The congenital varicella syndrome.

Two children, aged 18 months and 3 years, with the congenital varicella syndrome are described. Neuropathic bladder and lax abdominal wall musculature were present in one case. In the other, paralysis and marked hypoplasia of one arm, and an ipsilateral Horner's syndrome, were the main abnormalities. Extensive skin scarring corresponding to a dermatome distribution was present in both children. This constellation of abnormalities, which may best be explained on the basis of a developmental radiculopathy, emphasises the neuropathic effects on the fetus of varicella zoster (VZ) infection in earl

Gastroschisis update.

Sixty-four infants with gastroschisis have been managed in the 9-yr period, 1970-1979, with four postoperative deaths. The silo technique has been the standard method of management in this series, permitting initial expansion of the abdominal cavity without increased abdominal pressure and respiratory embarassment. Removal of the silo and complete closure of the abdominal wall deficit were possible 5-12 days later. A high proportion of the infants were below 2500 g in weight at birth (61%); and although 3 of the 4 postoperative deaths occurred in the low birth weight group, this did not appear

Long-term results after repair of coarctation of the aorta by patch grafting.

Six patients who had been operated upon for coarctation of the aorta with patch grafting between 1958 and 1960 were followed for a minimum of 17 years. One of these patients received an aortic homograft patch for an abdominal coarctation and was in excellent health 21 years later. An aortogram showed no aneurysm formation. One other patient was reoperated upon 1 month after the primary operation because of a pseudoaneurysm. The remaining four patients received either a Dacron graft (three patients) or an Orlon graft (one patient), and they all developed an aneurysm in the aortic wall opposite

The problem of "giant" omphalocele.

"Giant" omphalocele implies an abdominal wall defect that is 5 cm or more in diameter with the liver in a central position. Giant omphalocele is often associated with other significant anomalies. Technically it is often difficult to close the abdominal wall defect and a staged repair utilizing prosthetic materials may be necessary. The morbidity and mortality associated with this entity remain significant despite advances in management techniques. Hospitalization is often prolonged and costly. For the best outcome in managing patients with giant omphalocele early attention to hypothermia and o

Gastroschisis and omphalocele. An eight-year review.

Until recently confusion has existed concerning the clinical features and surgical treatment of gastroschisis and omphalocele. Since 1971 75 infants with these abdominal wall defects have been treated at our institution. Significant differences (p equal to 0.001 in all instances) were noted between the two diseases. Gastroschisis occurred twice as often as omphalocele and is increasing in frequency. Prematurity was commonly seen with gastroschisis (65%). While the overall incidence of malformations associated with gastroschisis was low (23%), the vast majority of the additional malformations w

Familial occurrence of gastroschisis. Four new cases and review of the literature.

In two unrelated families, there was familial occurrence of gastroschisis. In one family, a boy and girl were affected and there was a family history of stillbirth, abortion, prematurity, and esophageal obstruction. In the second family, two boys were affected and there was a family history of spontaneous abortion, inguinal hernia, and umbilical hernia. The recurrence of gastroschisis, generally considered a sporadic congenital effect, suggests that the condition may be genetic in nature. Furthermore, the pedigree of one of the families suggests that gastroschisis may be a severe expression of

Prognostic factors in omphalocele and gastroschisis.

Size of the abdominal wall defect, viscera exposed or herniated, birth weight, associated medical conditions and congenital anomalies, mode and complications of treatment, and the use of total parenteral alimentation was reviewed in 79 cases of omphalocele and 44 cases of gastroschisis treated in the past 10 yr. Sixty-seven percent of infants with omphalocele and 73% of those with gastroschisis survived. In omphalocele, the most important factors affecting mortality were the presence of other associated abnormalities and low birth weight. The size of the abdominal wall defect, the viscera hern

Prostatic maldevelopment in the prune belly syndrome: a defect in prostatic stromal-epithelial interaction.

The prune belly syndrome consists of a triad of abdominal wall musculature deficiency, cryptorchidism and urinary tract anomalies. A rarely described but fairly constant component of the syndrome is the lack of development of the epithelial elements of the prostate. We wish to suggest that this is a further expression of a postulated arrest in mesenchymal development responsible for other features of the syndrome.

[Macroscopical observations on a human anencephalous fetus with multiple malformations (author's transl)].

