systems, as opposed to gastroschisis, which is usually an isolated abnormality. Stage II ultrasound examination for the diagnosis of fetal abnormalities with an elevated amniotic fluid alpha-fetoprotein concentration.

Previous studies have indicated that patients with elevated levels of amniotic fluid alpha-fetoprotein (AFAFP) are at very high risk for fetal neural tube defects. Twenty-eight patients whose levels of AFAFP were greater than 3 standard deviations above the mean were evaluated with a Stage II ultrasound examination for detailed assessment of fetal anatomy. In 16 patients with AFAFP levels greater than 5 SD above the mean, six fetuses with open spina bifida were identified (37.5%). Moreover, in four fetuses in this group, the diagnosis was ventral wall defects (two with gastroschisis and two wi

Malrotation - the ubiquitous anomaly.

Although anomalies of rotation and fixation of the gastrointestinal tract are recognized to be associated frequently with anomalies of the abdominal wall and diaphragmatic hernia, the frequency with which these common anomalies are associated with other congenital and acquired lesions of the gastrointestinal tract has not been sufficiently emphasized. Sixty-two percent of our series of 34 patients were associated with such lesions as upper gastrointestinal atresias and stenoses, intussusception, and Hirschsprung's disease. The embryologic and anatomic features of rotational anomalies, their cl

Surgical correction and rehabilitation for children with "Prune-belly" syndrome.

Until the recent past, patients born with congenital absence of the abdominal musculature, and associated deformity of the urinary collecting system, have often had an unhappy life and an early death. Twelve years ago, a treatment protocol was devised in our institution which included the following major steps: 1) bilateral tubeless pyelostomy immediately after birth; 2) serial evaluation of renal status during the first year of life; 3) laparotomy at one year of age with orchiopexy, reconstruction of the urinary collecting system and reconstruction of the abdominal wall; 4) serial electromyog