The present report treats a female stillborn human anencephalous fetus with the craniorrhachischisis, cleft palate, left-sided congenital diaphragmatic hernia, hypoplasia of lungs, imperforate vagina and anus, absent left kidney and sternal muscle on the left side. In view of the rarity of the records of autopsied cases of anencephaly, authors made efforts to obtain informations on how all the organs in this fetus and their topographical relation in each other are affected by the anencephaly. The findings are summarized as follows. 1) This case falls under Anencephalus craniorrhachischisis in

Perinatal aspects of omphalocele and gastroschisis.

Forty-one infants treated for abdominal wall defects were studied retrospectively. Using established anatomic criteria the infants were identified as having either an omphalocele or gastroschisis. They were compared for maternal and infant characteristics to determine if distinct perinatal characteristics exist between the two groups. Gastroschisis patients had lower mean maternal age, were more likely to be first born, and to be of lower birth weight. Major malformations apart from those associated with the gastrointestinal tract and the anterior abdominal wall occurred 12 times more often in

Incidence of some surgically correctable congenital abnormalities in South Australia.

A retrospective 5 yr survey of the incidence of some neonatal surgically correctable congenital abnormalities in South Australia has been carried out. Meaningful figures have been obtained for the incidence, in terms of the live birth rate, of obstructive malformation of the gastrointestinal tract, major abdominal wall and diaphragmatic defects, and some anomalies of the genitourinary system. In the main, the incidence of these malformations is similar to that reported from other centers. However the incidence of exomphalos and small bowel obstructions is lower in this series than in others, a

Simplified treatment of large congenital ventral wall defects.

A preformed silicone polymer silo has been used in eleven infants with omphalocele and gastroschisis. This prosthesis has significant advantages over other forms of silo construction. Among these advantages are: (1) easy and rapid primary application; (2) a totally nonreactive and closed system which prevents peritoneum fluid loss and bacterial contamination; (3) a uniform constant stretching force applied to the abdominal wall which promotes expansion of the peritoneal cavity; and (4) the transparent material of the silo which permits constant evaluation of the contained viscera. These factor

One-stage reconstruction for exstrophy of the bladder in girls.

In an African female infant with exstrophy who was a candidate for complete reconstruction of the urinary tract, pelvis, and abdominal wall, the treatment program was simplified by performing iliac osteotomies at the same time as the other repairs, using a single transverse lumbosacral incision for access. A series of 3 wire loops held the symphysis in apposition. A plaster spica, followed by a plaster girdle, minimized the tension on the pubic wires. We believe that we obtained, by this method, superior strength and a better appearance of the abdominal wall and perineum.

Congenital defects of the gastrointestinal tract and abdominal wall. A three-year review.

One hundred forty-four infants with anomalies of the gastrointestinal tract and abdominal wall were treated at Children's Memorial Hospital between July 1970 and July 1973. The overall mortality, including unoperated infants and those dying from other causes up to six months later, was 17%. Associated defects, particularly of the heart and CNS, were the most common causes for death. Many infants were premature; however, this factor alone did not contribute to mortality. Differences in surgical technique did not affect mortality in patients with esophageal atresia, but one baby with total agang

Technical advances in resection and graft replacement of thoracic, abdominal, peripheral aneurysms.

Condition we now term aneurysm, deriving from the Greek word for "a sidening," has been recognized since the earliest days of medical history. The predilection of such lesions to rupture, with resultant hemorrhage, thrombosis, and distal ischemia, has led to constant attempts at surgical management, including ligation and incision, wrapping, wiring, plasticizing, packing, obliterative and reconstructive endoaneurysmorrhaphy, and a wide variety of procedures both ingenious and ingenuous. Modern treatment depends on minimal dissection, to obtain proximal and distal control, incision of only the

Amniocentesis dysmelia in rats.

Amniocentesis performed on Day 15½ of gestation caused dysmelia in 85 of 143 offspring of Sprague-Dawley and Wistar strain rats. Almost all defects were of reduction type, only 3 hind limb postaxial supernumerary digits being seen. Defect intensity graded from longitudinal splitting of the phalanges, through phalangeal suppression, to very severe growth retardation of the long limb bones and the limb girdle. The lesions in the 2 strains were indistinguishable from each other. Subcutaneous haemorrhages were frequently present and could take the form of blebs. Hind limbs were more frequently and
